1.Influencing factors for poor prognosis of drug-induced liver injury in Chinese population: a systematic review

OBJECTIVE To systematically evaluate the influencing factors affecting the poor prognosis of drug-induced liver injury (DILI) in the Chinese population, and to provide evidence-based support for early identification and interventions of DILI. METHODS Retrieved from PubMed, Medline, Embase, the Cochrane Library, CNKI, Wanfang database, China biomedical medicine database (CBM) and VIP, clinical studies (case-control studies, cohort studies) related to influencing factors for poor prognosis of DILI were collected from inception to May 31, 2025. After literature screening, data extraction and quality evaluation of included studies, meta-analysis was carried out by using RevMan 5.4 software. RESULTS A total of 17 literature were included, involving 4 078 DILI patients, of whom 673 were in the poor prognosis group and 3 405 were in the favorable prognosis group. Meta-analysis showed that history of liver disease (OR=2.47, 95%CI was 1.61-3.78, P <0.001), alcohol drinking history (OR=1.77, 95%CI was 1.22-2.56, P =0.003), Chinese herbal medicine/Chinese patent medicine (OR=1.87, 95%CI was 1.30-2.70, P <0.001), non-hepatocellular injury type (OR=1.70, 95%CI was 1.37-2.10, P <0.001), international normalized ratio (INR) elevated (OR=2.51, 95%CI was 1.97-3.19, P <0.001), and alanine transamine (ALT) elevated (OR=1.27, 95%CI was 1.14-1.41, P <0.001) were risk factors of poor prognosis in DILI. Higher albumin (ALB) level (OR=0.47, 95%CI was 0.39-0.57, P <0.001), elevated prothrombin activity (PTA) (OR=0.88, 95%CI was 0.85-0.91, P <0.001) and more than 2 kinds of hepatoprotective drugs (OR=0.62, 95%CI was 0.41-0.95, P =0.030) were protective factors for poor prognosis of DILI. CONCLUSIONS Patients with alcohol drinking history, history of liver disease, elevated INR, elevated ALT, taking Chinese herbal medicine/Chinese patent medicine, and non-hepatocellular injury type of DILI have a greater risk of poor prognosis, and higher ALB level, higher PTA and more than 2 kinds of hepatoprotective drugs can reduce the risk of poor prognosis of DILI.
2.Impact of number of positive regional lymph nodes in N1 stage on the prognosis of patients with non-small cell lung cancer: A propensity score matching study

Objective To explore the impact of number of positive regional lymph nodes (nPRLN) in N1 stage on the prognosis of non-small cell lung cancer (NSCLC) patients. Methods Patients with TxN1M0 stage NSCLC who underwent lobectomy and mediastinal lymph node dissection from 2010 to 2015 were screened from SEER database (17 Regs, 2022nov sub). The optimal cutoff value of nPRLN was determined using X-tile software, and patients were divided into 2 groups according to the cutoff value: a nPRLN≤optimal cutoff group and a nPRLN>optimal cutoff group. The influence of confounding factors was minimized by propensity score matching (PSM) at a ratio of 1 : 1. Kaplan-Meier curves and Cox proportional hazards models were used to evaluate overall survival (OS) and lung cancer-specific survival (LCSS) of patients. Results A total of 1316 patients with TxN1M0 stage NSCLC were included, including 662 males and 654 females, with a median age of 67 (60, 73) years. The optimal cutoff value of nPRLN was 3, with 1165 patients in the nPRLN≤3 group and 151 patients in the nPRLN>3 group. After PSM, there were 138 patients in each group. Regardless of before or after PSM, OS and LCSS of patients in the nPRLN≤3 group were superior to those in the nPRLN>3 group (P<0.001). N1 stage nPRLN>3 was an independent prognostic risk factor for OS [HR=1.52, 95%CI (1.22, 1.89), P<0.001] and LCSS [HR=1.72, 95%CI (1.36, 2.18), P<0.001]. Conclusion N1 stage nPRLN>3 is an independent prognostic risk factor for NSCLC patients in TxN1M0 stage, which may provide new evidence for future revision of TNM staging N1 stage subclassification.
3.Study on the influential factors for clinical efficacy of polymyxin B combined with other antibiotics in the treat-ment of carbapenem-resistant Acinetobacter baumannii pulmonary infection

