1.Redox-responsive nanoparticles reversing non-small cell lung cancer multidrug resistance via dual mechanisms

A redox-responsive hyaluronic acid-vitamin E polyethylene glycol succinate nanoparticle loaded with paclitaxel (HA-SS-TPGS@PTX) was designed to investigate its mechanism for overcoming multidrug resistance (MDR) in non-small cell lung cancer (NSCLC) in vitro. HA-SS-TPGS@PTX nanoparticles were prepared using an emulsion-ultrasonication method. Techniques such as flow cytometry and confocal laser scanning microscopy (CLSM) were employed to study their effects on apoptosis induction, mitochondrial function, and the regulation of P-glycoprotein (P-gp) expression in PTX-resistant lung cancer cells (A549/T). Results showed that HA-SS-TPGS@PTX nanoparticles significantly inhibited the proliferation of A549/T cells in vitro, with an IC50 of 1.35 μg/mL. The nanoparticles entered the cells via CD44 receptor-mediated endocytosis. The high intracellular concentration of glutathione (GSH) triggered the release of PTX and TPGS, which subsequently induced a decrease in mitochondrial membrane potential, leading to apoptosis. Meanwhile, HA-SS-TPGS@PTX also inhibited P-gp expression and ATP consumption, thereby blocking drug efflux. The design of HA-SS-TPGS@PTX provides a new strategy for overcoming MDR in NSCLC.
2.Research on the life stress and hypertension among young couples of childbearing age

Objective:To investigate the current status of life stress and hypertension among couples of childbearing age across diverse economic regions in China, and to explore relevant influencing factors.Methods:This study was a cross-sectional study, with subjects from the “Research on the standardized system of comprehensive prevention and control of birth defects based on preconception-prenatal-postnatal whole chain”. From February to May 2021, urban and rural couples of childbearing age (18-49 years old) from Beijing, Henan, and Gansu provinces were enrolled, representing the eastern, central, and western regions of China, respectively. The detection rate, cognition and control of hypertension in the general population, as well as the detection rate of hypertension in different genders and regions were analyzed. Subjects were divided into hypertensive group and non-hypertensive group based on whether their blood pressure was≥140/90 mmHg (1 mmHg=0.133 kPa), and the general clinical data of the two groups were compared. Subjects were also divided into prehypertension and hypertension group and normal blood pressure group based on whether their blood pressure was≥130/80 mmHg, and the general clinical data of the two groups were compared, with subgroup analyses conducted by gender. Multifactorial logistic regression model was applied to identify factors associated with prehypertension and hypertension in both males and females.Results:A total of 1 942 couples of childbearing age, comprising 3 884 individuals, aged (29.8±5.2) years were enrolled, with 1 942 males (50.0%). The overall hypertension detection rate was 6.3% (246/3 884), with a detection rate of 10.5% (203/1 942) in males and 2.2% (43/1 942) in females. The hypertension detection rates in Beijing, Henan, and Gansu were 6.2% (92/1 482), 11.6% (139/1 200), and 1.2% (15/1 202), respectively. The overall detection rate of prehypertension and hypertension was 40.5% (1 574/3 884). Multifactorial logistic regression analysis showed that life pressure factors had no effect on female blood pressure levels ( P>0.05), while a significant or high level of life/work pressure was a risk factor for prehypertension and hypertension in males ( OR=2.30, 95% CI 1.06-4.99, P<0.05). Conclusion:The detection rate of prehypertension and hypertension among young couples of childbearing age in China is high, with poor awareness and control of hypertension. There are sex differences in the relationship between life pressure and blood pressure levels. Comprehensive consideration of individual living environments and mental health factors is crucial in blood pressure management. Measures to reduce life stress and enhance mental resilience should be implemented to address this public health issue.
3.Efficacy and safety of an antioxidant gel containing tea polyphenols combined with narrow-band ultraviolet B in the treatment of vitiligo: a single-center randomized controlled trial

