1.Survey of awareness of influenza and influenza vaccine, influenza incidence, influenza vaccination status, related factors in primary and secondary school teachers in a district of Beijing

Objective:To understand the awareness of influenza and influenza vaccine, influenza incidence, influenza vaccination status and intention and related factors in primary and secondary school teachers.Methods:Cross-sectional study design and stratified random sampling method were used to select 5 teachers from each of 51 primary schools and 40 middle schools in a district of Beijing from April 17 to 21, 2024. A questionnaire survey was conducted in the selected teachers with an estimated sample size of 1 040. The survey collected the information about their demographic characteristics, awareness of influenza and influenza vaccine, influenza incidence, influenza vaccination status and intention. Multivariate logistic regression model was used to analyze the factors related to influenza vaccination status and intention.Results:In the 975 primary and secondary school teachers, the awareness of susceptibility and severity of influenza and the benefits of influenza vaccination were high. In terms of safety of influenza vaccine, 48.00% (468/975) of the teachers believed that adverse reaction would occur after the vaccination. Influenza like illness occurred in 41.13% (401/975) of the teachers during 2023-2024, and in these teachers, 40.15% (161/401) asked for leave from work due to illness, 40.90% (164/401) sought medical care and 32.93% (54/164) were influenza virus positive. In the 975 teachers, 13.95% (136/975) were vaccinated against influenza during 2023-2024 and 34.46% (336/975) were willing to be vaccinated against influenza during 2024-2025. Multivariate logistic regression analysis showed that being teacher in charge of a class (a OR=1.58, 95% CI: 1.06-2.35), believing that influenza vaccination could effectively prevent influenza (a OR=1.99, 95% CI: 1.05-3.80), believing that teachers' vaccination could improve students' willingness to be vaccinated (a OR=1.98, 95% CI: 1.12-3.50) were the positive factors for teachers' vaccination, and worrying about adverse reactions after the vaccination (a OR=0.52, 95% CI: 0.34-0.80) was the negative factor. Doctors' recommendation (a OR=2.62, 95% CI: 1.41-4.86) and free influenza vaccination (a OR=10.26, 95% CI: 5.17-20.39) were positively correlated with teachers' intention to influenza vaccination. Conclusions:Primary and secondary school teachers in a district of Beijing had high awareness of influenza and influenza vaccine, but their awareness of influenza vaccine safety should be improved. The incidence of influenza-related infection was high in the primary and secondary school teachers. The influenza vaccination rate was low and the willingness to receive influenza vaccination in following year was not strong. For the better prevention and control of influenza in schools, it is necessary to strengthen the health education in primary and secondary school teachers to improve their awareness of influenza vaccine safety and vaccination rate.
2.Adalimumab-induced cervical lymph node tuberculosis

A 38-year-old male patient with ankylosing spondylitis received subcutaneous injection of adalimumab 40 mg once every 2 weeks. After 21 months of medication, the patient developed fever, fatigue, swelling, and pain in the right neck lymph node and throat. Laboratory tests showed that the tuberculin test was strong positive, mycobacterium tuberculosis γ-interferon release test was 1 911.98 ng/L, and erythrocyte sedimentation rate was 27 mm/1 h. The biopsy of right neck lymph node showed granulomatous inflammation of the lymph node. The patient was diagnosed with cervical lymph node tuberculosis, which was considered to be related to adalimumab. The drug was stopped and anti-tuberculosis treatments were given. The next day, the patient′s temperature returned to normal. After 5 days, the swelling and pain of cervical lymph nodes and throat, and the fatigue were relieved gradually. After 45 days, the above symptoms in the patient disappeared.

