1.Correlations of PTTG1 and UBE2C with prognosis of liver cancer based on bioinformatics analysis

Objective To explore the T/NK cell-related differentially expressed genes(T/NK-DEGs)related to the prognosis of liver cancer based on the single-cell RNA-seq data,gene expression data and clinical information in the GEO and TCGA databases,and construct the prognostic model of liver cancer.Methods The single-cell RNA-seq data and gene expression matrices of liver cancer were obtained from the GEO database.The TCGA-LIHC cohorts,including mRNA expression data,clinical information,survival infor-mation,and somatic mutation data,were obtained from the TCGA database.Based on the two databases,the prognostic model of liver cancer patients was constructed by the bioinformatics method,and the performance of the model was predicted.Results Two T/NK-DEGs,PTTG1 and UBE2C,were identified to be associated with the prognosis of liver cancer and a prognostic model of liver cancer was constructed based on them.According to the risk score,the patients were divided into the high-risk score group and low-risk score group,in which the patients with high-risk score had a worse prognosis than those with low-risk score.The areas under the receiv-er operating characteristics(ROC)curve(AUCROC)of the prognostic model at 1-year,3-year and 5-year time points were 0.685,0.647 and 0.594,respectively.The higher risk score was correlated with the advanced pathological stage(Ⅰstage,Ⅱstage,andⅢstage)and T-stage(T1,T2,and T3)(P<0.05).The prognostic model was able to predict the proportion of tumor infiltrating immune cells,and the sensitivity of immunotherapy and chemotherapy drugs in patients with liver cancer.Conclusion The constructed prog-nostic model in this study has an important role in the prediction of individualized survival and clinical treatment response of patients with liver cancer.
2.Application of Jiugong abdominal meridian massage combined with Zhuang medicine line acupoint moxibustion in patients after radical resection of colorectal cancer

Objective:To explore the therapeutic effects of Jiugong abdominal meridian massage combined with Zhuang medicine line acupoint moxibustion in patients after radical resection of colorectal cancer.Methods:A convenience sampling method was used to select 200 patients who underwent radical resection of colorectal cancer at Huzhou First People's Hospital from January 2022 to December 2023. Patients were randomly divided into three groups using a random number table: the control group ( n=62), the Zhuang medicine line acupoint moxibustion group ( n=66), and the combined group ( n=72). The control group received conventional interventions, the Zhuang medicine line acupoint moxibustion group received Zhuang medicine line acupoint moxibustion in addition to the conventional intervention, and the combined group received Jiugong abdominal meridian massage in addition to the Zhuang medicine line acupoint moxibustion. Gastrointestinal function recovery and pain scores were compared among the three groups after the intervention. Results:After intervention, there were statistically significant differences in the recovery time of bowel sounds, time to tolerate solid food by mouth, time to first bowel movement, gastric body motility frequency, total gastrointestinal output rate, gastric emptying rate, and pain scores among the three groups ( P<0.05) . Conclusions:Jiugong abdominal meridian massage combined with Zhuang medicine line acupoint moxibustion intervention can effectively improve the gastrointestinal function indicators and reduce postoperative pain in patients after radical resection of colorectal cancer.
3.A qualitative study on the status of back patting and sputum drainage nursing by Neurological Intensive Care Unit nurses and the design requirements for a nursing robot

Objective:To explore the current status of back patting and sputum drainage nursing performed by nurses in the Neurological Intensive Care Unit (NICU) and their design requirements for a nursing robot, providing a reference for the design of nursing robots for back patting and sputum drainage.Methods:This descriptive qualitative study employed a purposive sampling method. From January to February 2024, semi-structured interviews were conducted with 15 nurses from the NICU of Xuanwu Hospital, Capital Medical University. Data were analyzed using Colaizzi 7-step analysis method.Results:A total of three major issues were identified regarding the current practice of back patting and sputum drainage nursing by NICU nurses: heavy workload and time-consuming; difficulties in patient positioning and maintenance; deficiencies in nursing care. The design requirements for a back patting and sputum drainage nursing robot were categorized into two main themes with 10 sub-themes: functional requirements (turning function, supporting function, intelligent recognition function, abnormality detection with alarm or pause feature, and evaluation of post-percussion effectiveness) ; design requirements (basic functional design, support component design, percussion component design, coordination between support and percussion functions, and overall structural design) .Conclusions:Understanding the design requirements of NICU nurses for a back patting and sputum drainage nursing robot provides valuable insights for developing a safe, scientific, reasonable, and effective robotic system.

