1.Efficacy of orthokeratology lenses in adolescent myopia patients with different diopters

AIM: To explore the control effects of wearing orthokeratology lens for 1 a on adolescent myopia patients with different diopters.METHODS: Prospective non-randomized controlled study. A total of 120 adolescent myopic patients(224 eyes), with an average age of 11.00±2.08 years old, who were fitted with orthokeratology lenses in the optometry department of our hospital from November 2022 to May 2023 were collected. There were 3 groups according to the spherical equivalent, including 86 eyes in the group of -0.50--2.00 D, 99 eyes in the group -2.25--4.00 D, and 39 eyes in group -4.25--6.00 D. Patients were followed up for 1 a to observe the changes of uncorrected visual acuity, axial length, corneal curvature, corneal central thickness and corneal endothelial cells density in the three groups after wearing lens for 1 a.RESULTS:A total of 113 cases(212 eyes)were followed up for 1 a, including 82 eyes in the group of -0.50--2.00 D, 95 eyes in the group of -2.25--4.00 D, and 35 eyes in the group of -4.25--6.00 D. There was no statistical difference in corneal central thickness and corneal endothelial cell density among the three groups of patients after wearing lens for 1 a(all P>0.05). Uncorrected visual acuity was significantly improved, and flat kerotometry(FK)and steep kerotometry(SK)were significantly flatter(both P<0.01). Furthermore, the growth of axial length in the three groups of patients after wearing lens for 1 a was 0.21±0.26, 0.13±0.21 and 0.09±0.10 mm, respectively(P<0.05). There were differences between the -0.50--2.00 D group and the -2.25--4.00 D group and -4.25--6.00 D group(P=0.028, 0.010), and there were no differences between the -2.25--4.00 D group and the -4.25--6.00 D group(P=0.344).CONCLUSION:It is safe and effective for young myopia patients to wear orthokeratology lenses, which can prevent the non-benign growth of the axial length and effectively delay the development of myopia, and the myopia control effect is better especially for myopia patients of above -2.0 D.
2.Clinical characteristics combined with inflammatory markers for predicting prognosis of patients with acute ischemic stroke after mechanical thrombectomy

Objective To explore the potential prognostic factors of patients with acute ischemic stroke(AIS)after undergoing endovascular mechanical thrombectomy and to construct an effective predictive model.Methods A retrospective analysis of clinical data was conducted on 202 patients with anterior circulation large vessel occlusion AIS from 2 stroke centers.All patients received endovascular mechanical thrombectomy treatment,with treatment and follow-up lasting at least 90 d.Basic demographic characteristics,medical records,and baseline blood biomarker data were collected,and the potential prognostic indicators for AIS after 90 d were screened using least absolute shrinkage and selection operator(LASSO)-logistic regression analysis.Results It was found that alcohol drinking(P=0.029),hypertension(P=0.001),diabetes mellitus(P=0.021),stroke or transient ischemic attack(P=0.049),systolic blood pressure on admission(P=0.009),diastolic blood pressure on admission(P=0.038),blood glucose(P=0.003),white blood cell count(P=0.001),neutrophil count(P=0.001),fibrinogen(P=0.010),systemic immune-inflammation index(P=0.008)and neutrophil-to-lymphocyte ratio(NLR)(P<0.001)were associated with adverse clinical outcomes.Nine significant prognostic determinants were screened through LASSO-logistic regression analysis.Multivariate logistic regression analysis revealed that male sex(P=0.008),smoking history(P=0.013),hypertension(P=0.011),lymphocyte(P=0.028),fibrinogen(P=0.016),and NLR(P<0.001)were significant predictive factors for poor prognosis in AIS patients after endovascular thrombectomy treatment.The constructed prognostic model had an accuracy of 76.2%,a sensitivity of 78.2%,a specificity of 71.7%,and a positive predictive value of 86.7%.Conclusion The predictive model established in this study can assist clinicians in identifying high-risk patients with AIS who have undergone endovascular thrombectomy,and it provide guidance for formulating individualized treatment strategies.
3.Application of centrifugal microfluidic chips based on loop-mediated isothermal amplification technology for rapid detection of foodborne pathogens

