1.Schroth therapy combined with core strength training improves scoliosis angle in patients with mild adolescent idiopathic scoliosis

BACKGROUND:The definitive cause of adolescent idiopathic scoliosis is not yet known.The search for a clinical approach to address adolescent idiopathic scoliosis is imminent.OBJECTIVE:To investigate the effect of Schroth therapy combined with core strength training on mild adolescent idiopathic scoliosis and to provide more bases for the clinical treatment of mild adolescent idiopathic scoliosis.METHODS:110 patients with mild adolescent idiopathic scoliosis attending the Department of Rehabilitation Medicine and Department of Spine Surgery of Affiliated Hospital of Nantong University from July 2022 to January 2024 were selected as the study subjects.They were divided into the trial group and the control group according to the wishes of the patients and their parents,with 55 cases in each group.The control group was observed and followed up,and the trial group underwent Schroth therapy combined with core strength training for 45 minutes a day for 24 weeks.The differences in imaging parameters,body surface indexes,three-dimensional ultrasound imaging angle,and quality of life were compared between the two groups before and after treatment.RESULTS AND CONCLUSION:(1)At 24 weeks after treatment,major curve Cobb,apical vertebral translation,and cervical lordosis were significantly improved in the trial group(P<0.05),while there was no significant difference in the control group(P>0.05).Major curve Cobb and apical vertebral translation in the trial group were significantly better than those in the control group(P<0.05).(2)At 24 weeks after treatment,angle of trunk rotation in the trial group was significantly lower than that before treatment(P<0.05),while there was no significant difference between before and after treatment in the control group(P>0.05),and angle of trunk rotation in the trial group was significantly lower than that of the control group(P<0.05).(3)At 24 weeks after treatment,the center of laminae angle of three-dimensional ultrasound imaging was significantly reduced in the trial group(P<0.05),while there was no significant difference in the control group before and after treatment(P>0.05).The center of laminae angle of three-dimensional ultrasound imaging was smaller in the trial group than that in the control group(P<0.05).(4)At 24 weeks after treatment,in terms of the quality of life,pain dimension score in the trial group was significantly increased(P<0.05).Both trial and control groups showed significantly higher scores in the self-image dimension compared with that before treatment(P<0.05).Both groups had significantly lower scores in the mental health dimension compared with that before treatment(P<0.05).In the dimensions of pain,self-image,mental health,and satisfaction,the trial group was significantly higher than the control group(P<0.05).(5)It is indicated that Schroth therapy combined with core strength training can improve the major curve Cobb,apical vertebral translation,and cervical lordosis angle,reduce the angle of trunk rotation,decrease the center of laminae angle of three-dimensional ultrasound imaging,and improve the quality of life,and it is effective in the treatment of mild adolescent idiopathic scoliosis.

3.Advances in the Targeted Delivery of Antibody Drug Conjugates

Targeted delivery of antibody bound to antigen is a precise drug delivery mode. It is regarded as one of the ideal targeted drug delivery modes due to its high specificity and affinity, which opens up a new way to successfully solve the problem of poor selectivity of chemotherapy drugs in antitumor therapy. Currently, the research on antibody drug conjugates(ADCs) that bind monoclonal antibodies to target antigens has become a research hotspot of molecular targeted therapy. This paper reviews the mechanism of action, targeting strategies and progress in the targeted delivery of ADCs, in order to provide reference for the clinical development of new ADCs.
4.Reliability and Validity of Morisky Medication Adherence Scale for Assessing Medication Adherence in Children with Inflammatory Bowel Disease

Abstract
OBJECTIVE To explore the reliability and validity of the Chinese version of the Morisky scale(including guardian version and minor version) applied in assessing medication adherence of children with inflammatory bowel disease, and to clarify the current status and features of medication adherence in children. METHODS The 141 children with inflammatory bowel disease were studied, and collect data through on-site distribution and collection of questionnaires using the Chinese version of the Morisky scale as an evaluation tool. Cronbach's a and factor analysis were used to evaluate the internal consistency and construct validity of scales, respectively, and Spearman test was used to evaluate the correlation between medication adherence and disease severity in children. RESULTS The internal consistency of guardian and minor version of Morisky scale determined by Cronbach's a were 0.701 and 0.738, respectively, while factor analysis indicated that the two scales were all composed of three factors which could explain 67.94% and 72.24% of total variance contribution rate, respectively. The adherence score of the 141 children was 6.75(4.75, 8.0). Among them, 58(41.1%), 39(27.7%) and 44(31.2%) children had poor, moderate and good medication adherence respectively; significant negative correlation was found between children's medication adherence and their disease severity(Rs=-0.286, P=0.001). CONCLUSION Both the guardian and minor version of the Chinese-version Morisky scale exhibit good reliability and validity in evaluating medication adherence in children with inflammatory bowel disease, thus can be applied to evaluate medication adherence in children. Nearly half of the children with inflammatory bowel disease have poor medication adherence, while forgetting to take medicine is the main barrier, and significant negative correlation is found between children's medication adherence and their disease severity, high attention should be given to clinical practice.
5.Prediction of EGFR mutant subtypes in patients with non-small cell lung cancer by pre-treatment CT radiomics and machine learning

