1.Integration dilemmas and intervention countermeasures for older adults in integrated medical and elderly care institutions

Taking the K integrated medical and elderly care institution in Fuzhou as a case, this study drew on ecosystem theory to analyze the structural characteristics of the elderly in family settings and elderly care institutions, revealed the integration dilemmas they encountered during the transition from family to the elderly care institutions, and explored the intervention countermeasures and practical outcomes of the “seven-day integration service” program led by social workers. The research found that after moving into the institution, the elderly commonly face challenges such as reduced spatial autonomy, standardized living rhythms, weakened emotional support networks, poor cross-system communication, unequal resource allocation, and disordered cultural identity. In the intervention practice, social workers conduct individualized need assessment and emotional support, collaborate with elder volunteers to carry out social guidance, promote cross-departmental resource integration and information linkage, create an equal and mutually supportive living atmosphere, and strengthen the elderly’s sense of subjectivity and social identity through narrative reconstruction and role reconstruction, thereby promoting the smooth transition and positive integration of their life from family to the institution. To a certain extent, this study contributes to expanding the understanding of the integration issues of the elderly in integrated medical and elderly care institutions and the intervention paths of social work, while also providing an experiential reference for the application of the ecosystem theory in local elderly social work practice.
2.Risk factors for overall postoperative complications in elderly patients undergoing gastrointestinal surgeries: a multicenter observational study.

OBJECTIVES:
To investigate the risk factors of overall postoperative complications in elderly patients undergoing gastrointestinal surgeries.
METHODS:
This study was conducted among a total of 1388 elderly patients, who underwent elective gastrointestinal surgeries at 17 centers across China between April, 2020 and April, 2022. The primary outcome was the incidence of postoperative complications within 30 days, including procedure-related, neuropsychiatric, respiratory, cardiovascular, and gastrointestinal complications as well as acute kidney injury. Baseline characteristics, preoperative psychological and functional status, intraoperative anesthesia and surgical factors, intraoperative medication, use of nerve block, and postoperative analgesia methods were compared between the patients experiencing one or more postoperative complications and those without complications. Univariate and multivariate logistic regression analyses were performed to identify the independent risk factors for postoperative complications. The relationship between postoperative acute pain and each type of complication were explored.
RESULTS:
The incidence of overall postoperative complications was 50.8% (705/1388) in these patients. Multivariate analysis showed that age (OR: 1.026; 95% CI: 1.006-1.046), prognostic nutritional index (OR: 0.998; 95% CI: 0.997-1.000), preoperative EuroQol-5 dimensions score (OR: 0.094; 95% CI: 0.018-0.500), blood loss (OR: 1.002; 95% CI: 1.001-1.003), and acute postoperative pain (OR: 1.308; 95% CI: 1.033-1.657) were significantly associated with the occurrence of postoperative complications. Specifically, patients experiencing severe postoperative pain had a significantly higher incidence of neuropsychiatric (27.2% vs 19.8%), procedure-related (17.3% vs 10.2%), and cardiovascular complications (3.6% vs 1.7%).
CONCLUSIONS
An advanced age, a low preoperative nutritional index, a poor quality of life score, a greater volume of intraoperative blood loss, and acute postoperative pain are independent risk factors for postoperative complications in elderly patients undergoing gastrointestinal surgeries. There is a significant association between acute postoperative pain and multi-system complications.

3.Construction status and development dilemmas of health think tanks in China

Health think tanks serve as a crucial intellectual support for the Healthy China strategy. The authors systematically analyzed the definitions, classifications, and operational status of health think tanks, categorizing them into five types: government-affiliated think tanks, university and research institute think tanks, social think tanks, hospital think tanks, and corporate think tanks. It was indicated that current health think tanks had significant room for improvement in both quantity and influence, particularly concerning deeper integration with policy decision-making and interdisciplinary collaboration. Although these think tanks played a unique role in supporting government decision-making, issues such as lagging institutional development, insufficient collaborative effectiveness, and difficulties in translating research findings into practice remained prominent. To enhance the ability of think tanks to serve the Healthy China strategy, this study proposed strategies including strengthening top-level design, improving participation mechanisms, optimizing management and evaluation systems, establishing data-sharing platforms, and deepening collaborative governance, with the aim of fostering the healthy development of China′s health think tanks.
4.Validation of UHPLC-MS/MS-Network Pharmacology Joint Experiment to Explore the Mechanism of Action of Tongluo Muzu Powder in Improving Cartilage Injury

OBJECTIVE To study and analyse the main components of Tongluo Muzu Powder transdermal absorption solution,as well as to investigate its mechanism of action in the treatment of osteoarthritis of the knee.METHODS The transdermal absorption solution of Tongluo Muzu Powder was extracted by Franz vertical diffusion cell,and its components were analyzed by UHPLC-MS/MS.Then TCMSP,Swiss target prediction,Gene Cards and other databases were introduced to predict the possible targets of the effec-tive components of transdermal absorption solution for the prevention and treatment of knee osteoarthritis,and Cytoscape software was used to construct the"drug-component-disease-target"visual network,and then the protein interaction network was obtained through the string database to find the core targets.Finally,AutoDock and PyMOL were used to verify the molecular docking of main active in-gredients and targets,and the gene ontology(GO)function analysis of core genes and the enrichment analysis of Kyoto Encyclopedia of genes and genomes(KEGG)pathway were conducted.Primary mouse chondrocytes were extracted,and the predicted targets of net-work pharmacology were verified by ELISA,Western blot,qPCR,etc.RESULTS A total of 48 effective components of the transder-mal absorption solution of Tongluo Muzu Powder and 88 possible targets for the treatment of knee osteoarthritis were screened out.The results of enrichment analysis showed that it was mainly involved in the process of apoptosis and oxidative stress.Molecular docking suggested that the main active ingredients and targets had good binding ability.The results showed that the lyophilized powder of the transdermal absorption solution of Tongluo Muzu Powder could reduce the concentration of inflammatory factors in the supernatant of chondrocytes after LPS intervention in a dose-dependent manner(P<0.05,P<0.01).The protein expression of p-PI3K/PI3K,p-AKT/AKT,MMP13 and p53 was decreased,and the protein expression of collagen Ⅱ was increased(P<0.05,P<0.01).At the same time,the mRNA expression of MMP13 and p53 was decreased and the mRNA expression of collagen Ⅱ was increased(P<0.05,P<0.01).In addition,it reduced the fluorescence intensity of TUNEL staining in cells(P<0.01).CONCLUSION By combining UH-PLC-MS/MS technology with network pharmacology,the main active ingredients of Tongluo Muzu Powder transdermal absorption solu-tion are preliminarily understood,and its potential mechanism of action in the treatment of knee osteoarthritis are predicted.

