1.Distribution characteristics of IgA in Zhuang blood donors and its influence on blood transfusion safety in Nanning

Objective: To analyze the distribution characteristics of immunoglobulin A (IgA) concentration in Nanning Zhuang blood donors by measuring the concentration of plasma IgA. Methods: Enzyme-linked immunosorbent assay (ELISA) was performed to measure the absorbance of 2 000 plasma samples from Zhuang blood donors. The IgA concentration in samples was calculated using the ELISA Calc regression/fitting technology program. Results: The standard curve demonstrated that ELISA detection of plasma IgA concentration exhibited good precision. The frequency of IgA deficiency was 0/2 000. No statistically significant difference in the distribution of IgA concentration was observed between males and females (P>0.05). The distribution of IgA concentration varied significantly across age groups: younger individuals (18-39 years old) had lower plasma IgA levels (mg/dL) compared to older individuals (40-56 years old): 5-89.99 mg/dL group, 8.80% (176/2 000) vs 17.20% (344/2 000); 90-450 mg/dL group,20.65% (413/2 000) vs 51.20% (1 024/2 000); >450 mg/dL group, 0.45%, (9/2 000) vs 1.70% (34/2 000), P<0.05. No significant difference in IgA concentration was found among different ABO blood types in Zhuang blood donors (P>0.05). Spearman correlation analysis revealed a positive correlation between age and IgA concentration (R
=0.114, P<0.05). Conclusion: No individuals with IgA deficiency were screened out among the Zhuang blood donors in Nanning area, and plasma IgA levels progressively increase with age.
2.Research progress of unilateral biportal endoscopy technology in cervical degenerative disease.

OBJECTIVE:
To review the application and progress of unilateral biportal endoscopy (UBE) technology in the treatment of cervical degenerative diseases, and to provide reference for clinical treatment decisions.
METHODS:
The literature related to UBE technology in the treatment of cervical spondylotic radiculopathy (CSR) and cervical spondylotic myelopathy (CSM) at home and abroad was extensively reviewed, and the surgical methods, indications, effectiveness, and safety were analyzed and summarized.
RESULTS:
UBE technology is effective in the treatment of CSR and CSM, and has the advantages of good surgical field, reducing the injury of the posterior structure of the cervical spine, and protecting the facet joint process, but in general, the indications are relatively narrow, limited to single-segment or adjacent double-segment lesions, and the requirements for the operator are relatively high, and the learning curve is long.
CONCLUSION
UBE technology can be applied to the treatment of CSR and CSM, but it needs to be carried out by experienced UBE surgeons for specific cases.

4.Effect of blood glucose control on lumbar facet joint osteoarthritis in patients with type 2 diabetes mellitus

