1.Association of abnormal expression of angiotensin-converting enzyme and lipoprotein(a) with hypertension-diabetes comorbidity

Objective To investigate the relationship between abnormal expression of angiotensin-converting enzyme(ACE)and lipoprotein(a)[Lp(a)]with hypertension-diabetes comorbidity.Methods A case-control analysis was conducted on 342 patients with metabolic diseases admitted in our hospital between January 2021 and May 2024,and they were divided into in a diabetes-hypertension comorbidity group(114 cases),a diabetes group(114 cases)and a hypertension group(114 cases).Another 114 matched healthy individuals who taking physical examination in our hospital during the same period were subjected and served as the control group.Clinical data were collected and serum levels of ACE and Lp(a)were measured in all the groups.Multivariate logistic regression analysis was applied to analyze the independent risk factors for hypertension-diabetes comorbidity,and ROC curve analysis was performed to evaluate and predict the efficacy.Results The serum levels of ACE and Lp(a)were significantly higher in the comorbidity group than the control,hypertension and diabetes groups(P<0.01),and in the hypertension and diabe-tes groups than the control group(P<0.01).After adjusting for waist-to-hip ratio,alanine amin-otransferase and triglyceride,multivariate logistic regression analysis revealed that serum ACE and Lp(a)still were risk factors for the occurrence of hypertension-diabetes comorbidity(P<0.01).ROC curve analysis demonstrated that when the cut-off value was 39.01 pg/L,ACE had an AUC value of 0.884(95%CI:0.789-0.980),a sensitivity of 76.67%and a specificity of 75.28%in predicting the comorbidity,and when the cut-off value for Lp(a)was 254.33 mg/L,the AUC value was 0.899(95%CI:0.799-0.919),the sensitivity was 85.83%,and the specificity was 82.22%.Their combination improved the AUC value to 0.993(95%CI:0.979-1.000),with a sensitivity of 95.00%and a specificity of 95.28%in the prediction.Conclusion Serum ACE and Lp(a)are independent risk factors for hypertension-diabetes comorbidity,and can be combined together and regarded as biomarkers for early identification of the comorbidity.
2.A randomized controlled trial on effects of Baduanjin and brisk walking on sleep quality in female college students

Objective:To explore the effects of Baduanjin and brisk walking on the sleep quality among fe-male college students.Methods:Ninety female college students with poor sleep quality[Pittsburgh Sleep Quality Index(PSQI)≥ 8]were recruited randomly assigned to Baduanjin,brisk walking,and control groups,with 30 par-ticipants in each.The Baduanjin and brisk walking groups participated in 10-week intervention(five 45-minute ses-sions per week),while the control group did not receive any intervention.Baseline and post-intervention assessments were conducted using the PSQI,a lung capacity test,echocardiography,and the Fatigue Scale(FS-14).Results:Af-ter 10 weeks,participants in both the Baduanjin and brisk walking groups got significantly lower PSQI and FS-14 total scores compared to baseline(Ps<0.001).Cardiopulmonary function indicators,including stroke volume(SV),forced expiratory volume in one second(FEV1.0),the vital capacity-to-body mass index(VC/W),and maximum voluntary ventilation per minute(MVV),also significantly improved(Ps<0.001).Furthermore,the Baduanjin group had significantly lower PSQI and FS-14 scores than both the brisk walking and control groups(P<0.001),along with superior improvements in cardiopulmonary function(P<0.001).Conclusion:This study in-dicates that Baduanjin is particularly effective in improving sleep quality,cardiopulmonary function,and reducing fatigue among female college students,showing advantages over brisk walking.
3.Summary of the best evidence for the prevention and management of chyle leak after thyroid cancer surgery

Objective:To retrieve and obtain relevant evidence at home and abroad on the prevention and management of chylous leakage after thyroid cancer surgery, and provide reference for clinical practice.Methods:Based on the 6S pyramid model system, evidence related to the prevention and management of chylous leakage after thyroid cancer surgery in the databases of UpToDate, BMJ best practice, Guidelines International Network (GIN), National Guideline Clearinghouse (NGC), PubMed, EMbase, Wanfang Database, China National Knowledge Infrastructure (CNKI) and other databases, including guidelines, expert consensus, systematic reviews, evidence summary, clinical decisions, and position statements. The retrieval time was from database establishment to June 1st 2024.Results:A total of 16 articles were included, including 5 guidelines, 9 expert consensus, and 2 systematic reviews. The 30 best pieces of evidence were summarized from 6 aspects: preoperative evaluation and preparation, intraoperative prevention, diagnosis, diet and nutrition management, monitoring and treatment, and health education.Conclusions:This study summarized relevant evidence and provides reference for medical staff to formulate intervention measures and management plans for chyle leakage after thyroid cancer surgery.

7.Influencing factors and epidemiological survey of diffuse toxic goiter with hyperthyroidism in Xining

Objective To investigate the epidemiological status of toxic diffuse goiter hyperthyroidism (abbreviated as hyperthyroidism) in Xining area and analyze its influencing factors. Methods Patients with toxic diffuse goiter and hyperthyroidism who were hospitalized in Class ⅲ Grade A hospitals in Xining from January 2020 to January 2023 were collected as the experimental group by random cluster sampling method. During the same period, 500 healthy people in each hospital were selected as the control group. The general data of the patients were collected and the levels of thyroid function indexes of the two groups were detected. Multivariate logistic regression was used to analyze the influencing factors of diffuse goiter with hyperthyroidism. Results A total of 718 questionnaires were collected in this study, and 705 questionnaires were collected after excluding invalid questionnaires. There were 234 males and 471 females in 705 patients with diffuse goiter and hyperthyroidism. The most common age was 41-50 years, followed by 51-60 years and 31-40 years. The serum TSH level of the experimental group was lower than that of the control group, and the levels of FT3 and FT4 were higher than those of the control group ( P < 0.05 ) . There was no significant difference in family history, thyroid texture and thyroid imaging between the two groups ( P > 0.05 ). There were significant differences in exophthalmos and thyroid weight between the two groups (P<0.05). Multivariate Logistic regression analysis showed that exophthalmos, thyroid weight ≥30g , TSH , FT3 and FT4 were independent risk factors for the experimental group ( P < 0.05) . Conclusion Gender, age, exophthalmos, thyroid weight and thyroid related hormone levels are the influencing factors of diffuse goiter and hyperthyroidism in Xining area . Thyroid function should be monitored for early prevention and treatment of the disease.
8.Application of IDEAS model-based preoperative nursing in patients with early esophageal cancer undergoing ESD for the first time

