1.Study on the correlation of peak blood concentrations of compound sulfamethoxazole and its metabolites with clinical efficacy and adverse reactions in critically ill patients

OBJECTIVE To analyze the correlation of the peak blood concentration (cmax) of compound sulfamethoxazole (TMP/SMZ) and its metabolite N-acetyl sulfamethoxazole (NSMZ) with clinical efficacy and adverse reactions in critically ill patients. METHODS The data of critically ill patients treated with TMP/SMZ in various ICU of Hainan General Hospital from December 2023 to January 2025 were retrospectively collected. The patients were divided into success group and failure group based on the treatment outcome. Simple linear regression and Spearman correlation analysis were used to analyze the correlation of TMP cmax, SMZ cmax, and NSMZ cmax with clinical efficacy and adverse reactions. The receiver operating characteristic curve (ROC) was used to determine the cutoff values of cmax for predicting the occurrence of adverse reactions. RESULTS Among critically ill patients with an acute physiology and chronic health evaluation Ⅱ (APACHE-Ⅱ) ≥15 points 24 h of check-in at ICU, SMZ cmax of success group was significantly higher than failure group (P<0.05). The daily total dose of TMP/SMZ was positively correlated with TMP cmax and SMZ cmax( P<0.05). TMP cmax was significantly correlated with hepatotoxicity and nephrotoxicity, SMZ cmax with hepatotoxicity, and NSMZ cmax with nephrotoxicity (P<0.05). The cutoff values of TMP cmax for predicting nephrotoxicity and hepatotoxicity were 7.25 μg/mL and 6.63 μg/mL, respectively. The cutoff value of SMZ cmax for predicting hepatotoxicity was 138.00 μg/mL, and that of NSMZ cmax for predicting nephrotoxicity was 60.76 μg/mL. CONCLUSIONS Among critically ill patients with an APACHE-Ⅱ ≥15 points 24 h of check-in at ICU, SMZ cmax is associated with treatment success. Hepatotoxicity risk significantly increases when TMP cmax ≥6.63 μg/mL or SMZ cmax ≥138.00 μg/mL; nephrotoxicity risk significantly increases when TMP cmax ≥7.25 μg/mL or NSMZ cmax ≥60.76 μg/mL.

Patients with periodontal disease often require combined periodontal-orthodontic interventions to restore periodontal health, function, and aesthetics, ensuring both patient satisfaction and long-term stability. Managing these patients involving orthodontic tooth movement can be particularly challenging due to compromised periodontal soft and hard tissues, especially in severe cases. Therefore, close collaboration between orthodontists and periodontists for comprehensive diagnosis and sequential treatment, along with diligent patient compliance throughout the entire process, is crucial for achieving favorable treatment outcomes. Moreover, long-term orthodontic retention and periodontal follow-up are essential to sustain treatment success. This expert consensus, informed by the latest clinical research and practical experience, addresses clinical considerations for orthodontic treatment of periodontal patients, delineating indications, objectives, procedures, and principles with the aim of providing clear and practical guidance for clinical practitioners.

Cemental tear is a rare and indetectable condition unless obvious clinical signs present with the involvement of surrounding periodontal and periapical tissues. Due to its clinical manifestations similar to common dental issues, such as vertical root fracture, primary endodontic diseases, and periodontal diseases, as well as the low awareness of cemental tear for clinicians, misdiagnosis often occurs. The critical principle for cemental tear treatment is to remove torn fragments, and overlooking fragments leads to futile therapy, which could deteriorate the conditions of the affected teeth. Therefore, accurate diagnosis and subsequent appropriate interventions are vital for managing cemental tear. Novel diagnostic tools, including cone-beam computed tomography (CBCT), microscopes, and enamel matrix derivatives, have improved early detection and management, enhancing tooth retention. The implementation of standardized diagnostic criteria and treatment protocols, combined with improved clinical awareness among dental professionals, serves to mitigate risks of diagnostic errors and suboptimal therapeutic interventions. This expert consensus reviewed the epidemiology, pathogenesis, potential predisposing factors, clinical manifestations, diagnosis, differential diagnosis, treatment, and prognosis of cemental tear, aiming to provide a clinical guideline and facilitate clinicians to have a better understanding of cemental tear.

