OBJECTIVES:
To study the epidemiological characteristics of respiratory syncytial virus (RSV) infection in hospitalized children with community-acquired pneumonia (CAP) in Hebei Province.
METHODS:
Hospitalized children with CAP who tested positive for RSV and were admitted to Hebei Children's Hospital from various cities and counties across Hebei Province between January 2019 and December 2023 were included in the study. Clinical data were collected and analyzed to assess epidemiological characteristics.
RESULTS:
The clinical data of 43 978 children with CAP were collected, with an overall RSV detection rate of 25.98%. The detection rate was higher during the implementation of non-pharmaceutical interventions (NPIs) (30.60%) than in the non-NPIs period. Winter and spring were the primary epidemic seasons for RSV each year except in 2022. The detection rate in males (26.62%) was higher than in females (25.06%) (P<0.001). The highest detection rate (59.18%) was found in infants aged 29 days to <1 year. Single RSV infection was more common, with rhinovirus being the most frequent co-infection.
CONCLUSIONS
The overall RSV detection rate in Hebei Province is influenced by NPIs, being higher during their implementation. RSV predominantly circulates in winter and spring. The detection rate of RSV is higher in males and infants. RSV infection is primarily single, most often co-occurring with rhinovirus.

Paclitaxel(PTX)is a first-line chemotherapy drug for breast cancer, but its resistance issues significantly impact clinical treatment efficacy.Fucosylation, especially core fucosylation,is closely related to tumor chemoresistance, resulting in poor chemotherapy responses and poor prognosis in patients. In this study,we investigated the effect and mechanism of the fucosylation inhibitor 2-fluorofucose(2-F-Fuc)on the chemosensitivity of paclitaxel-resistant breast cancer MCF-7/PTX cells. The drug resistanceindex(RI)of MCF-7/PTX cells was 8.49 by MTT assays.Western blotting,real-time PCR,enzyme-linked immu-nosorbent assay(ELISA)and Lens Culinaris Agglutinin(LCA)lectin imprinting showed that compared with MCF-7 cells, the expression of FUT8,MDR1and core fucosylation in MCF-7/PTX cells was high.Western blotting showed that 2-F-Fuc had a significant inhibitory effect on the growth of MCF-7/PTX cells, and the expression levels of FUT8 and MDR1 were significantly down-regulated after 2-F-Fuc treatment, and the down-regulation was more pronounced in the PTX and 2-F-Fuc combination group(P<0.05).Compared to the control, expression of PCNA in MCF-7/PTX cells in the PTX and the 2-F-Fuc group were down-regulated,and the apoptosis-related proteins, such as cleaved caspase-3 and Bax/Bcl-2 were in-creased.The level of p-PI3K and p-AKT were down-regulated,and the changes in the combination of 2-F-Fuc and PTX were more robust(P<0.05).The above results showed that the core fucosylation level of MCF-7/PTX cells was significantly increased, and 2-F-Fuc could reduce the core fucosylation level of MCF-7/PTX cells by inhibiting the expression of FUT8,and enhance the sensitivity of drug-resistant cells to PTX,which may correlate with the downregulation of PI3K/AKT signaling pathway proteins.

Objective To study the anti-reflux effect of conical gastric stump embedding in radical resection of esophageal cancer. Methods A total of 60 patients who planned to undergo radical resection of esophageal cancer in our hospital from June 2020 to June 2022 were selected as the study objects, and the patients were divided into the observation group and the control group by random number table method, with 30 cases in each group.Patients in both groups underwent laparoscopic radical resection of esophageal cancer and esophagogastric end-to-side mechanical anastomosis. The observation group adopted the conical gastric stump embedding technique after esophagogastric end-to-side mechanical anastomosis. The perioperative related indexes, postoperative complications and gastroesophageal reflux of patients in the two groups were compared. The postoperative anti-reflux effect was evaluated by reflux disease questionnaire(RDQ)score and 24-hour intraesophageal pH monitoring. Results The operation time and digestive tract reconstruction time of patients in the observation group were longer than those in the control group(P<0.05),while there was no statistically significant difference in the amount of intraoperative bleeding, the number of lymph node dissection, the first exhaust time,or the postoperative hospital stay of patients between the two groups(P>0.05).There was no statistically significant difference in the overall incidence of postoperative complications between the two groups(P>0.05).The severity of postoperative gastroesophageal reflux of patients in the observation group was lighter than that in the control group, and the difference was statistically significant(P<0.05).The RDQ score,24-hour reflux frequency,>5 minutes reflux frequency,pH<4 time, and longest reflux time of patients in the observation group was significantly lower/less/shorter than those in the control group(P<0.05).Conclusion The conical gastric stump embedding technique is safe and feasible in the radical resection of esophageal cancer.Although the operation time and digestive tract reconstruction time are slightly prolonged,it does not increase the perioperative risks, which can significantly reduce the occurrence and severity of postoperative gastroesophageal reflux of patients, and achieve a good anti-reflux effect.

Objective To develop a health belief model(HBM)based adolescent alcohol-related cognition scale to measure adolescent alcohol-related cognition and test its reliability and validity. Methods The adolescents'alcohol-related cognitive scale was developed based on HBM model.By using purposive sampling, three general high schools in Qingpu District,Shanghai were selected.One-third of the classes from grades 10 and 11 in each school were randomly selected, and the students from these classes were surveyed as the research subjects.Exploratory factor analysis and confirmatory factor analysis were used to analyze its reliability(internal consistency reliability and combination reliability)and validity(structural validity, convergent validity, discriminative validity and criterion validity). Results A total of 970 questionnaires were collected,of which 948 were valid, with an effective rate of 97.7%.The adolescents'alcohol-related cognitive scale contained 22 items.Five common factors were extracted from exploratory factor analysis, including perceived susceptibility, perceived severity, perceived benefits, perceived obstacles, and self-efficacy.The cumulative variance contribution rate reached 83.89%.The results of confirmatory factor analysis confirmed the overall fit of the model. The average variance extracted value(AVE)of each dimension was greater than 0.5,and the convergent validity of the model was ideal. The AVE square root of each dimension of the scale was greater than its correlation coefficient, indicating good discrimination validity.Cronbach's α coefficient of the total volume table was 0.892,indicating good overall reliability. Conclusion The adolescents'alcohol-related cognitive scale developed in this study has good reliability and validity, which can be used to measure adolescents'alcohol-related perceptions.

The assessment of adverse drug events is an important basis for clinical safety evaluation and post-marketing risk control of drugs, and its causality assessment is gaining increasing attention. The existing methods for assessing the causal relationship between drugs and the occurrence of adverse reactions can be broadly classified into three categories:global introspective methods, standardized methods, and probabilistic methods. At present, there is no systematic introduction of the operational details of the various methods in the domestic literature. This paper compares representative causality assessment methods in terms of definition and concept, methodological steps, industry evaluation and advantages and disadvantages, clarifies the basic process of determining the causality of adverse drug reactions, and discusses how to further improve the adverse drug reaction monitoring and evaluation system, with a view to providing a reference for drug development and pharmacovigilance work in China.
