ObjectiveTo clarify whether METTL14 mediates the core role of acupuncture at Neiguan (PC6) in promoting myelination and improving behavior in young autistic rats through gene intervention technology. MethodsThe ASD model was established by intraperitoneal injection of valproic acid (VPA) in pregnant rats. Male offspring were intracerebroventricularly injected with adenovirus-packaged METTL14 shRNA (sh-METTL14) or its control (sh-NC) on postnatal day 1, with a model group set as well. Subsequently, the juvenile rats were divided into model group, acupuncture group, acupuncture+sh-NC group, and acupuncture+sh-METTL14 group. The acupuncture group received acupuncture at Neiguan (PC6) from postnatal day 7, once daily for 21 consecutive days. Neurobehavioral changes were evaluated by behavioral tests; METTL14 knockdown efficiency and the expression of METTL14, METTL3, and PTEN were detected by quantitative real-time PCR (qRT-PCR) and Western blot (WB); PTEN m6A levels were measured by RNA immunoprecipitation-qPCR (RIP-qPCR); myelin ultrastructure, expression of myelin basic protein (MBP) and neurofascin 155 (NF155), and dendritic spine density were observed using transmission electron microscopy (TEM), enzyme-linked immunosorbent assay (ELISA), immunofluorescence, qRT-PCR, and primary neuron culture. ResultsBehaviorally, knockdown of METTL14 significantly counteracted the beneficial effects of acupuncture in improving self-grooming, open field exploration, three-chamber social interaction, and Morris water maze learning and memory (P<0.05, P<0.01). Compared with the acupuncture+sh-NC group, the acupuncture+sh-METTL14 group showed significantly decreased mRNA and protein expression of hippocampal METTL14 (P<0.01), and the upregulating effects of acupuncture on METTL3 and PTEN expression were reversed (P<0.01). Meanwhile, knockdown of METTL14 significantly inhibited the acupuncture-induced increase in PTEN m6A levels (P<0.01). Morphologically, knockdown of METTL14 attenuated the improvement of myelin structure by acupuncture, reversed the downregulation of MBP and upregulation of NF155 induced by acupuncture, and blocked the increase in dendritic spine density (P<0.05, P<0.01). ConclusionMETTL14 is a key molecule mediating the therapeutic effect of acupuncture at Neiguan. Acupuncture at Neiguan upregulates METTL14, thereby enhancing m6A methylation modification of PTEN mRNA to stabilize its expression, ultimately promoting myelin development and improving behavioral symptoms in ASD juvenile rats. This preliminarily reveals the modern biological connotation of “opening Xuanfu and dredging myelin”.

3.Effects of transcranial alternating current stimulation combined with sertraline on cognitive function in patients with depressive disorder

