1.Influence of Orthokeratology Lens Geometric Design Parameters on Corneal Biomechanics:A Finite Element Study

Objective To investigate corneal biomechanical differences in different geometric design parameters of orthokeratology lenses(OK lenses)and to further reveal the corneal shaping mechanism of OK lenses.Methods A coupled finite element model of the aspheric OK lens corneosclera was established for various geometric design parameters of the OK lens,corresponding to different degrees of myopia correction.The distribution trends of the profile and curvature of the corneal anterior surface,as well as the von Mises stress(VMS)and eye axis displacement on both the corneal anterior surface and superior corneal stroma surface were analyzed numerically.Results The stress concentration of the corneal anterior surface was observed in the mid-peripheral and peripheral zones,whereas that of the superior corneal stroma surface appeared in the mid-peripheral zones.The sagittal height of the base curve of the OK lens decreased with increasing degree of myopia correction.At myopia correction degrees of-2.0,-3.0,-4.0,-5.0,and-6.0 D,the maximum corneal VMS increased by 0.81%,1.86%,2.84%,3.81%,and 7.04%,respectively,compared with that at-1.0 D;the curvature of the corneal central zone was reduced by an average of 2.59,3.78,4.51,4.99,5.33,and 6.41 D compared with that without OK lenses.Conclusions The sagittal height of the base curve of the OK lens decreased with increasing degree of myopia correction,resulting in a flatter central curvature of the cornea.The base curve of the OK lens plays a crucial role in both correction and control of myopia.

3.Current Development Status and Consideration for Rare Hemorrhagic Disease Drugs

Hemophilia is the only rare hereditary hemorrhagic disorder included in the First Rare Diseases catalogue. However, rare bleeding diseases identified in the clinic are far more common than hemophilia. Most other rare hemorrhagic disorders have less effective treatment than hemophilia. Hemophilia has a history of successful drug development in rare hemorrhagic diseases, and the cycle between clinical research and drug development has been gradually realized. Drug research and pharmaceutical companies can refer to the drug research and development process in the field of hemophilia, learn from the experience of hemophilia drug research and develop treatments. The industry can increase drug development by strengthening basic research, focusing on the value of natural history research, the application of quantitative pharmacological tools and improving the efficiency of drug development to meet the urgent unmet medical needs of patients with rare hemorrhagic diseases.
4.Application of preoperative diet management model of enhanced recovery after surgery based on process management theory

Objective:To study how to construct the preoperative diet management mode of enhanced recovery after surgery (ERAS) based on process management theory, and the application effect of the new model in clinical practice.Methods:By convenience sampling method, 120 patients of colorectal surgery in the First Affiliated Hospital of Wenzhou Medical University from November 2018 to February 2019 were selected as the control group, and were treated with routine fasting and water deprivation management plan. A total of 118 patients from May to September 2019 were selected as the test group, and the new preoperative diet management process was used to guide nurses to prepare for preoperative fasting and water deprivation. The implementation effect of preoperative diet management and the satisfaction of patients and nurses to the process were evaluated.Results:The actual preoperative fasting time of the experimental group was (6.53±0.86) hours, and the time of water deprivation was (2.74±0.76) hours, which were both significantly shorter than those of the control group, and the differences were statistically significant ( t=20.85, 36.31; P< 0.01) . The satisfaction of patients in the experimental group to the new process was (98.03±1.81) , and the satisfaction of nursing staff was (98.47±1.42) , which were both higher than those of the control group, and the differences were statistically significant ( t=-33.57, -13.29; P<0.01) . Conclusions:The ERAS preoperative diet management model based on process management theory can provide reasonable and effective work path, improve the preoperative discomfort experience and patient satisfaction.

