1.Efficacy of different doses of methylprednisolone on AECOPD mice induced by influenza A virus infection

Objective To investigate the efficacy of varying doses of methylprednisolone(MP)on mice with acute exacerbations of chronic obstructive pulmonary disease(AECOPD)induced with influenza A virus(IAV).Methods Mouse model of COPD was established using LPS combined with smoking for 12 weeks,and then these COPD mice were treated with administration of 40 μL IAV via nasal drip to establish a AECOPD model.A total of 15 AECOPD mice were randomly divided into low-,medium-and high-dose MP groups,oseltamivir group and blank group.The body weight and survival time were monitored within 10 d after IAV infection.On days 1,3,and 5 post-treatment,lung function was assessed using whole-body plethysmography(WBP),inflammatory factors in bronchoalveolar lavage fluid(BALF)were quantified with ELISA,viral titers in BALF were determined using plaque assays,and colony-forming units were evaluated with blood agar plates.Immunofluorescence analysis:① Pulmonary immunofluorescence assay:Mice were randomly categorized into(n=4):LPS 1-day group,LPS 3-day group,and LPS+MP treatment group.All groups received an initial dose of LPS via atomization;subsequently,the LPS+MP treatment group received a single gavage dose of MP.Lung tissues were harvested from the 1-day LPS group on 1 d post-treatment,and from the 3-day LPS and LPS+MP groups on 3 d for immunofluorescence staining.② Cellular immunofluorescence assay:Mouse bone marrow neutrophils were classified into blank control(no intervention),LPS stimulation(LPS group),MP intervention with LPS stimulation(LPS+MP group),and MP intervention alone(MP group).The above cells were collected in 4 h after corresponding interventions for subsequent cellular immunofluorescence analysis.Results ①The medium-dose MP group demonstrated the most significant improvement in survival rate,weight recovery,and lung function when compared to other groups(P<0.05).② Treatment of medium-dose MP obviously reduced the levels of IL-6 and neutrophil extracellular traps(NETs)(P<0.05),while,elevated inflammatory factors and NETs were observed in the high-dose MP group on day 5 post-treatment.③ Notable decline in the lung injury score was found in the medium-dose MP group than the other groups(P<0.05).④The high-dose MP group exhibited substantial bacterial proliferation and delayed viral clearance since day 5 after treatment.Conclusion Medium-dose MP shows best efficacy in treatment of IAV-induced AECOPD,and the dose neither delays viral clearance nor increases the risk of bacterial infection following viral infection.
2.Epidemiological characteristics of human brucellosis in Yantai City, Shandong Province from 2020 to 2022

Objective:To investigate the epidemiological characteristics of human brucellosis in Yantai City of Shandong Province, and provide a basis for scientifically formulating brucellosis prevention and control strategies.Methods:A retrospective study was conducted to collect data on the epidemic situation of brucellosis in Yantai City from 2020 to 2022 from the "Infectious Disease Reporting Information Management System of China Disease Prevention and Control Information System". Descriptive analysis was conducted on the epidemic situation of brucellosis, the distribution of cases (region, time, population distributions), clinical symptoms and signs.Results:A total of 493 cases of brucellosis were reported in Yantai City from 2020 to 2022, including 129 cases in 2020, 172 cases in 2021, and 192 cases in 2022. The incidence rate was 1.905 3/100 000, 2.506 3/100 000, and 2.901 9/100 000, respectively. There were statistically significant differences between different years (χ 2 = 13.87, P = 0.001). The main distribution of brucellosis cases was in Laizhou City, Laiyang City, Zhaoyuan City, and Longkou City, accounting for 69.37% (342/493) of the total cases. Among them, Laizhou City had the highest proportion, accounting for 31.24% (154/493). There were reports of brucellosis cases in every month of the year, with the onset mainly concentrated from March to August, the cumulative cases accounted for 63.69% (314/493) of the total cases. In the population distribution, male cases were the main ones, with a male to female ratio of 2.24 ∶ 1.00 (341 ∶ 152). The age was (54.97 ± 13.59) years old, mainly concentrated in the age group of 45 - 59 years old, accounting for 43.20% (213/493). Farmers were the main occupation, accounting for 81.95% (404/493). The clinical symptoms of 493 cases of brucellosis were mainly fever, muscle and joint pain, and fatigue, accounting for 62.27% (307/493), 61.87% (305/493), and 61.46% (303/493), respectively. A few cases were accompanied by splenomegaly and hepatomegaly, accounting for 3.45% (17/493) and 1.22% (6/493), respectively. Direct contact with animals was the main route of infection for brucellosis cases, accounting for 57.00% (281/493). Conclusions:From 2020 to 2022, there are reports of brucellosis cases in every month of the year in Yantai City, with a high incidence in spring and summer. The main affected population is middle-aged male farmers, and direct contact with animals is the main route of infection. It is necessary to strengthen disease monitoring and health education for key populations during high incidence seasons.