Objective:To evaluate the clinical efficacy and safety of an antioxidant gel containing tea polyphenols combined with narrow-band ultraviolet B in the treatment of vitiligo.Methods:A single-center, randomized controlled clinical trial was conducted. From April 25 to June 27, 2024, patients with vitiligo were selected from the Department of Dermatology, Hangzhou Third People's Hospital. An open-label and researcher-blinded design was used. The patients were divided into 3 groups: a phototherapy group receiving phototherapy alone, a tea polyphenols combined group treated with an antioxidant gel containing tea polyphenols combined with phototherapy, and a positive control group treated with an antioxidant gel containing superoxide dismutase combined with phototherapy, with the treatment duration being 3 months. The efficacy was evaluated using the Vitiligo Area Scoring Index (VASI), and when the VASI was improved by ≥ 10%, the treatment would be considered effective. Changes in skin aging and skin barrier function indicators before and after treatment were assessed for 72 vitiligo lesions in the tea polyphenols combined group and for 72 lesions in the phototherapy group. Comparisons between the groups were performed using one-way analysis of variance, Fisher's exact test, chi-square test, or t test. Results:A total of 171 vitiligo patients with 307 target lesions were successfully followed up in this study, including 74 males and 97 females, and their ages ranged from 1 to 64 years. Among the 307 lesions, 95 were treated with phototherapy alone, of which 35 showed improvement, resulting in a total response rate of 36.8% and an average VASI improvement rate of 10.9%; adverse reactions occurred in 29 lesions (30.5%). Of 138 lesions treated with the antioxidant gel containing tea polyphenols combined with phototherapy, 73 showed improvement, resulting in a total response rate of 52.9% and an average VASI improvement rate of 24.0%; adverse reactions occurred in 10 lesions (7.2%). In the positive control group, 74 lesions were treated, and 40 showed improvement, resulting in a total response rate of 54.1% and an average VASI improvement rate of 18.3%; adverse reactions occurred in 5 lesions (6.8%). Compared with the phototherapy group, the tea polyphenols combined group showed a significantly increased total response rate and a VASI improvement rate (both P < 0.01), but a significantly decreased incidence rate of adverse reactions ( P < 0.001). No significant differences in the above indicators were observed between the tea polyphenols combined group and the positive control group (all P > 0.05). In addition, the changes in skin barrier function and skin aging indicators (except for wrinkle depth) before and after treatment were significantly reduced in the tea polyphenols combined group compared to the phototherapy group (all P < 0.05). After the phototherapy alone, the transepidermal water loss significantly increased ( P = 0.004), and the water content of the stratum corneum significantly decreased ( P = 0.012). However, no significant differences in skin barrier function or skin aging indicators were found between pre- and post-treatment in the tea polyphenols combined group ( P > 0.05) . Conclusion:The antioxidant gel containing tea polyphenols could effectively improve the efficacy of narrow-band ultraviolet B in the treatment of vitiligo, and alleviate skin aging and barrier damage caused by phototherapy.
4.Current status and influencing factors of self-disgust in female breast cancer patients

Objective:To explore the current status of self-disgust in female breast cancer patients and analyze its influencing factors, so as to provide reference for clinical intervention.Methods:Convenience sampling was used to select 283 female breast cancer patients who were hospitalized in Guangdong Provincial Hospital of Chinese Medicine from June to July 2024 for the study. A questionnaire survey was conducted using the General Information Questionnaire, Questionnaire for the Assessment of Self-Disgust (QASD), Family Avoidance of Communication about Cancer Scale, Body Image Scale, and the Chinese version of the Female Self-Advocacy in Cancer Survivorship. Factors influencing patients' self-disgust were analyzed using one-way analysis of variance and multiple linear regression.Results:The total score of QASD in female breast cancer patients was (33.77±7.64). Education level, sexual dysfunction after breast cancer, family avoidance of communication about cancer, body image and self-advocacy were influencing factors of self-disgust in female breast cancer patients ( P<0.05) . Conclusions:Self-disgust of female patients with breast cancer is at a medium to high level. It is recommended that nurses pay attention to the psychological status of patients with low level of education and sexual dysfunction after the disease, implement targeted psychological interventions to improve the family avoidance of communication about cancer and body image, and increase the awareness of self-advocacy, thus reducing the risk of self-disgust in female breast cancer patients.