7.Mechanism of Xibining Ⅱ Combined with ADSC-Exos in Improving Knee Osteoarthritis by Regulating Mitochondrial Autophagy

ObjectiveTo observe the effects of Xibining (XBN) and adipose stem cell exosome (ADSC-Exos) in the cases of separate or joint application on cartilage degeneration and mitochondrial autophagy and explore its mechanism of action to improve knee osteoarthritis (KOA). MethodSD rats were divided into a sham operation group (sham group), a model group, an ADSC-Exos group (Exos group), an XBN group, and an ADSC-Exos+XBN group (Exos+XBN group). KOA model was established by using anterior cruciate ligament transection (ACLT). The pain sensitivity status of rats was evaluated, and the degeneration degree of the knee joint and cartilage tissue was detected by Micro-CT and pathological staining. The expression of p62 and LC3B was observed by immunofluorescence, and the serum levels of TNF-α, IL-1β, IL-6, and IL-15 in rats were detected by ELISA. The Western blot was used to detect the protein expression levels of MMP-3, MMP-13, ADAMTS5, ColⅡ, TIMP, ACAN, PINK1, Parkin, p62, and LC3A/B. ResultCompared with the sham group, rats in the model group showed decreased cold-stimulated foot-shrinkage thresholds and mechanical pain sensitivity thresholds, varying degrees of abrasion and loss of cartilage tissue, degeneration of cartilage tissue, elevated serum IL-1β, IL-6, IL-15, and TNF-α levels (P<0.01), and increased protein expression of MMP-3, MMP-13, and ADAMTS5 in cartilage tissue. In addition, the protein expression of ColⅡ, TIMP1, and ACAN was decreased (P<0.01). Compared with the model group, rats in each treatment group showed higher cold-stimulated foot-shrinkage thresholds and mechanical pain sensitivity thresholds, reduced cartilage tissue degeneration, lower serum levels of IL-1β, IL-6, IL-15, and TNF-α (P<0.05,P<0.01), decreased protein expression of MMP-3, MMP-13, and ADAMTS5, and higher protein expression of Cold, TIMP1, and ACAN in cartilage tissue (P<0.05,P<0.01). Moreover, the changes were the most obvious in the Exos+XBN group. ConclusionBoth ADSCs-Exos and XBN can increase the level of mitochondrial autophagy in chondrocytes and delay cartilage tissue degeneration by promoting the expression of the PINK1/Parkin signaling pathway, and the combination of the two can enhance the therapeutic effect.
8.Problems and suggestions in the implementation of drug centralized volume-based procurement policies in the hospitals

Objective:To explore the challenges in the implementation of drug centralized volume-based procurement policies in hospitals and propose corresponding optimization suggestions.Methods:From August to December 2023, a purposive sampling was conducted to select 11 pharmaceutical experts from tertiary hospitals in China for Delphi method. The survey content included " policy recommendations for promoting the acceleration and expansion of national drug centralized procurement and retaining surplus medical insurance funds for centralized procurement" .Results:Survey participants gave feedback on a set of existing problems found in the implementation of drug centralized procurement policies and proposed corresponding optimization methods. Kendall′s W coefficient of the specialist consultation was 0.332( P<0.05), demonstrating good consistency and concentration of the expert opinions. Among the problems, the score of drug supply guarantee was the highest(mean value of importance was 4.45). At the same time, the recommendation of strengthening monitoring and early warning, coordination and dispatch, and effectively ensuring the supply of centralized drug procurement had the highest score and concentration(mean value of importance was 4.91, coefficient of variation was 0.06). Conclusions:Through Delphi method, this study revealed issues and optimization methods in the implementation of drug centralized procurement policies in hospitals. The findings could provide valuable insights for improvements in the pharmaceutical sector and future policy adjustments.
9.The influence of processing depth on sensorimotor simulation of verbs

Objective To explore the influence of processing depth of verb comprehension on the pro-cess of sensorimotor simulation of verbs and its automatization characteristics.Methods Thirty-six com-mon college students were selected,and required to complete verb semantic judgment tasks of Experi-ment 1 and non-semantic judgment tasks of Experiment 2.The changes in hand and foot action out-puts were also observed.Results In Experiment 1,in semantic judgment tasks,the reaction time of leg-related verbs was significantly shorter than for hand-related verbs(P=0.001),with no significant differences in true-false word judgment tasks.Moreover,in Experiment 2,no significant interaction ef-fect was found between verb types and action execution(P=0.199).Conclusion The sensorimotor simula-tion triggered by verb comprehension is modulated by processing of different depths,and individuals are more inclined to utilize sensorimotor simulation of verbs at higher levels of processing depth,non-automatically.
