Objective To investigate the association between nutritional status and serum hepcidin and pentraxin 3(PTX3)levels in patients undergoing maintenance hemodialysis(MHD).Methods A total of 76 patients with MHD who met the inclusion criteria were recruited from the hemodialysis center at the Affiliated Hospital of Chengde Medical University.Nutritional status was assessed using the Subjective Global Assessment(SGA),which categorizes patients into three grades:SGA-A,SGA-B,and SGA-C.Serum levels of PTX3 and hepcidin were mea-sured by enzyme-linked immunosorbent assay(ELISA).Statistical analyses were conducted using SPSS software.One-way analysis of variance(ANOVA)was applied to compare differences across the three SGA groups.Logistic regression analysis was performed to identify influencing factors, and receiver operating characteristic(ROC)curve analysis was used to evaluate diagnostic value.Among the participants,45 patients were classified as malnourished based on SGA-B and SGA-C scores, while those with SGA-A constituted the well-nourished control group. Results Among the 76 MHD patients,59.2%were malnourished. We then compared clinical characteristics across the three groups. The results showed that the malnourished group was older and exhibited significantly higher levels of hs-CRP,PTX3,and hepcidin(P<0.05),while serum albumin, creatinine, and phosphorus levels were significantly lower(P<0.05).Pearson correlation analysis revealed positive correlations between SGA grades and hs-CRP,PTX3,and hepcidin levels(all P<0.05).Logistic regression analysis with"malnutrition"as the dependent variable indicated that elevated hs-CRP,PTX3,and hepcidin levels, along with age and male gender, were associated with increased risk of malnutrition in MHD patients, whereas higher serum phosphorus and creatinine levels were protec-tive factors.Further multivariate logistic regression analysis demonstrated that serum PTX3 level was an independent risk factor for malnutrition(P=0.032),while higher creatinine level was an independent protective factor(P=0.047).ROC curve analysis showed that the combination of serum PTX3 and creatinine levels had a high diagnostic value for identifying malnutrition in MHD patients, yielding an AUC of 0.789(P<0.001),a Youden index of 0.448,sensitivity of 77.8%,and specificity of 71.0%.Conclusions Elevated levels of PTX3 and hepcidin, along with reduced serum creatinine levels, are associated with an increased risk of malnutrition in patients undergoing MHD.Notably, elevated serum PTX3 and decreased serum creatinine independently predict malnutrition in this population and demonstrate high predictive value.

Objective To explore the correlation between the expression level of interferon gamma(IFN-γ)and the severity of patients with septic shock and its prognostic value. Methods The clinical data and serum of 96 septic shock patients admitted to the Department of Critical Care Medicine, the First Affiliated Hospital of Guangxi Medical University from March 2022 to August 2023 were collected, and divided into survival group and death group according to the 28-day outcome;collected at the same time Sera from 30 healthy people undergoing physi-cal examination during the same period served as the control group. The enzyme-linked immunosorbent(ELISA)method was used to detect the expression levels of IFN-γ in the three groups, and the expression levels of IFN-γ and various clinical data between the groups were analyzed. Results The serum IFN-γ expression level of patients with septic shock was lower than that of healthy people undergoing physical examination,212.80(151.11,255.79)ng/L compared with 343.37(314.5,427.95)ng/L,P<0.01.Among the 96 cases of septic shock,There were 54 cases in the survival group and 42 cases in the death group. The clinical data of the two groups were com-pared.The gender, smoking history,SBP,DBP,SI,Lac,IL-6,IFN-γ,PCT,SOFA score, and APACHEⅡscore of the two groups of patients were compared. The difference is statistically significant(P<0.01).Logistic regression analysis showed that decreased expression of Lac,IFN-γ,and APACHEⅡ score were independent risk factors for death in patients with septic shock.[Odds ratio(OR)and 95%confidence interval(95%CI)were 6.491(1.404~30.004)respectively.0.954(0.954~0.999),3.476(1.210~9.984),P<0.05].The Spearman correlation analysis of INF-γ,Lac and APACHEⅡ showed a negative correlation, and the correlation coefficients were-0.38 and-0.35 respectively.Drawing the ROC curves of Lac,IFN-γ,and APACHEII,the AUCs of the three were 0.847,0.869,and 0.833 respectively. The AUC of the three joint predictions was 0.978.The joint prediction value of the three was better than that of a single indicator,P<0.001.Conclusion The decrease in IFN-γ expression level and the severity of septic shock patients have good prognostic value.

Objective To evaluate the therapeutic effects of metformin hydrochloride and rosiglitazone maleate tablets on insulin resistance and androgen concentration in obese and nonobese women with polycystic ovarian syndrome (PCOS).Methods Seventy women with PCOS were enrolled into this study according to the diagnostic criteria of Rotterdam conference (2003).The women with PCOS were divided into the over weight/obese group (body mass index ≥ 25 kg/m2,n =34) and the nonobese group (body mass index<25 kg/m2,n =36).All women with PCOS took metformin hydrochloride and rosiglitazone maleate tablets twice daily for 3 months. The following indexes were measured before and after the treatment:height, weight, waist circumference, hip circumference, fasting plasma glucose, fasting plasma insulin, luteinizing hormone (LH),follicle stimulating hormone (FSH),total testosterone, free testosterone, follicular number and ovarian area, body mass index, waist to hip ratio, and homeostasis model assessment for insulin resistance (HOMA-IR) were calculated. Results (1) Compared with the nonobese group, dramatic increases in HOMA-IR and follicle number were observed in over weight/obese group(P<0.05),while LH,LH/FSH,total testosterone, free testosterone showed no significant difference between two groups.(2) After the treatment with metformin hydrochloride and rosiglitazone maleate tablets, weight, body mass index, fasting plasma glucose, fasting plasma insulin,HOMA-IR significantly decreased in two groups (P < 0.05).There were also significant declines of LH,LH/FSH,free testosterone, ovarian area, follicular number, especially in the over weight/ obese women with PCOS (P<0.05),while total testosterone and FSH did not change significantly.(3) The incidence of gastrointestinal adverse reactions in nonobese and over weight/obese group was 9.3% and 6.3% respectively, and no liver toxicity, edema, hypoglycemia and other adverse reactions was observed during treatment. Conclusion Compared with the nonobese group, there were severer insulin resistance and more marked polycystic change of ovary in over weight/obese women with PCOS,while total testosterone and free testosterone showed no significant difference between two gruops.Metformin hydrochloride and rosiglitazone maleate tablets can ameliorate insulin resistance and polycystic ovary, and also reduce body mass index and free testosterone in women with PCOS particularly in the over weight/obese individuals.