Objective:To investigate the effect of blood glucose control on the imaging severity and clinical symptoms of facet joint osteoarthritis (FJOA) in patients with type 2 diabetes mellitus (T2DM).Methods:A total of 286 patients with lumbar degenerative diseases who were diagnosed and treated in the Department of Spinal Surgery of the Third Affiliated Hospital of Sun Yat-sen University from December 2021 to December 2022 were retrospectively collected. Patients were divided into diabetic and non-diabetic groups according to whether T2DM was diagnosed at admission. Age, gender, presence of hypertension, and body mass index (BMI) were recorded. The duration of diabetes was recorded. Fasting blood glucose and peak postprandial blood glucose were monitored for 3 consecutive days. Plasma glucose and glycosylated hemoglobin were assessed by blood biochemical results. Diabetic patients were divided into three sub-groups according to fasting blood glucose and glycosylated hemoglobin levels (HbA1c): ideal blood glucose control (HbA1c<6.5% and fasting blood glucose<6.1 mmol/L), good (6.5%≤HbA1c≤7.5% or 6.1 mmol/L≤fasting blood glucose≤7.0 mmol/L), and poor (HbA1c>7.5% and fasting blood glucose>7.0 mmol/L). Visual analogue scale (VAS) was used to assess the degree of low back pain. Pathria grading system was used to assess the severity of FJOA at different levels of the lumbar spine on lumbar CT. Mann-whitney U test was used to compare the difference of FJOA between L 1-S 1 segments in diabetic and non-diabetic patients. Logistic regression was used to analyze the effect of diabetes on FJOA. Kruskal-Wallis test was used to compare the difference of FJOA between different segments in diabetic patients among different sub-groups. Logistic regression was used to analyze the effect of blood glucose control on FJOA. Results:A total of 121 patients in the diabetic group and 165 patients in the non-diabetic group were included. L 4, 5 FJOA grade 3(2, 3) in diabetic patients was greater than grade 2(1, 3) in non-diabetic patients with significant difference ( Z=-3.179, P=0.001), and diabetes was an independent risk factor for L 4, 5 FJOA [ OR=1.767, 95% CI(1.032, 3.025), P=0.038]. There was no significant difference in age, BMI, sex ratio, prevalence of hypertension and blood glucose fluctuation values among different subgroups of glycemic control in the diabetic group. Patients in the poor glucose group had higher FJOA grades 2(1, 2), 3(3, 3) and 3(2, 4) at L 1, 2, L 4, 5 and L 5S 1 than those in the ideal glucose group at grades 1(1, 2), 2(1.5, 3) and 2(1, 2) with significant differences ( H=9.530, P=0.009; H=18.248, P<0.001; H=27.916, P<0.001). Patients in the poor glucose group had higher grades 3(3, 3) and 3(2, 4) of osteoarthritis of the L 4, 5 and L 5S 1 facet joints than those in the good glucose group, grades 3(2, 3) and 2(1, 2) with significant differences ( H=18.248, P<0.001; H=27.916, P<0.001). Low back pain was positively correlated with poor glycemic control, L 4, 5 and L 5S 1 FJOA ( r=0.512, P<0.001; r=0.383, P<0.001; r=0.484, P<0.001). Poor glycemic control was an independent risk factor for FJOA at L 4, 5 and L 5S 1 [ OR=4.963, 95% CI (1.095, 22.496), P=0.038; OR=6.010, 95% CI(1.061, 34.049), P=0.043]. Conclusion:Compared with non-diabetic patients, patients with type 2 diabetes have a higher risk of osteoarthritis in the facet joints of L 4, 5. Compared with diabetic patients with good or ideal glycemic control. Patients with poor glycemic control had more severe osteoarthritis of the L 4, 5 and L 5S 1 facet joints. Patients with severe facet joint degeneration and poor glycemic control often suffered more from severe low back pain.
5.Bibliometric analysis of the application of machine learning in pharmacovigilance

Objective To explore the application status and development trend of machine learning in the field of pharmacovigilance worldwide,and to provide reference for the research on the application of machine learning in the field of pharmacovigilance.Methods Relevant literature was searched in the Web of Science with the key words of"machine learning"and"pharmacovigilance"from the inception to March 1,2023.R language and other software were used to quantitatively analyze the literature data in this field.The clustering,co-occurrence and emergence visual analysis were carried out on the characteristics of annual published papers,institutions,countries,keywords and other aspects.Results A total of 904 literature were included.The number of literature published showed a fluctuating upward trend since 1994.There was cross-regional,cross-regional and cross-agency cooperation among the cooperative network institutions.The top 5 countries in the number of publications were the United States,China,Japan,South Korea and India,China and the United States had relatively close cooperation in this field.Signal detection,social media and electronic health records were high-frequency keywords in this field.Clustering and association rule analysis showed that this field focused on three aspects signal recognition,unstructured text mining and analysis,and processing and analysis of electronic medical information.At present,machine learning has made significant progress in signal recognition,social media information mining,and unstructured text processing of electronic medical information,which broaden the data sources of pharmacovigilance,improve the real-time monitoring ability of adverse drug reactions,bringing innovation impetus to the field of pharmacovigilance.Conclusion The rapid development of big data and artificial intelligence technologies has led to an increasing integration of machine learning into the field of pharmacovigilance,which promotes technical exchanges and cooperation and cross-disciplinary integration.It is necessary to optimize each machine learning algorithm to improve its accuracy and stability in pharmacovigilance,strengthen the protection measures of data privacy and security to ensure the safety of patient information.Integrating expertise in the fields of science,medicine,and data statistics with a view to promoting technological progress in the field of pharmacovigilance.
6.Study on the Characteristics of Supramolecular"Imprinting Template"of Curcuma kwangsiensis and Curcuma phaeocaulis

