1.Application and progress of dental pulp stem cells and their derivatives in dental pulp regeneration

BACKGROUND:Dental pulp stem cells,which derived from the dental pulp tissue,are one type of dental-derived mesenchymal stem cells,possess significant properties of self-renewal and multi-lineage differentiation.In recent years,series of researches focus on dental pulp stem cells and their derivatives such as extracellular vesicles,conditioned medium,and decellularized extracellular matrix,those have positive effects on the repair and regeneration of pulp tissue injury,showing an attractive clinical application.OBJECTIVE:To systematically review the researches and applications of dental pulp stem cells and their derivatives in dental pulp tissue engineering.METHODS:Literature searches were conducted in PubMed,China Biology Medicine(CBM),and China National Knowledge Infrastructure(CNKI)databases for articles published from January 2005 to June 2023.The search terms included"dental pulp stem cells,extracellular vesicles,exosomes,apoptotic bodies,conditioned medium,decellularized matrix,regeneration"in Chinese and English.After screening the titles and abstracts,duplicate and irrelevant studies were excluded.Finally,103 studies closely related to dental pulp regeneration were included for review and analysis.RESULTS AND CONCLUSION:Dental pulp stem cells and their derivatives are rich in lots of bioactive factors that can effectively promote odontogenic,angiogenic,and neurogenic differentiation,exhibiting significant potential in the formation of the pulp-dentin complex.However,the clinical translation of dental pulp stem cells and their derivatives still faces several challenges.Future researches should focus on optimizing preparation protocols,elucidating the underlying mechanisms of action,and refining safety assessments to provide novel therapeutic strategies for the repair of dental pulp injury.

3.Application research of nursing scheme based on self-regulation theory in patients with chronic pancreatitis

Objective:To explore the effect of nursing scheme based on self-regulation theory (SRT) on self-efficacy and quality of life of patients with chronic pancreatitis, and to provide basis for better promoting patients′ rehabilitation.Methods:Using a non-random historical control study, 55 patients with chronic pancreatitis admitted to The Fifth Affiliated Hospital of Southern Medical University from November 2021 to October 2022 were classified as the control group, and 55 patients with chronic pancreatitis admitted from November 2022 to October 2023 were classified as the research group. The control group received routine nursing intervention, while the research group received SRT-based nursing intervention. Both groups received a 3-month intervention. The General Self-Efficacy Scale, Exercise of Self-Care Agency Scale, Self Rated Abilities for Health Practices Scale, and Gastrointestinal Quality of Life Index were used to evaluate the self-efficacy, self-care ability, health behavior, and quality of life of two groups before and after the intervention, and statistical comparisons were made.Results:Both groups of patients completed the study. There were 28 males and 27 females in the control group. The age was (45.33 ± 12.26) years old. There were 30 males and 25 females in the research group. The age was (45.48 ± 12.61) years old. After the intervention, the score of the self-efficacy in the research group was (29.32 ± 2.88) points, which was higher than that in the control group (19.28 ± 1.95) points, the difference was significant ( t=21.41, P<0.05). The scores of self-care skills, self-responsibility, self-concept, health knowledge and the total score of the self-care ability in the research group were (35.32 ± 2.31), (22.85 ± 1.42), (25.88 ± 1.93), (64.32 ± 5.22), (148.34 ± 8.33) points respectively. They were all higher than those of the control group (28.66 ± 1.77), (17.22 ± 1.23), (18.67 ± 1.86), (60.08 ± 4.52), (124.56 ± 8.92) points. The differences were significant ( t values were 4.55-22.23, all P<0.05). The scores of exercise, nutrition, health responsibility, psychological comfort and the total score of the health behavior in the research group were (25.58 ± 1.41), (25.02 ± 2.03), (24.31 ± 2.34), (25.16 ± 1.63), (100.05 ± 7.36) points respectively. They were all higher than those of the control group (22.31 ± 2.03), (20.15 ± 2.82), (20.16 ± 2.89), (21.15 ± 2.17), (83.79 ± 7.84) points. The differences were significant ( t values were 8.28-11.21, all P<0.05). The scores of social activity, psychological state, physiological function, self-conscious symptoms and the total score of the quality of life in the research group were (16.78 ± 1.26), (17.03 ± 1.85), (22.43 ± 1.96), (17.09 ± 0.88), (73.41 ± 4.22) points respectively. They were all higher than those of the control group (15.32 ± 1.15), (15.34 ± 1.76), (20.86 ± 1.59), (16.03 ± 0.75), (67.52 ± 4.18) points. The differences were significant ( t values were 4.61-7.35, all P<0.05). Conclusions:SRT-based nursing scheme can effectively improve the self-efficacy and self-care ability of patients with chronic pancreatitis, and improve their health behavior and quality of life.
4.Preparation of vancomycin hydrochloride-loaded polydopamine nanoparticles and their antimicrobial effect in combination with photothermal therapy