Objective:To investigate the application value of centrifugal microfluidic chips based on loop-mediated isothermal amplification (LAMP) technology for rapid detection of foodborne pathogens.Methods:A retrospective analysis was conducted on 128 test samples collected by the Yan'an Center for Disease Control and Prevention from January 2022 to January 2024. All samples underwent bacterial culture and testing using centrifugal microfluidic chips based on LAMP technology. The detection time, spiking concentration, and accuracy of two detection methods were compared. Additionally, the diagnostic efficacy of both methods was analyzed using receiver operating characteristic (ROC) curves.Results:Bacterial cultures were mostly detected within 16-48 hours, with a detection limit concentrated at 10 000 CFU/25 g. In contrast, the LAMP-based centrifugal microfluidic chip technology typically yielded results within 5-16 hours, with a detection limit ranging from 1 000 to 10 000 CFU/25 g. The differences were statistically significant ( t = 14.86, 573.04, both P < 0.001). When compared with subsequent confirmation results, the positive rate for bacterial culture was 75.00% (24/32), with a diagnostic accuracy of 80.47% (103/128), showing a low consistency (Kappa = 0.51, P < 0.001). Meanwhile, the LAMP-based centrifugal microfluidic chip technology had a positive detection rate of 93.75% (30/32) and a diagnostic accuracy of 96.87% (124/128), also exhibiting low consistency (Kappa = 0.92, P < 0.001). In paired comparisons, the difference between the LAMP-based centrifugal microfluidic chips and bacterial culture was statistically significant ( χ2 = 53.14, P < 0.001). ROC curve analysis indicated that the area under the curve for bacterial culture in diagnosing foodborne pathogens was 0.77, while the AUC for LAMP-based centrifugal microfluidic chip technology was 0.95. Conclusions:When detecting foodborne pathogens, the use of LAMP-based centrifugal microfluidic chip technology allows for the rapid identification of pathogens in a short time period, offering advantages such as high efficiency, speed, and accuracy.
4.Analysis of intellectual property sharing in international cooperation agreements involving human genetic resources in medical institutions: taking Peking University Cancer Hospital as an example

Objective:To understand the current status and existing issues of intellectual property ownership arrangements in international cooperation agreements concerning human genetic resources, and to explore suggestions for medical institutions to strengthen the management of Sino-foreign cooperation agreements, in order to safeguard the rights of medical institutions to benefit-sharing and promote the sustainable development of international cooperation involving human genetic resources.Methods:This study reviewed the international cooperative scientific research projects approved or completed for filing by Peking University Cancer Hospital on the National Health Commission′s Government Service Platform from July 2019 to December 2024. This study analyzed the nature of the research and the provisions regarding patent rights and intellectual property rights of other scientific and technological achievements in the hospital′s international cooperation agreements with sponsors. Existing issues in intellectual property ownership arrangements was summarized, and corresponding recommendations were proposed.Results:A total of 390 international cooperation projects on human genetic resources were analyzed. Among them, there were 66 exploratory research projects, 138 marketing research projects, and 186 projects included both exploratory research and marketing research. Among the cooperation agreements containing exploratory research, 78.6% did not specify the specific connotation of exploratory research. All agreements stipulated that the hospital alone or jointly held the patent rights for the achievements generated from exploratory research. 15.1% of the agreements restricted the geographical scope of the patent rights, 13.1% restricted the hospital′s implementation of the patent rights, 8.7% unilaterally restricted the hospital's external licensing and transfer of the patent rights, and 43.7% did not stipulate the ownership of other scientific and technological achievements other than the patent rights.Conclusions:There is a lack of clear and standardized regulations regarding the scope of exploratory research. The intellectual property arrangements in the agreements show an interest-oriented tendency. The sponsors restrict the implementation, transfer, and licensing of shared patent rights by medical institutions through agreements. For other scientific and technological achievements derived from the cooperation, apart from patent rights, medical institutions have not fully exercised the rights stipulated by law. It is recommended that medical institutions clearly specify the scope of application of exploratory research. They should pay attention to the stipulations of specific rights such as the right to enforce, transfer, and license patents. They should also make full use of the enabling provisions of the law, clearly define in the agreement the ownership of other scientific and technological achievements and the distribution of rights and interests, so as to achieve a balance of interests with their partners.
5.Prediction value of peripheral blood circular RNA_0001449 combined with ABCD3-Ⅰ score on cerebral infarction after transient ischemic attack

