1.Effect of Tuina at "Weizhong (BL 40)" on Spinal Microglial Activation-related Proteins and the IL-10/β-EP Pathway in a Rat Model of Chronic Sciatic Nerve Compression Injury

ObjectiveTo investigate the analgesic effect of Tuina at the "Weizhong (BL 40)" on neuropathic pain in a rat model of chronic constriction injury (CCI) of the sciatic nerve and its potential central spinal mechanisms. MethodsThirty-two Sprague-Dawley rats were randomly divided into four groups (8 rats in each group), sham-operated group, model group, Tuina group, and blockade group. The CCI model was established in the model group, Tuina group, and the blockade group by ligating the sciatic nerve with catgut, while the sham-operated group underwent only sciatic nerve exposure without ligation. From postoperative day 4 to day 14, rats in the Tuina group and the blockade group received Tuina manipulation at the "Weizhong (BL 40)" using a dynamic pressure distribution measurement system (5 N pressure, 2 Hz frequency, 10 min per session, once daily). The blockade group also received intraperitoneal injections of the microglial inhibitor minocycline (10 mg/kg) once daily. The sham-operated and the model group underwent the same handling and fixation as the Tuina group without actual Tuina. Mechanical withdrawal threshold (MWT) and paw withdrawal latency (PWL) were measured before surgery and on day 3, 7, 10, and 14 post-surgery. Transmission electron microscopy was used to evaluate sciatic nerve injury and repair, measuring axon diameter and total myelinated fiber diameter to calculate the g-ratio. Western Blotting was performed to detect the protein levels of ionized calcium-binding adapter molecule 1 (Iba-1), CD206, CD68, interleukin-10 (IL-10), and β-endorphin (β-EP) precursor pro-opiomelanocortin (POMC) in the ipsilateral spinal dorsal horn. ResultsCompared with the sham-operated group, the model group showed significantly reduced MWT and PWL on day 3, 7, 10, and 14 (P<0.01). Compared with the model group, the Tuina group and the blockade group showed increased MWT and PWL on day 10 and 14 (P<0.05). Compared with the Tuina group, the blockade group exhibited higher MWT on day 7, 10, and 14, and higher PWL on day 10 (P<0.05). Sciatic nerve pathological morphology revealed intact and well-structured myelin in the sham-operated group, while the model group exhibited myelin collapse, distortion, and myelin ovoid formation. The Tuina group displayed partially irregular myelin with occasional myelin collapse, whereas the blockade group exhibited partial myelin irregularities and phospholipid shedding. Compared with the sham-operated group, the model group showed a decreased g-ratio and increased levels of Iba-1 and CD68 in the spinal dorsal horn (P<0.05 or P<0.01). Compared with the model group, the Tuina group and the blockade group exhibited an increased g-ratio and reduced Iba-1 and CD68 levels. Additionally, the Tuina group showed elevated levels of CD206, IL-10, and POMC, whereas the blockade group had decreased CD206 levels (P<0.05). ConclusionTuina at "Weizhong (BL 40)" alleviates neuropathic pain in CCI rats, potentially by regulating microglial activation in the spinal cord, inhibiting M1 polarization while promoting M2 polarization, and activating the IL-10/β-EP pathway to exert analgesic effects.
2.Epidemiological Analysis of Pathogens in Acute Respiratory Infections During the 2023-2024 Autumn-Winter Season in Beijing:A Case Series of 5556 Patients at Peking Union Medical College Hospital

Objective To analyze the epidemiological characteristics of acute respiratory infections(ARIs)during the autumn-winter season in Beijing,providing evidence for the prevention,control,diagnosis,and treatment of ARIs.Methods A convenience sampling method was employed,enrolling patients who visited Peking Union Medical College Hospital(PUMCH)between September 2023 and February 2024 due to ARIs.Na-sopharyngeal swabs were collected,and real-time fluorescence quantitative PCR was used to detect six common respiratory pathogens[influenza A virus(FluA),influenza B virus(FluB),human rhinovirus(HRV),Myco-plasma pneumoniae(MP),respiratory syncytial virus(RSV),and adenovirus(ADV)],as well as SARS-CoV-2 infection.The distribution patterns of pathogen infections were analyzed.Results A total of 5556 eligible patients were included.The overall positivity rate for the six common respiratory pathogens was 63.7%,with sin-gle-pathogen positivity at 54.0%,dual-pathogen positivity at 8.9%,and triple or more pathogen positivity at 0.7%.The predominant pathogens detected were FluA(16.1%)and RSV(15.7%),followed by ADV(11.1%),MP(11.1%),HRV(10.0%),and FluB(10.0%).No significant difference in overall pathogen positivity was observed between genders.However,significant differences were found between autumn and winter(x2=34.617,P<0.001)and among pediatric,young/middle-aged,and elderly patients(x2=422.38,P<0.001).Specifically,MP(x2=8.647,P=0.003),FluA(x2=131.932,P<0.001),and HRV(x2=174.199,P<0.001)exhibited significantly higher positivity rates in autumn than in winter,whereas FluB was more prevalent in winter(x2=287.894,P<0.001).In pediatric patients,MP,RSV,HRV,and ADV positivity rates were significantly higher than in young/middle-aged and elderly patients(all P<0.001),whereas FluB was more common in young/middle-aged patients(both P<0.001).The positivity rates of the six common respiratory pathogens significantly declined during the SARS-CoV-2 epidemic period,exhibiting an asynchronous seasonal pattern.Conclusions The prevalence of respiratory pathogens in Beijing is associated with age and season.Tar-geted preventive measures should be implemented in different seasons and for key populations.

