1.Effectiveness of orthopedic surgery for 247 patients with moderate and severe hallux valgus.

OBJECTIVE:
To summarize the effectiveness of orthopedic surgery for patients with moderate and severe hallux valgus and analyze its related influencing factors.
METHODS:
A clinical data of 247 patients (287 feet) with moderate and severe hallux valgus, who were admitted between January 2013 and October 2024 and met the selection criteria, was retrospectively analyzed. There were 39 males and 208 females, with a median age of 57 years (range, 19-89 years). There were 207 cases of single-foot involvement and 40 cases of double-foot involvement; 159 feet were moderate hallux valgus and 128 feet were severe hallux valgus. The disease duration ranged from 3 months to 25 years, with a median of 5 years and 8 months. The hallux valgus angle (HVA), the intermetatarsal angle (IMA), proximal articular set angle (PASA), and the American Orthopaedic Foot and Ankle Society (AOFAS) scores were measured before operation and at 6 months after operation, and the differences (change values) between pre- and post-operation were calculated. All patients were grouped according to the degree of preoperative hallux valgus deformity and age, and the patients with severe hallux valgus according to different surgical procedures, and the change values of HVA, IMA, and AOFAS scores were compared between groups. All patients were grouped according to postoperative HVA, then the postoperative AOFAS scores were compared between groups.
RESULTS:
All patients successfully completed the operations and were followed up 6 months to 11 years and 3 months, with an average of 4 years and 6 months. The HVA, IMA, PASA, and AOFAS scores at 6 months after operation showed significant improvement compared to preoperative levels, and the differences were significant ( P<0.05). The patients with severe hallux valgus had the higher change values of HVA, IMA, and AOFAS scores than the patients with moderate hallux valgus ( P<0.05). The elderly patients had the highest change values of HVA and AOFAS scores than the young and middle-aged patients ( P<0.05). The patients with postoperative HVA ranging from 0° to 5° had the highest AOFAS scores than the other patients at 6 months after operation ( P<0.05). Among different surgical procedures for severe hallux valgus, the metatarsophalangeal joint fusion had the highest change value of HVA, the Scarf osteotomy had the highest performance in correcting the IMA, and the first metatarsal base osteotomy had the highest improvement in the postoperative AOFAS score, and the differences were significant ( P<0.05).
CONCLUSION
Elderly patients show the better improvement in HVA and foot function after operation. The first metatarsal base osteotomy show the better improvement in foot function than other surgical procedures. A certain HVA is allowed to remain after hallux valgus correction, and the postoperative AOFAS score is higher when the corrected HVA is in the range of 0°-5°.

2.Unveiling the metabolic fate of drugs through metabolic reaction-based molecular networking.

Effective annotation of in vivo drug metabolites using liquid chromatography-mass spectrometry (LC-MS) remains a formidable challenge. Herein, a metabolic reaction-based molecular networking (MRMN) strategy is introduced, which enables the "one-pot" discovery of prototype drugs and their metabolites. MRMN constructs networks by matching metabolic reactions and evaluating MS2 spectral similarity, incorporating innovations and improvements in feature degradation of MS2 spectra, exclusion of endogenous interference, and recognition of redundant nodes. A minimum 75% correlation between structural similarity and MS2 similarity of neighboring metabolites was ensured, mitigating false negatives due to spectral feature degradation. At least 79% of nodes, 49% of edges, and 97% of subnetworks were reduced by an exclusion strategy of endogenous ions compared to the Global Natural Products Social Molecular Networking (GNPS) platform. Furthermore, an approach of redundant ions identification was refined, achieving a 10%-40% recognition rate across different samples. The effectiveness of MRMN was validated through a single compound, plant extract, and mixtures of multiple plant extracts. 3.Effect of muscle energy technique on dynamic postural control and lumbar neuromuscular function in patients with non-specific low back pain:a randomized controlled trial