OBJECTIVE To prepare vancomycin hydrochloride(VH)-loaded polydopamine(PDA)nanoparticles(VH@PDA nanoparticles),and study their antimicrobial effect in combination with photothermal therapy.METHODS Using PDA as the carrier,VH was loaded to prepare VH@PDA nanoparticles(PDA nanoparticles were prepared using the same method).The nanoparticles were characterized with laser particle size analyzer,transmission electron microscope,and UV visible absorption spectrometer;the drug loading capacity and encapsulation efficiency of the nanoparticles were determined.The near-infrared laser irradiation was adopted to determine their photothermal ability.Taking Staphylococcus aureus as the research object,the bactericidal properties of the nanoparticles in combination with photothermal therapy in vitro were clarified through plate colony coating method,live/dead staining analysis,crystal violet staining.Using L929 cells as the research object,the effects of VH@PDA nanoparticles at different mass concentrations(0,3.125,6.25,12.5,25,50 and 100 μg/mL)on cell viability were investigated to assess their biocompatibility.RESULTS VH@PDA nanoparticles were successfully loaded with VH,exhibiting uniform particle size(approximately 300 nm),distinct pore size,and a spherical structure.The drug loading capacity was 11.34%,the encapsulation efficiency was 32.00%,the photothermal conversion efficiency reached 23.55%,and they demonstrated stable photothermal performance.The antibacterial effect results of the combined photothermal therapy demonstrated that without near-infrared laser irradiation,the antibacterial effects of VH,PDA nanoparticles,and VH@PDA nanoparticles were not significant.However,when subjected to near-infrared laser irradiation,VH@PDA nanoparticles exhibited a pronounced antibacterial effect.The results of the cell experiments revealed that after treatment with VH@PDA nanoparticles at various mass concentrations,the cell viability rates remained above 80%.CONCLUSIONS VH@PDA nanoparticles are successfully prepared,which exhibit stable photothermal properties,significant antibacterial effects when combined with photothermal therapy,and good biocompatibility.
5.Effect of the DrCloudme platform on follow-ups of patients with intracranial aneurysms

Objective To evaluate the efficacy of the DrCloudme platform in follow-ups of intracranial aneurysm(IA)patients.Methods A randomized controlled trial was conducted with 120 IA patients treated at Hechi People's Hospital from July 2021 to June 2024.Participants were equally allocated via random number table method into two groups:The control group(n=60)received conventional telephone follow-up post interventional embolization,while the intervention group(n=60)uti-lized the DrCloudme platform for digital follow-up management.Outcome measures included medical compliance,social functio-ning(assessed by Social Disability Screening Schedule,SDSS),quality of life(evaluated using Quality of Life Instrument for Head and Neck Cancer,QLICP-HN),follow-up completion rates,and patient satisfaction.Results The observation group dem-onstrated significantly higher medical compliance,follow-up completion rate,and follow-up patient satisfaction compared to the control group(P<0.05).Additionally,the observation group had lower SDSS scores and higher QLICP-HN scores,indicating better social function and quality of life(P<0.05).Conclusion The DrCloudme platform can not only improve the follow-up completion rate for healthcare providers,but also enhance medical compliance,social functioning,and quality of life among dis-charged IA patients.Moreover,it significantly boosts patient satisfaction with follow-ups.
6.Development and Initial Use of a New Inflammatory Bowel Disease Clinical Database Integrating Both Eastern and Western Clinical Characteristics

