1.Preliminary exploration of X-ray imaging features in triple-negative breast cancer with different expression levels of human epidermalgrowth factor receptor 2

Objective To preliminary explore the imaging manifestations of digital breast tomosynthesis (DBT) and contrast-enhanced mammography (CEM) in triple-negative breast cancer (TNBC) patients with different levels of human epidermal growth factor receptor 2 (HER2) expression. Methods A retrospective analysis was conducted on TNBC patients who underwent preoperative DBT or CEM examinations at Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from January 2018 to December 2019 and Shanghai Second People’s Hospital from January 2022 to May 2025. Clinical data, pathological and immunohistochemical results, and imaging data were collected. Results A total of 69 TNBC patients pathologically confirmed as invasive ductal carcinoma were included, among which 34 underwent DBT and 35 underwent CEM. Among these patients, 34 (49.28%) had HER2-low expression and 35 (50.72%) had HER2-zero expression. DBT results showed that the proportion of spiculation signs in HER2-low group (n=14) was significantly higher than that in HER2-zero group (n=20; P=0.009, Padj=0.045). However, there were no significant differences in breast density type, mass shape, or calcification between the two groups. CEM results showed that on low-energy images, the proportion of spiculation signs in the HER2-low group (n=20) was higher than that in the HER2-zero group (n=15; P=0.011, Padj=0.077). Results of CEM showed that on reconstructed images, differences in background parenchymal enhancement and mass enhancement patterns between the two groups were not statistically significant; in both groups, heterogeneous enhancement was the most common, followed by homogeneous enhancement, with ring enhancement being the least common. Conclusions TNBC with low HER2 expression and TNBC with zero HER2 expression may have potential differences in the presentation of spiculation signs on DBT. However, the correlation between CEM manifestations and TNBC with different HER2 expression levels requires further research.
2.Clinical and Immunological Characteristics of Elderly Onset Rheumatoid Arthritis

To investigate the clinical features and peripheral blood immune cell subsets ofelderly (≥60 years old) onset rheumatoid arthritis (EORA) patients. The patients with rheumatoid arthritis (RA) who were hospitalized in the Department of Rheumatology and Immunology of the People's Hospital of Xinjiang Uygur Autonomous Region from March 2023 to December 2023 were selected as the study objects. The demographic data, clinical characteristics, extra-articular manifestations and laboratory examination results of the patients were collected. The patients were divided into EORA group and young adult (< 60 years old)onset rheumatoid arthritis (YORA) group, and the clinical features and peripheral blood immunological indexes of the two groups were compared. A total of 187 RA patients with an average age of (62.1±12.0) years were enrolled, including 89 patients in the EORA group and 98 patients in the YORA group. Compared with YORA group, EORA group had a higher proportion of male patients(39.3% Compared with YORA, EORA has higher male proportion, joint disease degree and disease activity. EORA is prone to interstitial lung disease, anemia and high inflammation, and the level of peripheral blood NK cells is higher.
3.Assessment of left ventricular wall shear stress at each segment in non-obstructive hypertrophic cardiomyopathy using vector flow mapping