OBJECTIVE To analyze the influential factors for clinical efficacy of polymyxin B combined with other antibiotics in the treatment of carbapenem-resistant Acinetobacter baumannii (CRAB) pulmonary infection. METHODS A retrospective analysis was conducted on the clinical data of patients with CRAB pulmonary infection in our hospital from May 2021 to October 2024. Information such as age, gender, admitting department, infection status, underlying medical conditions, mechanical ventilation time, combination anti-infective treatment regimens, and the Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE-Ⅱ) score 24 h before medication was compiled. Based on the effectiveness of the treatment, patients were divided into treatment-effective group and treatment-ineffective group. Univariate analysis and multivariate Logistic regression analysis were employed to identify independent factors influencing clinical efficacy. RESULTS A total of 156 patients were included, and 108 patients were treated effectively, with an effective rate of 69.23%. The results of univariate analysis indicated that there were statistically significant differences between 2 groups in terms of the duration of mechanical ventilation time, APACHE-Ⅱ score 24 h before medication, the number of complication types, the proportion of abnormal coagulation function, anti-infective treatment course, and hospital stay before medication (P<0.05). The results of multivariate Logistic regression analysis showed that APACHE-Ⅱ score≥15 points 24 h before medication [OR=2.965, 95%CI (1.284, 6.845), P=0.020], mechanical 20251606) ventilation time≥10 d [OR=3.577, 95%CI (1.185, 10.793), P=0.037] and hospital stay≥14 d before medication [OR=2.422, 95%CI (1.036, 5.654), P=0.041] were independent 15120420253@139.com risk factors, and anti-infective treatment course>7 d was a protective factor [OR=0.445, 95%CI (0.221, 0.895), P=0.043]. CONCLUSIONS This study shows that the effective rate of polymyxin B combined with other antibiotics in the treatment of CRAB pulmonary infection is less than 70%. The mechanical ventilation time≥10 d, APACHE-Ⅱ score≥15 points 24 h before medication, and hospital stay≥14 d before medication may lead to treatment failure, whereas anti-infective treatment course>7 d may be associated with treatment success.
4.The constituent elements, experiences, and popularization significance of the palliative care model of integrated elderly care and medical services

Under the trend of increasing aging, integrated elderly care and medical services is an important measure to optimize the supply of elderly care services and promote the good death of the elderly. Using the cooperative production theory and the classical grounded theory, a qualitative analysis was conducted on 38 cases of elderly palliative care and 25 cases of hospital-based palliative care under the integrated elderly care and medical services model from a hospital in Nanning City using Nvivo 20.0 software. This paper found that the integrated elderly care and medical services mode emphasized the deep integration of medical and elderly care services by integrating resources and improving service efficiency, to achieve the basic experience of comprehensive health care for the elderly. The promotion of these experiences has a positive significance for building a multi-agent cooperative production system, strengthening personnel training, perfecting the performance distribution mechanism, and further promoting the development of the national palliative care pilot.
5.Efficacy of blinatumomab as a bridge therapy for allogeneic hematopoietic stem cell transplantation in treatment of minimal residual disease-positive B-lineage acute lymphoblastic leukemia

Objective:To investigate effective strategies to reduce the high risk of recurrence after allogeneic hematopoietic stem cell transplantation(allo-HSCT)in patients with minimal residual disease(MRD)-positive B-lineage acute lymphoblastic leukemia(B-ALL).Methods:A retrospective analysis was performed for ten B-ALL patients with positive MRD before transplantation at our center from November 2022 to November 2024.There were four male patients and six female patients,with a median age of 30(7-56)years.Of all patients,four received transplantation from unrelated donors,and six received haploidentical transplantation from relatives.All ten patients received blinatumomab(28 μg/day for 14 days for patients≥45 kg;5 μg/m2/day for 14 days for patients<45 kg)and were bridged to the conditioning regimen for allo-HSCT within 7 days.Results:After treatment with blinatumomab,all ten patients achieved the clearance of MRD.After allo-HSCT,100%of the patients achieved donor hematopoietic reconstitution.The median time to neutrophil reconstitution was 12(9-22)days,and the median time to platelet reconstitution was 13.5(9-22)days.As of January 2025,the median observation time was 14(2-26)months;of all patients,two experienced recurrence,and one had positive MRD again,with a recurrence rate of 20%.Among the ten patients,nine survived and one died of disease recurrence.One patient achieved complete re-mission again after chimeric antigen receptor T-cell(CAR-T)therapy following recurrence,and the patient with positive MRD achieved the clearance of MRD again after interleukin-2 treatment;both patients were currently alive.During treatment,one patient developed cytokine release syndrome,one patient was comorbid with SARS-CoV-2 infection,one patient had herpes zoster and viral encephalitis,three patients developed grade Ⅰ-Ⅱ acute graft-versus-host disease,and four patients developed chronic graft-versus-host disease;no transplantation-associated thrombotic microangiopathy was observed.Conclusion:For patients with MRD-positive B-ALL,blinatumomab as a bridge therapy for allo-HSCT can significantly reduce recurrence after transplantation,with fewer complica-tions that are easy to control.Multicenter randomized controlled clinical studies can be performed to further verify its efficacy and safety.
6.Progress on CLCN5 gene in Dent disease type 1