Objective The characteristics of supramolecular"imprinting template"of volatile oil of Curcuma kwangsiensis and Curcuma phaeocaulis were analyzed and studied based on the supramolecular"qixi"theory of Chinese materia medica combined with chemometrics.Methods The volatile oil of C.kwangsiensis and C.phaeocaulis were extracted by steam distillation,and the fingerprint and composition information of each batch were obtained by GC-MS.Total statistical moment method was used to compare the imprinting characteristics of the"imprinting template"of C.kwangsiensis and C.phaeocaulis.The core index(CI)of each batch of essential oil of C.kwangsiensis and C.phaeocaulis was calculated,and the topological characteristics of types of"imprinting template"of C.kwangsiensis and C.phaeocaulis were compared by chemometrics.Results There was no significant difference in the extraction rate of volatile oil between C.kwangsiensis and C.phaeocaulis.The average values of total zero order moment(AUCT)were(1.907±0.177)×108,(1.979±0.413)×108 μV·s,respectively,showing that there was no significant difference in the total content of volatile oil between the two groups.The mean values of the total first order moment(MCRTT)were(30.969±0.962)and(33.198±0.409)min.The average value of total second order moment(VCRTT)was(56.176±11.368)and(43.891±4.113)min2,respectively,indicating that there were significant differences in the content ratio and species of volatile oils between the two groups.The similarity of total statistical moments of C.kwangsiensis and C.phaeocaulis was mostly lower than the defined value,indicating that the chemical composition and composition ratio of the volatile oil were different.Principal component analysis and orthogonal partial least squares discriminant analysis could obviously divide C.kwangsiensis and C.phaeocaulis into two categories.Through the analysis of P value and VIP value,the CI values of Xvp 4th order,Xvpc 5th order,Xvpc 6th order,Xvpc 7th order,Xvc 3rd order,Xvpc 4th order were the main difference values of C.kwangsiensis and C.phaeocaulis.Conclusion Through the characterization of"imprinting property"and"topological characteristics"of the supramolecular"imprinting template"and combining with chemometric analysis,it is possible to successfully distinguish C.kwangsiensis and C.phaeocaulis,and find the different CI values between two"imprinting templates".
7.Preoperative Adjacent Facet Joint Osteoarthritis Is Associated with the Incidence of Adjacent Segment Degeneration and Low Back Pain after Lumbar Interbody Fusion

Methods:
The study included patients who underwent LIF. The demographic characteristics and radiographic and surgical data were collected and evaluated. The included patients were divided into control group and FJOA group based on the preoperative adjacent facet joint Pathria grade. Preoperative and last follow-up LBP Visual Analog Scale (VAS) score, leg pain (LP) VAS, Oswestry Disability Index (ODI) and RASD were evaluated and compared. The improvement rates in VAS and ODI were calculated and compared between the two groups. Logistic regression was used to analyze the risk factors of LBP relief and incidence of RASD.
Results:
In total, 197 patients (control group, 86; FJOA group, 111) were included, and the median follow-up was 46 months. The VAS and ODI in both groups significantly improved after surgery. At the last follow-up, the FJOA group had higher VAS and lower VAS improvement rates of LBP than the control group (p<0.05). However, no significant difference in the LP VAS and ODI was found between the two groups. The incidence of RASD in the FJOA group was significantly higher than that in the control group (48.6% vs. 30.2%, p=0.034). Multivariate logistic regression analysis showed that preoperative adjacent FJOA was significantly associated with LBP relief (odds ratio [OR], 0.691; 95% confidence interval [CI], 0.498–0.958) and the postoperative incidence of RASD (OR, 1.406; 95% CI, 1.020–1.939).
Conclusions
The preoperative FJOA in the adjacent segments was significantly associated with LBP following LIF. Patients with preoperative FJOA were more likely to have RASD following lumbar fusion surgery.
8.Surgical approaches and clinical outcomes of endoscopic endonasal resection for trigeminal schwannomas

