1.Effects of Electro-Acupuncture Based on the Midnight-Noon Ebb-Low Method with Hour-Prescription on BMAL1 Protein and NETs Markers in Lung Tissue of Chronic Obstructive Pulmonary Disease Model Rats with Lung-Qi Deficiency Syndrome

ObjectiveTo explore the potential mechanism underlying electroacupuncture therapy with the midnight-noon ebb-low method with hour-prescription for chronic obstructive pulmonary disease (COPD) with lung-qi deficiency syndrome from the perspective of brain and muscle basic helix-loop-helix ARNT-like 1 (BMAL1) as well as neutrophil extracellular traps (NETs). MethodsThe experiment was conducted in two phases. For the therapeutic effectiveness observation, 24 rats were randomly allocated into control group 1, model group 1, midnight-noon electro-acupuncture group 1 and conventional electro-acupuncture group, with 6 rats per group. Rat models of COPD with lung-qi deficiency syndrome were established via cigarette smoke exposure combined with intratracheal instillation of lipopolysaccharide (LPS). After successful modelling, rats in midnight-noon electro-acupuncture group 1 received electro-acupuncture at acupoints "Taiyuan (LU 9)", "Feishu (BL 13)" and "Zusanli (ST 36)" during 05:00—07:00, 30 minutes per treatment once every other day for 14 consecutive days (7 sessions in total). Rats in the conventional electro-acupuncture group received identical electroacupuncture manipulation at random daytime hours (08:00—18:00). Pulmonary function parameters including forced expiratory volume in 0.3 second (FEV0.3), forced vital capacity (FVC) and FEV0.3/FVC ratio were detected. Pulmonary histopathological changes were observed via hematoxylin-eosin (HE) staining. Plasma levels of pro-inflammatory factors interleukin-1β (IL-1β) and tumor necrosis factor-α (TNF-α), as well as pulmonary reactive oxygen species (ROS) content, BMAL1 protein expression and the levels of neutrophil extracellular traps (NETs) biomarkers such as myeloperoxidase (MPO), neutrophil elastase (NE), and citrullinated histone H3 (CitH3) in lung tissue were determined. For the mechanistic verification phase, 36 rats were divided into control group 2, model group 2, midnight-noon electro-acupuncture group 2, overexpression empty group, overexpression BMAL1 group and overexpression with midnight-noon electro-acupuncture group, with 6 rats in each group. At the 5th week of model construction, rats in the overexpression BMAL1 group and overexpression with midnight-noon electro-acupuncture group received intratracheal instillation of 100 μl adenoviral suspension carrying overexpressed Bmal1 gene. Rats in the overexpression empty group were injected with an equal titer and equal volume of blank adenovirus without the Bmal1 coding sequence into the lung at the identical time point. Midnight-noon electro-acupuncture group 2 and overexpression with midnight-noon electro-acupuncture group were consistent with the aforementioned protocol. Finally, pulmonary levels of MPO, NE and CitH3 were measured in all groups. ResultsCompared with the control group 1, the model group 1 exhibited decreased FEV0.3, FVC and FEV0.3/FVC ratio, elevated plasma IL-1β, TNF-α and pulmonary ROS levels, downregulated lung BMAL1 protein expression, and increased contents of MPO, NE and CitH3 (all P<0.05); typical NETs-like structures with sparse granular substances attached to fibrous networks were observed under scanning electron microscopy; HE staining revealed damaged alveolar architecture accompanied by inflammatory cell infiltration. Compared with model group 1, both the midnight-noon electro-acupuncture group 1 and conventional electro-acupuncture group achieved obvious improvements in all above indicators, with superior therapeutic effects in midnight-noon electro-acupuncture group 1 (P<0.05). Consistently, midnight-noon electro-acupuncture group 1 showed more prominent alleviation of NETs-like structure formation and lung pathological injury compared to the conventional electro-acupuncture group. Mechanistic validation results demonstrated that compared with model group 2, the average optical density of MPO as well as the protein levels of MPO, NE and CitH3 were markedly reduced in midnight-noon electro-acupuncture group 2, overexpression BMAL1 group and overexpression with midnight-noon electro-acupuncture group (P<0.05); furthermore, these parameters were significantly lower in the overexpression with midnight-noon electro-acupuncture group than in midnight-noon electro-acupuncture group 2 (P<0.05). ConclusionElectro-acupuncture based on the midnight-noon ebb-low method with hour-prescription may alleviate pulmonary inflammation in COPD by upregulating the expression of clock protein BMAL1 and inhibiting excessive accumulation of NETs in lung tissue.
2.Design and application of a pressure control device for the continuous renal replacement therapy integrated in-series with extracorporeal membrane oxygenation.