Dent disease is a rare X-linked recessive renal tubular disease characterized by low molecular weight proteinuria,hypercalcemia and nephrocalcinosis. It is also a major cause of tubular proteinuria in children. According to different causative genes,Dent disease can be divided into three types:type 1 is caused by mutations in the CLCN5 gene,accounting for about 60%-70%;type 2 is caused by mutations in the OCRL gene,accounting for about 15%-20%;type 3 has a similar clinical phenotype but no known pathogenic gene mutations. CLCN5 encodes the voltage-dependent 2Cl -/1H +exchange channel CIC-5,which is involved in proximal renal tubule endocytosis. Its mutations can cause a variety of proximal tubular dysfunction symptoms,mainly including low molecular weight proteinuria. The use of gene detection technology has resulted in an increase in reports on Dent disease year after year. At present,the specific mechanism underlying Dent disease remains unknown. This article reviews the research progress of CLCN5,hoping to provide new insight for the mechanism research of CLCN5 and the specific treatment of Dent disease type 1.
7.Analysis of Related Factors Influencing One-year Recurrence of Polymyalgia Rheumatica

To identify factors associated with the recurrence of polymyalgia rheumatica(PMR) within one year. This study included 64 patients diagnosed with PMR at Peking Union Medical College Hospital between January 2019 and June 2024. The baseline characteristics of patients with and without recurrence were compared, and logistic regression analysis was performed to identify risk factors for recurrence. The mean age at onset was 65.1±7.9 years, with a male-to-female ratio of 1:3.3. The average duration from onset to diagnosis was 4.5±3.7 months. At baseline, the average erythrocyte sedimentation rate(ESR) was 67.0±29.2 mm/h, with 11 patients(17.2%) having an ESR > 100 mm/h, and the average C-reactive protein(CRP) level was 57.9±51.3 mg/L. Corticosteroids were used as the initial treatment in 95.3% of patients, with an average dose of 21.6±11.6 mg/day. During the 12-month follow-up, 35.9% of patients experienced recurrence, with the median time to first recurrence being 8.2±3.3 months.The cumulative recurrence rates at 3, 6, 9, and 12 months were 6.3%, 14.1%, 25%, and 35.9%, respectively. Comparisons between patients with and without recurrence revealed significant differences in age(68.7±6.5 The one-year recurrence rate of PMR is 35.9%. Older age and lower serum albumin levels are associated with recurrence, and age may be associated with disease recurrence within one year.
8.Population heterogeneity analysis of caries prevention service preferences among children in Anhui Province

Objective:
To determine the heterogeneity for caries prevention service preferences among children in Anhui Province, so as to provide reference for the promotion and popularization of caries prevention services for school age children.
Methods:
Based on a discrete selection experiment, a face to face questionnaire survey was administered using a multi stage sampling method among 785 parents with children 3-12 years of age who were hospitalized in the stomatology clinics of 7 prefectures and cities in Anhui Province from October 2021 to October 2022. A mixed Logit model was used to evaluate caries prevention service preferences for children.
Results:
Four discrete choice experiment attributes included in the study were statistically significant for choice preference ( P <0.05). Compared with the control group, parents with a high school education or above preferred caries prevention services with 70%-<80% preventive effectiveness, 2-<5 and <2 km from the service point, and a high service cost ( β =0.38, 1.66, 1.64, 0.00); female parents preferred preventive services with 70%-<80% preventive effectiveness and a high service cost ( β =0.35, 0.01 ); parents of children <7 years of age preferred services with 70%-<80% preventive effectiveness ( β =0.75); parents of children with oral health preferred preventive services during winter and summer vacations ( β =-0.28); parents of children with caries preferred preventive services with a high cost per denticle ( β =0.00)( P <0.05).
Conclusions
Parents with different education levels, gender, child age, and oral health status have heterogeneity in dental caries prevention service preferences. The provision of targeted and precise services can improve the participation and coverage of caries prevention services for school age children.
9.Resistance of adult Culex quinquefasciatus and Musca domestica to commonly used insecticides in Dazu District, Chongqing, 2023

