ObjectiveTo explore the effect of individualized intermittent catheterization (IC) guided by 24-hour dynamic urine volume monitoring on bladder function and urinary tract infection complications in patients with neurogenic bladder (NB). MethodsA total of 50 NB patients admitted to the First Affiliated Hospital of Nanjing Medical University from August, 2025 to June, 2026 were enrolled. The patients were randomly divided into control group (n = 25) and experimental group (n = 25). The control group received routine timed/empirical IC, and the experimental group adopted dynamic urine volume-guided individualized IC, for a week. Urine leukocyte positive rate, urine bacterial positive rate, frequencies of single catheterized volume > 400 mL, 24-hour catheterization and 24-hour urine leakage were recorded before and after intervention. ResultsAfter intervention, the positive rates of urine leukocytes and urine bacteria were lower in the experimental group than in the control group (χ² > 5.094, P < 0.05), while the frequencies of single catheterized volume > 400 mL, 24-hour catheterization and 24-hour urine leakage were less (t > 2.078, P < 0.05). ConclusionThe individualized IC guided by 24-hour dynamic urine volume monitoring can accurately match individual urine secretion rhythm, reduce over-distension of bladder and unnecessary catheterization, and lower the incidence of urine leakage and urinary tract infection.

Objective To investigate the needs of the main caregivers of patients with spinal cord injury following neurogenic bowel dysfunction to participate in the whole process of intestinal management.Methods Nine main caregivers of patients with spinal cord injury in Jiangsu Province Hospital from January to Decem-ber,2024 were selected with objective sampling method.They were investigated face-to-face semi-structured in-depth interviews,and the data were analyzed,summarized and extracted by Colaizzi 7-step analysis method.Results Three themes and eight sub-themes were extracted:the needs of professional knowledge and skills standards in intestinal management(lack of intestinal training knowledge,lack of ability to acquire knowledge and different standards of medical institutions);the needs of physical ability and decision making ability participating in intesti-nal training(lack of participation,single decision-making behavior,excessive dependence on medical personnel);the desire for therapeutic benefits and the needs for building psychological confidence(concerns about the pa-tient's prognosis,overdependence on medication).Conclusion Clinical medical staff should pay attention to the actual needs and difficulties faced by the main caregivers of patients with neurogenic bowel dysfunction to participate in the entire intestinal management,and construct targeted training programs,strengthen the skills and knowledge training,to enhance their autonomy and responsi-bility,to achieve the rehabilitation goal better.

Objective.To study the changes of microcirculation in patients with diabetic retinopathy(DR).Methods.Examination were performed in 153 cases of type Ⅱ diabetes mellitus, among them,72 cases were male,81 cases were female,mean age 57.0±10.0 years, mean disease course 8.2±7.5 years.All cases were examined fundi by ophthalmologist,urinary albumin excretion rate (UAE)in 24 hours was measured by radioimmunoassay.Moreover,we examined the blood glucose,blood pressure,blood viscosity and observed the changes of naifold microcirculation.Results.It was found that there were more evident disturbance of microcirculation,markedly slowed velocity of blood flow(P＜0.05),significantly increased aggregation of blood cells(P＜0.05)and exudation around the loop(P＜0.05)in the group with DR,compared with the group without DR.Conclusion.It was more evident disturbance of nailfold microcirculation in patients with diabetic retinopathy.
