1.Interproximal tunneling combined with customized connective tissue graft to improve severe papillary defects in the aesthetic zone: a case report and literature review

Objective:
To explore the treatment plan for severe papillary defects in the aesthetic zone caused by severe periodontitis, providing a reference for clinical practice.
Methods :
A patient with severe periodontitis leading to severe papillary defects in the upper anterior teeth from 12 to 23 was treated using interdental tunnel technique combined with personalized connective tissue grafting for periodontal plastic surgery, and stable soft tissue augmentation was achieved. Resin restoration was conducted to modify the crown shape of the aesthetic zone teeth, reconstruct white aesthetics, guide the shaping of the gingival papillae, reduce “black triangles,” and enhance the patient’s confidence in smiling.
Results :
The patient’s periodontal condition and the regeneration of soft tissues in the aesthetic zone were good, and the smile aesthetics were restored. After a 3-year follow-up, the gingival morphology, color, and texture were good, and the effect was stable. The literature review indicates that for papillary defects in the aesthetic zone, analysis should be conducted based on the following aspects: whether a defect is present in periodontal hard and soft tissues, crown shape, and the distance from the most apical part of the crown contact area to the top of the alveolar crest. Based on the analysis of aesthetic defects and surgical indications, a personalized treatment plan should be designed.
Conclusion
For patients with obvious papillary defects in the aesthetic zone due to the reduction of periodontal support tissues caused by severe periodontitis, factors such as periodontal hard and soft tissue defects, crown shape, and the distance from the most apical part of the crown contact area to the top of the alveolar crest should be fully considered, and a personalized treatment plan should be formulated after multidisciplinary joint consultation.

ObjectiveTo investigate the effect of Niuhuang Qingxinwan (NHQXW) in improving intracerebral hemorrhage (ICH) with Tanre Fushi (phlegm-heat and fu-organ excess) syndrome by maintaining blood-brain barrier (BBB) integrity, and to explore its potential mechanism. MethodsMale mice were administered with 15% autologous feces for 3 consecutive days to simulate spontaneous Tanre Fushi syndrome, followed by surgical induction of collagenase-induced ICH on the fourth day. Mice were randomly assigned to seven groups: Sham, Sham+NHQXW-H, collagenase, collagenase+feces, and NHQXW intervention groups at low (NHQXW-L, 0.225 g·kg-1), medium (NHQXW-M, 0.45 g·kg-1), and high (NHQXW-H, 0.9 g·kg-1) doses. Treatments were administered for 3 days after surgery. NHQXW effects on Tanre Fushi syndrome were assessed via fecal water content and small intestinal carbon propulsion rate. Protective effects of NHQXW against ICH with Tanre Fushi syndrome were evaluated by measuring hematoma volume, neurological deficits, and brain water content. BBB integrity was further assessed using Evans blue staining, hematoxylin-eosin (HE) staining, immunofluorescence, and Western blot for Claudin-5, plasmalemma vesicle-associated protein (PLVAP), matrix metalloproteinase (MMP)-2, and MMP-9. The potential mechanism of NHQXW was investigated by detecting protein expression of protein kinase B (Akt), extracellular signal-regulated kinase 1/2 (ERK1/2), signal transducer and activator of transcription 3 (STAT3), Yes-associated protein (YAP), and their phosphorylated forms. ResultsCompared with the collagenase+feces group, NHQXW-M and NHQXW-H significantly reduced fecal water content (P<0.05, P<0.01) and intestinal propulsion rate (P<0.01), alleviated neurological deficits (P<0.01), decreased hematoma volume (P<0.01) and Evans blue extravasation (P<0.01), increased Claudin-5 protein expression (P<0.05, P<0.01) and fluorescence intensity (P<0.01), and decreased PLVAP protein expression (P<0.01) and fluorescence intensity (P<0.05, P<0.01), as well as MMP-2 (P<0.05, P<0.01) and MMP-9 (P<0.01) expression. NHQXW-H downregulated p-Akt (P<0.05), p-ERK1/2 (P<0.05), p-STAT3 (P<0.01), and p-YAP (P<0.05), with the most significant effect observed on STAT3 phosphorylation. ConclusionNHQXW effectively alleviates neurological deficits and BBB injury in ICH mice with Tanre Fushi syndrome, primarily by inhibiting STAT3 activation.
4.Relationship between changes in classroom lighting environment and refractive progression among primary and secondary school students