Objective To explore the effect of muscle energy technique(MET)on dynamic posture control and lumbar neuromuscu-lar function in patients with non-specific low back pain. Methods From March to June,2022,30 college students with non-specific low back pain from Shandong Sport Universi-ty were randomly divided into control group(n=15)and intervention group(n=15).The control group received health education,and the intervention group received MET,for four weeks.They were assessed with Visual Ana-logue Scale(VAS)of pain,Oswestry Disability Index(ODI),Y-balance test and trunk flexion-relaxation test be-fore and after intervention. Results VAS scores decreased in both groups after intervention(|t|>2.449,P<0.05),and it was less in the intervention group than in the control group(t=-5.068,P<0.001);while ODI score decreased in the intervention group(t=4.785,P<0.001),and it was less in the intervention group than in the control group(t=-2.895,P=0.007);the performance of Y-balance test increased(t=-3.662,P=0.003)in the intervention group,as well as flexion-re-laxation ratio of multifidus(t=-2.460,P=0.029). Conclusion MET is effective on alleviating pain and lumbar dysfunction,improving dynamic posture control and en-hancing the function of the multifidus during flexion in patients with non-specific low back pain.
4.Effect of Orem self-care mode on disease management ability and quality of life in patients with diabetes

Objective To explore the effect of Orem self-care mode on disease management ability and quality of life of diabetic patients.Methods A total of 130 patients with diabetes were divided into the control group and the observation group by random number table method,65 cases in each group.The control group underwent routine nursing intervention,and the observation group were treated with Orem self-care mode.The intervention effects of the two groups were compared.Results After the intervention,the knowledge of diabetes management,the self-care ability and the improvement of quality of life in the observation group were better than those in the control group(P<0.05).Conclusion The application of Orem self-care mode can significantly improve patients'cognition of the disease and self-management ability,which is of great significance to improve patients'condition and improve their quality of life.
5.Effects of Muscle Electrical Stimulation Combined with Muscle Strength Training on Biomechanical Characteristics of Knee Joint in Patients with Patellofemoral Pain

Objective To investigate the effects of electrical stimulation combined with muscle strength training on knee joint biomechanical characteristics in patients with patellofemoral pain(PFP).Methods Forty-six patients with PFP were recruited and randomly assigned to the muscle strength training(MST)and electrical muscle stimulation with strength training(EMS)groups.The intervention was performed three times a week for six weeks.The anterior knee pain scale(AKPS)was used to measure the knee pain degree.Knee kinematics,dynamics,and surface electromyography(sEMG)data were collected using an infrared motion capture system,force platform,and sEMG system during drop jumps before and after the intervention.Two-way analysis of variance with repeated measures was applied to determine the differences between the dependent variables of the two groups before and after the intervention.Results Compared with pre-intervention,the AKPS score,vastus medialis oblique(VMO)activation,VMO/vastus lateralis(VMO/VL)activation,maximum knee flexion angle,and peak knee extension moment increased significantly in the EMS group;the maximum knee abduction,external rotation angle,and peak knee external rotation moment decreased significantly in the EMS group after intervention.Compared with pre-intervention,the AKPS score,maximum knee flexion angle,and peak knee extension moment increased significantly in the MST group after intervention,the peak knee abduction and external rotation moment significantly decreased in the MST group after intervention.Post-hoc comparisons indicated that compared with the MST group,the AKPS score,VMO activation,VMO/VL activation were significantly higher and the maximum knee abduction angle was significantly lower in the EMS group.Conclusions EMS contributes to the better balance muscle activation of the VMO and VL and corrects the excessive knee abduction angle during jump landing,which may be helpful in relieving pain and improving lower limb function in patients with PFP.
6.Effects of Wuwei Xiaoke Fang on intestinal flora and inflammatory factors in type 2 diabetes mellitus patients

Objective To explore the effect of Wuwei Xiaoke Formula on intestinal flora and inflammatory factors in patients with type 2 diabetes mellitus.Methods A total of 92 cases of type 2 diabetes patients with Qi-Yin deficiency syndrome were selected and randomly divided into treatment group(n=46)and control group(n=46).The control group was treated with basic treatment + metformin,and the treatment group was treated with Wuwei Xiaoke formula on the basis of the control group.The clinical efficacy,intestinal flora and inflammatory factor levels of the two groups were observed and compared.Results The two groups of patients were evenly distributed,with good baseline status and strong comparability.After treatment,the total effective rate of 91.30%in the treatment group was higher than that of 78.26%in the control group(P<0.05).After treatment,the number of Bacteroides,Lactobacillus and bifidobacterium groups in treatment group was higher than that in control group(P<0.05).The numbers of enterococcus,enterobacter and yeast were lower than those in control group(P<0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and C-reactive protein(CRP)in the treatment group were decreased more significantly than those in the control group(P<0.05).Conclusion Wuwei Xiaoke formula is helpful to improve the clinical symptoms of type 2 diabetes patients,regulate the disorder of intestinal flora in patients,reduce the level of inflammatory factors,and has good safety.
7.Study on the Mechanism of Gegen Qinlian Decoction for Lowering Blood Lipids and Preventing Blood Glucose Increase Based on Intestinal Flora