Patients requiring extracorporeal membrane oxygenation (ECMO) often need concurrent continuous renal replacement therapy (CRRT). At present, there are various connection methods between ECMO and CRRT circuits, among which in-series integration is the most common. However, ECMO blood flow and catheter type, pressure changes at the pre-pump, post-pump pre-oxygenator, and post-oxygenator segments frequently result in circuit pressures that exceed the alarm threshold of the device. Excessive negative or positive pressures may compromise blood withdrawal and return within the CRRT circuit, leading to frequent system alarms, interruptions in therapy, filter occlusion, and an increased risk of thrombus formation. To address this issue, the critical care nursing team of Zhongda Hospital Affiliated to Southeast University, developed a novel pressure-regulating clamp for CRRT vascular access in ECMO patient, which has been granted a National Utility Model Patent of China (patent number: ZL 2021 2 1496610.7). The device comprises opposing left and right clamp arms joined at the top by a flexible plastic bridge, with dual internal compression surfaces designed to fit CRRT tubing of various calibers. A locking mechanism and serrated strip at the base enable precise adjustment of the compression distance, thereby modulating the tubing's cross-sectional area. This configuration allows real-time regulation of blood flow and stabilization of pressures at blood withdrawal and return sites within the CRRT circuit. By reducing pressure-related alarms and extending filter life, the device may enhance the safety and efficiency of CRRT delivery during ECMO. It is user-friendly, cost-effective, and well-suited for broad clinical implementation, with the potential to alleviate the overall treatment burden on patients and their families.

Paclitaxel (PTX), a valuable natural product derived from Taxus species, exhibits remarkable anti-cancer properties. It penetrates nanopores in microtubule walls, interacting with tubulin on the lumen surface and disrupting microtubule dynamics, thereby inducing cytotoxic effects in cancer cells. PTX and its derivatives have gained approval for treating various diseases due to their low toxicity, high efficiency, and broad-spectrum application. The widespread success and expanding applications of PTX have led to increased demand, raising concerns about accessibility. Consequently, researchers globally have focused on developing alternative production methods and applying nanocarriers in PTX delivery systems to enhance bioavailability. This review examines the challenges and advancements in PTX sourcing, production, physicochemical properties, anti-cancer mechanisms, clinical applications, trials, and chemo-immunotherapy. It aims to provide a comprehensive reference for the rational development and effective utilization of PTX.

4.Determination of Related Substances in the Novel Gi Protein-biased Opioid Receptor(MOR) Agonist LPM3480392 Active Pharmaceutical Ingredients

OBJECTIVE
To establish a determination method for the related substances of LPM3480392, a novel Gi protein-biased opioid receptor(MOR) agonist.
METHODS
The separation was carried out with Waters Symmetry Shield RP18 (150 mm×4.6 mm, 3.5 μm) by gradient elution method, using a mixture of 0.002 5 mol·L–1 sodium 1-octanesulfonate monohydrate in 0.01 mol·L–1 potassium dihydrogen phosphate-water solution(containing 0.1% triethylamine, adjusted pH to 2.50 with phosphate acid) and acetonitrile as the mobile phase at a flow rate of 1.0 mL·min–1 and the UV detection wavelength was set at 210 nm.
RESULTS
The chromatographic peaks of LPM3480392 and impurity A, impurity B, impurity C, impurity E and impurity F could be completely separated, the linear relationship of LPM3480392 was good in 0.064 9−5.191 2 μg·mL–1, while impurity A, impurity B, impurity C, impurity E and impurity F showed good linear relationship within 0.066 6−7.610 4 μg·mL–1, 0.166 0−3.794 0 μg·mL–1, 0.209 2−4.463 2 μg·mL–1, 0.167 9−7.672 6 μg·mL–1 and 0.016 4−7.505 7 μg·mL–1, respectively. The recovery rate was within 93.0%−103.2%.
CONCLUSION
The method is suitable for the determination of related substances in LPM3480392, and can provide valuable reference for the follow-up research and development of LPM3480392.
5.Mining and research on the adverse event signal of exenatide microspheres based on FAERS database