Objective:
To analyze the changes in classroom lighting environment of schools in Suzhou and their impact on refractive progression among primary and secondary school students, so as to provide the basis for accurate provention and control of myopia.
Methods:
A baseline investigation was conducted in October 2022 by using a stratified cluster random sampling method to recruit primary and secondary school students from Suzhou. A follow up visit was performed in October 2023. A total of 12 302 students and 360 classrooms that participated in both surveys were included analysis. The visual acuity progression over one year and classroom lighting conditions were assessed. Group comparisons were performed by using the Wilcoxon or Kruskal-Wallis rank-sum, and Chi-square tests. Multivariate Logistic regression was employed to identify the major factors influencing refractive changes.
Results:
The compliance rate of average illuminance on classroom blackboard surface increased from 72.22% to 75.28%, while the compliance rate of average illuminance on desks decreased from 89.44% to 87.22%, the overall myopia rate among students rose from 59.63% to 66.99% from 2022 to 2023. The average annual progression of equivalent spherical power(SE) in the right eye of students was -0.25(-0.75,0.06)D. Significant statistical differences were observed in the annual mean changes across different school levels, regions, baseline refractive statuses, and classroom lighting environment change groups ( Z/H =316.59, -8.27, 38.80 , 51.01, all P <0.05). Multivariate Logistic regression analysis showed that pre myopia, low myopia, junior high school, senior high school, vocational high school, and improved classroom lighting environment were protective factors of reducing the risk of rapid progression in refractive error ( OR =0.58, 0.69, 0.81, 0.50, 0.28, 0.82, all P <0.05). Conversely, female students and rural students had higher risks of rapid myopia progression ( OR =1.09, 1.42, both P <0.05).
Conclusions
Over one year follow up, the complance rate of classroom lighting indicators in Suzhou remaines stable, while students refractive status shows a trend toward myopia. Improving classroom lighting environment can reduce the risk of rapid myopia progression.
5.Study on the influence of electronic medical record linkage time node reminder system on emergency treatment time node and prognosis of severe trauma patients

Objective To clarify the clinical application effect of electronic medical record linkage time node reminder system(hereinafter referred to as reminder system)in trauma center.Methods A total of 90 patients with severe trauma admitted to the Trauma Center of Huzhou First People's Hospital from June 2022 to March 2023 were selected,and 45 patients admitted from June to October 2022 were included in control group for routine pre-hospital in-hospital treatment.Forty-five patients with severe trauma admitted from November 2022 to March 2023 were included in experimental group,and reminder system was implemented to guide the treatment process.The indexes of medical quality control,emergency black hole procedure time and patient outcome were compared between two groups.Results Bedside FAST ultrasound examination time,CT scan completion time,emergency room stay time,nursing waiting time,make diagnosis and treatment plan and surgical talk time,and travel transport time of patients in experimental group were significantly shorter than those in control group(P<0.05).There were no significant differences in the first tranexamic acid administration time,completion time of damage control treatment,length of stay in intensive care unit,duration of invasive mechanical ventilation and success rate of treatment between two groups(P>0.05).Conclusion The application of reminder system is helpful to strengthen the cooperation of medical care,shorten the unnecessary waiting time for treatment,and accelerate the time node transformation in the process of severe trauma treatment.

Arsenic compounds are widely used for the therapeutic intervention of multiple diseases.Ancient pharmacologists discovered the medicinal utility of these highly toxic substances,and modern phar-macologists have further recognized the specific active ingredients in human diseases.In particular,Arsenic trioxide(ATO),as a main component,has therapeutic effects on various tumors(including leukemia,hepatocellular carcinoma,lung cancer,etc.).However,its toxicity limits its efficacy,and con-trolling the toxicity has been an important issue.Interestingly,recent evidence has pointed out the pivotal roles of arsenic compounds in phase separation and membraneless organelles formation,which may determine their toxicity and therapeutic efficacy.Here,we summarize the arsenic compounds-regulating phase separation and membraneless organelles formation.We further hypothesize their potential involvement in the therapy and toxicity of arsenic compounds,highlighting potential mecha-nisms underlying the clinical application of arsenic compounds.
7.Effect Analysis of Electroconvulsive Therapy Under Combined Anesthesia of Esketamine and Propofol in Patients with Major Depressive Disorder Accompanied with Suicidal Ideation