Objective:To research the development status of medical robot industry at home and abroad at present,and analyze the development dynamics,technological advances,market trends,and existing challenges of this field,so as to promote the long-term development of health industry of Chinese population. Methods:The systematic review was adopted. According to declaration standards of preferred reporting items for systematic reviews and meta-analyses (PRISMA) of systematic evaluation and meta-analysis in 2020,the relevant research literatures of medical robots from January 10th 2015 to January 10th 2024 was retrieved. The CNKI was used as the data source of Chinese literature,and the Web of Science (WOS) of American was used as data resource of English literature. The preliminary screening of 245 research papers were screened according to the inclusion and exclusion criteria. Finally,the screened 194 literatures were used as core samples to conduct qualitative analysis in industrial development situation,development scale and growth trend,policy and requirement of development and existing problem of medical robots. Results:The development of medical robots in China was later than that in foreign countries. As a result,the industrial collaborative innovation mechanism had not yet been fully established,and the core technique innovation capability was relatively weak. In recent years,the size of the market of medical robots in China has continuously expanded,and we had obtained significant breakthroughs in the development and application of single-port robots and micro-robots. Under the guidance and support of government policies,China's enterprises of medical robot had achieved critical technical advancements in the research and development of core components,and the degree of autonomy of industrial chain continuously was improved. As the advanced intelligent manufacturing,the technique of medical robot had played an increasingly important role in medical services and other fields in China. Conclusion:The industry of medical robots in China is currently in a period of strategic opportunities driven by high-speed growth and innovation. But it also facing a series of challenges such as upgrading core technique,optimizing industrial structures and intensifying international competition. In the future,China must further strengthen basic research,and enhance original innovation capabilities,and strengthen the integration of upstream and downstream of the industrial chain,so as to realize sustainable development with high quality in the industry of medical robots in China.

5.Effect of standardized lymph node sorting on surgical treatment of gastroesophageal junction malignant tumors

Objective To investigate the effect of standardized lymph node sorting on postoperative results of gas-troesophageal junction malignant tumors.Methods The data of all patients with malignant gastroesophageal junc-tion in gastric cancer database were analyzed retrospectively.Lymph nodes were sorted according to whether sur-geons were present immediately after surgery.Patients were divided into lymph node sorting group(sorting group)and lymph node unsorting group(unsorting group).General data included gender,age,body mass index(BMI),carcinogenic antigen(CEA),postoperative albumin level,preoperative hemoglobin,etc.Perioperative and patho-logical data included operation time,intraoperative blood loss,postoperative hospital stay,tumor differentiation,distance from superior incisal margin,total number of lymph nodes,number of positive lymph nodes,etc.Kaplan-Meier curve and Log-rank test were used for survival analysis,and propensity score matching analysis adjusted for confounding factors between groups.Results A total of 386 patients were included,including 133 in lymph node sorting group and 253 in non-sorting group.The median follow-up time was 40.18 months.The total number of lymph nodes and the number of positive lymph nodes in the sorting group were(26.38±12.18)and(6.63±10.14),respectively,while the total number and the number of positive lymph nodes in the non-sorting group were(12.25±7.06)and(3.07±3.77),respectively.There were statistically significant differences in the total num-ber of lymph nodes and the number of positive lymph nodes between the sorting group and the non-sorting group(P<0.05).There was no statistically significant difference in survival between the sorting group and the non-sorting group before matching.There were 112 and 203 patients with advanced gastric cancer in the two groups,respec-tively.The overall survival curve of patients in the sorting group was better than that in the non-sorting group,and the difference in median survival time was statistically significant(P<0.05).The caliper value was set to 0.02,and 94 pairs of patients were preferentially matched.After matching,the total number of lymph nodes and the num-ber of positive lymph nodes in the sorting group were(24.71±12.03)and(5.70±9.95),respectively,while the total number and the number of positive lymph nodes in the non-sorting group were(13.05±7.63)and(3.37±4.32),respectively.The difference between the two groups was statistically significant(P<0.05).The overall survival curve of patients in the sorting group was better than that in the non-sorting group,and the median survival time was statistically significant(P<0.05).Conclusion Postoperative lymph node sorting for gastric cancer can significantly increase the number of total lymph nodes and positive lymph nodes,reduce lymph node migration,and improve postoperative survival time.