6.Diagnosis and treatment process of a case of Streptomyces thermoviolaceus pneumonia and literature review

OBJECTIVE To report the diagnosis and treatment process of 1 case of Streptomyces thermoviolaceus pneumonia,and provide reference for the diagnosis and treatment of this type of infection by combining literature on Streptomyces pneumonia.METHODS A case study was conducted on a patient with S.thermoviolaceus pneumonia treated at the First Affiliated Hospital of Army Medical University.Additionally,a systematic literature review of Streptomyces pneumonia cases was performed.RESULTS The patient with S.thermoviolaceus presented with left lung consolidation and mass-like opacity.Initial diagnosis via matrix-assisted laser desorption/ionization time-of-flight mass spectrometry failed,but 16S rRNA gene amplification and sequencing confirmed S.thermoviolaceus as the causative pathogen.Six-month therapy with Amoxicillin capsules(1 g orally,three times daily)resulted in near-complete lesion resolution.The literature analysis of Streptomyces pneumonia revealed that 13 patients with Streptomyces pneumonia were included(including the patient reported in the article),age range of 18-77 years,more males(8 cases),and mostly suffering from underlying diseases.In terms of clinical symptoms,all enrolled cases exhibited cough,and some cases were accompanied by variable dyspnea.Imaging findings included that there was no characteristic predilection site for Streptomyces pneumonia lesions,and CT images commonly showed lung consolidation and bilateral nodules.Definitive diagnosis relied on 16S rRNA sequencing.Treatment regimens included tetracyclines,β-lactam drugs combined with enzyme inhibitors,ceftriaxone,aminoglycosides,macrolides,or carbapenems,administered for prolonged duration(6 months).Follow-up indicated a good prognosis,and only one mortality occurred.CONCLUSIONS 16S rRNA gene sequencing should be prioritized for diagnosing S.pneumonia.Effective antimicrobial options include tetracyclines,β-lactam drugs combined with enzyme inhibitors,ceftriaxone,aminoglycosides,macrolides,and carbapenems.Prolonged therapy correlates with favorable prognosis.

Gastric cancer(GC)is one of the most common malignant tumors of the digestive system.Human epidermal growth factor re-ceptor 2(HER2)is an important therapeutic target in GC.In recent years,anti-HER2 agents have made significant breakthroughs in GC treat-ment,providing more options for patients.To better guide the rational,effective,and safe application of anti-HER2 therapy in clinical prac-tice,the Gastric Cancer Committee of China Anti-Cancer Association assembled domestic experts in the field of GC.After multiple rounds of discussions,they systematically reviewed domestic and international evidence-based data and integrated it with clinical practices in China.As a result,they formulated the"Chinese expert consensus on anti-HER2 therapy for gastric cancer(2024 edition)"to address the use of anti-HER2 therapy in advanced and perioperative settings,conversion therapy,and key considerations during treatment.This consensus aimed to further standardize and guide the clinical practice of anti-HER2 therapy for GC.
8.The retrospective clinical study of asymptomatic primary hyperparathyroidism

Objective:To summarize and analyze the diagnostic and therapeutic characteristics of asymptomatic primary hyperparathyroidism (aPHPT).Methods:A retrospective analysis was conducted on the clinical data of 103 patients with aPHPT admitted to the Chinese PLA General Hospital from January 2012 to September 2023. The clinical characteristics, treatment modes, and prognoses of the patients were analyzed. GraphPad Prism 8.0 software was used for statistical analysis.Results:Among the 103 cases, there were 37 males and 66 females, aged from 25 to 78 years, with an average age of (53.81±11.34) years. Ninety-eight cases (95.15%) visited due to abnormal findings during physical examination and 5 cases (4.85%) due to hypertension, diabetes or other diseases. All patients underwent minimally invasive parathyroidectomy with small incision, with 96 cases (93.20%) pathologically diagnosed as adenomas and 7 cases as hyperplasia (6.80%). Postoperative mean serum calcium, parathyroid hormone (PTH) and alkaline phosphatase (ALP) levels were respectively significantly lower than preoperative levels, while postoperative serum phosphorus level was significantly higher than preoperative level ( P<0.05). The mean lesion volume was (3.32±6.72)cm 3 (range 0.05-49.50 cm 3). Patients with different lesion volumes had significant differences in preoperative serum calcium, PTH and ALP levels. Lesion volume was positively correlated to preoperative serum calcium(ρ=0.36, P<0.01), PTH(ρ=0.50, P<0.01) and ALP(ρ=0.39, P<0.01). Among 103 patients, 94 cases were followed up (91.26%), 9 cases were lost (8.74%), and the mean follow-up period was (60.15±29.23) months. The followed-up patients were alive and had no recurrence of lesions or complications, and their blood calcium levels were normal. Conclusion:aPHPT can be preliminarily diagnosed through blood biochemistry and imaging examination, and minimally invasive surgery can offer good prognosis without serious complications.

10.The risk and management of latent tuberculosis infection reactivation in patients with inflammatory bowel disease using biological agents and small molecule agents

Inflammatory bowel disease (IBD) is a kind of chronic inflammatory immune disease of the intestine. In recent years, biologics and small molecule agents with different targets have been used for the treatment of IBD. However, it also brings concerns about opportunistic infections, among which the reactivation of latent tuberculosis infection is more harmful. China is still one of the epidemic areas of tuberculosis, and clinicians attach great importance to the reactivation of latent tuberculosis infection induced by biologics and small molecule agents. This paper reviews the risk of latent tuberculosis infection reactivation associated with biologics and small molecule agents and the recommendation of the guideline. It is expected to provide a reference for the application of these medications in patients with inflammatory bowel disease complicated by latent tuberculosis infection.
