Objective:To explore the effect of Pegylated recombinant human granulocyte colony-stimulating factor (PEG-rhG-CSF) with chemotherapy on breast cancer patients who got agranulocytosis of 3 to 4 degree, agranulocytosis with fever (FN) and the influential factors of relative dose intensity (RDI) chemotherapy scheme. Meantime, the value of CD34 + and CD45 in peripheral blood on predicting agranulocytosis of 3-4 degree were investigated. Methods:A total of 104 women with breast cancer were treated at Huzhou Maternal and Child Health Hospital and Cancer Hospital of Zhejiang Province from Jan. 2022 to Sep. 2023. All subjects received primary prevention with PEG-rhG-CSF during chemotherapy. The clinical risk factors of agranulocytosis, FN and RDI were analyzed. The levels of CD34 + and CD45 in peripheral blood samples were analyzed by flow cytometry. Then the predictive value of the receiver characteristic curve (ROC) for Grade 3 to 4 agranulocytosis after primary prophylaxis with PEG-rhG-CSF for breast cancer chemotherapy was evaluated. Results:Among 104 breast cancer patients who received primary prevention of PEG-rhG-CSF during chemotherapy, 28 patients had agranulocytosis of 3 to 4 grade, 10 patients got FN, and 12 patients developed RDI<85%. The results of single factor analysis showed that CD34 +, CD45 and chemotherapy scheme were the influential factors of agranulocytosis of 3 to 4 degree, and low RDI of chemotherapy scheme ( OR=0.584, OR=0.999, OR=2.299, OR=0.100, OR=0.999, OR=3.088, P<0.05) . It also showed that CD34 + and chemotherapy scheme were the influential factors of FN ( OR=0.099, OR=2.667, P<0.05) . Multivariate Logistic regression analysis showed that CD34 +, CD45 and intensive chemotherapy were the independent risk factors of agranulocytosis of 3 to 4 degree after primary prevention with PEG-rhG-CSF ( OR=0.602, OR=0.999, OR=20.174, P<0.05) . CD34 + and intensive chemotherapy scheme were the independent influential factors of FN and RDI of chemotherapy scheme after primary prevention with PEG-RHG-CSF ( OR=0.072, OR=33.934, OR=0.086, OR=54.788, P<0.05) . The area under the curve (AUC) of CD34 + were 0.767 (95% CI:0.659-0.876) , AUC of CD45 were 0.743 (95% CI:0.644-0.842) , and the AUC of combined two indexes was 0.825 (95% CI:0.730-0.920) , which was higher than that of single index. So AUC of CD34 + and CD45 can be used for predicting agranulocytosis of grade 3 to 4 in breast cancer patients receiving primary prophylaxis with PEG-rhG-CSF. Conclusions:The levels of CD34 + and CD45 in peripheral blood of breast cancer patients with agranulocytosis of grade 3 to 4 receiving primary prevention with PEG-rhG-CSF during chemotherapy are lower. Combined detection of CD34 + and CD45 in peripheral blood can predict the occurrence of agranulocytosis of grade 3 to 4 in breast cancer patients after primary prevention with PEG-rhG-CSF. Also it can provide a reliable basis for assessing the risk of grade 3 to 4 agranulocytosis.

Objective:To observe the effect of combining low-frequency transcranial magnetic stimulation (rTMS) with melodic intonation therapy (MIT) in the treatment of aphasia soon after a stroke. Methods:Sixty survivors of a recent stroke with aphasia were randomly divided into a control group, an MIT group and an observation group, each of 20. All received conventional rehabilitation therapy, but for 4 weeks the MIT group was additionally given MIT while the observation group received MIT plus 1Hz rTMS applied over the right inferior frontal gyrus and the posterior superior temporal gyrus. The intensity was 80% of the resting motor threshold. Before and after the treatment, language function and communication ability were evaluated using the Chinese version of the Western Aphasia Battery (WAB) and Communicative Activities of Daily Living (CADL) scale. Results:Significant improvements in the WAB and CADL scores and aphasia quotient (AQ) were observed in both the MIT group and the observation group after the treatment compared to their pre-treatment levels, but the observation group demonstrated significantly higher scores than the other two groups on all three assessments. The MIT group′s average scores were superior to those of the control group. Conclusions:Applying rTMS and MIT in combination significantly improves the speech and the daily communication abilities of aphasics early post-stroke, reducing the severity of their aphasia. The therapy exhibits considerable promise for clinical implementation.