Background::The increasing incidence of inflammatory bowel disease (IBD) presents significant medical and societal challenges. A well-designed IBD database is crucial for both epidemiological studies and clinical management. However, inconsistencies between regional databases hinder cross-institutional and international research, especially between Eastern and Western societies.Methods::We developed a new IBD database, the 301 IBD database, integrating the IBD clinical characteristics from the Penn IBD database (USA) and the latest IBD guidelines and consensus and clinical practices of the Chinese PLA General Hospital (PLAGH). We applied this database to analyze clinical data of IBD inpatients at PLAGH from 2008 to 2023.Results::The 301 IBD database contains 490 items in 6 sections including demographic characteristics, personal history, clinical phenotype, disease activity, laboratory tests and examinations, and treatment. Features of the 301 IBD database include inpatient focus, biochemical indicators and opportunistic infection focus, and more about ulcerative colitis (UC)-associated complications. Single-center analysis revealed an increasing hospitalization trend, from 2.35% in 2008 to 3.94% in 2023. We found that the clinical characteristics of our UC inpatients are predominantly male (62.5%), extensive lesions (55.1%), low usage of biologics (4.1%), and a high incidence of UC-CRC (3.0%). The clinical characteristics of CD inpatients included male predominance (68.39%), early onset age (35.43 ± 14.75-year-old), and high rate of surgery (25.81%).Conclusion::The 301 IBD database, integrating Eastern and Western clinical data, provides a valuable tool for IBD clinical research. Future international, multicenter collaborations are expected to further enhance its utility.
7.Construction method and application of a cloud-based platform for full process closed-loop quality control management of healthcare-associated infection based on multi-source data fusion

Objective To construct a cloud-based platform for healthcare-associated infection(HAI)management based on multi-source data fusion and data visualization,and evaluate its application effectiveness.Methods A ter-tiary first-class cancer hospital was selected as the research object.Aligned with graded hospital accreditation stan-dards and based on grid-based management and responsibility zone mode,a platform was constructed in 2023 with low-code technology,multi-source data fusion,and visualization function.The self-comparison method was adopted to compare occurrence of HAI before and after the operation of the platform.A questionnaire survey was adopted to assess the experience and workload reduction feelings of full-time and part-time infection surveillance and control profe-ssionals using the platform.Results 81.56%of the surveyed respondents believed that the platform could re-duce the workload of infection surveillance and control.Compared with before the trial operation,the hospital achieved an annual reduction of 11 200 yuan in paper costs,with associated labor savings of approximately 4 482.5 hours.The incidence of HAI cases in the whole hospital decreased from 0.67%to 0.45%.The pathogen detection rate before therapeutic use of antimicrobial agents increased from 51.26%to 71.54%.Differences were both statis-tically significant(both P<0.05).The detection number and proportion of carbapenem-resistant Pseudomonas aeruginosa(CRPA)and extended-spectrum β-lactamase-producing Klebsiella pneumoniae(ESBL-KP)in HAI cases decreased(54.55%vs 36.47%and 51.14%vs 32.50%,respectively,both P<0.05).Conclusion The construc-tion and application of smart HAI management cloud-based platform can reduce cost,improve management quality,and provide a theoretical basis and technical paradigm for the construction of smart HAI management system.

9.Prognostic value of PNI combined with serum PRX3 in patients with advanced high-grade serous ovarian cancer

