The catalytic activity of 3-mercaptopyruvate (3MP) sulfurtransferase (MPST) converts 3MP to hydrogen sulfide (H2S). However, the regulatory mechanisms governing MPST and its impact on the brain remain largely unexplored. Our study reveals the neuroprotective role of endothelial MPST-generated H2S, regulated by protein phosphatase 2A (PP2A). Bioinformatics analysis and RNA sequencing demonstrated that endothelial PP2A is associated with neurodegenerative disease pathways. Cerebral ischemic mice exhibited significant inactivation of endothelial PP2A, evidenced by the reduction of PP2Acα in the brain endothelium. Mice with endothelium-specific null PP2A (PP2AEC-cKO) exhibited neuronal loss, cognitive dysfunction, and long-term potentiation deficits. Postnatal inactivation of endothelial PP2A also contributes to cognitive dysfunction and neuronal loss. However, regaining endothelial PP2A activity by overexpressing Ppp2ca rescued neuronal dysfunction. Mechanistically, PP2A deficiency is intricately linked to the MPST-H2S signaling pathway. A robust reduction in endothelial MPST-dependent H2S production followed PP2A deficiency. Exogenous H2S treatment and AAV-mediated overexpression of MPST in brain endothelial cells significantly mitigated neuronal dysfunction in PP2AEC-cKO mice. Furthermore, PP2A deficiency promotes an increase in calcium influx and calpain2 phosphorylation, subsequently leading to MPST degradation. The PP2A activator (FTY720) and MPST activator (3MP sodium) both remarkably restored endothelial MPST-dependent H2S production, subsequently rescuing ischemia-induced neurological deficits. In conclusion, our study demonstrates that endothelial PP2A deficiency leads to MPST degradation by activating calpain2, thus damaging neuronal function.

Cervical spinal cord injury without fracture-dislocation (CSCIWFD) is referred to as a special type of cervical spinal cord injury characterized by traumatic spinal cord dysfunction and no significant bony structural abnormalities on imagines. Duo to the high risk of missed diagnosis during the initial consultation, CSCIWFD may lead to progressive neurological deterioration or even complete paralysis, severely impacting patients′ prognosis. Currently, there are no established consensuses over the diagnosis and treatment of CSCIWFD, such as the lack of evidence-based standards for indications of non-surgical treatment and risk of secondary neurological injury, as well as debates over the optimal timing for surgical intervention and indications for different surgical approaches. To address these issues, the Spine Trauma Group of the Orthopedic Branch of the Chinese Medical Doctor Association organized experts in the relevant fields to formulate Diagnosis and treatment guideline for acute cervical spinal cord injury without fracture- dislocation in adults ( version 2025) . Based on evidence-based medicine and the principles of scientific rigor and clinical applicability, the guidelines proposed 11 recommendations covering terminology, diagnosis, evaluation treatment, and rehabilitation, etc., aiming to standardize the management of CSCIWFD.

Self-limited epilepsy with centrotemporal spikes(SeLECTS)is the most common idiopathic focal epilepsy in childhood with self-limited course,characterized by centrotemporal spike-slow wave discharges on electroencephalography.Children with SeLECTS often experience language dysfunction,which is closely related to abnormalities in brain structure and function.The progresses of MRI researches of language function in children with SeLECTS were reviewed in this article.
6.Treatment of multi-finger degloved defects with 7 free flaps from a leg: a case report

In January 2023, a patient with soft tissue degloving defect of right index, middle, ring and little fingers was treated in the Department of Hand Surgery, Suzhou Ruihua Orthopaedic Hospital. Seven free flaps from a leg were harvested to reconstruct the defected wound of fingers in primary surgery. Flap thinning and plastic surgery were performed in stage-II surgery. Over the 22 months of postoperative follow-up, the flaps in right index, middle, ring and little fingers survived well with the colour and texture close to proximal skin. There was no obvious swelling of the flaps and sensation of the flaps recovered to S 3. The donor sites healed well and the donor leg walked normally.

10.Comparison of clinical effects between percutaneous locking plate and interlocking intramedullary nail in treating segmental tibial shaft fracture

Objective To compare the clinical effects of minimally invasive percutaneous plate osteosyn-thesis(MIPPO)and interlocking intramedullary nail(IMN)internal fixation in the treatment of segmental tibial shaft fractures.Methods A total of 41 patients with segmental tibial shaft fractures in the Affiliated People's Hospital of Jiangsu University and Northern Jiangsu People's Hospital were selected as the study subjects,including 22 cases in the MIPPO group and 19 cases in the IMN group.All patients were followed up for 1 year.The clinical data such as the intraoperative bleeding volume,total blood loss volume(the difference value of hemoglobin before operation and on postoperative 1 d),postoperative early loading time,fracture healing time,incidence rate of postoperative complications and Johner Wruhs functional score in postoperative 1 year were compared between the two groups.Results Compared with the MIPPO group,the fracture heal-ing time in the IMN group was shorter[(17.39±2.10)weeks vs.(19.34±3.22)weeks,P<0.05)],postoper-ative loading time was early[(6.93±1.04)weeks vs.(8.71±1.38)weeks,P<0.05],total blood loss volume was greater[(32.89±6.55)g/L vs.(18.36±4.27)g/L,P=0.177)],but the intraoperative average blood loss had no statistical difference between the two groups[(99.33±31.62)mL vs.(87.34±24.15)mL,P=0.177].There was 1 case of malunion with a forward angle of 8° in the IMN group,1 case of infection,and the incidence rate of complications was 10.53%.In the MIPPO group,there were 3 cases of bone nonunion,2 ca-ses of infection and the incidence rate of complications was 22.73%.The complication incidence rate and the excellent and good rate of Johner Wruhs functional score in postoperative 1 year had no statistical difference between the two groups.Conclusion IMN could provide the stronger fixation,shorter fracture healing time,and early weight bearing functional exercise,which is the better choice for treatment of segmental tibial frac-tures.