Objective To explore the risk signals of adverse events(ADE)in the use of exenatide microspheres by the FDA adverse event reporting system(FAERS),and provide reference for clinical rational drug use and drug safety.Methods With exenatide microspheres as the target drug,the search keywords were Exenatide Microspheres for Injection,LY05006,AC 2 993 LAR and Bydureon.SAS software was used to extract the ADE report data from January 2,2012 to March 31,2023 in the FAERS database and the duplicates were removed.Data mining of exenatide microspheres-related ADE reports was performed by the reporting odds ratio method and the comprehensive standard method.Results A total of 27 248 exenatide microspheres-related ADE reports were retrieved,involving 27 SOCs,of which 4 719 were severe ADE reports.The reporting personnel were mainly consumers(18 435 cases,67.66%),the United States was the mainly reporting country(26 295 cases,96.50%).A total of 163 ADE risk signals were obtained by reporting odds ratio method and comprehensive standard method,including new adverse reactions such as abnormal blood cholesterol,elevated lipase and mixed hyperlipidemia.Conclusion Based on the FAERS database,the post-marketing ADE of exenatide microspheres was mined and analyzed,which could provide reference for clinical medication safety and improvement of patients'medication compliance.
6.Clinical characteristics and virus identification of 4 cases of monkeypox confirmed in Zhejiang province

Objective:To analyze the clinical characteristics of monkeypox patients.Methods:The clinical data and laboratory findings of 4 patients with monkeypox patients diagnosed at Yiwu Central Hospital in July 2023 were analyzed. Herpes fluid and skin tissue samples were collected, the viruses were isolation and cultured in African green monkey kidney cells (Vero) and identified with whole gene sequencing.Results:All four patients were male, aged 24-35 years. All patients had male-to-male behavior within 21 days before onset of the disease. Among them, one patient has AIDS and one patient has syphilis. Four patients presented with perineal skin lesions with itching, and 3 patients were found to have enlarged lymph nodes upon admission. Laboratory testing: lymphocyte abnormality (4.57×10 9/L) in 1 case; increased procalcitonin (0.25 ng/mL) in 1 case; elevated IL-10 levels ( 7.11 ng/L and 9.42 ng/L) in 2 cases; increased IL-6 (66 ng/L) and IL-4 (3.24 ng/L) in 1 case, respectively. One case had abnormal myocardial zymogram with a elevated lactate dehydrogenase level of 313 U/L. The monkeypox virus was isolated from lesion tissue and herpes fluid, and the whole gene sequencing identified it as the B. 1.3 subtype of the IIb evolutionary branch, exhibiting typical pathological effects on Vero cells. Conclusion:The clinical manifestations of the 4 monkeypox patients confirmed in Zhejiang province are mild, patients had a definitive history of male-to-male sexual behavior and the virus strains belong to the B. 1.3 lineage of the IIb evolutionary branch.
7. Advances in immunotherapy of extranodal NK/T cell lymphoma

Extranodal NK/T-cell lymphoma (ENKTCL) is a relatively rare group of highly aggressive non-Hodgkin′s lymphoma (NHL). The disease has rapid clinical progress, high degree of malignancy and poor prognosis. Traditional chemoradiotherapy regimens have not shown good efficacy. In recent years, the immunotherapy of tumors has developed rapidly. At present, it has shown strong therapeutic activity in the treatment of various solid tumors such as non-small cell lung cancer, prostate cancer, melanoma and kidney cancer. Multiple tumor immunotherapy drugs have been approved by the US Food and Drug Administration (FDA) for clinical use. This article reviews recent novel immunotherapeutic regimens of ENKTCL, hoping to change the treatment modality of this malignant disease.
8.Hippocampal subfield volume alteration in post-traumatic stress disorder: a magnetic resonance imaging study.