Objective:To assess the clinical utility of vector flow mapping(VFM)for evaluating left ventricular wall shear stress(WSS)in individuals with non-obstructive hypertrophic cardiomyopathy(nHCM).Methods:Forty patients with non-obstructive HCM(nHCM)and 46 healthy volunteers in Tangdu Hospital of Air Force Medical University from May 2020 to September 2023 were enrolled prospectively in this study. The left ventricular WSS of each of 18 segments was measured and analyzed by VFM during rapid filling phase(T1),slow filling phase(T2),isovolumetric contraction phase(T3)and rapid ejection phase(T4).Results:① WSS decreased gradually from basal to apical segments in control group,while there was no such change pattern in nHCM group. ② Diastolic phase:When compared to the control group,the nHCM group demonstrated significantly increased WSS across all segments of the inferoseptal regions,the basal anteroseptal segment,and the middle and apical inferior segments during the T1 stage. Additionally,at the T2 stage,the WSS was significantly higher in the nHCM group across all segments of the inferoseptal and anteroseptal regions,the basal anterolateral segment,and the apical inferolateral and anterior segments(all P<0.05). Conversely,during the T1 stage,the basal anterolateral segment,basal inferolateral segment,and the middle and apical anteroseptal segments,as well as during the T2 stage,the basal and middle inferior segments,exhibited lower WSS values compared to the control group,with these differences being statistically significant(all P<0.05). ③Systolic Phase:During the T3 stage,the WSS values in all segments of the anteroseptal and anterolateral regions,as well as the apical inferior and anterior segments,were significantly elevated in the nHCM group compared to the control group(all P<0.05).At the T4 stage,WSS in each segment of the inferolateral and inferior regions,as well as the basal and middle anterior segments,was significantly elevated in the nHCM group compared to the control group(all P<0.01). Conversely,during the same stage,the WSS values in the basal inferolateral segment,the basal and middle anterolateral segments,and the basal and middle anteroseptal segments were significantly lower in the nHCM group than in the control group,with all differences reaching statistical significance(all P<0.01).Regarding global WSS,the left ventricular global WSS in the nHCM group was significantly higher than that in the control group at both the T2 and T3 stages(all P<0.001). Furthermore,the global WSS at the T3 stage was negatively correlated with left ventricular end-systolic volume,end-diastolic volume,and left ventricular stroke volume( r=-0.226, P=0.036; r=-0.345, P=0.001; r=-0.303, P=0.005). Conclusions:Significant differences in WSS are observed between patients with nHCM and control groups at various phases. WSS may serve as an early indicator of changes in cardiac function in nHCM patients.
4.Clinical efficacy of segmented peroral endoscopic myotomy for the treatment of achalasia

To explore the clinical efficacy of the modified segmented peroral endoscopic myotomy (POEM) for the treatment of achalasia (AC), AC patients admitted to the Jianhu People's Hospital from December 2019 to December 2021 were included in a retrospective cohort study. Patients received segmented POEM were included in the segmented POEM group, while patients treated with progressive POEM were included in the progressive POEM group. Perioperative indicators (including Eckardt scores preoperatively and at 6 months postoperatively), clinical efficacy, and complications were compared between the two groups. A total of 65 AC patients were included, among them, 28 patients were in the segmented POEM group, and 37 patients in the progressive POEM group. There were no significant differences in the general information of the two groups, including gender, age, lesion type, or Eckardt scores before the operation ( P>0.05). There were significant differences in the incidence of postoperative complications, including subcutaneous emphysema [3.6% (1/28) VS 24.3% (9/37), P=0.035] and small pleural effusion [7.1% (2/28) VS 27.0% (10/37), χ2=4.186, P=0.041] between the two groups. Postoperative chest pain duration (36.5±10.5 hours VS 44.3±11.8 hours, t=2.765, P=0.008), severity (4.1±2.1 VS 6.2±2.3, t=3.783, P<0.001), and fever days (2.1±1.3 days VS 3.1±1.5 days, t=2.816, P=0.007) also showed statistical difference. However, there was no significant difference in the incidence of postoperative pneumothorax between the two groups [0.0% (0/28) VS 2.7% (1/37), P=1.000]. At the 6-month follow-up, the segmented POEM group had a lower incidence of retrosternal pain and gastroesophageal reflux compared to the progressive POEM group ( P<0.05), and the Eckardt score was significantly improved (2.2±1.1 VS 2.9±1.3, t=2.294, P=0.025). Both segmented and progressive POEM are effective treatments for AC, but segmented POEM has higher safety, lower incidence of postoperative chest pain and gastroesophageal reflux complications, and better clinical efficacy.
5.Correlation analysis of urinary sodium excretion and early renal functional impairment in patients with primary hypertension