Objective To analyze the effects of different doses of esketamine combined with electroconvulsive therapy on patients with major depressive disorder accompanied with suicidal ideation.Esketamine,as an electroconvulsive anesthetic with antidepressant properties,has advantages and disadvantages in the treatment of patients with major depressive disorder accom-panied by suicidal ideation.It provides a reference for the better application of esketamine in clinical practice.Methods In all,120 patients were randomly divided into control group,low-dose group,and high-dose group using random number table meth-od,with 40 cases in each group(n=40).The control group was treated with slow infusion of 1.2-1.5 mg/kg propofol,the low-dose group was treated with slow infusion of 0.3 mg/kg esketamine and 1.0 mg/kg propofol,and the high-dose group was trea-ted with slow infusion of 0.6 mg/kg esketamine and 0.5-0.8 mg/kg propofol.The therapeutic effect,safety indicators,and im-pact on cognitive function were evaluated during the treatment process.Results The results of this study showed that there was no significant difference in HAMD-24 scores among control group,low-dose group,and high-dose group patients before treat-ment.After treatment with non-convulsive electroconvulsive therapy(MECT),the scores decreased among three groups.There were differences in various observation indicators between the low-dose group and high-dose group compared to the control group(all P<0.05),and there were differences in various observation indicators between the low-dose group and high-dose group(all P<0.05).The Repetitive Neuropsychological State Test(RBANS)was used to evaluate the cognitive function of three groups of patients.The results showed that the RBANS scores of the high-dose group were significantly higher than those of the low-dose group and control group after treatment(all P<0.05).There was no serious adverse reactions among the 120 patients in the three groups,and the incidence of adverse reactions among the three groups was not statistically significant(P>0.05).The Beck Suicide Intention Scale was used to evaluate the efficacy of suicide ideation in three groups of patients after treatment.The results showed that the Beck Suicide Intention Scale scores in the high-dose group were significantly lower than those in the low-dose group and control group(P<0.05).Conclusion The symptoms of severe depression and suicidal ideation in all three groups of patients have been improved.The efficacy of using different anesthetics and doses on patients with severe depression accompanied by suicidal ideation shows significant differences with dose changes,and no serious adverse reactions have occurred.The safety of its clinical application has been verified,and it is worth further promotion in clinical practice.
8.Hypericin reduces myocardial ischemia-reperfusion injury in rats by modulating AMPK/Nrf2/HO-1 signaling pathway

AIM:To investigate the effect of hypericin(Hyp)on the heart of rats with myocardial ischemia-re-perfusion injury(MIRI),and to explore its mechanism.METHODS:Thirty SPF male SD rats were divided into 5 groups:sham group,MIRI group,low-dose Hyp(L-Hyp)group(MIRI+L-Hyp group),high-dose Hyp(H-Hyp)group(MIRI+H-Hyp group),and positive control trimetazidine(TMZ)group(MIRI+TMZ group),with 6 rats in each group.Apart from the sham group,the MIRI model was established by ligating the anterior descending branch of the left coronary artery and then recanalizing it in the remaining four groups of rats.The success of the modeling was determined by monitor-ing the electrocardiogram.We assessed the cardiac function in rats using echocardiography.TTC staining was employed to measure the area of myocardial infarction in rats,and HE staining was utilized to observe the morphological traits of rat myocardium.We assayed the activities of lactate dehydrogenase(LDH)and superoxide dismutase(SOD)and the levels of malondialdehyde(MDA)in rat serum using biochemical kits.ELISA kits were applied to assess the contents of cardiac troponin I(cTnI)and reactive oxygen species(ROS)in rat serum.Western blot analysis was perfomed to measure the pro-tein expression levels of AMPK,p-AMPK,Nrf2,and HO-1 in rat myocardial tissues.RESULTS:The rats in MIRI group exhibited increased myocardial tissue injury,larger myocardial infarction areas,decreased left ventricular ejection fraction(LVEF),and reduced left ventricular fractional shortening(LVFS)compared with sham group,as shown by echo-cardiography.Additionally,there were increases in LDH activity,cTnI,MDA and ROS levels,along with significant de-creases in SOD activity,and p-AMPK,Nrf2 and HO-1 protein levels(P<0.05).Compared with MIRI group,the rats in MIRI+L-Hyp,MIRI+H-Hyp and MIRI+TMZ groups showed decreased myocardial histopathological damage and reduced myocardial tissue infarction area,increased LVEF and LVFS,and lowered serum levels of LDH activity,cTnI,MDA and ROS,while SOD activity,p-AMPK,Nrf2 and HO-1 protein levels were elevated(P<0.05).CONCLUSION:Hypericin attenuates myocardial ischemia-reperfusion injury in rats,possibly by modulating the AMPK/Nrf2/HO-1 signaling path-way.