7.Mechanism of Qianyang Yuyin Granules Regulating NR3C2/ROS/ERK Pathway to Alleviate Aldosterone-induced Podocyte Injury

ObjectiveTo investigate the protective mechanism of Qianyang Yuyin granules (QYYY) on aldosterone-induced podocyte injury. MethodA total of 30 C57BL/6J mice were randomly divided into five groups: control group, model group, QYYY low dose (QYYY-L) group, QYYY high dose (QYYY-H) group, and spironolactone (SPL) group, with six mice in each group. Except for the control group, mice were implanted with osmotic minipumps and injected continuously with aldosterone (300 μg·kg-1·d-1) to induce renal injury. The drug administration group was given low and high doses (2.6, 5.2 g·kg-1·d-1) of QYYY and SPL (18 mg·kg-1·d-1) for 28 days. The renal pathological changes of mice were observed by hematoxylin-eosin (HE) staining and Masson staining. The expression levels of Nephrin, Desmin, B-cell lymphoma-2 (Bcl-2), Bcl-2-associated X (Bax), cleaved Caspase-3, nuclear receptor subfamily 3 group C member 2 (NR3C2), extracellular regulated protein kinases (ERK), and phospho-ERK (p-ERK) in kidney tissue were detected by Western blot. The apoptosis levels of kidney tissue were detected by TdT-mediated dUTP nick and labeling (TUNEL) staining, and the superoxide dismutase (SOD) levels were detected. In vitro, the mice were divided into five groups: Control group, model group (aldosterone concentration of 200 nmol·L-1), QYYY-L group, QYYY medium dose (QYYY-M) group, and QYYY-H group (25, 50, and 100 mg·L-1). The effect of different concentrations of QYYY on the relative viability of aldosterone-induced podocytes was detected by cell proliferation and viability assay (CCK-8). The expressions of Nephrin, Desmin, Bax, Bcl-2, cleaved Caspase-3, NR3C2, and p-ERK/ERK were detected by Western blot. AnnexinV-FITC/PI flow cytometry was used to detect the apoptosis levels of podocytes. Reactive oxygen species (ROS) in podocytes were observed by DCFH-DA. ResultCompared with the control group, the model group showed structural pathological changes and fibrotic conditions in the kidney, increased apoptosis levels (P<0.01), and decreased SOD levels (P<0.01). Aldosterone concentration at 200 nmol·L-1 showed a significant decrease in podocyte activity (P<0.05). Podocytes in the model group showed structural pathological changes, disordered arrangement of intercellular microfilaments, increased apoptosis levels (P<0.01), and increased intracellular ROS levels (P<0.01). The protein expressions of Nephrin, Bcl-2, and p-ERK/ERK in kidney tissue and podocytes were decreased (P<0.05, P<0.01). The protein expressions of Desmin, Bax, cleaved Caspase-3, and NR3C2 were increased (P<0.05, P<0.01). Compared with the model group, QYYY alleviated the structural damage and fibrosis of the kidney, decreased the apoptosis levels (P<0.05, P<0.01), and enhanced the SOD content of the kidney (P<0.05, P<0.01). QYYY improved the activity of podocytes (P<0.05, P<0.01), restored the foot process structure of podocytes, and decreased apoptosis levels (P<0.01) and ROS levels of podocytes (P<0.01). The protein expressions of Nephrin, Bcl-2, and p-ERK/ERK in kidney tissue and podocytes were increased (P<0.05, P<0.01), and the protein expressions of Desmin, Bax, cleaved Caspase-3, and NR3C2 were down-regulated (P<0.05, P<0.01). ConclusionQYYY improves aldosterone-induced podocyte injury by regulating the NR3C2/ROS/ERK pathway.
8.Effect of remimazolam combined with nalbuphine on postoperative recovery in elderly patients un-dergoing fibrobronchoscopy

Objective To observe and compare the effect of remimazolam combined with nalbuphine and midazolam combined with nalbuphine on postoperative recovery in elderly patients undergoing fibrobronchoscopy.Methods A total of 112 elderly patients undergoing fibrobronchoscopy,56 males and 56 females,aged ≥ 65 years,BMI 18.5-28.0 kg/m2,ASA physical status I or Ⅱ,were ran-domly divided into two groups:midazolam group and remimazolam group,56 patients in each group.The midazolam group received midazolam 0.03 mg/kg combined with nalbuphine 0.1 mg/kg for anesthesia.The remimazolam group received remimazolam 0.1 mg/kg combined with nalbuphine 0.1 mg/kg for anesthesia.HR,MAP,SpO2,and RR were recorded after entering room,before anesthesia induction,5 minutes after anesthesia induction,and at the end of the operation.The onset time of anesthesia,the time of awakening,the time of operation,the time of discharge,the number of successful cases of sedation,the number of suc-cessful cases of endoscopy,and the occurrence of adverse events were recorded.The satisfaction of the pa-tient,anesthesiologist,and endoscopist were also recorded.Results Compared with the midazolam group,HR and MAP were significantly increased 5 minutes after induction and the end of operation in the remima-zolam group(P<0.05),the onset time of anesthesia and the time of awakening were significantly short-ened(P<0.05),the incidence of hypotension,respiratory depression and nausea and vomiting were sig-nificantly reduced(P<0.05),and the rae of very satisfaction of the operating physician was significantly increased(P<0.05).There were no significant differences in the time of separation,the success rate of sedation and the success rate of endoscopy between the two groups.Conclusion Compared with midazolam,remimazolam combined with nalbuphine can be safely used for flexible bronchoscopy in elderly patients,reduce recovery time and adverse reactions,which is conducive to rapid postoperative recovery.
9.Expert consensus on plasma exchange nursing for severe autoimmune encephalitis