Objective:To evaluate the feasibility and clinical value of pre-treatment non-enhanced chest CT radiomics features and machine learning algorithm to predict the mutation status and subtype (19Del/21L858R) of epidermal growth factor receptor (EGFR) for patients with non-small cell lung cancer (NSCLC).Methods:This retrospective study enrolled 280 NSCLC patients from first and second affiliated hospital of University of South China who were confirmed by biopsy pathology, gene examination, and have pre-treatment non-enhanced CT scans. There are 136 patients were confirmed EGFR mutation. Primary lung gross tumor volume was contoured by two experienced radiologists and oncologists, and 851 radiomics features were subsequently extracted. Then, spearman correlation analysis and RELIEFF algorithm were used to screen predictive features. The two hospitals were training and validation cohort, respectively. Clinical-radiomics model was constructed using selected radiomics and clinical features, and compared with models built by radiomics features or clinical features respectively. In this study, machine learning models were established using support vector machine (SVM) and a sequential modeling procedure to predict the mutation status and subtype of EGFR. The area under receiver operating curve (AUC-ROC) was employed to evaluate the performances of established models.Results:After feature selection, 21 radiomics features were found to be efffective in predicting EGFR mutation status and subtype and were used to establish radiomics models. Three types models were established, including clinical model, radiomics model, and clinical-radiomics model. The clinical-radiomics model showed the best predictive efficacy, AUCs of predicting EGFR mutation status for training dataset and validation dataset were 0.956 (95% CI: 0.952-1.000) and 0.961 (95% CI: 0.924-0.998), respectively. The AUCs of predicting 19Del/L858R mutation subtype for training dataset and validation dataset were 0.926 (95% CI: 0.893-0.959), 0.938 (95% CI: 0.876-1.000), respectively. Conclusions:The constructed sequential models based on integration of CT radiomics, clinical features and machine learning can accurately predict the mutation status and subtype of EGFR.
6.Preliminary application study of 5G-based robotic remote ultrasound diagnosis system in musculoskeletal joint injuries

Objective:To explore the value of 5G-based robotic remote ultrasound diagnosis system in musculoskeletal joint injuries.Methods:From March to December 2020, 58 volunteers at a training base who felt musculoskeletal pain or paresthesia were selected and performed both robotic remote ultrasound (remote ultrasound group) and conventional ultrasound (portable ultrasound group). The two types of examinations were compared, the consistency of the two diagnosis results was analyzed by the Kappa test, and the the difference of the diagnosis results was compared by McNemar test.Results:Among the 58 volunteers, 40 cases were positive by both methods and 11 volunteers had 2-3 positive results. There were 59 positive results in the remote ultrasound group and 64 positive results in the portable ultrasound group. The positive rate of the examination sites from high to low was knee joint>foot and ankle joint >hand and wrist joint >shoulder joint>elbow joint, calf and hip. The diagnosis results of the two groups were in good consistency (Kappa=0.782, P<0.001), and there was no statistically significant difference in the diagnosis results between the two groups (χ 2=3.2, P=0.063). Five more diseases with positive results were detected in the portable ultrasound group: 1 meniscus injury, 1 medial collateral ligament injury, 1 soft tissue injury around the metatarsal, 1 biceps tendinitis with effusion and 1 cubital ulnar nerve subluxation. Conclusions:The 5G-based robotic remote ultrasound system has good consistency with conventional ultrasound in the diagnosis of musculoskeletal injures. It can be applied to the ultrasound diagnosis of musculoskeletal joint injuries in remote areas.
7.Clinical biological characteristics of metastatic carcinoma of the thyroid: a report of 30 cases

