1.Literature Based Analysis on Adverse Reactions in Simultaneously Clinical Use of Banxia (Rhizoma Pinelliae)- Wutou (Aconitum)

ObjectiveTo analyze the adverse reactions associated with the clinical use of Banxia (Rhizoma Pinelliae)- Wutou (Aconitum) in the same formula, with the aim of providing a reference for the safety of their clinical application. MethodsLiterature on the clinical application of antagonistic herbs "Banxia-Wutou" used in the same formula, published from January 1st, 2014, to June 30th, 2023, was retrieved from databases including CNKI, VIP, Wanfang, SinoMed, PubMed, Cochrane Library, and Embase. A database was established, and information related to adverse reactions was extracted, including descriptions, classifications, specific manifestations, management and outcomes, patients' primary diseases (western medicine diseases and traditional Chinese medicine diagnoses and syndromes), and medication information (dosage, ratio, administration routes, and dosage forms). ResultsA total of 79 researches simultaneously used antagonistic herbs Banxia-Wutou in the same formula and reported associated advers reactions. Gastrointestinal adverse reactions were the most common, with 8 studies reporting management of adverse reactions and 3 studies reporting improvement with no intervention. Among the 11 researches, the adverse reaction relieved to extant, while other 69 researches didn't report the managment of adverse reaction and its prognosis. For the primary disease in western medicine system, chronic bronchitis and chronic obstructive pulmonary disease (COPD) were most common, while gastric pain was the most common symptom in traditional Chinese medicine with spleen and kidney deficiency and spleen stomach cold deficiency being the most frequent syndromes. The most common Banxia dosage was 10 g, while for the Wutou, Fuzi (Radix Aconiti Lateralis Praeparata) was predominant with the highest dose at 15 g. The most frequent herbal combination was Banxia-fuzi, with a 1∶1 ratio. The main administration route was oral, and the primary dosage form was decoction. ConclusionGastrointestinal adverse reactions are the most common in the clinical use of Banxia-Wutou antagonistic herb combinations. Research on the safety of "Banxia-Wutou" combinations should focus on respiratory system diseases and spleen-stomach related conditions.
2.Construction of a risk prediction model for delivering macrosomic infants in pregnant women with gestational diabetes mellitus

Objective:To investigate the factors affecting the delivery of macrosomic infants in pregnant women with gestational diabetes mellitus, and utilize data to construct a nomogram model and validate it.Methods:A total of 1 493 pregnant women with gestational diabetes mellitus who received treatment at Shaanxi Provincial People's Hospital from January 2023 to January 2024 were included in this study. A retrospective study was conducted to analyze the proportion of pregnant women with gestational diabetes mellitus who delivered macrosomic infants. The influential factors in pregnant women with gestational diabetes who delivered macrosomic infants were analyzed using both univariate and multivariate analyses. A predictive model for macrosomia in pregnant women with gestational diabetes mellitus was established, and its predictive efficiency was evaluated.Results:Among the 1 493 pregnant women with gestational diabetes mellitus included in this survey, 51 delivered macrosomic infants, accounting for 3.42%. Univariate analysis revealed that weight gain during pregnancy [(14.11 ± 3.25) kg vs. (10.62 ± 2.72) kg, t = 8.94], pre-pregnancy body mass index [(24.31 ± 2.51) kg/m2 vs. (23.25 ± 2.13) kg/m2, t = 8.94], gestational week [(39.14 ± 0.42) weeks vs. (38.92 ± 0.51) weeks, t = 3.04], fasting blood glucose [(5.15 ± 0.41) mmol/L vs. (4.75 ± 0.35) mmol/L, t = 7.97], blood glucose level during the oral glucose tolerance test at 0 hours [(5.71 ± 0.42) mmol/L vs. (5.49 ± 0.41) mmol/L, t = 3.76], and insulin resistance index [(0.54 ± 0.13) vs. (0.41 ± 0.10), t = 9.02] had a significant impact on the delivery of macrosomic infants (all P < 0.05). Multivariate logistic analysis indicated that weight gain during pregnancy, fasting blood glucose, and insulin resistance index were independent risk factors for delivering macrosomic infants in pregnant women with gestational diabetes mellitus ( OR = 1.685, 27.113, 25.816, all P < 0.05). The multivariate logistic regression model demonstrated good goodness of fit (Hosmer-Lemeshow χ2 = 10.34, P > 0.05). Based on the factors identified through multivariate analysis, a nomogram risk model was constructed, yielding a C-index of 0.742. The independent risk factors from the logistic regression model and their prediction probabilities were utilized to generate the receiver operating characteristic curve for predicting the likelihood of delivering macrosomic infants among pregnant women with gestational diabetes mellitus. The areas under the curve were 0.815, 0.779, 0.795, and 0.938, respectively. Conclusion:The predictive model established based on weight gain during pregnancy, fasting blood glucose levels, and the insulin resistance index demonstrates significant predictive value for the delivery of macrosomic infants in pregnant women with gestational diabetes mellitus.
3.Effects of frontoparietal transcranial alternating current stimulation on working memory in different pos-tures

