1.Epidemic Characteristics and disease burden trend of cervical cancer in Guangxi Zhuang Autonomous Region, 2010-2017

Objective:To analyze the changing trend of cervical cancer epidemiological characteristics and disease burden in cancer registration areas of Guangxi Zhuang Autonomous Region (Guangxi) from 2010 to 2017, and to provide scientific basis for the development of cervical cancer prevention and control strategies in Guangxi.Methods:Using descriptive analysis method, based on the incidence and death data of cervical cancer in the tumor registration areas of Guangxi from 2010 to 2017, Crude morbidity, crude mortality, age-standardized morbidity and mortality (referred to as the winning rate), disability adjusted life years (DALYs) rate and the annual percentage change (APC) and average annual percentage change (AAPC) of the above indicators were calculated, and stratified analysis was conducted for urban and rural areas and different age groups.Results:From 2010 to 2017, the crude incidence rate of cervical cancer in Guangxi showed a significant upward trend, rising from 10.31/10 5 in 2010 to 19.94/10 5 in 2017, with an average annual growth rate of 7.9% ( P<0.05). However, after age standardization, the trend of the age-standardized incidence rate of cervical cancer was not statistically significant ( P>0.05). During the same period, the crude mortality rate of cervical cancer increased from 2.69/10 5 to 6.21/10 5, with an average annual growth rate of 13.1% ( P<0.05), and the trend of the age-standardized mortality rate was basically consistent with that of the crude mortality rate. The analysis of urban-rural differences showed that the growth rates of the crude incidence rate and crude mortality rate of cervical cancer in rural areas were higher than those in urban areas from 2010 to 2017 (AAPC incidence rate: 21.3% vs. 2.3%; AAPC mortality rate: 20.1% vs. 8.4%). The analysis of age differences showed that the crude incidence rate and crude mortality rate of cervical cancer in all age groups increased to varying degrees, among which the growth rate of the incidence rate (AAPC=16.2%, P<0.05) and mortality rate (AAPC=14.7%, P<0.05) of cervical cancer in women aged 65 and above was the fastest. In addition, the DALYs rate of cervical cancer in Guangxi increased from 50.6/10 5 in 2010 to 111.0/10 5 in 2017, with an average annual increase of 11.9% ( P<0.05). The growth rate of the DALYs rate in rural areas was higher than that in urban areas, and the growth rate of the DALYs rate in the 50-59 age group was higher than those in other age groups. Conclusions:From 2010 to 2017, the incidence rate, mortality rate and DALYs rate of cervical cancer in Guangxi showed an upward trend. Comprehensive prevention and control measures for cervical cancer, such as improving the early diagnosis and treatment system, promoting the popularization of HPV vaccination and strengthening health education, should be taken to reduce the disease burden of cervical cancer.
2.Incidence and Mortality of Liver Cancer in Fusui County of Guangxi Zhuang Autonomous Region in 2019 and Trend from 2011 to 2019

[Purpose]To analyze the incidence and mortality of liver cancer in Fusui County of Guangxi Zhuang Autonomous Region in 2019 and the trend of change from 201 1 to 2019.[Meth-ods]Based on the incidence and mortality data of liver cancer in Fusui County from 2011 to 2019,the crude incidence rate,crude mortality rate,age-standardized rate by Chinese standard population(ASIRC/ASMRC),age-standardized rate by world standard population(ASMRW/ASM-RW),disability-adjusted life years(DALY),years of life lost(YLL),years of lived with disability(YLD)were calculated.The Joinpoint linear regression model was used to calculate the average annual percentage change(AAPC)of the ASIRC/ASMRC and DALY rate of liver cancer in Fusui County from 2011 to 2019.[Results]In 2019,there were 345 new cases and 298 deaths of liver cancer in Fusui County,accounting for 28.49%and 35.86%of all new cases and death cases of malignant tumors in Fusui County,respectively.The crude incidence rate,ASIRC and ASIRW were 74.71/105,67.55/105 and 65.63/105,respectively;the crude mortality rate,ASMRC and ASMRW were 64.53/105,58.78/105 and 56.88/105,respectively.In 2019,ASIRC/ASMRC of men were higher than those of women(105.32/105 and 95.67/105 for men;27.26/105 and 19.11/105 for women).The peak incidence and mortality rates were in the 65~69 and 80~84 age groups,re-spectively.The incidence rate of liver cancer in Fusui County declined from 82.50/105 in 2011 to 67.55/105 in 2019,with an AAPC of-3.01%(P=0.024),but the mortality rate did not have sig-nificant changes(AAPC=-1.44%,P=0.224).The rate of DALY decreased from 14.51/105 to 10.36/105,with an AAPC of-2.82%(P=0.050).[Conclusion]The incidence rate and DALY rate of liv-er cancer in Fusui County showed an overall decreasing trend from 2011 to 2019,but still re-mained at a high level.It is necessary to further strengthen the prevention,screening and early treatment of liver cancer in this region,especially for men and the elderly.

5.Therapeutic effects on ovulation and reproduction promotion with acupuncture and clomiphene in polycystic ovary syndrome.

