Objective:
To investigate the association between overweight, obesity, central obesity and hypertension, so as to provide the basis for formulating targeted hypertension prevention and control strategies.
Methods:
Permanent residents aged ≥18 years were selected in Wenzhou City, Zhejiang Province from June 2023 to August 2024 by a multistage cluster random sampling method. Data on demographic information, lifestyle, height, weight, waist circumference (WC), blood pressure, and blood biochemical indicators were collected through questionnaire surveys, physical examinations, and laboratory tests. The prevalence of hypertension was calculated and standardized using the data of the Sixth National Population Census in 2010. Body mass index (BMI) was calculated to determine overweight and obesity, while WC was used to identify central obesity. The association between overweight, obesity, central obesity and hypertension were analyzed using multivariable logistic regression models.
Results:
A total of 38 593 residents were surveyed, including 19 481 (50.48%) males and 19 112 (49.52%) females. The median age was 46.00 (interquartile range, 26.00) years. The rates of overweight, obesity, and central obesity were 32.74% (12 634 individuals), 10.27% (3 963 individuals), and 27.87% (10 755 individuals), respectively. There were 11 813 cases of hypertension, with a prevalence and standardized prevalence of 30.61% and 24.41%, respectively. Multivariable logistic regression analysis showed that after adjusting for demographic information, lifestyle, diabetes and dyslipidemia, the likelihood of hypertension in the overweight and obesity groups was 1.927 (95%CI: 1.815-2.045) times and 3.724 (95%CI: 3.404-4.073) times that of the normal BMI group, respectively. The likelihood of hypertension in the central obesity group was 2.346 (95%CI: 2.214-2.486) times that of the normal WC group. The likelihood of hypertension in the central obesity only, overweight only, overweight with central obesity, obesity only and obesity with central obesity groups was 1.586 (95%CI: 1.391-1.809), 1.704 (95%CI: 1.582-1.835), 2.433 (95%CI: 2.254-2.626), 1.768 (95%CI: 1.424-2.194), and 4.466 (95%CI: 4.053-4.921) times that of the normal BMI and WC group, respectively.
Conclusions
Overweight, obesity and central obesity were all associated with hypertension among adult residents. The highest likelihood of hypertension was observed among adult residents with both general obesity and central obesity.

Heart failure(HF)is the final stage of various cardiovascular diseases. The period of high incidence of death and readmission in patients with HF within 2 to 3 months after discharge is defined as the"vulnerable period".The management of vulnerable period focuses on how to identify high-risk groups with the need of close follow-up and more active intervention.Despite the application of various effective medical treatment and nursing strategies, readmissions in HF patients remain high, which not only poses a significant financial burden on the healthcare system but also carries the prognostic impact of worsening survival associated with repeated hospitalizations.Remote monitoring is a promising approach for early detection of worsening HF and intervention before apparent decompensation.Remote monitoring includes patient reporting of remote vital signs monitoring, wearable devices, information from cardiac implantable electronic devices and invasive remote hemodynamic monitoring.Remote cardiac monitoring devices have developed rapidly.A wide variety of monitoring devices have been developed, and trials have been conducted to explore the availability and safety of remote monitoring devices and their effectiveness in reducing the risk of HF re-hospitalization.Wearable devices are easy to be accepted by patients because of their non-invasive,portable,cost and other factors.

Breast cancer is the main cause of death from cancer in women. Early diagnosis and treatment are very important to improve patient prognosis. As an important means of breast cancer screening, ultrasound plays a key role in the clinical diagnosis and treatment of cancer, but the accuracy of ultrasound diagnosis is greatly affected by subjective and objective, and it is easy to miss diagnosis and misdiagnosis. In recent years, with the development of big data and computer algorithms, the research and application of artificial intelligence (AI) technologies such as imaging omics and deep learning in the field of ultrasound have become increasingly extensive, making accurate and efficient imaging evaluation possible. This article mainly reviews the application status and clinical application potential of AI in ultrasound diagnosis of breast cancer.