Objective:To investigate the effect of theta-transcranial alternating current stimulation(θ-tACS)over the frontal-parietal cortex on working memory(WM)performance under different postural conditions(sitting and stand-ing)in healthy adults. Method:The study was a randomized,crossover,double-blind design.Sixteen healthy adults received tACS or sham stimulation in random order,with at least 72 hours interval between two sessions.Seven electrodes were placed in the left frontoparietal critical brain region with 6Hz for 21 mins.The sham stimulation used the same stimulation time and frequency as tACS group,but only 1 minute of stimulation(30s at the beginning and 30s at the end of the stimulation).WM performance was assessed before and immediately after stimulation,in-cluding accuracy,reaction time and inverse efficiency scores.Wilcoxon rank sum tests were used to observe between-and within-group differences on WM performance. Result:Fifteen participants completed the experiment and there were no significant differences in side effects be-tween two groups.There was a significant improvement in reaction time and inverse efficiency scores for sit-ting WM as well as reaction time for standing WM after the frontoparietal θ-tACS(P<0.05).There was a trend towards improvement in inverse efficiency scores for standing WM(P=0.057),while there was no signif-icant difference in accuracy,reaction time and inverse efficiency scores of WM during the sitting and standing conditions in the sham stimulation(all P>0.05). Conclusion:Frontoparietal θ-tACS can improve WM performance in both sitting and standing postures.This helps healthy adults cope with real-life multitasking cognitive activities and has important practical application value.
4.Establishment of a genetically diverse mouse model of hypertension and analysis of gene transcription regulation

Objective To investigate the differences in blood pressure phenotypes,renal pathological changes,and related pathogenic pathways in genetically diverse hypertensive mice obtained from 13 strains.Methods The genotypes of Cckbr+/+,Cckbr+/-and Cckbr-/-were obtained by hybridization of 13 strains of genetically diverse mice with Cckbr-/-mice.Blood pressure was measured with a noninvasive blood pressure analysis system(BP-2000).The expression of CCKBR protein in mouse kidney tissue was detected by Western Blot,and the pathological changes in mouse kidney tissue were detected by hematoxylin-eosin(HE)staining and immunohistochemistry(IHC).The pathogenic pathways related to essential hypertension were screened by RNA sequencing.Results In three specific mouse strains(A/J,LOT,and FIM),the systolic blood pressure(SBP)was significantly different between the Cckbr-/-and Cckbr+/+groups.HE staining and IHC showed that hypertension caused a certain degree of renal injury in the mice.Gene Ontology(GO)and pathway enrichment analysis showed that differentially expressed genes were enriched in metabolic processes and circadian rhythm regulation.Conclusions Genetically diverse mice can effectively simulate the genetic background of the population and provide a new resource for studying the pathogenic genes related to essential hypertension.
5.Multimodal ultrasound manifestations of port-wine stains and their application in efficacy assessment of photodynamic therapy

Objective:To comprehensively evaluate the skin thickness, blood flow grades, skin stiffness, and post-treatment changes in ultrasound indices in patients with different clinical types of port-wine stains (PWS) by multimodal ultrasound, and to explore the application value of multimodal ultrasound in the quantitative evaluation of and efficacy assessment for PWS.Methods:An observational study was conducted, including 298 patients with PWS. High-frequency ultrasound was used to measure the thickness of and blood flow signals in skin lesions, and shear wave elastography to measure the maximum Young's modulus (Emax) and maximum shear wave propagation velocity (Cmax) of the skin layers. Differences in various ultrasound indices were analyzed between skin lesions and surrounding normal skin. After photodynamic therapy, ultrasound re-evaluation was performed on 124 patients to analyze changes in ultrasound indices before and after treatment. Statistical analysis was conducted using t test, analysis of variance, and rank sum test. Results:In 106 patients with pink PWS, the skin thickness was 1.41 ± 0.28 mm, the blood flow was mainly rated as grade 0 (81%), the Emax was 16.24 ± 1.82 kPa, and the Cmax was 2.28 ± 0.09 m/s. There were no significant differences in the above indices between normal skin and treated skin (all P > 0.05). In the 121 patients with purplish red PWS, the skin thickness was 1.86 ± 0.23 mm; the blood flow was mainly rated as grade 1 (45%), which was higher than that of normal skin; after treatment, both the skin thickness and blood flow grades were decreased ( P < 0.05) ; there were no significant differences in the Emax and Cmax between purplish red PWS lesions (16.41 ± 1.82 kPa, 2.32 ± 0.13 m/s, respectively) and normal skin or treated skin (all P > 0.05). In the 71 patients with thickened PWS, the skin thickness was 2.51 ± 0.32 mm, the blood flow was mainly trated as grade 3 (55%), the Emax was 22.22 ± 2.46 kPa, and the Cmax was 2.75 ± 0.11 m/s, which were all significantly higher than those of normal skin (all P < 0.05) ; all the indices were decreased after treatment. Conclusions:Multimodal ultrasound can be used for the quantitative assessment of different clinical types of PWS and their treatment outcomes. Purplish red PWS and thickened PWS showed thicker skin layers and higher blood flow grades, and thickened PWS showed higher skin stiffness values. All ultrasound indices were decreased after treatment.
6.Exploration and practice of one-stop patient service hotline in a certain hospital