OBJECTIVE:To study the ef fects of Gegen qinlian decoction (GGQLD)on blood lipid and blood glucose of hyperlipidemia(HLP)model rats ,and to explore its mechanism from the perspective of intestinal flora. METHODS :Totally 48 rats were randomly divided into blank control group (n=8)and modeling group (n=40). For consecutive 5 weeks,model group was given high-lipid diet to induce HLP model ;blank control group was given routine diet. After modeling ,30 modeling rats were randomly divided into model group ,simvastatin group (positive control ,10 mg/kg),GGQLD high-dose ,medium-dose and low-dose groups (14.85,4.95,1.65 g/kg,by crude drug ),with 6 rats in each group. Blank control group and model group were given constant volume of normal saline intragastrically ;administration groups were given relevant medicine intragastrically ,once a day,for consecutive 11 weeks. At the same time ,each group was continuously given corresponding diet. After the last medication , body mass and body length of rats were determined ,and Lee ’s index was calculated. Serum levels of TG ,TC,HDL-C,LDL-C and fasting blood glucose (FBG)were determined in rats. DNA of rat caecum content was extracted for 16S rRNA V 3-V4 region sequencing. The Two-part model was used to analyze the correlation between intestinal flora with lipids and blood glucose. RESULTS:After 11 weeks of administration ,compared with blank control group ,the body mass ,body length ,Lee’s index , serum levels of TC ,TG,HDL-C and FBG of model group were increased significantly (P<0.05 or P<0.01),while the level of HDL-C was decreased significantly (P<0.05). Compared with model group ,body mass and Lee ’s index and serum levels of TG , FBG of rats in GGQLD high-dose group ,and serum levels of TC ,TG in GGQLD medium-dose group ,as well as serum level of TG of rats in GGQLD low-dose group was decreased significantly (P<0.05 or P<0.01). Correlation analysis with intestinal flora showed that TC and TG shared 3 operational taxonomic units (OTU),including OTU 559,OTU701 and OTU 135(OTU135 was also shared with FBG ),which were all positively correlated with the level of TC ,TG and FBG (P<0.01). The three OTU were annotated as Tyzzerella of Spirillaceae ,Anaerotruncus of Verrucaceae and Peptoclostridium of Streptococcidae ,respectively. High-dose and low-dose GGQLD had a down-regulating effect on Tyzzerella and Anaerotruncus(P<0.05 or P<0.01),while had up-regulating effect on Peptoclostridium(P<0.01). CONCLUSIONS :High-dose GGQLD (14.85 g/kg)can effectively reduce the body mass and blood lipid of HLP model rats ,and can prevent the abnormal increase of blood glucose of model rats. The mechanism may be associated with that the reduction of intestinal flora (Tyzzerella,Anaerotruncus)content.
8.Questionnaires analysis on present status of ventilation therapy in cardiopulmonary resuscitation in Chinese emergency physicians

