Objective:To establish a set of reasonable, reliable and easy-to-operate evaluation index system for the clinical application of ventilators,so as to carry out all-round clinical evaluation for application of ventilators. Methods:A research group on the evaluation index system for the clinical application of ventilators was formed by 5 clinical engineers, who came from the Department of Medical Engineering of the First Affiliated Hospital of Zhejiang University School of Medicine,to form a primary evaluation index system for the clinical application of ventilators through literature review and internal discussion, and invited 14 experts from different provinces and cities engaged in respiratory therapy or ventilator maintenance management, after two rounds of online discussion and optimization, the importance scores of each evaluation index were obtained by the Delphi method, and the relative importance of each evaluation index was calculated by analytic hierarchy process. An evaluation index system for the clinical application of ventilators was established from three dimensions:technical performance, clinical effect and after-sales service. The ventilators of brand A and brand B used in the hospital during the same period were selected, and the clinical application of the two different brands of ventilators was compared and evaluated by using the evaluation index system. Results:The constructed evaluation index system for clinical application of ventilator included 3 first-level indicators(technical performance, clinical effect and after-sale service),and 8 second-level indicators and 35 third-level indicators. The combined weight of indicators showed that the ratio of importance of"technical performance"of the first-level indicators accounted for more than half, and the relatively important indicators in the second-level indicators were successively design for safety, static performance, reliability, service quality and applicability. The comprehensive average score of ventilator A was 4.468 points, and that of ventilator B was 4.117 points. The overall performance of ventilator A was better than that of ventilator B in the evaluation of this round, which was consistent with the actually clinical application of the two ventilators. Conclusion:The evaluation index system for clinical application of ventilator can conduct comprehensive evaluation for the effect of clinical application of various kinds of ventilators, and clarify the advantages and disadvantages of various kinds of ventilators, and provide reference basis for medical institutions in selecting ventilator's brands, and for manufacturers in improving product's performance.

Cloud technology and the Internet of things ( IOT) technology can be used in building the patient-centered critical care medical information platform. The platform could inenable medical staff to efficiently obtain critical care medical information, and raise critical patients′ cure rates. The authors introduced the critical process of rapidly building and improving the urgent care medical information integration platform in a campus of the First Affiliated Hospital of Zhejiang University Medical School. The IOT platform could effectively interconnect a variety of critical care medical equipments, and features such functions as vital signs monitoring and life support, bedside treatment data retrieval, remote diagnosis and treatment, and cloud sharing. The platform featured the advantages of complete information integration, fast construction, and satisfactory system extensibility. It could improve the medical work efficiency, improve the effect of critical care, and reduce the frequency of contact with patients, hence providing references for rapidly establishing a vital care treatment platform for public health events.

Objective To evaluate the clinical application of superselective renal artery embolization in the treatment of iatrogenic renal hemorrhage. Methods The iatrogenic renal hemorrhage in fifteen patients was first demonstrated by renal arterigraphy to diagnose the rupture site and degree of injury. And then they were treated by percutaneous catheterized superselective renal arterial embolization through guidance of guide wire. Results In all 15 patients, the occlusive ruption of bleeding arteries and disappearance of extravasation staining were found after the superselective catheterized renal arterial embolization. Clinically, the bloody urine turned clear without macroscopic hematuria during 1-3 days in all patients after the procedure. Conclusions Superseclective renal arterial embolization treatment signified a less invasion, less complications, good hemostatic efficacy, and maximal preservation of renal tissue and function for iatrogenic renal hemorrhage. The treatment is worthy to be recommended.

Objective To study the efficacy and safety of transurethral plasmakinetic resection of prostate (PKRP) in treatment of benign prostate hyperplasia in elderly patients aged 80 years and over. Methods Retrospective clinical analysis of 180 case of high risk of benign prostate hyperplasia treated by PKRP in patients aged 80 years and over. Results All the 180 patients underwent the operation successfully. The average time for operation was (45.5±23.3) min and the resected prostate was in an average of (60.3±23.3) g. Neither of blood transfusion during the operation nor aductor reflex and transurethral resection syndrome occurred. International prostate symptom score, residual urine and quality of life decreased from (29.5±5.3) to (10.2±2.8),from (130.5±45.5) ml to (13.5±7.1)ml and from (7.2±1.1) to (1.0±0.5) respectively. The maximum flow rate elevated from (6.2±1.8) ml/s to (24.5±3.1) ml/s. The differences in the above indicators were statistically significant between pre- and post- operation (P<0.05). Conclusions Transurethral plasmakinetic prostatectomy is a reliable and effective surgical method, especially for the aged patients with benign prostate hyperplasia.
