1.Semantic segmentation for segmenting brain infarct lesions on diffusion weighted imaging

Objective To observe the effectiveness of semantic segmentation for segmenting brain infarct lesions on diffusion weighted imaging(DWI).Methods DWI data of 675 patients with newly occurred stroke were retrospectively analyzed.Taken manually depicted ROI of brain infarct lesions as gold standards,the effectiveness of semantic segmentation,threshold segmentation and local entropy information segmentation for segmenting brain infarct lesions on DWI were evaluated with Dice similarity coefficient(DSC)under 10-fold cross-validation and the area under the curve(AUC)of receiver operating characteristic(ROC)curve.Results DSC of semantic segmentation,threshold segmentation and local entropy information segmentation was 0.822,0.647 and 0.728,with AUC of 0.905,0.778 and 0.849,respectively.Conclusion Semantic segmentation had curtain clinical application value for segmenting brain infarct lesions on DWI.

3.Mechanism study of impaired homocysteine transsulfuration metabolism in sepsis

Objective:To investigate the clinical significance and possible mechanisms of elevated homocysteine(Hcy) levels in peripheral blood of children with sepsis.Methods:The clinical data of 51 children with sepsis (sepsis group) admitted to PICU at Xuzhou Children′s Hospital from January 2019 to December 2019 were analyzed, and the levels of Hcy in plasma were compared with 50 non-septic children (common infection group) and 50 healthy children (healthy control group) during the same period.The possible mechanism of metabolic disorders about Hcy was analyzed by detecting the levels of the key rate-limiting enzymes cystathionine-β-synthase(CBS) and cystathionine-γ-lyase(CSE), which were in the downstream of metabolism in septic mouse model induced by lipopolysaccharide.Results:The level of Hcy in plasma was (12.62±5.46)μmol/L in sepsis group, which was significantly higher than those in common infection group[(9.42±2.28) μmol/L] and healthy control group[(8.14±1.60) μmol/L]( P<0.05). The level of Hcy in plasma of 12 children with acute kidney injury in sepsis group was significantly higher than that of 39 children without acute kidney injury in sepsis group[(16.48±5.87)μmol/L vs.(11.62±4.74) μmol/L, P<0.05]. The level of Hcy in plasma of six children with acute liver failure in sepsis group was significant higher than that of 45 children without acute liver failure in sepsis group[(18.35±7.10) μmol/L vs.(11.84±4.78) μmol/L, P<0.05]. The level of Hcy in serum significantly increased in septic mouse models ( P<0.01). The transcription and protein expression levels of key rate-limiting Hcy transcription enzymes CBS and CSE in liver and kidney tissues of septic mouse were significantly down-regulated ( P<0.05). Conclusion:The level of Hcy in peripheral blood of children with sepsis increases, which is more obviously in children with acute kidney injury or acute liver injury.When patients developed sepsis, the expression of CBS and CSE will be restrained, leading to disorders related to transsulfuration metabolism and elevated level of Hcy in peripheral blood.
4.Perioperative management of splenic abscess with Mediterranean anemia complicated by portal vein thrombosis

The authors summarize the perioperative management provided to a splenic abscess patient with Mediterranean anemia complicated by portal vein thrombosis. Key aspects of perioperative management include multidisciplinary comprehensive management of the patient, implementation of a bundled infection control program, personalized thrombosis management, attention to the patient′s psychological well-being, provision of continuous care, and assistance with home management. Following a series of treatments and perioperative management, the patient′s condi-tion was improved, and discharged after 42 days of hospitalization.
5.Application of clinical nursing pathway based on enhanced recovery after surgery concept in perioperative nursing of gastric cancer in the elderly

Objective:To explore the application effect of clinical nursing pathway based on enhanced recovery after surgery (ERAS) concept in perioperative nursing of gastric cancer in the elderly, so as to provide basis for the selection of clinical nursing plan.Methods:A total of 92 patients with gastric cancer who underwent surgical treatment in Hefei Second People′s Hospital from January 2018 to July 2020 were enrolled. They were divided into the observation group and the control group by random number table method, 46 cases in each group. The control group was given routine nursing intervention, while the observation group was given clinical nursing pathway based on ERAS concept for intervention. The postoperative recovery, improvement of nutritional indexes, postoperative complications were compared between the two groups.Results:The first bowel sound time, first exhaust time, first defecation time, leaving bed time, indwelling time of urinary catheter, and postoperative hospitalization time were (18.43 ± 1.80) h, (43.67 ± 8.79) h, (53.06 ± 5.18) h, (22.65 ± 4.95) h, (29.08 ± 2.69) h, (13.93 ± 2.19) d in the observation group, while (22.96 ± 2.24) h, (47.98 ± 8.46) h, (57.97 ± 5.65) h, (31.30 ± 5.73) h, (35.40 ± 3.66) h, (15.48 ± 2.40) d in the control group, there were significant differences between the two groups ( t values were 2.40-10.69, all P<0.05). There was no significant difference in the nutritional indexes before intervention between the two groups( P>0.05). After intervention, the levels of serum total protein, serum albumin and hemoglobin were (64.43 ± 6.22), (35.43 ± 3.07), (125.88 ± 4.75) g/L in the observation group, while (55.97 ± 5.25), (29.96 ± 2.64), (120.05 ± 5.07) g/L in the control group, there were significant differences between the two groups ( t=7.05, 9.16, 5.69, all P<0.05). The incidence of postoperative complications was 4.35%(2/46) in the observation group, 19.57%(9/46) in the control group, there was significant difference between the two groups ( χ2=5.06, P<0.05). Conclusions:The clinical nursing pathway based on ERAS concept can effectively ensure the nutrition needed by patients with gastric cancer during postoperative rehabilitation, accelerate rehabilitation process, and reduce the incidence of postoperative complications.
6.Spatial distribution characteristics of severe fever with thrombocytopenia syndrome in Yantai City of Shandong Province from 2015 to 2020