Objective To investigate the current practice of ventilation during cardiopulmonary resuscitation (CPR) in Chinese emergency physicians. Methods Self-designed questionnaires were used to survey mainly the present situation of CPR ventilation practice performed by 800 physicians who participated in the Peking Union International Summit for Emergency Medicine from April 17th to 19th, 2015. Results A total of 800 questionnaires were distributed and 638 (79.75%) valid questionnaires were taken back; the responders joining the survey came from 29 provinces and regions, including Beijing, Hebei, Shandong, Guangdong, Liaoning, etc. There were 331 males and 307 females; 91.54% (584 responders) were emergency physicians and 77.90% (497 responders) came from tertiary hospitals, 17.55% (112 responders) came from the secondary hospitals. Regarding ventilation during CPR, 86.4% (551 responders) declared the patients was routinely given endo-tracheal intubation; after intubation, 25.8% (142 responders) adopted bag-mask ventilation, and 74.2% (409 responders) applied mechanical ventilation. When a ventilator was used, 301 (73.6%) responders used the volume controlled ventilation mode, 334 (81.7%) responders set the respiratory rate (RR) lower than 15 bpm, while 89.2% (365 responders) used the tidal volume set at a range of 400-500 mL. When adopted the flow triggering sensitivity, 79.7% (326 responders) set the sensitivity at 1-6 L/min, while 16.4% (67 responders) selected the default parameter, not adjusting the flow triggering parameter; when adopted the pressure triggering sensitivity, 75.1% (307 responders) set the sensitivity between -1 to -6 cmH2O (1 cmH2O = 0.098 kPa) and 20.3% (83 responders) selected the default value, not adjusting the pressure triggering parameter. When the mechanical ventilation (MV) was adopted, 84.8% (347 responders) declared often experiencing problems with MV, such as airway high peak pressure alarms [39.6% (162/409)], lower ventilation volume per minute alarms [24.9% (102/409)], higher respiratory frequency alarms [21.3% (87/409)], but only 67.2% (275 responders) would again adjust the ventilation mode related parameters and only 59.2% (242 responders) would observe the actual respiratory frequency. Conclusions With regards to artificial ventilation during CPR, the majority of emergency physicians tend to adopt endotracheal intubation and commonly use the volume controlled mode of mechanical ventilation; among the ventilator parameter setting, the RR is not strictly in accordance with the CPR guidelines, and most of the inspiration triggering sensitivity setting was too low, very easily to induce hyperventilation; simultaneously, the emergency physicians often neglect the practical RR; although there are many problems with ventilation such as frequent alarms, only 67.2% of the emergency physicians would again adjust the ventilation parameters.
9.A novel human immune system mice model for assessing the immunogenicity of cancer vaccines

Human immune system(HIS)mice are usually used to evaluate the ability of tumor vaccines to induce cytotoxic T lymphocyte(CTL)effects, but they failed to accurately reflect the ability of cancer vaccines to induce humoral immune responses. In this study, human peripheral blood mononuclear cells were isolated by density gradient centrifugation and co-transplanted into NCG mice with in vitro differentiated dendritic cells(DCs)to establish a DC-HIS mouse model. In DC-HIS mice, co-transplanted antigen-presenting cells(HLA-DR+CD11c+)could colonize the spleen of model mice. Moreover, co-transplantation of DCs significantly increased the proportion of activated human CD4+ T/CD8+ T cells and B cells in HIS mice, indicating that DC-HIS mice could better mimic the human immune responses. The immunogenicity of the targeted HER2 protein vaccine(NitraTh-HER2)was evaluated using the DCs-HIS mice. The results showed that the NitraTh-HER2 vaccine was able to induce the production of HER2-specific human IgG antibodies with a significant antibody-dependent cell-mediated cytotoxicity(ADCC)effect and the lysis rate of target cell SK-BR-3 reached 47. 1%. The NitraTh-HER2 vaccine was able to produce antigen-specific CTL effect, and the lysis rate of target cell SK-BR-3 reached 14. 6%. Taken together, the DC-HIS mouse model provides an effective method for predicting the immunogenicity of human tumor vaccines.

10. Analysis on the incidence of occupational pneumoconiosis of migrant workers in Shiyan city from 2013 to 2017

Objective:
To explore the incidence regularity and characteristics of pneumoconiosis among migrant workers in Shiyan city, and to provide a basis for making policies about pneumoconiosis prevention and treatment among migrant workers.
Methods:
In September 2018, 338 cases of pneumoconiosis among migrant workers and 354 cases of pneumoconiosis among non-migrant workers were collected between 2013 and 2017. The proportions of pneumoconiosis types, the age and seniority of onset workers, the severity grade of pneumoconiosis, the type of pneumoconiosis, the type of work and regional distribution, etc. were analyzed.