Spinal cord injury is a catastrophic injury causing lifelong severe disabilities, and poses a great burden to the individuals, families and society. In order to promote the standardization in treatment of traumatic spinal cord injury, the consensus on the evaluation, treatment and rehabilitation of traumatic spinal cord injury was suggested by experts, who came from authoritative multicenter in China. The expert consensus, which formed a standardization process from the first aid clinical treatment to rehabilitation of spinal cord injury, shall give a better practical guide for clinic and rehabilitation physicians.
7.Finite element analysis of the three and four cannulated screws for Pauwels III femoral neck fractures

BACKGROUND:Femoral neck fracture is mainly fixed by three inverted triangle cannulated screws. Scholars have proposed to add a cannulated screw to enhance the fixation strength of femoral neck fracture of Pauwels III type based on three cannulated screw fixation, but the stability is not verified. OBJECTIVE:To analyze the biomechanical stability and stress of the three and four cannulated screws for the treatment of the Pauwels III femoral neck fractures. METHODS:The CT imaging results of the fourth generation of artificial bone sawbones were imported into the Mimics software wherein a three-dimensional finite element model of the proximal femur was prepared and introduced in the 3-matic software. Models of middlesegment of femoral neck with Pauwels III fractures were established. Cannulated screw models were established with UG 8.0 software and introduced in the fractures models. Finally, finite element models of Pauwels III femoral neck fractures fixed with three and four screws were established. In the same condition, an axial load of 411 N was applied on the femoral head with Abaqus software. The displacement of two markers of the broken ends and internal fixation system Von Mises stress distribution were compared between the two models. RESULTS AND CONCLUSION:(1) The displacement was 0.42 mm in three screws model, and 0.17 mm in the four screws model. (2) Von Mises stress peak was 547 MPa and 27.8 MPa in both models. The peak value was lower in models of fourscrews than that of three screws. Stress concentration position was at the fracture site in both models. The stress range of models of four screws was more extensive and scattered. (3) Finite element analysis results demonstrated that four-screw implantation for Pauwels III femoral neck fractures had strong anti-shearing force and biomechanical stability. Clinical advantages need further clinical comparative study.
8.The relationship between breakfast frequency and dyslipidemia

Objective To investigate the prevalence of dyslipidemia and the relationship between breakfast frequency and dyslipidemia in residents over 40 years old in Yiling area of Yichang City in Hubei Province. Methods A random sampling was conducted, and 10 420 inhabitants were investigated during 2011 to 2012. Results The morbidity of dyslipidemia was 64. 0%. It was significantly higher in female than in male (65. 9% vs 60. 6%). Compared with regular breakfast eaters, non-breakfast eaters had significantly higher morbidity of higher blood low density lipoprotein-cholesterol(LDL-C) and hypertriglyceridemia(P<0. 05). The risk of elevated serum LDL-C was higher in the non-breakfast eaters group(OR=2. 382, 95%CI 1. 300-4. 367, P=0. 019) after adjusted by age, sex, smoking, drinking, etc. Conclusions Compared with regular breakfast eaters, breakfast skippers had significantly higher morbidity of dyslipidemia. Eating breakfast on daily basis may have a significant protective effect on preventing dyslipidemia.

Objective TALE-TFs were adopted to provide a new way in detection of the expression result ofβ-ca-sein gene promoter-interesting gene expression cassettes in mouse fibroblasts.Methods TALE-TFs of eukaryotic expres-sion plasmid and expression cassette withβ-casein gene promoter and red fluorescent protein reporter gene were co-nucleo-fected into mouse fibroblasts by Amaxa nucleofector.Results and Conclusion β-casein gene promoter was activated by artificial TALE-TFs in the mouse fibroblasts.The way is a new expression verification system instead of mammary epithelial cells with fibroblasts.
10.Influence of psychological intervention on visual acuity, quality of life and satisfaction degree of nurse- patient relationship in patients with cataract during perioperative period

ObjectiveTo investigate the influence of psychological intervention on visual acuity,quality of life and satisfaction degree of nurse-patient relationship in patients with cataract during perioperative period. Methods120 cases of cataract patients from September 2009 to March 2010 during perioperative period in our hospital were enrolled,and randomly divided into the experimental group and the control group with 60 cases in each group.The control group received routine nursing,the experimental group was given psychological intervention based upon routine nursing.The postoperative visual acuity,the quality of life and the satisfaction degree with nurse-patient relationship were compared between the two groups. ResultsThe postoperative visual acuity,quality of life and satisfaction degree with nurse-patient relationship were higher in the experimental group than those in the control group. ConclusionsThe psychological intervention for cataract patients can improve their visual acuity,quality of life and satisfaction degree with nurse-patient relationship,and it is worthy of wide application.
