1.Epidemiological characteristics of influenza in Yuhang District

Objective:
To investigate the epidemiological characteristics of influenza in Yuhang District, Hangzhou City from 2019 to 2023, so as to provide the reference for formulating influenza prevention and control measures.
Methods:
Influenza case data with current address in Yuhang District was collected through the Chinese Disease Prevention and Control Information System from 2019 to 2023. Influenza-like illness data was collected through the Hangzhou Epidemiological Investigation System. Time distribution, population distribution and pathogen detection of influenza-like illness were descriptively analyzed.
Results:
There were 118 319 influenza cases reported in Yuhang District from 2019 to 2023, with an average annual reported incidence rate of 2 316.80/105. The highest incidence rate was seen in 2023, with a reported incidence rate of 5 736.82/105. The peak incidence of influenza occurred from November to March of the following year, presenting the winter and spring epidemic. The cases were mainly distributed in the age groups of 7 to 14 years and 25 to 59 years, with 31 310 and 34 470 cases, accounting for 26.46% and 29.13%, respectively. Influenza cases were reported in all 12 towns (streets) in Yuhang District, with the top two being Wuchang Street and Liangzhu Street, with the average annual incidence rates of 17 346.08/105 and 14 945.80/105, respectively. From 2019 to 2023, there were 103 868 cases of influenza-like illness, and 1 482 throat swab samples were collected. Among them, 260 positive samples of influenza virus were detected, with a positive rate of 17.54%. The peak detection period for positive specimens was from November to March of the following year, with 222 influenza virus positive specimens detected. The influenza virus types included H1N1, H3N2, and Victoria B, accounting for 25.77%, 38.85% and 35.38%, respectively.
Conclusions
The peak of influenza outbreak in Yuhang District from 2019 to 2023 was in winter and spring, with children and adolescents being the main affected objectives. H1N1, H3N2 and Victoria B were alternately prevalent. Prevention and control measures such as influenza vaccination should be strengthened.
2.Research on the detection algorithm of electrocardiogram characteristic wave based on energy segmentation and stationary wavelet transform.

The detection of electrocardiogram (ECG) characteristic wave is the basis of cardiovascular disease analysis and heart rate variability analysis. In order to solve the problems of low detection accuracy and poor real-time performance of ECG signal in the state of motion, this paper proposes a detection algorithm based on segmentation energy and stationary wavelet transform (SWT). Firstly, the energy of ECG signal is calculated by segmenting, and the energy candidate peak is obtained after moving average to detect QRS complex. Secondly, the QRS amplitude is set to zero and the fifth component of SWT is used to locate P wave and T wave. The experimental results show that compared with other algorithms, the algorithm in this paper has high accuracy in detecting QRS complex in different motion states. It only takes 0.22 s to detect QSR complex of a 30-minute ECG record, and the real-time performance is improved obviously. On the basis of QRS complex detection, the accuracy of P wave and T wave detection is higher than 95%. The results show that this method can improve the efficiency of ECG signal detection, and provide a new method for real-time ECG signal classification and cardiovascular disease diagnosis.

Neoadjuvant chemotherapy plays an important role in the multimodality therapies of breast cancer. After neoadjuvant chemotherapy, some patients with breast cancer were down-staged from positive axillary lymph node (cN+ ) to clinically negative (cN0). For these patients, the feasibility and safety of sentinel lymph node biopsy remains controversial. However, with the application of new technologies, SLNB is expected to become the main treatment for breast cancer patients with stage cN0 after neoadjuvant chemotherapy.