4.Study on the scientific research development of primary hospitals empowered by urban medical groups from a collaborative perspective

Objective:This study aims to explore how urban medical groups can empower the scientific research development of primary hospitals through collaborative efforts, providing experience reference for the research construction of medical consortiums.Methods:Taking the Shenzhen Dapeng New District Medical and Health Group as an example, the study examined the impact of the group′s establishment on the research capabilities of primary hospitals in terms of research spirit, team and facilities. It summarized the group′s efforts to promote primary hospital research outcomes through various measures, including establishing funds, building mechanisms, setting up platforms and implementing stratified training.Results:The number of research projects approved by the group had significantly increased, with participation in national key research and development projects as sub-centers for cohort studies. The publication volume of SCI papers had continuously climbed. The integration of research and clinical practice had promoted the introduction of new technologies, significantly enhancing diagnostic and treatment capabilities and public satisfaction.Conclusions:Urban medical groups can address challenges such as the scarcity of research resources in primary hospitals through collaborative mechanisms, offering a replicable pathway for research capacity improvement. However, there is still room for improvement, including deepening the cultivation of scientific research talents and improving the support system to promote the sustainable development of primary hospital research.
5.Application of mobile health in the home management of patients with urinary incontinence after radical prostatectomy:a scoping review

Objective To conduct a scoping review on the application of mobile health in the home management of patients with urinary incontinence after radical prostatectomy,providing references for future research and clinical practice.Methods A literature search was performed in the PubMed,Web of Science,Embase,CINAHL,Cochrane Library,China National Knowledge Infrastructure(CNKI),China Biomedical Literature Database(SinoMed),Wanfang Database,VIP Database,the website of the American Urological Association and so on from the establishment of databases to March 7,2025.The included literature was summarized and analyzed.Results A total of 19 pieces of literature were included.The forms of intervention included applications,online platforms,and communication software.The intervention contents covered health education,functional exercise,self-management,communication and interaction,and follow-up management.The outcome indicators included physiological indicators,psychological indicators,quality of life,self-management,patient experience.Conclusion The application of mobile health in the home management of patients with urinary incontinence after radical prostatectomy has certain feasibility.Howe ver,its forms of intervention,contents and evaluation indicators still need to be further expanded and improved.In the future,it is possible to explore the integration of emerging technologies,combine the multi-disciplinary collaboration model,and construct a more scientific and systematic home management plan for urinary incontinence patients.
6.Construction of a predictive model for efficacy of neoadjuvant immunotherapy combined with chemotherapy in gastric cancer based on CT radiomics

Objective:To investigate the value of a computed tomography(CT)radiomics-based model for predicting the efficacy of neoad-juvant immunotherapy combined with chemotherapy for locally advanced gastric cancer(LAGC).Methods:Data on 114 patients with LAGC who underwent radical surgery after neoadjuvant immunotherapy combined with chemotherapy at The Second Affiliated Hospital of Xingtai Medical College between June 2019 and June 2021 were retrospectively collected.These patients'data were divided into a training set(n=67)and a validation set(n=47)based on the time of admission.High-throughput features were extracted from baseline portal phase CT images of all patients,and the selected features were used to construct the radiomics prediction model.The predictive performance of the model was evaluated using receiver operating characteristic(ROC)and calibration curves.The prognostic ability of the model was assessed using Kaplan-Meier curves.Results:Based on the maximum relevancy min-redundancy(mRMR)algorithm and least absolute shrinkage and selection operator(LASSO)regression model,5 out of 584 assessed features were incorporated into the radiomics(Rad)score.The respect-ive areas under the curve for predicting pathological complete response(pCR)in the training and validation sets were 0.865 and 0.830,re-spectively,and good fits were obtained(Hosmer-Lemeshow test:P>0.05).The optimal cut-off value for the Rad score was determined based on the Youden index.Patients with high Rad scores had significantly higher 3-year recurrence-free survival rates(82.7%vs.60.4%in the training set and 78.9%vs.53.8%in the validation set)and 3-year overall survival rates(78.9%vs.60.2%in the training set and 79.3%vs.50.0%in the validation set)than those with low Rad scores(P<0.05).Conclusions:The CT radiomics prediction model effectively predicted the pathological response and prognosis of patients with LAGC after neoadjuvant immunotherapy combined with chemotherapy and is ex-pected to serve as a practical clinical tool.