10.Effects of Total Intravenous Anesthesia on Circadian Rhythms in Patients Undergoing Cardiac Transcatheter Closure

Objective To evaluate the effects of total intravenous anesthesia on the circadian rhythms in the patients undergoing cardiac transcatheter closure.Methods Thirty patients undergoing cardiac transcathe-ter closure under elective intravenous anesthesia were included in this study.Paired t-tests were performed to com-pare the mRNA levels of the genes encoding circadian locomotor output cycles kaput(CLOCK),brain and mus-cle ARNT-1 like protein-1(BMAL1),cryptochrome1(CRY1),and period circadian clock 2(PER2),the Munich Chronotype Questionnaire(MCTQ)score,and the Pittsburgh Sleep Quality Index(PSQI)score be-fore and after anesthesia.Multiple stepwise regression analysis was performed to screen the factors influencing sleep chronotype and PSQI total score one week after surgery.Results The postoperative mRNA level of CLOCK was higher[1.38±1.23 vs.1.90±1.47;MD(95%CI):0.52(0.20-0.84),t=3.327,P=0.002]and the postoperative mRNA levels of CRY1[1.56±1.50 vs.1.13±0.98;MD(95%CI):-0.43(-0.81--0.05),t=-2.319,P=0.028]and PER2[0.82±0.63 vs.0.50±0.31;MD(95%CI):-0.33(-0.53--0.12),t=-3.202,P=0.003]were lower than the preoperative levels.One week after surgery,the pa-tients presented advanced sleep chronotype[3:03±0:59 vs.2:42±0:37;MD(95%CI):-21(-40--1),t=-2.172,P=0.038],shortened sleep latency[(67±64)min vs.(37±21)min;MD(95%CI):-30.33(-55.28--5.39),t=-2.487,P=0.019],lengthened sleep duration[(436±83)min vs.(499±83)min;MD(95%CI):62.80(26.93-98.67),t=3.581,P=0.001],increased sleep efficiency[(87.59±10.35)%vs.(92.98±4.27)%;MD(95%CI):5.39(1.21-9.58),t=2.636,P=0.013],decreased sleep quality score[1.13±0.78 vs.0.80±0.71;MD(95%CI):-0.33(-0.62--0.05),t=-2.408,P=0.023],and declined PSQI total score[6.60±3.17 vs.4.03±2.58;MD(95%CI):-2.57(-3.87--1.27),t=-4.039,P<0.001].Body mass index(BMI)(B=-227.460,SE=95.475,t=-2.382,P=0.025),anesthesia duration(B=-47.079,SE=18.506,t=-2.544,P=0.017),and mRNA level of PER2(B=2815.804,SE=1080.183,t=2.607,P=0.015)collectively influenced the sleep chronotype,and the amount of anesthesia medicine(B=0.067,SE=0.028,t=2.385,P=0.024)independently influenced the PSQI one week after surgery.Conclusions Total intravenous anesthe-sia can improve sleep habits by advancing sleep chronotype.BMI,anesthesia duration,and mRNA level of PER2 collectively influence sleep chronotype one week after surgery.The amount of anesthesia medicine independently influences the PSQI total score one week after surgery.