AIM:To investigate the mechanism by which muscle segment homeobox 2(Msx2)regulates the differentiation of outer enamel epithelial cells in the enamel organ.METHODS:Tissue paraffin sections were prepared and subjected to hematoxylin-eosin(HE)staining to analyze the effect of Msx2 deficiency on the differentiation status of epithelial cells in the enamel organ at the morphological level. At the ultrastructural level, alterations in cell structure were analyzed. The intermediate steps mediating cell differentiation were identified.Transcriptome sequencing analysis was performed to validate the molecular mechanisms underlying the observed phenomena.RESULTS:Msx2 deficiency was innovatively found to induce severe squamous epithelial hyperplasia in outer enamel epithelial cells of enamel organ, accompanied by dynamic restructuring of the cell cytoskeleton and alterations in cell adhesion at the ultrastructure level. As a transcriptional repressor, the loss of Msx2 expression results in significant increases(P<0.05 or P<0.01)in the mRNA expression levels of integrin β2(Itgβ2),ItgαM,Itgα4,Rac family small GTPase 2(Rac2),Rac/Cdc42 guanine nucleo-tide exchange factor 6(Arhgef6)and protein tyrosine phosphatase receptor type C(Ptprc).CONCLUSION:Msx2 regu-lates cytoskeleton structure and cell-cell interaction through the Rho GTPases signaling pathway, thereby influencing the differentiation state of outer enamel epithelial cells. This study reveals the mechanism through which Msx2 regulates the differentiation of outer enamel epithelial cells, providing a theoretical foundation for the prevention and treatment of enam-el-related clinical dental diseases.

Neuronal intranuclear inclusion disease(NIID)is a rare hereditary neurodegenerative disorder that can affect multiple systems. However,it is uncommon for urinary dysfunction to be the initial symptom. This article reports five cases. The five patients began to experience voiding dysfunction such as frequent urination, weak urination, and incomplete urination at the mean ages of 55.4(47 - 65)years old. Four months to twelve years after urinary onset, neurological symptoms such as headache, memory decline, transient loss of consciousness, and unsteady gait began to appear. Four of the five cases had a family history. Brain MRI revealed the “ribbon sign” or “crest sign” in all cases. Skin biopsy revealed eosinophilic inclusions in the cell nuclei, and NOTCH2NLC gene testing identified abnormal GGC mutations. Three of the five patients underwent cystostomy due to secondary hydronephrosis, while the other two received no special treatment. After a follow-up of 18 to 35 months since diagnosis, the patients who underwent cystostomy had normal renal function. Neurological symptoms in all five patients worsened to varying degrees.

Objective:To construct a comprehensive performance evaluation index system based on diagnosis related groups(DRG) and analyze the overall performance of a hospital before and after the implementation of DRG payment using an improved radar chart method. Methods:A literature analysis method was employed to preliminarily determine the performance evaluation framework in combination with the actual situation of the hospital. Subsequently, the Delphi method was used to establish a comprehensive performance evaluation index system based on DRG. Data related to inpatients from 2019 to 2022 (with 2019 being the period before the implementation of DRG payment) were extracted from the hospital information system and medical insurance system of a tertiary general hospital in Liaoning Province. The improved radar chart method was then utilized to conduct a comprehensive performance evaluation of the hospital. Results:The comprehensive performance evaluation index system based on DRG included five first-level indicators (service capacity, service efficiency, medical quality, medical safety, and revenue and costs) and 19 second-level indicators. The weights of medical quality and medical safety were the highest, both at 0.212. The evaluation results using the improved radar chart method showed that the comprehensive performance values of the hospital from 2019 to 2022 were 0.427 0, 0.463 3, 0.505 2, and 0.545 0, respectively. Overall, the five first-level indicators showed an improving trend, but the development of service capacity, service efficiency, medical quality, and revenue and costs was more balanced compared with medical safety. Conclusions:With the implementation of DRG payment reform, the performance of the hospital has improved and was trending towards more balanced development. The evaluation index system constructed in this study is scientifically sound and feasible. Combined with the improved radar chart method, it can more intuitively reflect changes in hospital performance and can serve as a reference for performance evaluation in public hospitals.
