1.A prospective study of association between physical activity and ischemic stroke in adults

Objective:To explore the prospective associations between physical activity and incident ischemic stroke in adults.Methods:Data of China Kadoorie Biobank study in Tongxiang of Zhejiang were used. After excluding participants with cancers, strokes, heart diseases and diabetes at baseline study, a total of 53 916 participants aged 30-79 years were included in the final analysis. The participants were divided into 5 groups according to the quintiles of their physical activity level. Cox proportional hazard regression models was used to calculate the hazard ratios ( HR) for the analysis on the association between baseline physical activity level and risk for ischemic stroke. Results:The total physical activity level in the participants was (30.63±15.25) metabolic equivalent (MET)-h/d, and it was higher in men [(31.04±15.48) MET-h/d] than that in women [(30.33±15.07) MET-h/d] ( P<0.001). In 595 526 person-years of the follow-up (average 11.4 years), a total of 1 138 men and 1 082 women were newly diagnosed with ischemic stroke. Compared to participants with the lowest physical activity level (<16.17 MET-h/d), after adjusting for socio-demographic factors, lifestyle, BMI, waist circumference, and SBP, the HRs for the risk for ischemic stroke in those with moderate low physical activity level (16.17-24.94 MET-h/d), moderate physical activity level (24.95-35.63 MET-h/d), moderate high physical activity level (35.64-43.86 MET-h/d) and the highest physical activity level (≥43.87 MET-h/d) were 0.93 (95% CI: 0.83-1.04), 0.87 (95% CI: 0.76-0.98), 0.82 (95% CI: 0.71-0.95) and 0.76 (95% CI: 0.64-0.89), respectively. Conclusion:Improving physical activity level has an effect on reducing the risk for ischemic stroke.
2.Circadian Rhythm of Blood Pressure in Patients with Masked Hypertension and its Relationship with Twelve Two-hour Periods: A Cross-sectional Study

ObjectiveTo explore the characteristics of circadian rhythm of blood pressure in patients with masked hypertension (MH) and its relationship with twelve two-hour peirods, providing a basis for early detection and intervention of MH. MethodsPatients who underwent 24-hour ambulatory blood pressure examination in the ambulatory blood pressure room of Jiangsu Province Hospital of Chinese Medicine from January to December 2022 were enrolled, and according to their outpatient blood pressure measurements, 24-hour ambulatory blood pressure monitoring and follow-up survey results, the MH, essential hypertension (EH) and normal blood pressure groups were classified, with 50 cases in each group. The general data, office blood pressure and 24-hour ambulatory blood pressure monitoring data were collected. The circadian rhythm of blood pressure including 24-hour average systolic blood pressure (24h SBP), daytime average systolic blood pressure (d SBP), nighttime average systolic blood pressure (n SBP), 24-hour average diastolic blood pressure (24h DBP), daytime average diastolic blood pressure (d DBP), and nighttime average diastolic blood pressure (n DBP) were compared among the groups, and the nighttime blood pressure dipping percentage was calculated. The type of circadian rhythm was determined based on the nighttime blood pressure dipping percentage. The variability of blood pressure including 24h SBP standard deviation (24h SBP-SD), d SBP standard deviation (dSBP-SD), n SBP standard deviation (nSBP-SD), 24h DBP standard deviation (24h DBP-SD), d DBP standard deviation (dDBP-SD), and n DBP standard deviation (nDBP-SD) were compared among groups, and the corresponding coefficient of variation (CV), that is, 24h SBP-CV, d SBP-CV, n SBP-CV, 24h DBP-CV, d DBP-CV and n DBP-CV, were calculated. Based on the 24-hour ambulatory blood pressure monitoring results, the twelve two-hour average SBP and DBP in each group were calculated and compared. Simultaneously, patients with EH were divided into grades 1, 2, and 3 for further stratified analysis. ResultsThe age of the MH group was significantly higher than that of the EH group and the normal blood pressure group (P<0.01). The body mass index (BMI) and the proportion of smoking and alcohol consumption in the MH group and the EH group were significantly higher than those in the normal blood pressure group (P<0.05 or P<0.01). In the normal blood pressure group, there were 49 dipper patterns (98.0%) and one non-dipper pattern (2.0%); in the MH group, there were two dipper patterns (4.0%), 29 non-dipper patterns (58.0%) and 19 reverse-dipper patterns (38.0%); in the EH group, there were 20 dipper patterns (40.0%), 23 non-dipper patterns (46.0%) and seven reverse-dipper patterns (14.0%). Compared to the normal blood pressure group, the groups of MH and EH had significantly decreased proportion of dipper pattern and increased proportion of non-dipper and reverse-dipper pattern (P<0.01); the proportion of dipper pattern in the MH group was lower than that in the EH group, while the proportion of reverse-dipper pattern was higher (P<0.01). Compared to those in the normal blood pressure group, n SBP and n DBP in the MH group, as well as the the average SBP and average DBP at Zi hour (子时, 23:00-1:00), Chou hour (丑时, 1:00-3:00), Yin hour (寅时, 3:00-5:00), Mao hour (卯时, 5:00-7:00) and average SBP at Hai hour (亥时, 21:00-23:00) in the MH group increased,while the average DBP at Si hour (巳时, 9:00-11:00) decreased (P<0.01); 24h SBP, 24h DBP, d SBP, d DBP, n SBP, and n DBP,d SBP-SD, n SBP-SD,n DBP-SD increased, as well as the average SBP and average DBP at twelve two-hour periods increased in the EH group,while the 24h SBP-CV, 24h DBP-CV,and d DBP-CV in the EH group decreased(P<0.05 or P<0.01). The EH group had higher 24h SBP, 24h DBP, d SBP, d DBP, n SBP, n DBP, 24h DBP-SD and n DBP-SD , as well as higher average SBP and DBP at all twelve two-hour periods, and lower d DBP-CV than the MH group(P<0.05 or P<0.01). The EH group had 18 cases of grade 1 (36.0%), 19 cases of grade 2 (38.0%) and 13 cases of grade 3 (26.0%), with no significant differences among groups (P>0.05). ConclusionThe circadian rhythm of blood pressure in MH patients are mostly non-dipper and reverse-dipper patterns, and the abnormal elevation of blood pressure is obvious at Zi hour, Chou hour, Yin hour and Mao hour (23:00-7:00).
3.Unicompartmental knee arthroplasty in treatment of medial compartment osteoarthritis of knee joint:joint function and changes in inflammatory cytokines