OBJECTIVETo compare the difference in the therapeutic effects on ovulation disorder in polycystic ovary syndrome (PCOS) between the combined therapy of electroacupuncture and clomiphene and the single medication of clomiphene and to explore the effect mechanism.
METHODSA total of 80 patients of PCOS were randomized into an acupuncture plus medication group (40 cases) and a medication group (40 cases). In the medication group, since the 5th day of menstruation or medicine-withdrawal bleeding, clomiphene was prescribed for oral administration, continuously for 5 days. In the acupuncture plus medication group, the medication was the same as the medication group. Additionally, 30 min electroacupuncture (continuous wave, 2 Hz) was applied to Qihai (CV 6), Guanyuan (CV 4), Zigong (EX-CA 1), Dahe (KI 12), Sanyinjiao (SP 6), Zhongji (CV 3), Diji (SP 8), Shenshu (BL 23), Sanjiaoshu (BL 22) and Ciliao (BL 32), etc. The treatment started on the 5th day of menstruation or medicine-withdrawal bleeding, once every two days, 3 times a week. One cycle of menstruation or 1 month was taken as one course. The treatment cycles were 3 months in the two groups. The level of estradiol (E) and progesterone (P) in the serum, the endometrial thickness and morphology, ovulation rate and clinical pregnancy rate, as well as the clinical therapeutic effects were compared after treatment in the two groups.
RESULTSTwo cases were dropped out because the herbal medicine was taken simultaneously in the acupuncture plus medication group. In the medication group, the medication was discontinued in 3 cases due to gastrointestinal adverse reactions. The total effective rate was 86.8% (33/38) in the acupuncture plus medication group, better than 64.9% (24/37) in the medication group (<0.05). The ovulation rate in the acupuncture plus medication group was higher than that in the medication group [(86.8%, 33/38) vs (64.9% 24/37), <0.05]. The pregnancy rate in the acupuncture plus medication group was higher than the medication group in tendency, without significant difference [21.1% (8/38) vs 16.2% (6/37), >0.05]. After treatment, the results of endometrial thickness and morphology (A-type rate) in the acupuncture plus medication group were better than those in the medication group (<0.01, <0.05). After treatment, regarding the levels of E and P in the serum in the window period of implantation, the results in the acupuncture plus medication group were higher remarkably than those in the medication group (both <0.01).
CONCLUSIONThe combined intervention of electroacupuncture and clomiphene achieves the definite effects of ovulation and pregnancy promotion in PCOS. It remarkably increases the levels of E and P in the serum, the endometrial thickness and A-type rate. The therapeutic effects of the combined intervention are remarkably better than those of the simple application of clomiphene. This combined intervention is safe and tolerable for the adverse reactions. The effect mechanisms may be related to the improvements of estrogen, progestin and endometrial receptivity.

6.Study on treatment of tibial avulsion fractures of posterior cruciateligament combined with anterior cruciate ligament

Objective To explore the clinical efficacy of one-stage cannulated screw fixation in treatment of tibial avulsion fracture at the insertion of posterior cruciate ligament(PCL) under posterior direction version and in treatment of tibial avulsion fracture at the anterior cruciate ligament (ACL)under arthroscopy.Methods From January 2010 to June 2015,25 patients with tibial avulsion fractures of the posterior cruciate ligament combined with the anterior cruciate ligament in our hospital were retrospectively analyzed.Firstly,arthroscopic exploration was conducted in all patients in their horizontal position,aiming to observe whether combined injures existed or not and to confirm fracture fragments and the degree of tibial avulsion fractures of the cruciate ligaments.All patients recieved arthroscopic fragment fixation by using cannulated screws,then,were treated with a minimally posteromedial incision in the prone position by cannulated screw fixation.All knee joints were protected by adjustable orthopedic brace for 4-6 weeks.The clinical knee joint function of patients were measured by using International Knee Documentation Committee (IKDC) scoring scale and Lysholm scoring scale.The X-ray or CT image were used to evaluate healing process of the fractures.Results All patients were followed up for 6-24 months,the median follow-up time was 15 months.All fractures healed in one stage,time of fracture healing ranged from 9 to 24 weeks,and the median time was 12 weeks.The average IKDC scores at the finial follow-up were as follows:claudication (4.88±0.29)points,support (4.65±0.36)points,pain (4.78±0.49)points,swelling (4.88±0.52)points,squat (4.85±0.35)points,all were higher than those before operation,there were statistically significant differences when compared with those assessed before operation (P<0.05).The average Lysholm score at the finial follow-up was (93.4±2.8)points,the rate of excellent and fine was 96.0%,no statistically significant difference was found compared with that at the healthy side (P>0.05).Conclusion The one-stage cannulated screw fixation in treatment of tibial avulsion fracture at the insertion of PCL under posterior direction version and in treatment of tibial avulsion fractures at the ACL under arthroscopy is easy and convenient to operate,which has the advantages of reliable fixation and satisfactory recent curative efficacy.
7.Clinical application of needle retractable syringe

Objective To explore the effects of needle retractable syringe.Methods 400 patients undergoing injection treatment were randomly divided into the experiment group and the control group in equal number.The conventional syringe was used in the control group and the needle retractable syringe was adopted.The pains in the patients,time for puncturing and incidence of puncturing-induced injuries were compared between two groups.Result The pains,time for puncturing and incidence of puncturing-induced injuries in the observation group were all significantly lower than those of the control group(all P < 0.05). Conclusion Needle retractable syringes may help to alleviate the pain,reduce the workload of nurses and lower the incidence of needle stick injuries.