8.Constructing Ideas for a Self-Service Traditional Chinese Medicine Syndrome Differentiation and Home Sleep Scenario Solution

Objective To construct a self-service traditional Chinese medicine(TCM)syndrome differentiation-based home sleep scenario and establish a home-based TCM sleep aid environment.Methods Based on medical principles and modern sleep-related technologies,this study divided the home sleep scenario into four dimensions:Perception,syndrome differentiation,intervention,and evaluation.It proposed a construction plan for a self-service TCM sleep aid home scenario that integrated these four dimensions.Results The proposed TCM sleep aid scenario solution enables collaborative collection data from various devices in a home setting.This involved the setup of hardware at the perception level,the development of software at the syndrome differentiation level,the integration of hardware at the intervention level,and data cleaning,fusion,and analysis at the evaluation level.These four components are interconnected,allowing for the extraction of physiological data that is valuable for TCM-based population classification and intervention,which can be used to update algorithms and improve accuracy through iterative upgrades.Conclusion The construction of a home sleep scenario based on TCM syndrome differentiation is feasible and can provide a reference for the promotion and application of appropriate TCM techniques in home settings.
9.Molecular epidemiological characterization of influenza A(H3N2) virus in Fengxian District, Shanghai, in the surveillance year of 2023

ObjectiveTo understand the epidemiological distribution and gene evolutionary variation of influenza A (H3N2) viruses in Fengxian District, Shanghai, in the surveillance year of 2023, and to provide a reference basis for influenza prevention and control. MethodsThe prevalence of influenza virus in Fengxian District in the 2023 influenza surveillance year (April 2023‒March 2024) was analyzed. The hemagglutinin (HA) gene, neuraminidase (NA) gene, and amino acid sequences of 75 strains of H3N2 influenza viruses were compared with the vaccine reference strain for similarity matching and phylogenetic evolutionary analysis, in addition to an analysis of gene characterization and variation. ResultsIn Fengxian District, there was a mixed epidemic of H3N2 and H1N1 in the spring of 2023, with H3N2 being the predominant subtype in the second half of the year, and Victoria B becoming the predominant subtype in the spring of 2024. A total of 75 influenza strains of H3N2 with HA and NA genes were distributed in the 3C.2a1b.2a.2a.2a.3a.1 and B.4 branches, with overall similarity to the reference strain of the 2024 vaccine higher than that of the reference strain of the 2022 and 2023 vaccine. Compared with the 2023 vaccine reference strain, three antigenic sites and one receptor binding site were changed in HA, with three glycosylation sites reduced and two glycosylation sites added; where as in NA seven antigenic sites and the 222nd resistance site changed with two glycosylation sites reduced. ConclusionThe risk of antigenic variation and drug resistance of H3N2 in this region is high, and it is necessary to strengthen the publicity and education on the 2024 influenza vaccine and long-term monitoring of influenza virus prevalence and variation levels.

With the widespread adoption of low-dose CT screening and the extensive application of high-resolution CT, the detection rate of sub-centimeter lung nodules has significantly increased. How to scientifically manage these nodules while avoiding overtreatment and diagnostic delays has become an important clinical issue. Among them, lung nodules with a consolidation tumor ratio less than 0.25, dominated by ground-glass shadows, are particularly worthy of attention. The therapeutic challenge for this group is how to achieve precise and complete resection of nodules during surgery while maximizing the preservation of the patient's lung function. The "watershed topography map" is a new technology based on big data and artificial intelligence algorithms. This method uses Dicom data from conventional dose CT scans, combined with microscopic (22-24 levels) capillary network anatomical watershed features, to generate high-precision simulated natural segmentation planes of lung sub-segments through specific textures and forms. This technology forms fluorescent watershed boundaries on the lung surface, which highly fit the actual lung anatomical structure. By analyzing the adjacent relationship between the nodule and the watershed boundary, real-time, visually accurate positioning of the nodule can be achieved. This innovative technology provides a new solution for the intraoperative positioning and resection of lung nodules. This consensus was led by four major domestic societies, jointly with expert teams in related fields, oriented to clinical practical needs, referring to domestic and foreign guidelines and consensus, and finally formed after multiple rounds of consultation, discussion, and voting. The main content covers the theoretical basis of the "watershed topography map" technology, indications, operation procedures, surgical planning details, and postoperative evaluation standards, aiming to provide scientific guidance and exploration directions for clinical peers who are currently or plan to carry out lung nodule resection using the fluorescent microscope watershed analysis method.