8.Comparing the efficacies of different treatments for T lymphoblastic lymphoma

Objective:To explore the differential efficacies of conventional chemotherapy, autologous hematopoietic stem cell transplantation (auto-HSCT) and allogeneic hematopoietic stem cell transplantation (allo-HSCT) for T lymphoblastic lymphoma (T-LBL) .Method:From January 2012 to December 2022, the relevant clinical data were retrospectively reviewed for 82 T-LBL patients hospitalized at Affiliated Tongji Hospital. According to different treatments, they were assigned into two groups of non-transplantation (49 cases) and transplantation (33 cases). The transplantation group was divided further into two groups of allo-HSCT (22 cases) and auto-HSCT (11 cases) according to different transplantation modes. In non-transplantation group, remission was induced mostly by cyclophosphamide+messosodium+doxorubicin+dexamethasone+vincrine/methotrexate+Hyper CAVD A/B. Six patients achieved remission based upon cyclophosphamide+cytarabine+6-mercaptopurine (CAT), etoposide+vincristine+doxorubicin+cyclophosphamide+cyclophosphamide+ prednisone (EPOCH), high-dose methotrexate+dexamethasone and vincristine+pirubicin+ cyclophosphamide+ pemasase+prednisone (VDCLP). The transplantation group underwent HSCT after multi-drug combination intensive induction therapy. Efficacy and survival were analyzed by observing the rates of overall survival (OS) and progression-free survival (PFS) .Result:There were 64 males and 18 females with a median age of 23 (11~74) year. Among them, 62 cases (75.61%) had clinical stage Ⅲ~Ⅳ. And 43 cases (53.44%) had systemic symptoms (B symptom) of fever, night sweats and weight loss at an onset of disease. Fifty cases (61.00%) had an involvement of bone marrow and 33 cases (80.5%) belonged to Ann Arbor stage Ⅲ and above. There were 65 cases (79.27%) with Eastern Cooperative Oncology Group (ECOG) score ≤2 and 17 cases (20.73%) with ECOG score >2. International Prognostic Index (IPI) was ≤3 (63 cases, 76.83%) and >3 (19 cases, 23.17%). Follow-up period was 27.5 (5~118) month. And 3-year OS and PFS were 53.64% (95% CI: 42.35%~64.62%) and 47.56% (95% CI: 36.53%~58.82%). Significant inter-group difference existed in 3-year OS[42.86% (95% CI: 29.12%~57.71%) vs 69.70% (95% CI: 51.13%~83.79%), P=0.014]and 3-year PFS was 38.76% (95% CI: 25.54%~53.76%) and 60.61% (95% CI: 42.24%~76.57%). And the difference was statistically significant ( P=0.032) . Conclusion:As a consolidation therapy, HSCT may improve the long-term outcomes of T-LBL patients as compared with chemotherapy alone.
9.Nursing experience of extracorporeal membrane oxygenation combined with inhaled nitric oxide in the treatment of morbid obesity with severe acute respiratory distress syndrome