Objective:To analyze the spatial distribution characteristics and spatial aggregation of the epidemic of severe fever with thrombocytopenia syndrome(SFTS) in Yantai City of Shandong Province, and to provide basis for formulating effective SFTS prevention and control measures.Methods:The epidemic data of SFTS confirmed cases in each township (street) in Yantai City, Shandong Province from 2015 to 2020 were collected from the "China Disease Prevention and Control Information System Infectious Disease Monitoring and Reporting System", and ArcGIS 10.2 software was used for spatial autocorrelation analysis.Results:From 2015 to 2020, a total of 839 SFTS cases were reported in Yantai City, including 124 deaths; with an average annual incidence rate of 2.14/100 000, and a total case fatality rate of 14.78%. Global spatial autocorrelation analysis showed that the distribution of SFTS cases in Yantai City from 2015 to 2020 showed a positive spatial correlation, with the highest spatial correlation in 2015 (Moran's I = 0.25, Z = 5.66, P < 0.001), and the lowest in 2018 (Moran's I = 0.16, Z = 3.69, P < 0.001). Local spatial autocorrelation and hotspot analysis showed that the epidemic areas of SFTS were mainly in some mountainous and hilly townships (streets) of Laizhou City, Penglai District, Qixia City, Zhaoyuan City, and Haiyang City. Conclusions:The distribution of SFTS epidemic in Yantai City has obvious regional clustering. Intervention measures such as publicity, education and monitoring should be strengthened in high-incidence areas to reduce the incidence of the disease.

Objective To explore the nursing intervention of adverse reactions of Iohexol Injection using CT scanning. Methods According to the different nursing intervention mode of patients in our hospital in January January 2015 to January 2017 performed CT scanning using the Iohexol Injection group in 50 cases as control group with routine nursing intervention, the observation group used routine nursing quality nursing intervention;adverse reaction a detailed record of the two groups of patients, and the incidence of adverse reactions compared to investigate the effect of nursing intervention of adverse reactions using the Iohexol Injection CT scan. Results Routine nursing quality nursing intervention (observation group) is better than the clinical effect of CT scanning using Iohexol Injection in patients with routine nursing intervention (control group) clinical the effect and adverse reaction rates in patients with lower than the control group, the difference was statistically significant (P<0.05). Conclusion CT scan. With Iohexol Injection patients regular nursing care, high quality nursing mode intervention effect is remarkable, can effectively reduce the incidence of adverse reactions in patients, it is worthy of clinical widely used.
10.Inhibition of ursolic acid on angiogenesis and xenografts in zebrsfish (danio rerio)

Aim To investigate the anti-angiogenesis and anti-xenograftes of UA in zebrafish larvaes. Meth-ods 24 hour post-fertilization ( hpf ) TG zebrafish was treated with different concentration of UA for 24h, and the number of intersegmental vessels( IVS) were detec-ted under fluorescent microscope respectively;then the models of AB/TG zebrafish xenografts were established by be micro-injected with SMMC-7721 or HT-29 cell at 48hpf respectively, and after cocultured with UA for 48h, optical density (OD) of the SMMC-7721 cell and subintestinal veins ( SIVs) induced by HT-29 were e-valuated under confocal microscope. Results ISV as-say showed that UA could cause IVS missing or disap-perance, inhibition ratio reaching 20. 25% and 26. 65%. UA blocked the spread of SMMC-7721 cells in zebrafish and OD value,and inhibition ratios at three concentrations were 38. 01%, 54. 69%, 61. 88%, re-spectively; in another SIVs assay induced by xeno-grafts, UA at concentration 10 and 15mg·L-1 showed that SIVs were inhibited (P<0. 01) obviously. Con-clusion UA could inhibit the angiogenesis and the growth of SMMC-7721/HT-29 xenografts,and the anti-tumor mechanism may be related with VEGFR2 expres-sion.