Objective:To form an expert consensus on plasma exchange nursing for severe autoimmune encephalitis ( " Consensus" for short) and standardize the practice of plasma exchange nursing for severe autoimmune encephalitis. Methods:After reviewing and summarizing the literature related to plasma exchange of severe autoimmune encephalitis, the relevant nursing evidence was extracted and the draft of Consensus was formed. From September to October 2023, a total of 20 experts were selected to conduct two rounds of expert letter consultation, and then the expert results were analyzed, sort outed and modified to form the final draft of the Consensus. Results:The expert positive coefficient was 100.00% (20/20), the expert judgment basis coefficient was 0.89, the expert familiarity coefficient was 0.85, the expert authority coefficient was 0.87, and the Kendall harmony coefficient was 0.15-0.22 ( P<0.05). The final Consensus included plasma exchange quality control management, preparation and evaluation before plasma exchange, setting and monitoring during plasma exchange, and evaluation and maintenance after plasma exchange. Conclusions:This Consensus is practical and can provide guidance for clinical nursing practice and quality control of plasma exchange in severe autoimmune encephalitis.
10.Effects of nutritional support on chemotherapeutic efficacy and safety in patients with acute myeloid leukemia

Objective:To investigate the effects of nutritional support on chemotherapeutic efficacy and safety in patients with acute myeloid leukemia.Methods:A total of 130 patients with acute myeloid leukemia who received treatment in Lishui Municipal Central Hospital from January 2021 to December 2021 were included in this study. They were divided into a control group and an observation group ( n = 65/group) according to different nutritional support methods. Patients in the control group were given routine intervention, while patients in the observation group were given nutritional support based on routine intervention. These two interventions were administered till 1 month after chemotherapy. Chemotherapeutic efficacy and safety were compared between the two groups. Results:Total response rate in the observation group was significantly higher than that in the control group [92.3% (60/65) vs. 78.5% (51/65), Z = 4.91, P < 0.05]. After chemotherapy, waist-to-hip ratio, arm girth, and body mass index in the observation group were (0.9 ± 0.1), (25.7 ± 1.2) cm, (21.9 ± 2.1) kg/m 2, respectively, which were significantly greater than (0.8 ± 0.1), (24.4 ± 1.1) cm, (20.6 ± 2.1) kg/m 2 in the control group, respectively ( t = 4.13, 6.63, 3.64, all P < 0.05). Transferrin, albumin, prealbumin, and total serum protein in the observation group were (1.4 ± 0.3) g/L, (27.5 ± 3.1) g/L, (171.3 ± 11.3) mg/L, and (61.2 ± 4.3) g/L, respectively, which were significantly higher than (1.3 ± 0.3) g/L, (25.2 ± 2.9) g/L, (154.3 ± 10.3) mg/L, (56.6 ± 4.0) g/L respectively in the control group ( t = 2.24, 4.48, 8.93, 6.31, all P < 0.05). The scores of emotional state, social status, role cognition, and somatic perception in the observation group were (57.5 ± 4.6) points, (64.5 ± 3.8) points, (56.5 ± 4.1) points, (62.0 ± 4.2) points, which were significantly higher than (47.9 ± 4.2) points, (56.4 ± 3.2) points, (47.7 ± 4.5) points, (55.5 ± 5.4) points in the control group ( t = 12.34, 13.04, 11.55, 7.65, all P < 0.05). The total incidence of adverse reactions in the observation group was 9.2% (6/65), which was significantly lower than 24.6% (16/65) in the control group ( χ2 = 4.43, P < 0.05). Conclusion:Nutritional support can substantially improve chemotherapeutic efficacy in the treatment of acute myeloid leukemia, decrease the incidence of adverse reactions, and is safe. Therefore, nutritional support for patients with acute myeloid leukemia deserves clinical promotion.