With the progress of society and the continuous improvement of people′s living standards in China, the public′s demand for medical services is becoming increasingly diversified. How to move hospital services forward and improve medical services centered on patients has become a key consideration for hospitals to enhance patients′ sense of medical satisfaction. A certain hospital has established a one-stop patient service hotline, integrating functions such as number inquiry, medical consultation, appointment registration, appointment examination, praise and suggestions, complaint follow-up, etc., injecting a complaint handling management mode, and responding to and solving patient feedback problems in a timely manner. Since the launch of the patient service hotline, it has effectively solved the problems that patients encountered during their visits, effectively reduced the hospital′s complaint rate, and initially formed a service closed-loop management. From March to October 2023, the demand ratio of the 12345 hotline in the hospital has continuously decreased, and was significantly lower than the average level of 22 municipal hospitals in Beijing. In the future, we should further improve the communication skills between doctors and patients, focus on managing appeals and services, and continue to strengthen proactive governance.
7.QL1604 plus paclitaxel-cisplatin/ carboplatin in patients with recurrent or metastatic cervical cancer:an open-label, single-arm, phase II trial

Objective:
QL1604 is a highly selective, humanized monoclonal antibody against programmed death protein 1. We assessed the efficacy and safety of QL1604 plus chemotherapy as first-line treatment in patients with advanced cervical cancer.
Methods:
This was a multicenter, open-label, single-arm, phase II study. Patients with advanced cervical cancer and not previously treated with systemic chemotherapy were enrolled to receive QL1604 plus paclitaxel and cisplatin/carboplatin on day 1 of each 21-day cycle for up to 6 cycles, followed by QL1604 maintenance treatment.
Results:
Forty-six patients were enrolled and the median follow-up duration was 16.5 months. An 84.8% of patients had recurrent disease and 13.0% had stage IVB disease. The objective response rate (ORR) per Response Evaluation Criteria in Advanced Solid Tumors (RECIST) v1.1 was 58.7% (27/46). The immune ORR per immune RECIST was 60.9% (28/46).The median duration of response was 9.6 months (95% confidence interval [CI]=5.5–not estimable). The median progression-free survival was 8.1 months (95% CI=5.7–14.0). Fortyfive (97.8%) patients experienced treatment-related adverse events (TRAEs). The most common grade≥3 TRAEs (>30%) were neutrophil count decrease (50.0%), anemia (32.6%), and white blood cell count decrease (30.4%).
Conclusion
QL1604 plus paclitaxel-cisplatin/carboplatin showed promising antitumor activity and manageable safety profile as first-line treatment in patients with advanced cervical cancer. Programmed cell death protein 1 inhibitor plus chemotherapy may be a potential treatment option for the patient population who have contraindications or can’t tolerate bevacizumab, which needs to be further verified in phase III confirmatory study.
8.Comparative efficacy of vNOTES and LESS in the management of benign uterine lesions in obese patients

Objective To compare the outcomes of transvaginal natural orifice transluminal endoscopic surgery(vNOTES)and laparoendoscopic single-site surgery(LESS)for total uterine excision in obese patients with benign uterine lesions,and to investigate the utility of vNOTES in this patient population.Methods A total of 100 obese patients(BMI>28.0 kg/m2)diagnosed with benign uterine lesions requiring total uterine and bilateral salpingectomy between January 2022 and January 2023 were included in this study.They were randomly assigned to two groups:the LESS group(n=51)and the vNOTES group(n=49).Patient demographics,surgical duration,intraoperative blood loss,changes in hemoglobin levels,pain scores,time to first flatus postoperatively,length of hospital stay,pelvic floor function,sexual quality of life,and postoperative complications were compared between the two groups.Results The two groups did not show any statistically significant differences in terms of blood loss,pre-and postoperative hemoglobin changes,pelvic floor function,sexual quality of life,or postoperative complications(P>0.05).However,the vNOTES group exhibited shorter surgical durations,time to first flatus postoperatively,and length of hospital stay compared to the LESS group(P<0.05).Additionally,the vNOTES group demonstrated lower intraoperative pain scores than the LESS group.(P<0.05).Conclusions In obese patients with benign uterine lesions,vNOTES total uterine excision surgery demonstrated shorter surgical durations and postoperative hospital stays,lower postoperative pain scores,and better adherence to the principles of en-hanced recovery after surgery(ERAS),indicating its potential for broader application.
9.Analysis of thyroid hormone indicators among interventionaland diagnostic radiologists