Objective:To analyze the correlation between 24-hour urinary sodium excretion and early renal function impairment in patients with primary hypertension.Methods:This cross-sectional study included patients with primary hypertension who were admitted to the Department of Cardiology of the Chinese People′s Liberation Army General Hospital between January 2021 and October 2024. Patients were divided into low-sodium, medium-sodium, and high-sodium groups based on their 24-hour urinary sodium excretion. General clinical data were collected using the electronic medical record system. Urinary sodium, protein, and microalbumin excretion were analyzed from 24-hour urine samples. Spearman correlation analysis was used to explore the relationship between 24-hour urinary sodium excretion and urinary microalbumin excretion. A multiple linear regression model was used to further assess the independent association between these variables. Subgroup analyses were conducted based on age were performed to determine whether age influenced the relationship between urinary sodium excretion and renal function impairment.Results:A total of 1 065 patients with primary hypertension were included, with a mean age of (55.26±14.06) years, including 568(53.33%) males. The low-sodium, medium-sodium, and high-sodium groups included 223, 579, and 263 patients, respectively. The 24-hour urinary microalbumin excretion in the high-sodium group was significantly higher than in the medium-sodium and low-sodium groups, and this trend remained consistent across different age groups (all P<0.05). Spearman correlation analysis showed a positive correlation between 24-hour urinary sodium excretion and urinary microalbumin excretion ( r=0.220, P<0.001), and this relationship was observed in all age groups (all P<0.05). Multiple linear regression analysis confirmed an independent association between 24-hour urinary sodium excretion and urinary microalbumin excretion (all P<0.001), which persisted across different age groups (all P<0.05). Conclusion:In patients with primary hypertension, 24-hour urinary sodium excretion is closely associated with microalbumin excretion, suggesting a potential link to early renal function impairment.
6.Clinical effect of active risk prevention nursing on the prevention of skin injury in patients with esophageal cancer during radiotherapy

Objective To explore the clinical effect of active risk prevention nursing on the prevention of skin injury in patients with esophageal cancer during radiotherapy.Methods A total of 102 patients who underwent radiotherapy for esophageal cancer in Jiangsu Cancer Hospital from December 2022 to November 2023 were enrolled.They were assigned to control group(n=51,routine nursing)and study group(n=51,routine nursing+active risk prevention)according to the random number table.The skin injury,quality of life,and nursing satisfaction were compared between the two groups.Results The skin injury in the study group at 20 Gy,40 Gy and at the end of radiotherapy was significantly slighter than that in the control group(P<0.05).After 4 weeks of intervention,the total score of SF-36 and scores in all dimensions in the study group were higher than those in the control group,and the degree of nursing satisfaction in the study group was significantly higher than that in the control group(72.55%vs 94.12%,χ2=8.541,P<0.05).Conclusion Active risk prevention can reduce skin injury,improve quality of life,and help to improve nursing satisfaction in patients with esophageal cancer during radiotherapy.
7.Cryo-EM structures of Nipah virus polymerase complex reveal highly varied interactions between L and P proteins among paramyxoviruses.

Nipah virus (NiV) and related viruses form a distinct henipavirus genus within the Paramyxoviridae family. NiV continues to spillover into the humans causing deadly outbreaks with increasing human-bat interaction. NiV encodes the large protein (L) and phosphoprotein (P) to form the viral RNA polymerase machinery. Their sequences show limited homologies to those of non-henipavirus paramyxoviruses. We report two cryo-electron microscopy (cryo-EM) structures of the Nipah virus (NiV) polymerase L-P complex, expressed and purified in either its full-length or truncated form. The structures resolve the RNA-dependent RNA polymerase (RdRp) and polyribonucleotidyl transferase (PRNTase) domains of the L protein, as well as a tetrameric P protein bundle bound to the L-RdRp domain. L-protein C-terminal regions are unresolved, indicating flexibility. Two PRNTase domain zinc-binding sites, conserved in most Mononegavirales, are confirmed essential for NiV polymerase activity. The structures further reveal anchoring of the P protein bundle and P protein X domain (XD) linkers on L, via an interaction pattern distinct among Paramyxoviridae. These interactions facilitate binding of a P protein XD linker in the nucleotide entry channel and distinct positioning of other XD linkers. We show that the disruption of the L-P interactions reduces NiV polymerase activity. The reported structures should facilitate rational antiviral-drug discovery and provide a guide for the functional study of NiV polymerase.