4.Construction of pressure injury nursing index system based on evidence-based evidence in patients with stroke hemiplegia

Objective To construct the stress injury cluster nursing index system in stroke hemiplegic patients to provide the basis for the standardized nursing of stress injury in the patients with stroke hemiple-gia.Methods The preliminary pre-investigation plan was formed through prophase literature research and multi-center questionnaire survey.The stress injury cluster nursing index system in stroke hemiplegic patients was formed by 2 rounds of Delphi expert consultation.Results The effective recovery rate of the two rounds of expert consultation was 100.0%,the expert total authority coefficient was 0.827,the first and second rounds Kendall's W was 0.216 and 0.212.The finally formed stress injury cluster nursing index system in stroke hemiplegic patients includes 4 first-level indicators,25 second-level indicators and 90 third-level indica-tors.Conclusion The constructed stress injury cluster nursing index system in stroke hemiplegic patients is scientific and reasonable.
5.Application of acceptance and commitment therapy in treating patients with maintenance hemodialysis

Objective To explore the effects of acceptance and commitment therapy(ACT)on anxiety,depression and self-management abilities of patients with maintenance hemodialysis(MHD).Methods A total of 140 MHD patients from June 2021 to December 2023 were selected as research subjects,and were randomly divided into control group and study group,with 70 cases in each group.The control group received routine nursing,while the study group received ACT intervention based on routine nursing care.Score of Self-rating Anxiety Scale(SAS),score of Self-rating Depression Scale(SDS),and score of self-management ability were compared before and after intervention in both groups.Results After intervention,the SAS and SDS scores in both groups decreased significantly,with the study group having significantly lower SAS and SDS scores than the control group(P<0.05).After intervention,the self-management ability scores of all dimensions in both groups were sig-nificantly higher than those before intervention,with the study group having significantly higher scores in all dimensions than the control group(P<0.05).Conclusion ACT can improve anxiety and de-pression in MHD patients,enhance their self-management abilities,and improve their quality of life.
6.Relationship between RDW and neurological function prognosis in patients with cardiac arrest

Objective To investigate the prognostic value of red cell distribution width(RDW)on neurological prognosis in patients with cardiac arrest(CA).Methods Retrospective analysis the case data,RDW and biochemical indicators of 107 CA patients treated in our department(January 2020 to November 2023).According to cerebral performance category(CPC),patients were divided into good neurological prognosis group and bad neurological prognosis group.The differences in clinical data and RDW between the two groups were compared.Multivariate Logistic regression analysis was used to investigate the risk factors on neurological prognosis.Predictive effect of RDW on poor neurological prognosis in CA patients was assessed by receiver operating characteristic(ROC)curve.According to the optimal cut-off value of RDW,it was divided into high RDW group(≥ 13.45%)and low RDW group(<13.45%)to compare the incidence of poor neurological prognosis.Results Totally 107 CA patients were included,34 patients in the good neurological prognosis group and 73 patients in the bad neurological prognosis group,the incidence of poor prognosis of neurological function was 68.22%.Compared with good neurological prognosis group,the acute physiology and chronic health evaluation Ⅱ score,RDW,cardiopulmonary resuscitation(CPR)time,age,serum sodium and glutamic-pyruvic transaminase in bad neurological prognosis group were significantly increased(P<0.05).Multivariate Logistic regression analysis showed that RDW,age and CPR time were independent risk factors for poor prognosis of neurological function(P<0.05).ROC curve analysis showed that the area under the curve of RDW to predict poor neural function prognosis in CA patients was 0.766.When the optimal threshold of RDW was 13.45%,the sensitivity was 73%and the specificity was 71%.The incidence of poor prognosis of neurological function in high RDW group was 84.13%,which was significantly higher than 45.45%in low RDW group(P<0.05).Conclusion Elevated RDW is an independent risk factor for increased risk of poor neurological outcomes in patients with CA.It has certain predictive value for neurological outcomes in patients with CA.
7.Study on Tongue Image and Formation of Mucus Plug in Airway of Mycoplasma Pneumoniae Pneumonia in Children

Objective To investigate the role of tongue diagnosis in the evaluation of airway mucus plug formation in mycoplasma pneumoniae pneumonia(MPP).Methods Totally 418 MPP children with MPP were included in this study.According to the formation of mucus plug under the microscope,the children with MPP were divided into mucus plug group(n=108)and non-mucus plug group(n=310).The purpose of this study was to compare the tongue features(tongue color,coating color,coating quality,tongue fluid)and levels of related inflammatory factors C-reactive protein(CRP),lactate dehydrogenase(LDH),interleukin(IL)-6,etc between the two groups,and to explore the role of tongue examination in evaluating the formation of mucus plugs in MPP.Results The tongue color of MPP mucus plug group was mainly crimson tongue,the coating color was mainly yellow fur,the coating quality was mainly thick greasy coating,and the tongue fluid was mainly fluid.The tongue image score of MPP mucus plug group was significantly higher than that of non-mucus plug group(P<0.05).Multivariate logistic regression analysis showed that CRP,IL-6,IL-17,γ interferon(IFN-γ),LDH and tongue total score were independent risk factors for mucus plug formation in MPP.The results of receiver operating characteristic(ROC)curve showed that the area under the curve(AUC)of CRP,tongue image score,LDH,IL-6,IL-17 and IFN-γ to predict the formation of mucus plug in MPP were 0.893,0.830,0.783,0.794,0.627 and 0.602,respectively.The optimal cut-off values were 37.445(mg/L),4.5(pionts),358.673(U/L),27.174(pg/mL),4.855(pg/mL)and 26.427(pg/mL),respectively.The AUC of MPP mucous plug formation was 0.930,the sensitivity was 0.907,and the specificity was 0.827.Conclusion There are differences in tongue image characteristics in MPP children with mucus plug formation.Total tongue image score can better predict mucus plug formation in MPP,and combined analysis has greater value in predicting MPP mucus plug formation.
8.Application of bronchoscope in diagnosis and treatment of hemoptysis in children