Objective:
To learn the levels of thyroid hormone indicators among interventional and diagnostic radiologists, so as to provide insights into occupational health monitoring of radiologists.
Methods:
Demographic information, body mass index (BMI), thyroid hormone indicators and cumulative individual doses over 5 years among interventional and diagnostic radiologists were collected through the personal dosimetry monitoring database of Ningbo Center for Disease Control and Prevention and the Occupational Health Examination Center of a tertiary hospital in Ningbo City. The thyroid hormone indicators of different job types and cumulative individual doses over 5 years were analyzed, and factors affecting the thyroid hormone indicators were identified using a multivariable ordinal logistic regression model.
Results:
There were 159 interventional and 159 diagnostic radiologists included, and the proportions of abnormal thyroid hormone indicators were 9.43% and 12.26%, respectively, with no statistically significant difference (P>0.05). The levels of triiodothyronine and free triiodothyronine in interventional radiologists were lower than those in diagnostic radiologists [(1.54±0.41) vs. (1.68±0.34) nmol/L, (5.13±1.07) vs. (5.55±0.87) pmol/L; both P<0.05]. The levels of triiodothyronine and free triiodothyronine were significantly different among radiologists with cumulative individual doses over 5 years of <1.5 mSv, 1.5-<3.0 mSv and ≥3.0 mSv [(1.69±0.31), (1.69±0.40) vs. (1.52±0.41) nmol/L, (5.60±0.83), (5.32±0.94) vs. (5.14±1.09) pmol/L; both P<0.05]. Multivariable ordinal logistic regression analysis identified BMI (<18.5 kg/m2, OR=0.111, 95%CI: 0.028-0.436) and cumulative individual doses over 5 years (<1.5 mSv, OR=6.259, 95%CI: 2.368-16.547) as the factors affecting triiodothyronine, and job types (diagnostic radiologists, OR=3.171, 95%CI: 1.529-6.574), BMI (18.5-<24.0 kg/m2, OR=0.393, 95%CI: 0.184-0.842), and gender (men, OR=3.449, 95%CI: 1.294-9.190) as the factors affecting free triiodothyronine.
Conclusion
Occupational exposure has a certain impact on the thyroid hormone indicators among interventional and diagnostic radiologists, and the main influencing factors include BMI, radiation dose, job type and gender.
10.Status quo survey of nutrition work ability in primary medical institutions of Chongqing City

Objective To understand the status quo of nutrition working ability in primary medical and health institutions of Chongqing City.Methods Three primary medical institutions were randomly extracted from each of 39 districts and counties of Chongqing City,and Primary Nutrition Service Capacity Survey Form and Nutrition Work Capacity Survey System were adopted to conduct the questionnaire survey.Then the sur-vey results were analyzed.Results A total of 117 primary medical institutions were surveyed,in which 86 in-stitutions(73.50%)undertook the nutritional work.The number of engaging the nutrition full-time work in the units was 0(0,0).Among the nutritional staff,the age in 164 persons was 30-<40 years old,accounting for 48.38%,180 persons(53.10%)had the primary title,232 persons(68.44%)had the bachelor degree,287 persons(84.66%)had the medical related background,but only 2 persons had the nutritional related profes-sional background.There were 57(48.72%),75(64.10%),77(65.81%)primary medical institutions in carry-ing out the nutrition and health management of pregnant women,children aged 0-6 years old and elderly peo-ple,34 institutions(29.10%)in carrying out nutritional monitoring,and 17 institutions(14.50%)had the clinical nutrition work ability.Compared with the villages and towns,the proportion of urban area primary medical institutions in carrying out the blood routine items in children aged 0-6 years old,hemoglobin,blood routine and urine routine items in elderly people was higher,the number of published popular science works on nutrition was more,and the differences were statistically significant(P<0.05).Conclusion The nutrition work system of primary medi-cal institutions in Chongqing City is temporarily imperfect,the specialized persons still lack and the nutritional health service level needs to be further strengthened.