8.Efficacy study of eltrombopag in the treatment of systemic lupus erythematosus with refractory thrombocytopenia

Objective:To evaluated the efficacy and safety of eltrombopag in patients with systemic lupus erythematosus (SLE)-related refractory immune thrombocytopenia (ITP).Methods:Nine patients with severe SLE-associated refractory ITP were recruited the People's Hospital of Xinjiang Uygur Automomous Region from Septermber 2021 to February 2024 and received eltrombopag 50 mg orally daily for 1 month; basic patient information, platelet counts, other laboratory parameters and adverse events were recorded. Normally distributed continuous data were expressed as mean±standard deviation ( Mean±SD) and compared using paried samples t-test; non-normally distributed data were presented as median (interquartile range) [ M( Q1, Q3)]; categorical variables were described as n (%). Results:Eight participants (88.9%) completed the study, and at 1 month, five patients showed significant improvement in subcutaneous bleeding and erythema of the skin and mucous membranes, and three patients showed significant relief of joint and muscle involvement, and the patients′ platelet counts were (16.9±9.6)×10 9/L before treatment and increased to (114.0±53.9)×10 9/L. Differences in platelet levels before and after treatment in intergroup comparisons ( t=-4.881 , P=0.002), of which 4 patients achieved complete remission of platelet involvement, 3 patients achieved partial remission of platelet involvement, 4 cases were significantly efficacious and 3 cases efficacious. Follow-up data showed that platelet counts remained normal with no significant adverse events and one patient discontinued the study after 2 weeks of eltrombopag due to abnormal blood counts. Conclusion:Eltrombopag may be a promising new option for patients with SLE-associated refractory ITP with satisfactory remission rates, but large sample studies are needed for validation.
9.Hepatitis B virus serological marker and liver function index among patients with primary liver cancer

Objective:
To analyze the hepatitis B virus serological markers (HBVM) and abnormal rates of liver function indexes among primary liver cancer (PLC) patients with different HBVM profiles, so as to provide a reference for risk stratification and optimization of diagnosis and treatment strategies for PLC patients.
Methods:
Patients diagnosed with PLC at Qidong People's Hospital between January 2017 and June 2024 were selected for this study. Basic information such as gender and age was collected through the hospital information management system. Venous blood samples were drawn to test for HBsAg, anti-HBs, HBeAg, anti-HBe, and anti-HBc, as well as ten liver function indexes such as alanine aminotransferase (ALT), aspartate aminotransferase (AST), lactate dehydrogenase (LDH), cholinesterase (CHE), and adenosine deaminase (ADA). Compare the abnormal rates of liver function indexes among the six HBVM profiles: "big three yang" (HBsAg+, HBeAg+, anti-HBc+), "small three yang" (HBsAg+, anti-HBe+, anti-HBc+), triple antibody positive (anti-HBs+, anti-HBe+, anti-HBc+), s/c antibody positive (anti-HBs+, anti-HBc+), e/c antibody positive (anti-HBe+, anti-HBc+), and all negative.
Results:
A total of 1 434 patients with PLC were enrolled in this study. Among them, 1 043 (72.73%) were males and 391 (27.27%) were females. The median age was 64.00 (interquartile range, 16.00) years. The positive rates for HBsAg, anti-HBs, HBeAg, anti-HBe, and anti-HBc were 51.95%, 29.43%, 10.81%, 60.32%, and 88.42%, respectively. The "big three yang", "small three yang", triple-antibody positive, s/c antibody positive, e/c antibody positive, and all-negative profiles accounted for 85 (5.93%), 491 (34.24%), 170 (11.85%), 148 (10.32%), 100 (6.97%), and 121 (8.44%) cases, respectively. The abnormal rates of ALT among PLC patients with six HBVM profiles were 26.19%, 28.33%, 13.94%, 22.60%, 20.41%, and 14.91%, respectively. The abnormal rates of AST were 33.33%, 36.17%, 23.03%, 24.66%, 22.45%, and 18.42%, respectively. The abnormal rates of LDH were 62.16%, 68.22%, 53.73%, 61.19%, 60.00%, and 68.42%, respectively. The abnormal rates of CHE were 0%, 1.81%, 0%, 2.11%, 2.22%, and 3.88%, respectively. The abnormal rates of ADA were 59.09%, 57.27%, 24.27%, 33.33%, 45.00%, and 37.04%, respectively. These differences were statistically significant (all P<0.05).
Conclusions
In this study, the HBVM profiles were mainly characterized by "small triple positive" among PLC patients. The significant differences in liver function indexes abnormal rates among PLC patients with six HBVM profiles could reflect the liver injury status.