Hemoptysis is one of the acute and critical conditions in pediatrics,which can cause asphyxia,shock and even death,and needs timely and effective treatment.Bronchoscopy plays an important role in clarifying the etiology of hemoptysis,determining the bleeding site,evaluating the amount of bleeding,stopping bleeding locally under the microscope,assisting in isolating the blood source,clearing the airway and ensuring effective ventilation.However,bronchoscopy can also induce bleeding,aggravate hypoxia and even lead to death.In order to avoid risks,standardize diagnosis and treatment,and effectively play the role of bronchoscopy,this review focused on the standardized process and intervention timing of bronchoscopy in the diagnosis and treatment of hemoptysis in children.
9.Bioceramic scaffolds with two-step internal/external modification of copper-containing polydopamine enhance antibacterial and alveolar bone regeneration capability

Magnesium-doped calcium silicate(CS)bioceramic scaffolds have unique advantages in mandibular defect repair;however,they lack antibacterial properties to cope with the complex oral microbiome.Herein,for the first time,the CS scaffold was functionally modified with a novel copper-containing polydopamine(PDA(Cu2+))rapid deposition method,to construct internally modified(*P),externally modified(@PDA),and dually modified(*P@PDA)scaffolds.The morphology,degradation behavior,and mechanical properties of the obtained scaffolds were evaluated in vitro.The results showed that the CS*P@PDA had a unique micro-/nano-structural surface and appreciable mechanical resistance.During the prolonged immersion stage,the release of copper ions from the CS*P@PDA scaffolds was rapid in the early stage and exhibited long-term sustained release.The in vitro evaluation revealed that the release behavior of copper ions ascribed an excellent antibacterial effect to the CS*P@PDA,while the scaffolds retained good cytocompatibility with improved osteogenesis and angiogenesis effects.Finally,the PDA(Cu2+)-modified scaffolds showed effective early bone regeneration in a critical-size rabbit mandibular defect model.Overall,it was indicated that considerable antibacterial property along with the enhancement of alveolar bone regeneration can be imparted to the scaffold by the two-step PDA(Cu2+)modification,and the convenience and wide applicability of this technique make it a promising strategy to avoid bacterial infections on implants.
10.Evaluation value of red cell volume distribution width in prognosis of patients with cardiac arrest

Objective To evaluate the value of red cell volume distribution width(RDW)in the prognosis of patients with cardiac arrest(CA).Methods Clinical data of 146 CA patients admitted to intensive care unit(ICU)of Huzhou Central Hospital from January 2018 to October 2022 were retrospectively analyzed,and the patients were divided into survival group and death group according to the prognosis during ICU stay.The clinical data of two groups were compared,and the risk factors affecting the prognosis of CA patients were analyzed by multivariate Logistic regression.Receiver operating characteristic(ROC)curve was used to evaluate the predictive efficacy of RDW in CA patients.The survival curve was plotted by Kaplan-Meier method.Results A total of 146 CA patients were included in the analysis,among which 49 patients survived and 97 patients died,the mortality rate of CA patients in ICU was 66.44%.The scores of acute physiology and chronic health evaluation Ⅱ,RDW,blood sodium and alanine aminotransferase in death group were significantly higher than those in survival group,the time of cardiopulmonary resuscitation was significantly longer than that in survival group,and the length of ICU stay was significantly shorter than that in survival group(P<0.05).Multivariate Logistic regression analysis showed that RDW and time of cardiopulmonary resuscitation were independent risk factors for predicting death during ICU stay in CA patients(P<0.05).ROC curve results showed that RDW predicted death in CA patients with the area under curve of 0.742,the best cut-off value was 13.95%,the sensitivity was 59.8%,and the specificity was 85.7%.Patients with low RDW had significantly higher one-year cumulative survival rate than those with high RDW(χ2=18.757,P<0.001).Conclusion RDW was an independent risk factor for predicting death during ICU stay in CA patients.