In the current study, we aim to investigate whether post-traumatic stress disorder (PTSD) is associated with structural alterations in specific subfields of hippocampus comparing with trauma-exposed control (TC) in a relatively large sample. We included 67 PTSD patients who were diagnosed under Diagnostic and Statistical Manual of Mental Disorders (4th Edition) (DSM-Ⅳ) criteria and 78 age- and sex-matched non-PTSD adult survivors who experienced similar stressors. High resolution T1 weighted images were obtained via a GE 3.0 T scanner. The structural data was automatically segmented using FreeSurfer software, and volume of whole hippocampus and subfield including CA1, CA2-3, CA4-DG, fimbria, presubiculum, subiculum and fissure were extracted. Volume differences between the two groups were statistically compared with age, years of education, duration from the events and intracranial volume (ICV) as covariates. Hemisphere, sex and diagnosis were entered as fixed factors. Relationship between morphometric measurements with Clinician-Administered PTSD Scale (CAPS) score and illness duration were performed using Pearson's correlation with SPSS. Comparing to TC, PTSD patients showed no statistically significant alteration in volumes of the whole hippocampus and all the subfields ( > 0.05). In male patients, there were significant correlations between CAPS score and volume of right CA2-3 ( = 0.197, = 0.034), right subiculum ( = 0.245, = 0.016), and duration statistically correlated with right fissure ( = 0.247, = 0.016). In female patients, CAPS scores significant correlated with volume of left presubiculum ( = 0.095, = 0.042), left subiculum ( = 0.090, = 0.048), and left CA4-DG ( = 0.099, = 0.037). The main findings of the current study suggest that stress event causes non-selective damage to hippocampus in both PTSD patients and TC, and gender-specific lateralization may underlie PTSD pathology.
9.Clinical study on the treatment of allergic rhinitis by rhinitis desensitization prescription with nebulization suction

Objective To observe the clinical effect of rhinitis desensitization prescription with nebulization suction in the treatment of allergic rhinitis.Methods 100 patients of allergic rhinitis were randomly divided into control group(50 cases)and study group(50 cases).The control group was given conventional treatment,study group on the basis of conventional therapy was given rhinitis desensitization atomization inhalation.The clinical efficacy was compared between the two groups.Results After 1 course,three course,1 month of treatment,the total effective rates of the study group were 90%,92%,96%,which of the control group were 86%,88%,and 90% respectively,the differences between the two groups were statistically significant (χ2 =3.87,4.10,4.30,all P <0.05).Conclusion Rhinitis desensitization atomization inhalation in the treatment of allergic rhinitis has good curative effect,and it has high clinical application value.
10.Prokaryotic expression of coxsackievirus group A type 16 VP1 protein

Objective To construct the recombinant plasmid of coxsackievirus group A type 16 (CA16)VP1,express in E.coli and identify expressed product. Methods The CA16 VP1 gene was amplified by PCR,The constructed recombi-nant plasmid pET21b-CA16 VP1 was transformed into E.coli BL21(DE3) for expession and purification,expressed and purified product was identified by SDS-PAGE,Indirect ELISA tested Antigenicity and validity of recombinant proteins CA16 VP1. Results The coxsackievirus group A type 16 VP1 protein was expressed in peokaryotic cells and purified, which showed specific reaction with mice antiserum against CA16 virus,high reactivity with part serums of the elderly population in Taiyuan city. Conclusion CA16 VP1 protein was successfully expressed, had good Antigenicity and va-lidity,which will lay a foundation for study on the CA16 diagnostic kits.