BACKGROUND:Unicompartmental knee arthroplasty has been paid more and more attention because of its advantages of fewer traumas,less blood loss,less complications,faster recovery,excellent knee function and high prosthetic survival rate.Most of the relevant reports focused on its clinical effect,but the changes in the levels of relevant inflammatory cytokines in patients after unicompartmental knee arthroplasty were rarely reported. OBJECTIVE:To investigate the effect of unicompartmental knee arthroplasty in patients with medial compartment osteoarthritis of the knee and the changes of interleukin-1,interleukin-6,and tumor necrosis factor-α levels. METHODS:A total of 30 patients with medial compartment osteoarthritis of the knee who received unicompartmental knee arthroplasty from September 2021 to March 2023 and met the inclusion criteria were selected as the study subjects.Medial angle of proximal tibia,femoral and tibial angle,pain visual analog scale score,hospital for special surgery score,knee joint range of motion,synovial fluid interleukin-1,interleukin-6,and tumor necrosis factor-α levels before and after surgery were compared and statistically analyzed. RESULTS AND CONCLUSION:(1)All 30 patients had grade I wound healing.All patients were followed up for more than 6 months after surgery.There were no complications such as prosthesis loosening and dislocation of prosthesis liner in all patients.(2)The medial proximal tibia angle and the femoral and tibial angles measured after operation were significantly improved(P<0.05).(3)The visual analog scale score,hospital for special surgery score and range of motion measured at 1,3,and 6 months after operation were better than those before operation(P<0.05),and showed a gradual improvement(P<0.05).(4)The levels of interleukin-1,interleukin-6,and tumor necrosis factor-α in synovial fluid at 3 and 6 months after operation were decreased gradually compared with those before operation(P<0.05)and showed a gradual downward trend at each time point after operation.(5)It is indicated that unicompartmental knee arthroplasty can effectively treat medial compartment osteoarthritis of the knee,significantly relieve pain,restore knee joint function,improve knee varus deformity and lower limb strength line,and reduce the levels of inflammatory cytokines interleukin-1,interleukin-6,and tumor necrosis factor-α in joints to a certain extent.

