ObjectiveThis study aimed to provide a scientific basis for the safety management of drinking water by assessing the contamination levels of 21 per- and polyfluoroalkyl substances (PFASs) in water samples collected from the Yangtze River Delta region. MethodsSolid-phase extraction coupled with ultra-high-performance liquid chromatography-tandem mass spectrometry (UHPLC-MS/MS) was employed to determine 21 PFASs in 6 source water samples, 20 treated water samples, and 60 tap water samples collected from 5 administrative districts in the Yangtze River Delta region across four seasons in 2023. The Kruskal-Wallis H test was used to compare differences in PFAS concentrations among the three types of water samples, followed by post hoc pairwise comparisons with Bonferroni correction. Removal efficiencies of target compounds were also calculated. ResultsAmong the 86 samples, 14 PFASs were detected, with 7 compounds detected in 100% of samples. The total concentrations of PFASs in source water, treated water, and tap water ranged from 63.85 to 190.54 ng·L-¹, 55.75 to 774.36 ng·L-¹, and 40.72 to 689.80 ng·L-¹, respectively. The compounds with the highest median concentrations were perfluorobutanoic acid (PFBA) (40.19, 15.20, and 14.05 ng·L-¹), perfluorohexanoic acid (PFHxA) (29.69, 32.62, and 34.24 ng·L-¹), perfluorooctanoic acid (PFOA) (17.65, 39.45, and 36.83 ng·L-¹), and potassium perfluorobutanesulfonic acid (PFBS) (16.99, 24.38, and 23.89 ng·L-¹). Notably, concentrations of PFOA (7.71‒71.63 ng·L-¹) and PFOS (

ObjectiveTo explore the influencing factors of tumor regression patterns after neoadjuvant chemotherapy (NACT) in breast cancer, construct and validate a predictive model for tumor regression patterns. MethodsA total of 1 314 breast cancer patients who received NACT between March 1, 2019, and March 1, 2024, were selected as study subjects and randomly divided into a validation cohort (n=301) and a study cohort (n=603). Within the study cohort, patients were further stratified based on post-NACT tumor regression patterns into a concentric regression group(case group)(n=387) and a non-concentric regression group(control group)(n=216). General clinicopathological data were collected and compared between the two groups. Unconditional logistic regression was used to analyze the influencing factors of tumor regression patterns post-NACT and establish a predictive model. The Hosmer-Lemeshow test and receiver operating characteristic (ROC) curve were employed to evaluate the model's goodness-of-fit and predictive performance, followed by external validation. ResultsAmong all subjects in the study cohort, 174 patients (28.9%) achieved pathological complete response (pCR), 213 (35.3%) showed unifocal regression, 92 (15.3%) had multifocal regression, 78 (12.9%) presented main residual lesions with satellite lesions, 43 (7.1%) had stable disease, and 3 (0.5%) experienced disease progression. Significant differences were observed between the two groups in age, estrogen receptor(ER), progesterone receptor(PR), Ki-67, human epidermal growth factor receptor 2(HER2), histological grade, prognostic nutritional index(PNI), and chemotherapy regimen (all P < 0.05). Logistic regression analysis revealed that age (OR=0.563, 95% CI: 0.358–0.885), ER (OR=0.521, 95% CI: 0.350–0.773), PR(OR=0.552, 95% CI: 0.379–0.806), HER2(OR=3.729, 95% CI: 2.488–5.590), Ki-67 (OR=1.804, 95% CI: 1.071–3.039), PNI (OR=2.285, 95% CI: 1.307–3.997) and histological grade (Ⅲ) (OR=2.194 95% CI: 1.283–3.751) were independent influencing factors for concentric regression post-NACT (all P < 0.05). The Hosmer-Lemeshow test for the logistic regression model yielded a P-value of 0.927, and the area under the ROC curve was 0.731 (95%CI: 0.690-0.772, P < 0.001). External validation demonstrated that the area under the ROC curve of this prediction model was 0.764 (95% CI: 0.709–0.820, P<0.001), with a sensitivity of 75.0% and a specificity of 65.7%. The Youden's index was 0.407. ConclusionThis logistic regression model exhibits high predictive value and provides a reference for clinicians to predict tumor regression patterns in breast cancer patients after NACT.