BackgroundCognitive function is closely related to an individual's quality of life and social functioning, with approximately 20%~35% of patients with depressive disorder experiencing some degree of cognitive impairment even after clinical symptom remission. Existing evidence suggests that tACS can improve specific cognitive domains, such as memory function, while its effects on other cognitive dimensions, such as executive functioning, attention, and information processing speed, remain unclear. ObjectiveTo explore the effects of tACS on the multidimensional cognitive functions and emotional problems of patients with depressive disorder, thus to provide references for the treatment of depressive disorder. MethodsForty-nine patients with depressive disorder who were hospitalized in the Fourth People's Hospital of Wuhu from November 2022 to October 2024 and met the diagnostic criteria for depressive disorder outlined in the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5), were selected as study participants. Subjects were randomly divided into study group (n=23) and control group (n=26) based on Microsoft Excel. Both groups received sertraline treatment. The initial dose was 50 mg/day, which gradually titrated upward based on individual variability, drug tolerance, and therapeutic response, with a maintenance dose ranging from 100 to 200 mg/day. In addition, the study group underwent tACS therapy for 4 weeks, with 5 sessions per week, each lasting 20 minutes. The control group received sham stimulation, in which the stimulus was interrupted after the first 30 seconds. At baseline, the 4th week, and the 12th week of treatment, patients were assessed using the Hamilton Depression Scale-17 item (HAMD-17), Hamilton Anxiety Scale (HAMA), and MATRICS Consensus Cognitive Battery (MCCB). ResultsRepeated measures analysis of variance indicated that both the time effect and the time×group interaction effect for HAMD-17 scores were statistically significant between the two groups (F=260.437, 25.309, P<0.01). At week 12 of treatment, the HAMD-17 score in the study group was lower than that in the control group (t=4.236, P<0.01). For HAMA scores, the time effect, group effect, and time×group interaction effect were all statistically significant between the two groups (F=248.082, 4.506, 9.500, P<0.05 or 0.01). At weeks 4 and 12, study group reported lower HAMA scores compared with control group (t=4.580, 2.608, P<0.05 or 0.01). Regarding the MCCB scores for attention/vigilance, verbal learning, and overall composite, the time effect, group effect, and time×group interaction effect were all statistically significant between the two groups (F=70.331, 27.882, 51.679, 5.560, 10.948, 7.860, 8.490, 3.874, 5.025, P<0.05 or 0.01). After intervention, the study group showed significantly higher MCCB scores for attention/vigilance, verbal learning, and overall composite at both week 4 (t=-2.149, -3.530, -2.740, P<0.05) and week 12 (t=-3.534, -3.576, -3.838, P<0.01) when compared to the control group. ConclusionThe combined tACS and sertraline therapy may demonstrate superior efficacy to pharmacotherapy alone in the short term for improving attention/vigilance, verbal learning, overall cognitive function, and anxiety symptoms in patients with depressive disorders. Based on the 12-week outcomes, the combined tACS and sertraline therapy not only sustaine its previously observed advantages in improving cognitive domains and anxiety symptoms, but also demonstrate potentially superior efficacy over monotherapy in alleviating depressive symptoms. 4.Characteristics and management of pediatric pemphigus vulgaris

Pemphigus vulgaris (PV) is the most common subtype of pemphigus. It predominantly affects adults, with pediatric cases being exceedingly rare. Despite advancements in clinical treatment, the mortality rate of pediatric PV (PPV) has historically been alarmingly high, ranging from 70% to 100% in the absence of proper diagnosis and treatment. Although recent improvements in therapeutic strategies have led to a gradual decline in mortality, early and appropriate intervention remains crucial, particularly for children with acute onset and rapid disease progression, to prevent severe complications. However, due to the rarity of PPV, no standardized diagnostic and treatment guidelines are currently available. This study retrospectively analyzed 104 PPV cases recorded in the PubMed and China National Knowledge Infrastructure (CNKI) databases between 1969 and 2024, with the aim of providing insights for the standardized diagnosis and management of PPV. PPV presents with flaccid blisters affecting both cutaneous and mucosal surfaces. Upon rupture, these blisters result in painful, sharply demarcated erythematous erosions, accounting for approximately 1.4%-3.7% of all reported PV cases. The age of onset ranges from 1.5 to 18 years, with an average of 12.4 years, and no significant gender differences have been observed. In pediatric patients, the oral mucosa is typically the earliest and most frequently affected site, with an involvement rate as high as 87.3%, and it most commonly affects the buccal mucosa (27.9%). Other mucosal sites are affected in 52.9% of cases, with genital (28.8%) and perianal (6.7%) involvement being more frequent than in adult patients. Skin lesions are present in 80.4% of pediatric cases, a significantly higher rate than 16.0%-68.4% observed in adults. If lesions are relatively localized, local glucocorticoid therapy can be attempted first, with 8.3% of children achieving complete remission through local treatment alone. Systemic glucocorticoid therapy is the preferred option for cases that respond poorly to local therapy. Among these cases, 75.3% of pediatric patients were treated with prednisone, with 85.1% starting at an oral dose of 0.5-1.5 mg/kg/day, while 14.9% received an initial dose of 2 mg/kg/day. Alternative treatments, such as immunosuppressants, biologics, or other adjuvant medications, may be considered for pediatric patients who exhibit an inadequate response to glucocorticoid therapy or experience severe adverse effects. The most commonly used agents include azathioprine (24.0%), dapsone (21.7%), and rituximab (12.5%). The follow-up period for pediatric patients ranged from 1 to 120 months, with an average duration of 38 months. Prognosis in pediatric patients was more favorable compared to adults, with 43.8% achieving complete remission (cessation of treatment), 37.5% achieving partial remission (low-dose maintenance therapy), 9.6% still undergoing treatment, and only 1.1% succumbing to pneumonia or sepsis. Compared to adults, prolonged corticosteroid use in children poses a greater risk to physiological and psychological well-being, making them more susceptible to adverse effects related to growth, metabolism, and ocular health. Severe adverse reactions occurred in 22.1% of pediatric patients receiving corticosteroids, with Cushingoid facies (73.9%) and weight gain (39.1%) being the most common. In addition, 30.4% experienced growth and skeletal abnormalities, including growth retardation (17.4%), osteoporosis (8.7%), and fractures (4.3%). While PPV shares certain etiological, clinical, and histopathological characteristics with adult PV (APV), early diagnosis and timely intervention remain critical for optimal outcomes. Multidisciplinary collaboration is often necessary to ensure comprehensive management, improve treatment adherence, and safeguard the physical and psychological health of pediatric patients.
5.Research on hepatitis E virus antigen screening among voluntary blood donors