5.Curative Effect of Cocktail Combined with Tranexamic Acid on Total Knee Arthroplasty under the Concept of Enhanced Recovery after Sur-gery

Objective To investigate the curative effect of cocktail combined with tranexamic acid(TXA)on total knee arthroplasty(TKA)under the concept of enhanced recovery after surgery(ERAS).Methods Seventy patients with knee osteoarthritis who received unilateral TKA treatment admitted to the Second Affiliated Hospital of Bengbu Medical College from September 2020 to July 2022 were ret-rospectively analyzed.According to different treatment methods,they were divided into experimental group and control group,with 35 ca-ses in each group.Under the guidance of ERAS concept,the experimental group received injected intraarticular mixture of cocktail and TXA during the operation,while the control group received injected intraarticular mixture of cocktail and normal saline.Postoperative drainage volume,hemoglobin(Hb)decline and total blood loss at 3 days after operation,blood transfusion rate,first ambulation time,length of hospital stay,postoperative pain visual analogue scale(VAS)、knee range of motion(ROM)and HSS score at different time points after operation,delayed wound healing,wound infection and incidence of deep vein thrombosis were compared between the two groups.Results The postoperative drainage volume,decline of Hb and total blood loss at 3 days after operation,blood transfusion rate in the experimental group were less than those of the control group,the differences were statistically significant(P<0.05).The first ambu-lation time and length of hospital stay in the experimental group were significantly lower than those of the control group,the difference were statistically significant(P<0.05).There were no significant differences in pain VAS score at 1 day,2 days,3 days,7 days,14 days af-ter operation between two groups(P>0.05).There were no significant differences in ROM at 7 days,14 days after operation and HSS score at 4 weeks,12 weeks after operation between two groups(P>0.05).Conclusion Under the concept of ERAS,intraarticular in-jection of cocktail combined with TXA in TKA can effectively reduce postoperative blood loss,improve postoperative pain,reduce periop-erative risks,and promote better early postoperative rehabilitation,the clinical effect is significant.

The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection has spread worldwide and threatened human's health. With the passing of time, the epidemiology of coronavirus disease 2019 evolves and the knowledge of SARS-CoV-2 infection accumu-lates. To further improve the scientific and standardized diagnosis and treatment of maternal SARS-CoV-2 infection in China, the Chinese Society of Perinatal Medicine of Chinese Medical Association commissioned leading experts to develop the Recommendations for the Diagnosis and Treatment of Maternal SARS-CoV-2 Infection under the guidance of the Maternal and Child Health Department of the National Health Commission. This recommendations includes the epidemiology, diagnosis, management, maternal care, medication treatment, care of birth and newborns, and psychological support associated with maternal SARS-CoV-2 infection. It is hoped that the recommendations will effectively help the clinical management of maternal SARS-CoV-2 infection.

Hip fracture is considered as the most severe osteoporotic fracture characterized by high disability and mortality in the elderly. Improved surgical techniques and multidisciplinary team play an active role in alleviating prognosis, which places higher demands on perioperative nursing. Dysfunction, complications, and secondary impact of anaesthesia and surgery add more difficulties to clinical nursing. Besides, there still lack clinical practices in perioperative nursing for elderly patients with hip fracture in China. In this context, led by the Orthopedic Nursing Committee of Chinese Nursing Association, the Expert consensus on clinical practice in perioperative nursing for elderly patients with hip fracture ( version 2023) is developed based on the evidence-based medicine. This consensus provides 11 recommendations on elderly patients with hip fracture from aspects of perioperative health education, condition monitoring and inspection, complication risk assessment and prevention, and rehabilitation, in order to provide guiding advices for clinical practice, improve the quality of nursing and ameliorate the prognosis of elderly patients with hip fracture.

Cerebral hemorrhage has a high incidence and numerous complications.Pulmonary in-fection is one of the serious complications of cerebral hemorrhage.Domestic and foreign researchers have developed pulmonary infection prediction models for patients with cerebral hemorrhage,which can provide references for clinical medical staff to identify high-risk groups in the early stage and formulate intervention strategies as soon as possible.This article summarized the risk factors of pulmonary infec-tion,the overview of risk prediction models,the current research status of pulmonary infection predic-tion models,and the significance of prediction,aiming to provide references for the evaluation and pre-vention of pulmonary infection in patients with cerebral hemorrhage.

This study elaborates the composition and method of this training mode, and compares the number and quality of published papers and thesis before and after its implementation. Results show that the "COME" training mode can significantly improve the capacity for scientific research of master degree candidates in plastic surgery.