6.Study on Nasal Mucosa Absorption of Paeonol of Xingbi Microemulsion-based Thermo- sensitive Gel in Rats

Objective To study the nasal mucosa absorption of paeonol ofXingbi microemulsion-based thermo-sensitive gel in rats; To evaluate the nasal mucosa absorption of medicine and the rationality of preparation form.Methods According to the nasal administration method, the blood medicine concentration of paeonol was determined at different time points.Results The nasal administration bioavailability of paeonol in Xingbi microemulsion-based thermo-sensitive gel was 78.68%, the plasma concentration reached the maximum (0.3404 μg/mL) after 3 min of administration, and the average residence time was 9.23 h. After reaching Cmax, The plasma concentration changed similar to intravenous administration of paeonol. The concentration data of paeonol in plasma was consistent with a two-compartment model, with a weight of 1.Conclusion Xingbi microemulsion-based thermo- sensitive gel has a higher bioavailability, and the preparation form was reasonable.
7.Study on Quality Standards and Vitro Dissolution of Gualou Guizhi Dropping Pills

Objective To establish the quality standards of Gualou Guizhi Dropping Pills; To investigate the vitro dissolution. Methods HPLC was used to determine the contents of paeoniflorin, albiflorin, cinnamic acid, 6-gingerol, liquiritin, glycyrrhizic acid and liquiritigenin in the dropping pills. The vitro dissolution rate of dropping pills was determined by rotating basket method. Results The calibration curves of paeoniflorin and albiflorin were in a good linearity in the range of 0.690–6.900 μg, 0.300–2.996 μg, respectively, and the average recoveries were 101.12% and 98.52%, respectively, with RSD of 2.24%, 1.37%, respectively. Cinnamic acid was linear in the range of 0.023–0.348 μg and the average recovery was 98.21% with RSD of 2.00%. 6-Gingerol was linear in the range of 0.025–0.382 μg, and the average recovery was 99.19% with RSD of 2.18%. The calibration curves of liquiritin, glycyrrhizic acid and liquiritigenin were in a good linearity in the ranges of 0.120–2.498 μg, 0.150–2.253 μg, and 0.010–0.147 μg, respectively, and the average recoveries were 99.80%, 100.38%, and 100.62%, respectively with RSD of 2.10%, 1.91%, 1.66%, respectively. The accumulated dissolution rates of seven kinds of elements in the dropping pills all reached more than 98% in 15 min. Conclusion The method is simple and accurate, with repeatability, which can be applied to the quality control of Gualou Guizhi Dropping Pills. The vitro dissolution rates meet relevant standards.
8.Clinical research on the expression of X chromosome coupled ZFX expression in the serum and pathology of NSCLC patients

Objective To discuss the expression of X chromosome coupled zinc finger protein(ZFX) in the serum and pathology of patients with advanced non-small cell lung cancer before and after treatment and its evaluation in the chemotherapy efficacy.Methods Forty cases(NSCLC group)with non-small cell lung cancer treated in Baotou Central Hospital from January 2013 to October 2014 were retrospectively analyzed.The control group included 40 normal people who may have tumor by normal physical examination.Based on the blood tests of research subjects before and after treatment,the peripheral blood ZFX content was detected by the quantitative detection, ZFX expression was detected by tissue morphological identification and immunohistochemical methods.Results The level of adenocarcinoma ZFX serum was(15.32± 2.01)μg/L, squamous cell carcinomas ZFX serum level was(11.65±4.12)μg/L,the difference between the two groups was statistically significant(t=3.216,P<0.05); the ZFX serum level of non-small cell lung cancer group was (17.55±0.37)μg/L before treatment,and was(6.35± 0.06)μg/L after treatment which was significantly lower,the difference was statistically significant(t=188.97,P<0.05); the serum level of non-small cell lung cancer group before treatment was(17.55±0.37)μg/L,after treatment was(6.35±0.06)μg/L,compared with (2.29± 0.01)μg/L,(2.29 ± 0.01)μg/L in the control group,the differences were statistically significant (before treatment:t=260.75,after treatment t=422.14,P<0.05); the expression of ZFX in adenocarcinoma was(15.32±2.01)ug / L,higher than that of squamous cell carcinoma((11.65±4.12)μg/L),the difference was statistically significant(t=3.216,P<0.05);the expression of ZFX in CR+PR group before treatment was (17.35±0.46)μg/L,higher than that after treatment((6.24±0.11)μg/L),the difference was statistically significant(t=142.88,P<0.05).Conclusion The expression of ZFX in peripheral blood serum and pathology may be a marker for the diagnosis of non-small cell lung cancer,and it has guiding significance for the diagnosis and curative effect evaluation of lung cancer.
9.The effect of hyperthermia on spatial working memory:an fMRI study