ObjectiveTo explore the biological mechanism of bone loss caused by perfluorooctanoic acid (PFOA) through transcriptomic analysis, and to provide new insights into regulating perfluoroalkyl substances (PFAS) applications and the prevention of hazards affecting bone health. MethodsMouse embryonic osteoblast precursor cells (MC3T3-E1) were exposed to 0.1, 1, 10, and 100 μmol·L-¹ PFOA for 24 hours to assess the effects on cell viability and alkaline phosphatase (ALP) activity, and to determine the critical concentration of PFOA toxicity. The transcriptome sequencing (RNA-seq) was performed to identify differentially expressed genes (DEGs) induced by PFOA. Gene ontology (GO) analysis and gene set enrichment analysis (GSEA) were conducted to identify significantly affected gene pathways. Additionally, Seahorse XF metabolic phenotyping and reverse transcription polymerase chain reaction (RT-PCR) were used to validate the key pathways. ResultsExposure to 10 and 100 μmol·L-¹ PFOA significantly reduced the cell viability and ALP activity of MC3T3-E1 cells. Therefore, the results of transcriptomic analysis for 10 μmol‧L-1 PFOA exposure found that a total of 80 DEGs were identified, including 32 upregulated genes and 48 downregulated genes. According to GO analysis, PFOA mainly affected cellular components such as mitochondrion and nucleus, molecular functions involving GTPase activity and GTP binding, as well as biological process related to mRNA processing. GSEA identified the downregulation of the β-oxidation of fatty acid pathway in mitochondria. Metabolic phenotyping reserches showed that PFOA indeed reduced mitochondrial aerobic respiration capacity and adenosine triphosphate (ATP) production, and the ratio of ATP production from cellular aerobic respiration to glycolysis was significantly decreased as well. The mRNA expression of glucose metabolism-related genes (GK, G6PD, and CS), as well as fatty acid metabolism-related genes (CPT1A and CPT2), were significantly downregulated. ConclusionPFOA reduces bone formation by inhibiting energy metabolism and β-oxidation of fatty acid pathways in osteoblasts, whihc lays the foundation for revealing the mechanism of PFOA exposure induced bone loss.

Thoracolumbar spine fracture often leads to severe pain, functional impairments, and neurological deficits, for which open reduction and internal fixation can effectively restore the spinal structural stability. Open decompression and reduction with internal fixation can help relieve spinal cord compression and improve spinal function in cases of concomitant cord injury. Although spinal stability can be restored through surgery, patients often face chronic pain and functional impairments postoperatively. A postoperative rehabilitation program is critical in optimizing therapeutic outcomes, reducing complications, and minimizing the risk of secondary injuries. However, current rehabilitation methods, such as physical therapy, functional training, and pain management, are confronted with problems in clinical practice, including significant variation in efficacy, poor patient adherence, and prolonged rehabilitation period. There is an urgent need for a unified rehabilitation strategy to address these problems. To this end, the Spinal Trauma Group of the Orthopedic Physicians Branch of the Chinese Medical Association and the Spine Health Professional Committee of the Chinese Human Health Technology Promotion Association organized experts from relevant fields to formulate Evidence-based guidelines for rehabilitation treatment after internal fixation of thoracolumbar spine fracture in adults ( version 2025) by integrating evidences from clinical researches and advanced rehabilitation concepts at home and abroad. A total number of 14 recommendations concerning the rehabilitation treatment with multimodal analgesia, psychological intervention, deep vein thrombosis prevention, core muscle and extremity exercise, appropriate use of braces, early weight-bearing, device-aided rehabilitation exercise, neuroregulatory therapy, rehabilitation team were put forward, aiming to standardize the post-operative rehabilitation process following internal fixation, promote the functional recovery, and enhance patients′ quality of life.