Objective To summarize the successful experience of extracorporeal membrane oxygenation (ECMO) combined with inhaled nitric oxide (iNO) in the treatment of severe acute respiratory distress syndrome (ARDS) after empyema surgery in a morbid obesity patient,and to explore the nursing points. Methods On July 3,2023,a patient was admitted to the department of intensive care unit (ICU) of Guangdong Provincial People's Hospital (Guangdong Academy of Medical Sciences),Southern Medical University following the failure of closed thoracic drainage and catheterization at another hospital. Using the rapid integration of the WeChat group,a multidisciplinary team (MDT) model was built. This approach enabled the professional and standardized integration of clinical data and the implementation of targeted treatments,significantly reducing response times and optimizing the overall nursing process. Results A 27-year-old male patient with morbid obesity was admitted to the hospital due to dyspnea and chest pain for more than 7 days. ① Treatment process:on July 4,the patient underwent video-assisted thoracic surgery (VATS),including right chest exploration,pleural adhesion release,and empyema,performed under general anesthesia. Two thoracic drainage tubes were retained and water-sealed bottles were connected. The drainage fluid was purulent. After the operation,the patient was short of breath and the condition was aggravated and transferred to ICU. On admission,the patient's bedside chest X-ray showed that more moist rales were heard in both lungs,especially on the right side. At 18:30 on July 10,the pluse oxygen saturation (SpO2) was 0.75-0.80,and fiberoptic bronchoscopy was performed immediately. The ventilator parameters were up-regulated,the position was changed,and 20 mg of furosemide injection was injected intravenously,the effect was not good. Attempted to perform prone position ventilation,SpO2 did not improve. At 21:30,the SpO2 gradually decreased to 0.60,and the extracorporeal circulation was immediately decided. After veno-venous ECMO (VV-ECMO) at 2:30 on July 11,the SpO2 was 0.90,and the blood gas was stable after multiple reexaminations. During July 12,there was still shortness of breath and poor oxygenation index. According to the MDT consultation opinion,during the emergency treatment combined with iNO treatment,oxygenation improved rapidly to 172 mmHg (1 mmHg≈0.133 kPa) and 190 mmHg after 1 hour and 2 hours,respectively. After 6 days,the oxygenation index stabilized between 222-285 mmHg. On July 17,the iNO support was gradually reduced and successfully removed. On July 21,a chest X-ray showed that the patient's lung lesions were significantly improved,and ECMO support parameters were gradually reduced until ECMO treatment was successfully discontinued. On August 3,the patient's consciousness returned to normal,and the indicators returned to normal. The ventilator-assisted breathing was stopped,and the high-flow oxygen therapy was observed after extubation. He was transferred from the ICU on August 8 and was discharged on August 15. ② Nursing points:we focus on personalized analgesia and sedation,and adjust the types and doses of sedative drugs in stages to reduce oxygen consumption and reduce complications. For the treatment of ARDS with ECMO combined with iNO to improve oxygenation,close monitoring and supportive care were carried out. Special attention was paid to the fixation of ECMO pipeline and tracheal intubation in patients with morbid obesity and individualized fine skin care was implemented. Actively prevent the potential complications of ICU acquired myasthenia,and carry out phased psychological nursing to establish rehabilitation confidence. Conclusion ECMO combined with iNO treatment requires professional teamwork,close observation,effective nursing and perfect monitoring technology to ensure the safety of patients with severe ARDS morbid obesity,reduce complications and improve prognosis,which has important reference significance for relevant medical practice.

Attempted to perform prone position ventilation,SpO2 did not improve. At 21:30,the SpO2 gradually decreased to 0.60,and the extracorporeal circulation was immediately decided. After veno-venous ECMO (VV-ECMO) at 2:30 on July 11,the SpO2 was 0.90,and the blood gas was stable after multiple reexaminations. During July 12,there was still shortness of breath and poor oxygenation index. According to the MDT consultation opinion,during the emergency treatment combined with iNO treatment,oxygenation improved rapidly to 172 mmHg (1 mmHg≈0.133 kPa) and 190 mmHg after 1 hour and 2 hours,respectively. After 6 days,the oxygenation index stabilized between 222-285 mmHg. On July 17,the iNO support was gradually reduced and successfully removed. On July 21,a chest X-ray showed that the patient's lung lesions were significantly improved,and ECMO support parameters were gradually reduced until ECMO treatment was successfully discontinued. On August 3,the patient's consciousness returned to normal,and the indicators returned to normal. The ventilator-assisted breathing was stopped,and the high-flow oxygen therapy was observed after extubation. He was transferred from the ICU on August 8 and was discharged on August 15. ② Nursing points:we focus on personalized analgesia and sedation,and adjust the types and doses of sedative drugs in stages to reduce oxygen consumption and reduce complications. For the treatment of ARDS with ECMO combined with iNO to improve oxygenation,close monitoring and supportive care were carried out. Special attention was paid to the fixation of ECMO pipeline and tracheal intubation in patients with morbid obesity and individualized fine skin care was implemented. Actively prevent the potential complications of ICU acquired myasthenia,and carry out phased psychological nursing to establish rehabilitation confidence. Conclusion ECMO combined with iNO treatment requires professional teamwork,close observation,effective nursing and perfect monitoring technology to ensure the safety of patients with severe ARDS morbid obesity,reduce complications and improve prognosis,which has important reference significance for relevant medical practice.
