Osteoporosis is a systemic metabolic disease characterized by decreased bone mass, leading to an increased risk of fractures. Although osteoporosis in children and adolescents is rare, its incidence in younger populations is showing an increasingly notable trend. The diagnostic criteria for osteoporosis in children and adolescents include a bone mineral density(BMD) Z-score of≤-2.0 accompanied by a significant fracture history, defined as two or more long bone fractures before the age of 10, three or more long bone fractures before the age of 19, or the presence of low-energy vertebral compression fractures even in the absence of low BMD. The genetic causes and underlying mechanisms of pediatric osteoporosis remain largely unknown, requiring further research to elucidate the molecular pathways involved. Such advances could help reduce the disease′s impact on growth and development and improve the quality of life in affected children and adolescents.

3.Research on Influence of Cervus and Cucumis Polypeptide on the Bone Biomechanical Properties of Ovariectomized Rats

[Objective]To study whether Cervus and Cucumis Polypeptide(CCP ,Lugua polypeptide) can treat osteoporosis or not. [Methods]Forty 4-month-old female SD rats were randomized into four groups(10 in each group) ,including SHAM,0VX, OVX+CCP1 and OVX+CCP2 groups. Male rats in OVX groups take the ovariectomized surgery. Since the 13th week, rats in groups A and B got 0.9% physiological saline injection once in a day, group C was treated with CCP 0.4mL/(kg·d)injection and group D treated with CCP0.8mL/(kg·d) injection once one day. Twelve weeks later, al rats were sacrificed for obtaining specimen. Took right femurs, measured the bone biomechanical properties. [Results ]After 12 weeks since injection, the biomechanical property of right femurs in group OVX was significantly lower than that of the SHAM group( P<0.01); parameters such as maximum load and elastic load in group OVX+CCP1 and OVX+CCP2 were higher than that of group OVX(P<0.05), but lower than that of group SHAM(P<0.05). The distinction of the above parameters was significant( P<0.05)between group OVX+CCP1 and OVX+CCP2. [Conclusion]1.CCP can significantly im-prove the bone internal biomechanical properties in ovariectomized(OVX) rats, so it has function to treat osteoporosis caused by drop in estrogen level. There are differences of bone biomechanical properties aspects when treated with different dosage of CCP, which shows the tendency of dose-dependent effect. So we can conclude that CCP may have dose-dependent effect.
4.Risk evaluation of failed internal fixation for intertrochanteric fracture

Objective To analyze and summarize the risk factors of failed internal fixation for intertrochanteric fracture.Methods From April 2008 to April 2011,267 patients with intertrochanteric fractures in 4 hospitals were treated with internal fixation.The relationship between the failure of internal failure and possible factors as age,gender,hypertension,diabetes,the abuse of alcohol and tobacco,use of glucocorticoid,the degree of osteoporosis and fractures type were studied.According to the surgical risk assessment table,the patients were divided into low-risk,mid-risk,and high-risk group.The rate of internal fixation failure was compared in the 3 groups.Results We found 42 cases which showed radiographic failures.The internal fixation failure directly related with advanced age,diabetes,severe osteoporosis,unstable type fracture,but not gender,hypertension,the abuse of alcohol and tobacco,use of glucocorticoid.Risk factors of internal fixation failure included diabetes,osteoporosis degree,and fracture stability.Failed intertrochanteric fracture fixation mainly occurred in the mid-risk and high-risk groups.Conclusion Severe osteoporosis,unstable fracture,diabetes are risk factors of failure of intertrochanteric fracture fixation.These factors will affect the quality of surgery.For the patient with intertrochanteric fractures in the low-risk groups,internal fixation should be the first choice for treatment.For the patients in the mid-risk and high-risk group,internal fixation should be applied cautiously.For the aged patients in high-risk groups,hip arthroplasty is a wise option.
5.Effects of the Principium of Replenishing Qi and Nourishing Kidney Deficiency on Collum Femoris of Bone Histomorphometry in Ovariectomized Rats

[Objective] To explore the bone histomorphology effect of QiangGuYin on neck of femur of ovariectomized rats.[Methods]Ninety femal SD rats,weight 230~280 gram,were rangdonmized into three groups:QiangGuYin group(treatment group),Miacalcic group(contrast group)and control group.All rats were variectomized.After 10 weeks,the osteoporosis model succeed,the treatment group,contrast group were given QiangGuYin,Miacalcic respectively.The treatment group used QiangGuYin everyday,1ml/g.The contrast group used Miacalcic 0.72u/kg,everyday hyodermic.And 3,4.5,6 months after the treatment,kill 10 rats to explore.The neck of femur were cut and stined with Masson Golder Trichrome for bone histomorphomemetric contrast group analysis.[Result]The treatment group ovariectomized rats' in 3m,BV/TV compared with statistical significance;compared with control group in Tb.sp,BV/TV,Tb.Th with statistical significance.After 4.5m treatment group Tb.Th compared with contrast group have statistical significance.Give the medicine 6m,Tb.sp,Tb.Th compared with contrast group all have statistical significance.[Conclusion]Giangguyin can effectively improve the histomorphometry index.The mechanism probably is stimulating osteogenesis and supperssing the cytoactive of osteoclast and trend of the high bone transform.