Objective To explore the prediction value of peripheral blood circular RNA_0001449 combined with ABCD3-Ⅰ score on cerebral infarction after TIA.Method According to the occurrence of cerebral infarction 90 days after TIA,204 patients diagnosed with TIA for the first time were divided into cerebral infarction progression group and non-cerebral infarction group.The clinical data,ABCD3-Ⅰ score and peripheral blood circular RNA_0001449 expression level of the two groups were compared.The Logistic multivariate regression model analyzed the risk factors of cerebral infarction in TIA patients.ROC curve was used to analyze the predictive value of ABCD3-Ⅰscore and peripheral blood circular RNA_0001449 expression level on cerebral infarction of TIA patients.Results The fasting blood glucose level,ABCD3-Ⅰ score and peripheral blood circular RNA_0001449 expression of cerebral infarction progression group were significantly higher than those of non-cerebral infarction group(all P<0.05).Logistic multivariate regression analysis showed that increased ABCD3-Ⅰ score and increased peripheral blood circular RNA_0001449 expression were risk factors for cerebral infarction of TIA patients(OR=1.975,95%CI:1.352-2.883,P<0.001;OR=22.475,95%CI:1.460-345.868,P=0.026).ROC curve analysis showed that the area under the curve(AUC)of ABCD3-Ⅰ score and peripheral blood RNA_0001449 expression independently predict cerebral infarction of TIA patients were 0.805 and 0.840,respectively(all P<0.05),the AUC of combination prediction was 0.881(P<0.05).The AUC of combination prediction was significantly higher than that of ABCD3-Ⅰ score and peripheral blood RNA_0001449 expression independent prediction(Z=4.851,P=0.011;Z=4.236,P=0.018).Conclusion Peripheral blood circular RNA_0001449 combined with ABCD3-Ⅰscore has predictive value for cerebral infarction 90 d after the onset of TIA.

In paired comparisons, the difference between the LAMP-based centrifugal microfluidic chips and bacterial culture was statistically significant ( χ2 = 53.14, P < 0.001). ROC curve analysis indicated that the area under the curve for bacterial culture in diagnosing foodborne pathogens was 0.77, while the AUC for LAMP-based centrifugal microfluidic chip technology was 0.95. Conclusions:When detecting foodborne pathogens, the use of LAMP-based centrifugal microfluidic chip technology allows for the rapid identification of pathogens in a short time period, offering advantages such as high efficiency, speed, and accuracy.

8.Clinical application of laparoscopic iliopubic tract-to-arcuate inferior edge of transversus abdominis suture tightening of the internal ring combined with high ligation of the hernia sac in pediatric giant inguinal hernias

Objective:To investigate the clinical efficacy of laparoscopic iliopubic tract-to-arcuate inferior edge of transversus abdominis suture tightening of the internal ring combined with high ligation of the hernia sac in pediatric giant inguinal hernias.Methods:Randomized controlled study.Clinical data of 62 hernia sides in 61 children with inguinal hernia admitted to the Department of Minimally Invasive Urology, Children′s Hospital Affiliated to Shandong University from March 2021 to June 2024 were retrospectively analyzed.The maximum internal ring diameter, measured intraoperatively using an equal proportional indirect measurement technique, was >1.5 cm, defining a giant hernia.One child had bilateral giant hernias.Patients were divided into 2 groups based on surgical technique: the control group [29 sides (28 patients)] underwent traditional laparoscopic high ligation of the hernia sac; the study group [33 sides (33 patients)] underwent the laparoscopic iliopubic tract-to-arcuate inferior edge of transversus abdominis suture tightening of the internal ring combined with high ligation of the hernia sac.Independent sample t-test, Mann-Whitney U test, and Fisher′s exact test were used to compare preoperative characteristics and postoperative complication rates between the two groups. Results:The study group had a median age of 38.00 (20.00, 107.50) months, 93.9%(31/33) were male, and median maximum internal ring diameter was 1.61(1.53, 1.82) cm.The control group had a median age of 32.50 (22.25, 65.00) months, 78.6%(22/28) were male, and median maximum internal ring diameter was 1.58 (1.54, 1.71) cm.There were no statistically significant differences in baseline characteristics between groups (all P>0.05). The operative time for the suture tightening/high ligation component was longer in the study group [(21.73±9.81) minutes] compared to the simple high ligation time in the control group [(16.69±7.36) minutes], with statistical significance ( t=-2.262, P=0.027). In terms of recurrence rate, there was a statistically significant difference between the study group(0) and the control group (4 cases, 13.80%) ( P=0.043). There was no statistical difference between the two groups in the incidence of other complications including intraoperative bleeding, groin area edema, or pain (all P>0.05). Conclusions:The laparoscopic iliopubic tract-to-arcuate inferior edge of transversus abdominis suture tightening of the internal ring combined with high ligation of the hernia sac can effectively reduces postoperative recurrence when applied to the treatment of large hernias and is safe, making it a technique worthy of promotion.