ObjectiveTo determine the resistance of adult Culex quinquefasciatus and Musca domestica to commonly used insecticides in Dazhu District of Chongqing in 2023, and provide scientific evidence for rational use of hygienic insecticides. MethodsDrug resistance was determined by using adult mosquito contact tube method and adult fly drop method. ResultsIn 2023, the 24-hour mortality rate of adult Culex quinquefasciatus in Dazu District, Chongqing, against permethrin (0.25%), beta-cypermethrin (0.025%), deltamethrin (0.025%), malathion (5%) and propoxur (0.1%) was 37.35%, 21.92%, 28.33%, 96.43%, and 95.38%, respectively. The median lethal dose (LD50) value of female adult Musca domestica against beta-cypermethrin (95.8%), chlorpyrifos (97%), deltamethrin (90%) and dichlorvos (95%) was 2.572 μg, 0.329 μg, 0.406 μg, and 0.492 μg, respectively, with resistance of 829.68, 7.65, 53.42, and 46.42 folds to the above tested insecticides. ConclusionAdult Culex quinquefasciatus and Musca domestica in Dazu District show various degrees of resistance to commonly used insecticides. It warrants suspending the insecticides with high resistance, whereas mixedly and alternatively administering the insecticides with possible resistance and low resistance. Additionally, we should continue to monitor the resistance and guide the rational use of insecticides.
10.Comparison of the clinical efficacy of laparoscopic cholecystectomy via umbilical single hole and three holes based on propensity score matching in the treatment of acute cholecystitis

Objective To explore the comparison of the clinical efficacy of laparoscopic cholecystectomy based on propensity score matching through the umbilical single hole method and three holes method in the treatment of acute cholecystitis.Methods 106 patients with acute cholecystitis who underwent laparoscopic cholecystectomy in our hospital from January 2020 to December 2022 were selected as research subjects.According to different treatment methods,they were divided into the study group(n=64)and the control group(n=42).The patients in the study group were treated with single port via umbilical laparoscopic cholecystectomy,while the control group was treated with three port laparoscopic cholecystectomy.Compare the surgical related indicators,postoperative recovery,and postoperative complications between the two groups of patients.Results Comparing the general information of the two groups of patients,there was no significant difference in body mass index(BMI),BMI,and gallbladder ultrasound results(P>0.05),but the average age of the study group patients was greater than that of the control group patients(P<0.05).By comparing the general data of the two groups of patients after propensity score matching,it was found that there was no significant difference in gender,age,BMI,and gallbladder ultrasonography results between the two groups(P>0.05).The surgical time for the study group patients was(61.48±10.24)minutes,while for the control group,it was(40.42±8.35)minutes;And the visual analog scale(VAS)scores at 6 and 24 hours after surgery were(5.63±1.04)and(3.42±0.65)for the study group,respectively,while(7.22±1.61)and(5.48±1.02)for the control group.The pain sensation was more pronounced(P<0.05);However,there was no statistically significant difference in blood loss during surgery between the two groups of patients(P>0.05).The hospitalization time and gastrointestinal function recovery time of the study group patients were(2.05±0.42)days and(33.46±4.30)hours,respectively,while the control group was(3.95±1.02)days and(47.58±7.62)hours,respectively;And the satisfaction score of the study group patients with incision scars was(4.58±0.42)points,while the control group was(3.02±0.38)points(P<0.05).The total incidence of complications in the study group was 2.78%,while in the control group it was 16.67%,significantly lower in the study group(P<0.05).Conclusion Patients with acute cholecystitis treated with the umbilical single hole method have significantly lower postoperative pain,shorter hospital stay,and recovery time of gastrointestinal function compared to the three hole method.Patients also have higher scores of satisfaction with incision scars,and the overall incidence of postoperative complications is also lower.
