1.Impact of Temperature Intervention on Pain and Catheterization Outcomes in Premature Infants Undergoing PICC Placement

Objective To investigate the impact of temperature intervention on pain and catheterization outcomes in premature infants undergoing peripherally inserted central catheter(PICC)placement.Methods A total of 70 premature infants who underwent PICC placement and met the inclusion criteria were selected from the Neonatology Department of a tertiary children's hospital in Kunming,Yunnan,from January 2023 to December 2023.They were randomly divided into a control group(n=35)and an observation group(n=35)using a random number table.The control group underwent the standard PICC placement procedure;the observation group underwent temperature intervention measures based on the control group,which included using iodine tincture warmed to 37℃for disinfection,applying warm compresses to the catheterization site with heated hands of the operator,and using saline warmed to 37℃during catheter insertion and skin cleansing.Pain scores measured by the Premature Infant Pain Profile(PIPP),blood oxygen saturation,heart rate,success rates of single puncture and single catheter insertion,incidence of complications,and occurrence of hypothermia post-catheterization were compared between the two groups.Results The PIPP during disinfection,puncture,catheter insertion,and cleansing were significantly lower in the observation group than in the control group(P<0.05);the average blood oxygen saturation in the observation group was higher than that in the control group,with a lower difference compared to the control group(P<0.05);the difference and average heart rate during catheterization in the observation group were both lower than those in the control group(P<0.05);the success rates of single puncture and single catheter insertion,as well as the incidence of hypothermia post-catheterization in the observation group,were both lower than those in the control group(P<0.05),while the comparison of complication rates between the two groups showed no statistically significant difference(P>0.05).Conclusion Implementing temperature intervention during PICC placement in premature infants can effectively reduce pain,maintain stable vital signs,improve the success rates of single puncture and catheter insertion,and effectively prevent the incidence of hypothermia.
2.A case of atypical cholesterol side chain defect possibly caused by CYP11A1 heterozygous mutation

This paper reports a case of atypical cholesterol side chain enzyme deficiency caused possibly by CYP11A1p.A359V heterozygous mutation. The patient was diagnosed as congenital adrenal hyperplasia because he was born with gonadal dysplasia and there were manifestations of salt losing and pigment changing, combined with high level of plasma ACTH. Further genetic screening revealed CYP11A1p.A359V heterozygous mutations in patients with a final diagnosis of cholesterol side chain enzyme deficiency which was treated effectively to glucocorticoid.
3.Application of meticulous management in safety construction of intravenous infusion

For the safety problems of internal medicine intravenous infusion,we carry out meticulous management,establish quality control system of intravenous infusion safety management,and formulate quality control plan.We collect and analyse the potential unsafety factors in the various departments of internal medicine.We have revised the intravenous infusion system and procedures,formulated standards for safety inspection of intravenous fluids,and standardized the admission system for nurses.We carry out training on intravenous infusion related knowledge for nursing staff,carry out meticulous management of venous transfusion links,and actively carry out learning and communication.All these measures have greatly enhanced the safety awareness of the nursing staff.The safety index of intravenous infusion for liver diseases was preliminarily summarized.The use rate of the safe indwelling needle was increased from 30.60% to 92.30%,the rate of appropriate rate of drop speed increased from 68.45% to 93.20%,the three sign standard rate rose from 75.20% to 95.10%,and the patient's satisfaction with infusion increased from 85.60% to 96.82%.Meticulous management can improve the safety of the internal medicine intravenous infusion as a whole.
4.Application of a new type of look-up table method in the management of diet health for patients ;with type 2 diabetes mellitus

Objective To discuss the feasibility and effect of the new look-up table method in the diet health management of type 2 diabetes patients. Methods 200 patients with type 2 diabetes in department of endocrinology in our hospital were divided into experimental group and control group according to the random number table method and 100 cases in each group. The new look-up table method and food exchange method were used respectively to intervene. Before the intervention and 6 months after the intervention, the clinical related metabolic indexes and diet self-management scores of patients in two groups were monitored, meanwhile, the time for nurses to guide patient to learn self-catering arrangements was calculated. Results Fasting blood glucose, postprandial 2 hours blood glucose, glycosylated hemoglobin of patients in experimental group were respectively (9.64±4.62) mmol/L, (12.37±4.88) mmol/L, (9.26 ± 2.56) % before intervention and were respectively (6.74 ± 3.87) mmol/L, (9.16 ± 4.76) mmol/L, (7.21 ± 3.21) % at 6 months after intervention, the difference was statistically significant (t=4.81, 4.71, 4.99, P < 0.01). However, fasting blood glucose, postprandial 2 hours blood glucose, glycosylated hemoglobin of patients in control group were respectively (9.14 ± 5.28) mmol/L, (12.95 ± 4.65) mmol/L, (9.12±3.31)%before intervention and were respectively (7.29±4.98) mmol/L, (10.01±5.56) mmol/L, (7.36± 2.86)%at 6 months after intervention, the difference was statistically significant (t=2.55, 4.06, 4.02, P<0.05 or 0.01). The diet self-management scores of patients were significantly improved in two groups, the scores of experimental group were (9.52±5.45) points before intervention and (13.70±7.11) points after 6 months intervention, the difference was statistically significant (t= 4.44, P < 0.01). The scores of control group were (9.18±6.32) points before intervention and (11.27±6.38) points after 6 months intervention, the difference was statistically significant (t=2.33, P<0.05). Meanwhile, the difference between two groups was statistically significant (t=2.54, P<0.05). The time needed for the education of experimental group was less than that of control group, (22.47 ± 5.84) min in experimental group and (32.23 ± 7.66) min in control group respectively, the difference was statistically significant (t=10.13, P < 0.01). Conclusions The new look-up table method is more convenient and can improve diet self-management ability for patients. It can also improve the work efficiency of clinical nurses and has the feasibility and applicability in the diet management of diabetes patients.