Objective:To investigate the surgical approaches and clinical outcomes of endoscopic endonasal approaches (EEA) for trigeminal schwannomas(TSs).Methods:Clinical data, surgical videos and outpatient follow-up notes of 41 patients with TSs and underwent EEA between January 2013 to October 2022 in the Department of Neurosurgery of the First Affiliated Hospital of Nanchang University were retrospectively studied. The patients were 19 males and 22 females, with an average age of 45.3 (22-63) years old. Twenty-four patients had TSs on the left and 17 on the right. According to Jeong's classification, for 6 TSs with type MP, 2 tumours were resected by trans-Meckel's cave approach (TMCA) alone, and the remaining 4 TSs were resected by combined transclival approach (TCA). For the 4 tumours that involved infratemporal fossa(2 of type E3, 1 of type mE3 and 1 of type Mpe3), the surgery were performed via a trans-prelacrimal recess approach(TPRA), of which the operation for type Mpe3 was combined with a TMCA. The trans-laminal papyracea approach (TLPA) was applied to remove 2 tumours of type E1. The rest of 29 patients received the surgery by TMCA alone to remove tumours including 15 of types M, 10 of type Mp, 1 of type ME2, 2 of type E2 and 1 of type MpE2. Gross total tumour resection was achieved in 40 patients(97.6%), with only 1 patient (2.4%) had a subtotal tumour resection.Results:A total of 40 patients had completed the long-term follow-up, with 1 patient lost in follow-up. The average follow-up period was 34(3-101) months; No tumour recurrence or progression was observed over follow-up. After the surgery, preoperative symptoms were improved in 34 patients(89.5%). The main improved symptoms were: facial numbness(78.9%), facial pain(70.0%), headache(88.2%), mastication weakness (50.0%), poor vision (60.0%), diplopia (83.3%), and abducens nerve palsy (100%). Transient and permanent neurological deficits occurred in 8 (19.5%) and 9 (22.0%) patients, respectively. Cerebrospinal fluid leakage and internal carotid artery injury occurred in 1 patient each.Conclusion:According to the location of a tumour, an appropriate EEA should be selected and satisfactory results can be achieved for all types of tumours, except the TSs that has the main body of the tumour located in the posterior cranial fossa.
9.Design and application of performance salary management system in public hospitals based on job value

Scientific performance-based salary system is of great significance for hospitals to implement effective internal management, improve the enthusiasm of medical staff, and promote the high-quality development of public hospitals. Based on the requirements of the national salary system reform, a tertiary general hospital has been designing and implementing a performance-based salary system since 2016. The hospital has established a job evaluation index system to stratify various job sequences in the hospital, and determined the job value coefficients for different sequences and levels. It has also established a performance salary project system that covered job performance salary, specific performance salary, and innovative performance salary. In addition, the hospital has established a performance-based salary management system that covered salary standard formulation, performance salary accounting and distribution, and dynamic adjustment of the performance-based salary management system. The application of this performance-based compensation management system has achieved the matching of employee value, job hierarchy, and medical services. From 2016 to 2020, employees′ overall satisfaction with performance-based salary exceeded 85%. At the same time, the system could enhance the operational efficiency and quality of the hospital, drive technological development and scientific research innovation, playing a positive incentive role in the high-quality development of the hospital.
10.A cohort study of HIV infection in club drug abusers among men who have sex with men in Qingdao

Objective:To understand and analyze the incidence of HIV infection in club drug abusers among men who have sex with men (MSM) in Qingdao and provide a reference for the prevention and intervention of this population.Methods:From March 2017 to July 2022, club drug abusers among MSM who are HIV-negative were recruited by snowball sampling of MSM social organizations in Qingdao, a prospective cohort was established, and a follow-up survey was conducted every six months, with the sample size estimated to be 436. The survey contents included demographic characteristics, sexual characteristics, AIDS knowledge awareness, club drug abuse, HIV testing, and other information. HIV infection was the outcome-dependent variable, and the interval between the recruitment into the cohort and the HIV infection was the time-dependent variable. Cox proportional hazards regression analysis was conducted to identify the related factors for HIV infection.Results:A total of 987 participants of club drug abusers among MSM were recruited during the baseline survey and 826 eligible participants were enrolled in this cohort. A total of 46 HIV infection cases were found, and the cumulative follow-up time was 1 960.68 person-years. The incidence of HIV infection was 2.35/100 person-years. The multivariate Cox regression analysis showed that <30 (a HR=3.04, 95% CI: 1.62-5.71) non-Shandong residents (a HR=2.29,95% CI:1.20-4.39) found their partners through offline (a HR=4.62,95% CI:2.05-10.41), homosexual sexual partners >4 (a HR=3.06,95% CI:1.59-5.88), condom broken during sex (a HR=2.24,95% CI:1.21-4.17) and hemorrhage sexual intercourse (a HR=2.56,95% CI:1.31-5.03) were significantly associated with HIV infection in this cohort. Conclusions:The incidence of HIV infection in club drug abusers among MSM in Qingdao is generally low, but knowledge and practice have separated, and the risk of HIV infection is high. The related factors of HIV infection included younger age, non-Shandong residents, finding their partners offline, multiple sexual partners, condom break during sex, and hemorrhage sexual intercourse; HIV-related behavioral intervention should be strengthened.