Objective:To explore the application effect of IDEAS model-based preoperative nursing in early esophageal cancer patients undergoing endoscopic submucosal dissection (ESD) for the first time.Methods:A total of 130 patients with early esophageal cancer who underwent ESD for the first time in Affiliated Hospital of Jining Medical University from May 2021 to June 2023 were selected as the study objects by the convenient sampling method. The patients were divided into the experimental group and the control group by random number table method, with 65 cases in each group. The control group received routine preoperative care, while the experimental group received the IDEAS model-based preoperative care based on the control group. Chinese version of Medical Coping Questionnaire, heart rate, diastolic blood pressure, systolic blood pressure and Nursing Satisfaction Questionnaire were used to evaluate the effect of intervention.Results:30 minutes before entering the gastroscopy room, the score of face dimension in Chinese version of Medical Coping Questionnaire in the experimental group was higher than that in the control group, while the scores of avoidance and submission dimensions were lower than those in the control group, and the differences were statistically significant ( P<0.01). After entering the gastroscopy room, the heart rate, diastolic blood pressure and systolic blood pressure of the experimental group were lower than those of control group, and the differences were statistically significant ( P<0.01). The total satisfaction of nursing in the experimental group was higher than that in control group, and the difference was statistically significant ( P<0.05) . Conclusions:The IDEAS model-based preoperative care can increase the positive coping strategies of early esophageal cancer patients undergoing ESD surgery for the first time, reduce their cardiovascular stress response and improve patients' satisfaction with nursing.
9.Effect of anesthesia on the recurrence and metastasis of hepatocellular carcinoma

Hepatocellular carcinoma (HCC) is one of the leading causes of cancer death. There is a high risk of recurrence and metastasis after surgery. Anesthesia methods, anesthesia-related drugs and intraoperative anesthesia management can affect the biological behavior of HCC cells or the body's immunity, thus affecting the recurrence and metastasis of HCC. Paying attention to the effect of anesthesia on recurrence and metastasis of HCC and optimizing anesthesia management are expected to improve the long-term survival of patients.
10.Relationship between the prognosis of late-onset depression in the elderly and lncRNA expression levels in peripheral blood and coping styles

Objective:To investigate the correlation between the prognosis of late-onset depression(LOD)in the elderly and lncRNA expression levels and coping styles.Methods:Differential expression of lncRNAs in peripheral blood of LOD 92 patients was detected by a real-time quantitative PCR(qPCR)detection system, and the Hamilton Depression Scale(HAMD)and the Trait Coping Style Questionnaire(TCSQ)were used for psychological assessment.Results:Compared with the control group, the expression levels of TCONS_00019174(7.55 vs.4.36), ENST00000566208(6.48 vs.3.26), ENST00000517573(8.33 vs.5.32)and NONHSAT142707(6.78 vs.3.26)in elderly patients of the LOD group were significantly down-regulated( Z=5.09, 5.87, 4.35, 6.44, P<0.05); Compared with the low-expression subgroup, scores of anxiety/somatization[(3.83±1.40) vs.(6.39±2.35)], diurnal variation[(0.22±0.42) vs.(0.83±0.94)], retardation[(5.74±0.96) vs.(6.48±1.28)], hopelessness[(2.78±0.67) vs.(4.52±1.56)]and HAMD[(20.39±1.75) vs.(26.83±4.88)]in the high-expression subgroup were significantly lower( t=-4.50, -2.84, -2.22, -4.90, -5.96, P<0.05). The ΔCT value of TCONS_00019174 was negatively correlated with the reduction rates of anxiety/somatization, diurnal variation, retardation, sleep disturbance, hopelessness and HAMD( r=-0.40-0.66, P<0.05). The ΔCT value of ENST00000566208 was negatively correlated with the reduction rates of anxiety/somatization, sleep disturbance, hopelessness and HAMD( r=-0.47-0.62, P<0.01). The ΔCT values of ENST00000517573, NONHSAT034045 and NONHSAT142707 were negatively correlated with the reduction rates of retardation, sleep disturbance, hopelessness and HAMD( r=-0.39-0.76, P<0.05). The positive coping style was positively correlated with the reduction rates of HAMD, anxiety/somatization, retardation, sleep disturbance and hopelessness( r=0.38-0.55), while the negative coping style was negatively correlated with the reduction rates of HAMD, anxiety/somatization, sleep disturbance and hopelessness( r=-0.39-0.67, P<0.05). When TCONS_00019174, ENST00000566208, NONHSAT034045, NONHSAT142707, positive coping and negative coping were taken into the regression equation as variables for HAMD reduction, it was found that they were able to explain 32.4% of the variance for the reduction rate of the total HAMD score( t=-8.713, -3.584, -3.864, -2.257, 5.675, -2.357, P<0.05). Conclusions:TCONS_00019174, ENST00000566208, NONHSAT034045, NONHSAT142707, positive coping style and negative coping style are predictors of the prognosis of LOD in the elderly.
