A 20-year-old male patient presented to the Department of Dermatology of Peking Union Medical College Hospital with complaints of an 8-year history of facial scarring, swelling of the lower limbs, and a 4-year history of scalp thickening. Physical examination showed thickening furrowing wrinkling of the skin on the face and behind the ears, ciliary body hirsutism, blepharoptosis, and cutis verticis gyrate. Both lower limbs were swollen, especially the knees and ankles. The skin of the palms and soles of the feet was keratinized and thickened. Objective To study the current status of the randomized controlled trial(RCT)outcome indicators of acupuncture and moxibustion in the treatment of chronic fatigue syndrome(CFS);To provide guidance and references for clinical research and protocol design.Methods RCT of acupuncture and moxibustion for CFS were retrieved from CNKI,Wanfang Data,VIP,SinoMed,PubMed,Embase,Web of Science databases from January 1,2015 to December 9,2024.Through literature screening and data extraction,Excel 2019 was used to make a descriptive analysis of the basic characteristics,diagnostic criteria,intervention measures,classification of outcome indicators,use frequency and evaluation time points of the included studies.Origin2021 software was used to draw bubble charts for visual display.Results Totally 119 RCT were included,involving 8 445 subjects,5 diagnostic criteria of Western medicine,5 diagnostic criteria of TCM,and 6 outcome indicators,namely clinical symptoms/signs,symptoms/syndrome of TCM,physical and chemical examination,quality of life,safety evaluation,and patient satisfaction.A total of 58 outcome indicators were reported,with a cumulative frequency of 430 times.Conclusion The RCT of acupuncture and moxibustion treatment of CFS have some problems,such as incomplete baseline assessment,lack of sample size calculation basis,high risk of bias,ignorance of primary and secondary indicators in the selection of outcome indicators,numerous indicators selection and measurement points,lack of long-term follow-up,inconsistent evaluation criteria for TCM syndrome efficacy,and insufficient attention to health economics evaluation and safety outcome indicators.

Since the discovery of anti-phospholipase A2 receptor antibodies, the diagnosis of primary membranous nephropathy has evolved from reliance on renal tissue biopsy to the use of serum assays in certain patients, owing to continuously developing and improving methodologies. The advancement of detection methods and the discovery of new antigens are driving primary membranous nephropathy to personalized diagnosis and treatment. This article summarizes the evolution of antigens and antibodies detection techniques related to primary membranous nephropathy, aiming to explore the advantages, disadvantages, and possible future directions of methodology.
4.Construction and application of an evaluation index system for compact county medical community development

Objective:To construct an evaluation index system for the development of compact county medical communities, so as to provide evaluation tools for promoting the high-quality development of county medical communities in China.Methods:Based on the evaluation criteria and monitoring system related policy documents for the construction of county medical communities released at the national level, an evaluation index system for compact county medical community development was constructed through policy text analysis and expert discussions. The system was applied to evaluate the construction of medical communities in Shaxian County of Fujian Province, A County of Shanxi Province, and B County of Yunnan Province.Results:The evaluation index system included four primary indicators: responsibility community, management community, service community, and interest community, covering 11 secondary indicators and 32 tertiary indicators. The evaluation results showed that the construction level of the compact county medical community in Shaxian County (91.3 points) was slightly higher than that of A County and B County (83.1 and 83.9 points), and its advantages were mainly reflected in management community and interest community. There was still room for improvement in the construction of compact medical communities in all three regions.Conclusions:The evaluation system in this study combined scientific rigor and practical guidance value. It could provide guidance for promoting the construction of compact county medical communities in various regions of China, and provide a basis for relevant policy formulation and optimization.

Paroxysmal nocturnal hemoglobinuria (PNH) is an acquired clonal hematopoietic stem cell disease caused by abnormal expression of glycosylphosphatidylinositol (GPI) on the cell membrane due to mutations in the phosphatidylinositol glycan class A(PIGA) gene. It is commonly characterized by intravascular hemolysis, repeated thrombosis, and bone marrow failure, as well as multiple systemic involvement symptoms such as renal dysfunction, pulmonary hypertension, swallowing difficulties, chest pain, abdominal pain, and erectile dysfunction. Tuberous sclerosis complex(TSC)is a rare genetic disease that can lead to benign dysplasia in multiple organs such as the skin, brain, eyes, oral cavity, heart, lungs, kidneys, liver, and bones. Its main symptoms include epilepsy, intellectual disabilities, skin depigmentation, and facial angiofibromas, whilst incidence is approximately 1 in 10 000 to 1 in 6000 newborns. This case presents a middle-aged woman who initially manifested with epilepsy and nodular depigmentation. Later, she developed a lower abdominal mass, elevated creatinine, and severe anemia. Based on clinical features and whole exome sequencing, the primary diagnosis was confirmed as TSC. Laboratory and imaging examinations revealed that the lower abdominal mass originated from the uterus. CT-guided biopsy pathology and surgical pathology suggested a combination of leiomyoma and abscess. With the involvement of multiple organs and various complications beyond the main diagnosis, the diagnostic and therapeutic process for this patient highlights the importance of rigorous clinical thinking and multidisciplinary collaboration in the diagnosis and treatment of rare and challenging diseases.
10.The 508th case: recurrent edema of bilateral lower extremities with proteinuria

A 31-year-old man sought medical evaluation for a 2-year history of edema and proteinuria, with prior pathology suggesting atypical membranous nephropathy (MN). Despite treatment with a combination of steroids, calcineurin inhibitors, and four courses of rituximab (1 g, intravenous injection), the patient′s nephrotic syndrome showed no relief (24 h urine protein peaked at 31.18 g/d), indicating refractory nephrotic syndrome. Later in the disease course, a sudden surge of creatinine level (322.5 μmol/L) prompted a renal biopsy, which revealed concurrent acute interstitial nephritis. Further treatment involving steroids, cyclophosphamide, and a fifth rituximab infusion (1 g, intravenous injection) resulted in improvement in renal function (serum creatinine: 322.5?147 μmol/L), but the MN failed to achieve partial relief. Subsequent treatment with the novel humanized CD20 monoclonal antibody obinutuzumab (1 g, intravenous injection) was initiated. In the latest follow-up, anti-phospholipase-A2-receptor antibody (PLA2R) antibody were negative, B cells were eliminated, serum albumin was 36 g/L, urine protein-to-creatinine ratio was 4 810 mg/g, and serum creatinine was 162 μmol/L. This case underscores the potential efficacy of obinutuzumab in refractory MN. For advanced MN cases, prompt identification of the cause of acute kidney injury is crucial, emphasizing the need for targeted interventions to potentially stall renal function decline.