Objective To explore the changes of different brain regions of the human beings during completion of spatial working memory tasks under hyperthermia condition with functional magnetic yesonance.Methods Twenty-eight right-handed healthy young male volunteers were divided into control group and hyperthermia group randomly,and working memory task using block design was performed during behavioral test and fMRI scanning on both conditions.Activation intensity and location of different brain areas were compared in control and hyperthermia group.Results The activations of frontal,parietal,occipital and insular lobe were seen in both of the control group and the hyperthermia group.Right lateralization was shown in control group during spatial working memory (Li=0.05 1).While,in hyperthermia group the effect of the right lateralization significantly was enhanced (Li=0.103). Moreover,activation of bilateral middle frontal gyrus and right parietal lobe were significantly enhanced.Conclusion It is found for the first time that the hyperthermia could affect bilateral middle frontal gyrus and right parietal lobe,and enhance the effect of right lateralization in the spatial working memory task.
10.Effect and clinical significance of immunological state upon telomerase activation of bone marrow mononuclear cells in hemopoietic microenvironment of immune related hematocytopenia (IRHS)

Objective:To study the effect of immunological molecules expressive state upon the telomerase activation ( TA) of bone marrow mononuclear cells ( BMMNC ) in the hemopoietic microenvironment of patients with immune related hematocytopenia ( IRHS) ,and to explore the immunologic mechanism as well as the clinical significance of hematoclasis in marrow of IRHS patients .Methods:①TRAP-PCR-ELISA method was performed to detect the TA of BMMNC in marrow of 366 IRHS patients before and after therapy.②The molecules HLA-DR,anti-hunman IgG,FcγⅡR,mannose receptor ( MR),IL-17A and its receptor ( IL-17AR) were analysed by immunochemistry and immunofluorescence staining .③The flow cytometric ( FCM) was used to analyse the proportion of CD3+CD4+T cells as well as CD3+CD8+T cells ,CD3-CD19+B cells and CD3-CD16/56+NK cells in peripheral blood lymphocyte.60 cases of health examination were selected as the control group , and 30 cases hypoferric anemia patients were selected as disease control.The differences between patient group and control group were analysed with statistic method .Immunochemistry and immunofluo-rescence staining were performed to in situ analyze the activation-characteristics of immunocyte in bone marrow slides of IRHS ,and the dependablity of cellular immunologic injury was also checked.Results: ①The levels of TA was 0.261 7 ±0.021 6 before treatment , higher than the disease control group (0.061 6±0.031 3 ,P<0.01).Among of them HLA-B27+patients were higher than HLA-B27-patients (0.301 3±0.020 6 vs.0.192 3±0.012 9,P<0.05).Serious IRP patients with HLA-B27+IgG+were obviously higher than HLA-B27-IgG+patients (0.401 6±0.017 2 vs.0.221 1±0.011 0,P<0.01).②In marrow of HLA-B27+IgG+patients,both cell immunity and humoral immunity were in disorder in the hemopoietic microenvironment ,and immonocyte in marrow expressed HLA-DR, FcγⅡR,IL-17A,IL-17RA and MR,and Th,Ts,B cell and NK cell in peripheral blood increased in different degree ,inducing the in-flammation of haemocyte and lead to destruction.③Humoral immunity was in the dominant level in morrow;humoral immunity of HLA-B27-IgG+patients,immonocyte expressed FcγⅡR in high level,but IL-17A was seldom expressed,only CD19+B cell was increased slightly ,the antibody dependent cellular cytotoxicity ( ADCC) was the main mode of destruction.After therapy glucocorticoids associated with ciclosporin A ,the TA level of BMMNC decreased to 0 with devitalization.Conclusion: The telomerase activation of bone marrow mononuclear cells in IRHS is related with the immune state of hemopoietic microenvironment and the pathologic lesion degree of hema -topoietic cell in marrow.It is viral infection and immunological activation as well as a variety of inflammatory factors play a part in the immunologic injury that might be an important factor of the enhancement in TA.