Overweight and obesity have emerged as a significant public health concern globally.While factors such as genetics,diet,and physical activity are insufficient to fully account for the rise in overweight and obesity,recent studies have indicated a link between environmental pollutants and the development of obesity.Microplastics,a novel type of environmental pollutant,are pervasive in various environmental media and daily life,entering organisms through multiple pathways including the digestive tract,respiratory tract,skin,among others.Evidence from studies has revealed the presence of microplastics in human tissues,organs,and biological samples,suggesting potential health risks to humans.This review outlines the pathways and distribution of microplastics within the human body while summarizing current research progress in relation to obesity.This article aims to raise awareness within society regarding the detrimental effects of microplastics and provide a theoretical foundation for medical professionals addressing public health issues.
8.Reliability of ultrasound in the assessment of diaphragm function/

Objective:To analyze the consistency and repeatability of ultrasonography in diaphragm function evaluation. Method:Sixty healthy subjects participated in the present study,using a PA12A portable color doppler untra-sound diagnostic system.The diaphragm data were recorded during calm breathing and deep breathing,includ-ing diaphragm muscle mobility during calm breathing and deep breathing,diaphragmatic thickness during calm exhalation and calm inspiration,and diaphragmatic thickness during deep exhalation and deep inspiration.The in-tra-test reliability,inter-tester reliability and retest reliability of the above indexes were analyzed respectively. Result:For intra-test reliability,the intra-class correlation coefficient(ICC)values for diaphragm muscle mo-bility during calm breathing,diaphragm muscle mobility during deep breathing,diaphragmatic thickness during calm exhalation,diaphragmatic thickness during calm inspiration,diaphragmatic thickness during maximal exha-lation and diaphragmatic thickness during maximal inspiration were 0.99,0.99,0.99,0.99,0.99,0.99 and 0.99,respectively.The ICC values for the six indicators were 0.84,0.80,0.82,0.82,0.83,and 0.77 for in-ter-tester reliability.For retest reliability,the ICC values for the six indicators were 0.98,0.86,0.98,0.97,0.94,and 0.86 for relative reliability.The standard error of measurement(SEM)for the absolute reliability of the six indicators were 0.09,0.39,0.01,0.02,0.02,0.03,SEM%were 5.23%,8.40%,5.38%,5.7%,8.17%,8.77%,and the minimal detectable change(MDC)were 0.25,1.08,0.03,0.04,0.04,0.09,and MDC%were 14.49%,23.28%,14.9%,15.8%,22.64%,24.31%,respectively.Bland-Altman graphical analy-sis showed no systematic errors. Conclusion:Ultrasound has good reliability in diaphragm function evaluation and can provide objective evi-dence in clinical application.
9.Research Progress in Anti-tumor Mechanisms of Prunellae Spica and Its Active Components

As a common clinical Chinese medicine, Prunellae Spica has the effects of clearing liver-fire, improving eyesight, resolving massesand detumescence, and has strong anti-tumor effects against thyroid cancer, breast cancer, liver cancer and other cancers. Extracts of Prunellae Spica and its active components can play an anti-tumor role in a variety of ways, including cell apoptosis, inhibiting cell invasion and metastasis, inhibiting cell proliferation, inducing autophagy, anti tumor angiogenesis, reversing tumor multidrug resistance and regulating immune function, by regulating miRNA and Wnt/β-catenin, PI3/AKT, AMPK/mTOR/ULK1, RANKL/RANK/OPG and other signal pathways . In this paper, the anti-tumor mechanism of Prunellae Spica extract was reviewed, in order to provide reference for further research and application.
10.Advances on Chemical Constituents of Volatile Oil from Angelicae Sinensis Radix and Their Pharmacological Effect

Angelicae Sinensis Radix, drived from a medicinal and edible plant Angelica sinensis, with the reputation of "nine Angelica recipes out of ten". The volatile oils from Angelicae Sinensis Radix was the main medicinal component of Angelicae Sinensis Radix, mainly including benzene phthalides, terpenoids and alkanes, its chemical composition was complex. Such factors as growth environment, concoction process, extraction methods and other factors all can trigger changes in volatile oil constituents and content from Angelicae Sinensis Radix. Angelicae Sinensis Radix essential oil has diverse pharmacological activities such as anti-hypotension, protection of ischemia-reperfusion injury, asthma, anti-inflammatory and anti-cancer, etc., implying its high clinical application value. This paper reviewed the literature on the volatile oil of Angelicae Sinensis Radix in the past ten years, the chemical components of Angelicae Sinensis Radix were sorted out and the factors affecting the chemical components were summarized, focusing on its anti-hypotensive, ischemia-reperfusion injury protection, asthma and other active effects, in order to provide reference for the further development and utilization of the volatile oil of Angelicae Sinensis Radix.