Objectives To study the clinical characteristics,diagnosis,treatment and prognosis of cancer metastases to the thyroid gland.Methods At Tianjin Medical University Cancer Hospital,from 1980 to 2016 30 patients were identified with a metastatic malignant tumor of the thyroid gland.Results Primary tumor sites were in the esophagus (26.7%),lung (16.7%),and kidney (13.3%).The median age at discovery of thyroid metastasis was 56 years old.The time lapse ranged from 0 to 108 months.There were 18 patients with metachronous metastasis with median survival of 36 months,12 patients were with synchronous metastasis,and the median survival was 8 months (P < 0.01).20 patients were treated with thyroid surgery,and of which 15 patients were given post-op radiotherapy and chemotherapy.Patients who underwent thyroid resection had a median survival of 15 months,while that was 8 months in those without thyoid surgery (P < 0.01).Conclusions Metastases to the thyroid gland is a rare clinical phenomenon,the prognosis of patients is mainly related to the biological behavior of primary tumors,but surgical resection and combined treatment can increase the survival rate.
8.Preliminary application study of using tele-ultrasound robot system in volunteers

Objective To explore the accuracy of ultrasonic diagnosis using the tele-ultrasound robot system . Methods During the period from May to June 2018 ,42 volunteers were consecutive selected to participate in this study ,and their digestive system and urinary system were examined using the robot method manufactured by GI Tech Co . ,Ltd ,Shenzhen and the traditional method . The results including the ultrasound diagnosis and the inspection time of two methods in each patient were compared . The ultrasonographic diagnosis of each volunteer was obtained by two methods and the time was recorded . The ultrasound images of the two methods were mixed together ,and the quality of the images was evaluated double-blindly by two senior doctors . Results There were actually 25 positive lesions in 42 volunteers . In the ultrasonic diagnosis of the two methods ,22 lesions were positive detected by robot method ,with 5 lesions misdiagnosed . In the traditional method , 24 lesions were positive detected , with 1 lesions misdiagnosed . Using the Wilcoxon signed rank test of paired sample comparison ,the score obtained by the robot method was 4 .79 ± 0 .57 ,and the score obtained by the traditional method was 4 .81 ± 0 .54 ( Z =0 .775 ,P= 0 .439) ,the difference was not statistically significant . There was no statistically significant difference in the images'qulity between the robot method and the traditional method using the chi-square test of the four-grid data( P >0 .05) . The check time for volunteers with negative result was ( 8 .64 ± 2 .95) min in robotic method and ( 2 .55 ± 0 .74 ) min in the traditional method ,the difference was statistically significant ( t =15 .161 , P =0 .000) . Conclusions The robot method has high value in common disease diagnosis and high quality in image acquisition ,and can be used in real-time diagnosis of the remote areas or community medical .
9.Functions of lamin B1 and the new progress of its roles in neurological diseases and tumors.

Lamin B1 is one of the essential members of the nuclear lamina protein family. Its main function is to maintain the integrity of nuclear skeleton, as well as to participate in the cell proliferation and aging by affecting the chromosome distribution. gene expression, and DNA damage repair. The abnormal expression of lamin B1 is related to certain diseases, including neurological diseases [e.g. neural tube defects (NDTs), adult-onset autosomal dominant leukodystrophy (ADLD)] and tumors (e.g. pancreatic cancer). It is also a potential tumor marker as well as drug target. Further research on lamin B1 will help people understand the molecular mechanism of the emergence and development of neural system diseases and tumors, and define a new future in drug target.

10.Risk factors of the lumbar injuries during parachutists' training and relevant biomechanics researches

Objective To review the risk factors,mechanical characteristics and the research methods of spine biomechanics of lumbar injuries during parachuting training and to analyze the biomechanics and protective mechanisms of lumbar muscles in order to provide research strategy for reducing the lumbar injuries during parachuting training.Literature resource and selection The published domestic and foreign articles in the related fields.Literature quotation Fifty-one articles published at home and abroad were cited.Literature synthesis The rate of lumbar injuries during parachuting training was high in the training injuries.Acute injuries mainly happened during the process of altitude parachuting landing and chronic injuries mostly occurred in the long-term ground training of simulated parachuting landing.The early methods of biomechanics measuring of lumbar injury were simple and the data error was big and those could not completely reflect the injury mechanisms and mechanical characteristics.Modern biomechanical research methods,including motion capture,biological modeling,finite element analysis,electromyography signal,etc..integrating the static and dynamic analyses,could accurately calculate the lumbar force,provide data reference for the study of lumbar injuries during parachuting training and develop protective equipment.Conclusions Modern biomechanical research methods can be utilized to accomplish accurate biomechanical testing and lumbar mechanical analysis,as well as to properly improve training methods in order to reduce the incidence of lumbar injuries and non-combat depletion in parachuting training.