Objective:To compare the efficacy of double reverse traction reduction combined with minimally invasive percutaneous plate osteosynthesis (MIPO) and traditional reduction combined with MIPO in treating distal femoral fractures in the elderly. Methods:A retrospective cohort study was conducted to analyze the clinical data of 78 elderly patients with distal femoral fractures admitted to Third Hospital of Hebei Medical University from January 2021 to June 2023, including 16 males and 62 females, aged 60-85 years [(74.5±7.1)years]. The bone mineral density T-score was (-2.1±0.9)SD. According to the Orthopedic Trauma Association (OTA) classification, the fractures were classified as type 33-A1 in 27 patients, type 33-A2 in 36, and type 33-A3 in 15. Forty-three patients underwent traditional reduction combined with MIPO (traditional reduction group), while 35 patients received double reverse traction reduction combined with MIPO (double reverse traction group). The two groups were compared in terms of operation time, intraoperative blood loss, number of intraoperative fluoroscopies, time to initial callus formation, radiographic healing time, range of motion of knee flexion and extension and Knee Society score (KSS) at 1 and 3 months postoperatively and at the last follow-up, and the incidence of postoperative complications. Results:All the patients were followed up for 6-18 months [(14.4±2.6)months]. The operation time, intraoperative blood loss and number of intraoperative fluoroscopies were (73.7±7.6)minutes, (112.4±32.3)ml, and (9.8±4.5)times in the double reverse traction group, which were significantly reduced compared with those in the traditional reduction group [(95.2±10.0)minutes, (139.7±49.5)ml, (15.2±3.9)times] in the traditional reduction group ( P<0.01). There was no significant difference in the time to initial callus formation between the two groups ( P>0.05). The radiographic healing time in the double reverse traction group was (25.9±5.1)weeks, shorter than (29.6±8.2)weeks in the traditional reduction group ( P<0.05). At 1 month postoperatively, range of motion of knee flexion and extension in the double reverse traction group was (96.4±5.0)°, greater than (93.9±3.7)° in the traditional reduction group ( P<0.05), and there was no significant difference between the two groups at 3 months postoperatively or at the last follow-up ( P>0.05). KSS scores at 1 and 3 months postoperatively and at the last follow-up showed no significant difference between the two groups ( P>0.05). No malunions occurred in the double reverse traction group, while 9.3% (4/43) in the traditional reduction group had malunion ( P>0.05). No nonunion or infection was observed in either group. Conclusion:Compared with traditional reduction combined with MIPO, double reverse traction reduction combined with MIPO for elderly distal femoral fractures can shorten operation time, reduce intraoperative blood loss and the number of fluoroscopies, promote fracture healing, and facilitate early recovery of knee joint function.

Objective:To investigate the value of arterial enhancement fraction in the diagnosis of hepatocellular carcinoma in patients with liver cirrhosis. Methods:The clinical data of 80 liver cirrhosis patients with pathologically and clinically confirmed hepatocellular carcinoma who received treatment from April 2019 to April 2021 in Benxi Central Hospital were retrospectively analyzed. The data from phase III enhanced CT scans were input into the PACS system. Two physicians independently measured the CT values of hepatocellular carcinoma and liver cirrhosis using a double blind method. The AEF values were calculated. According to Child-Pugh classification, the severity of liver disease was divided into class A ( n = 33), class B ( n = 29), and class C ( n = 18). The AEF value of hepatocellular carcinoma was compared with that of liver cirrhosis in the same group. The AEF value of liver cirrhosis was compared between different classes of liver cirrhosis. The AEF value of hepatocellular carcinoma was compared between different classes of liver cirrhosis. Results:The inter-observer agreement of mean AEF values was high ( ICC = 0.95, 95% CI = 0.93-0.97). The mean AEF value of hepatocellular carcinoma was (54.79 ± 10.95)% for patients with class A liver cirrhosis, (54.90 ± 9.99)% for patients with class B liver cirrhosis, and (54.16 ± 7.19)% for patients with class C liver cirrhosis. The mean AEF value of liver cirrhosis was (39.94 ± 6.50)% for patients with class A liver cirrhosis, (44.97 ± 4.31)% for patients with class B liver cirrhosis, and (54.11 ± 4.63)% for patients with class C liver cirrhosis. In patients with class A and class B liver cirrhosis, the AEF value of hepatocellular carcinoma was significantly higher than that of liver cirrhosis (class A: t = 5.18, P = 0.001; class B: t = 3.94, P < 0.001). There was no significant difference in the AEF value between hepatocellular carcinoma and liver cirrhosis in patients with class C liver cirrhosis ( t = 0.02, P = 0.982). Conclusion:The AEF value has an important reference value for the differential diagnosis of hepatocellular carcinoma from liver cirrhosis.