Objective:To explore the predictive value of prognostic nutritional index (PNI) combined with serum peroxiredoxin 3 (PRX3) level for the prognosis of patients with advanced high-grade serous ovarian cancer (HGSOC) .Methods:A total of 103 patients with advanced HGSOC admitted to Affiliated Hospital of Jining Medical University from Jan. 2020 to Dec. 2022 were retrospectively selected and included in the study. Serum albumin (Alb), PRX3 levels and peripheral blood lymphocyte count (LY) levels were measured. The patients were followed up for 2 years, and they were divided into good prognosis group (70 cases) and poor prognosis group (33 cases). Multivariate stepwise Logistic regression analysis was used to analyze the prognostic factors. The receiver operating characteristic (ROC) curve was used to analyze the predictive efficacy of PNI combined with PRX3 on prognosis.Results:The age, proportion of postoperative residual lesions, proportion of lymph node metastasis, proportion of stage Ⅲ, and PRX3 level in the poor prognosis group were higher than those in the good prognosis group, and the PNI level was lower than that in the good prognosis group ( P<0.05). Multivariate Logistic regression analysis showed that postoperative residual lesions ( OR=2.731, 95% CI: 1.562-4.774), clinical stage ( OR=3.526, 95% CI: 1.503-8.271), PNI level ( OR=0.148, 95% CI: 1.503-8.271) 0.057-0.387) and PRX3 level ( OR=4.011, 95% CI: 1.972-8.154) were independent influencing factors for the prognosis of patients ( P<0.05). ROC curve analysis showed that the area under the curve (AUC) of PNI, PRX3 and their combination for predicting the prognosis of advanced HGSOC patients were 0.807, 0.781 and 0.874, respectively ( P<0.05). The survival curve of patients with high PNI and PRX3 levels was different from that of patients with low PNI and PRX3 levels ( P<0.05) . Conclusion:PNI and PRX3 are independent prognostic factors for advanced HGSOC patients, and the combination of PNI and PRX3 has a better prognostic value in patients with advanced HGSOC.
10.Analysis of clinical effect of cervical cerclage in twin pregnancies with cervical length ≤15 mm at different gestational ages

Objective:To investigate the clinical effect of cervical cerclage at different gestational weeks in twin pregnancy with cervical length (CL)≤15 mm.Methods:This was a retrospective cohort study. Eighty-three twin pregnant women with CL≤15 mm detected by transvaginal ultrasound at 16-27 +6 weeks of gestation in Fujian Maternity and Child Health Hospital from January 2017 to December 2023 were enrolled. According to different treatment methods, they were divided into cervical cerclage group (47 cases) and conservative treatment group (36 cases), and stratified according to the gestational age of CL≤15 mm diagnosis (<26 weeks, ≥26 weeks). The differences in pregnancy outcomes between the two groups were compared. Multivariate logistic regression was used to analyze the effect of cervical cerclage on preterm birth in twin pregnant women with different CL≤15 mm diagnosis weeks. Results:(1) The gestational age at delivery and prolonged gestational age in the cervical cerclage group were longer than those in the conservative treatment group (median gestational age at delivery: 35.3 vs 33.0 weeks; median prolonged gestational age: 10.4 vs 7.2 weeks), and preterm birth rates before 34, 32 and 28 weeks were lower than those in the conservative treatment group [34 weeks: 23% (11/47) vs 53% (19/36); 32 weeks: 15% (7/47) vs 39% (14/36); 28 weeks: 4% (2/47) vs 25% (9/36)], the differences were statistically significant (all P<0.05). There were no significant differences in the rates of preterm birth before 37 weeks of gestation and preterm premature rupture of membranes between the two groups (all P>0.05). (2) When gestational age at CL≤15 mm diagnosis was <26 weeks, pregnancy outcomes in the cervical cerclage group were better than those in the conservative treatment group, including gestational age at delivery (median: 35.4 vs 31.3 weeks) and prolonged gestational age (median: 11.1 vs 5.6 weeks), neonatal birth weight [(2 246±519) vs (1 594±691) g], incidence of adverse neonatal outcomes [19% (13/68) vs 56% (19/34)], and proportion of live births [100% (68/68) vs 82% (28/34)], respectively; the differences were statistically significant (all P<0.05). When the gestational age of CL≤15 mm diagnosis was ≥26 weeks, there were no statistically significant difference in pregnancy outcomes between the two groups (all P>0.05). (3) Multivariate logistic regression analysis showed that when the gestational age of CL≤15 mm diagnosis was <26 weeks, cervical cerclage reduced the risk of preterm birth before 34 weeks (a OR=0.10, 95% CI: 0.02-0.51; P<0.05), 32 weeks (a OR=0.11, 95% CI: 0.02-0.58; P<0.05) and 28 weeks (a OR=0.04, 95% CI: 0.01-0.65; P<0.05). When the gestational age of CL≤15 mm diagnosis was ≥26 weeks, cervical cerclage did not reduce the risk of preterm birth before 34, 32 and 28 weeks of gestation (all P>0.05). Conclusions:Cervical cerclage might reduce the risk of preterm birth and improve neonatal outcomes in twin pregnancies with CL≤15 mm before 26 weeks. However, cerclage showed no advantage over conservative treatment in twin pregnancies with CL≤15 mm over 26 weeks.