Objective: To investigate prevalence of hepatitis E virus (HEV) infection among voluntary blood donors in Dalian and provide evidence for enhancing blood screening strategies. Methods: A total of 3 277 blood donor samples collected between December 2023 and February 2024 at Dalian Blood Center underwent routine blood screening (ALT, HBsAg, anti-HCV, HIV Ag/Ab, anti-TP, and HBV/HCV/HIV NAT). Subsequently, HEV antigen (Ag) was detected using chemiluminescence immunoassay (CLIA). HEV-Ag reactive samples were further tested for HEV RNA, IgM and IgG antibodies. Blood donors with repeated reactive HEV Ag results were followed up to clarify the status of infection. Results: Among the 3 277 blood donor samples, 6 (0.18%) were repeatedly reactive for HEV Ag. However, supplemental testing for HEV RNA, anti-HEV IgM, and anti-HEV IgG on these samples yielded non-reactive results. One of these six blood donors was successfully followed up. On day 218 after the initial detection of HEV Ag reactivity, HEV Ag, HEV RNA, HEV IgM and IgG antibody were found to be non-reactive. Conclusion: The reaction rate for HEV antigen screening among voluntary blood donors in Dalian is low. CLIA method for detecting HEV antigen is easy to operate and cost-effective, but demonstrates some false reactivity. Improving the specificity of the assay and combining it with nucleic acid testing (NAT) would be valuable for implementing a selective HEV screening strategy for blood donors.
6.Analyzing the influencing factors of occupational burnout among disease control and prevention staffs in Sichuan Province

Objective To assess the situation and influencing factors of occupational burnout among the staff at the Center for Disease Control and Prevention (CDC) in Sichuan Province. Methods A total of 1 038 CDC staff members in Sichuan Province were selected as the study subjects using the stratified random sampling method. Occupational burnout of the staff was assessed using the Maslach Burnout Inventory General Survey via an online questionnaire. Results The detection rate of occupational burnout was 42.3% (439/1 038). Binary logistic regression analysis result showed that, after controlling for confounding factors such as education level and alcohol consumption, CDC staffs aged at 20-<31, 31-<41, and 41-<51 years were at higher risk of occupational burnout compared with those ≥51 years (all P<0.05). CDC staffs with 5-<10 or ≥10 years of service had higher occupational burnout risk compared with those with <5 years (both P<0.05). CDC staffs with poor or fair health status, irregular diet, and poor sleep quality had higher risk of occupational burnout compared with those healthy, have regular diet, and good sleep quality (all P<0.05). The risk of occupational burnout increased with higher overtime frequency (all P<0.05). Conclusion Occupational burnout among CDC staffs in Sichuan Province is relatively high. Age, years of service, health status, diet, sleep quality, and overtime frequency are key influencing factors.
7.Immunoregulatory mechanisms,development and application of terpenoid compounds from the Isodon genus

Plants of the Isodon genus are an important source of terpenoids, with their constituents exhibiting rich structural diversity and remarkable biological activities (such as anticancer, antimicrobial, and anti-inflammatory properties), demonstrating significant potential in the field of immunomodulation. This review summarizes recent advances in the immunomodulatory mechanisms, development and application of terpenoid compounds from the Isodon genus. It has been found that these compounds can modulate key inflammatory signaling pathways, such as nuclear factor-kappa B (NF-κB) and mitogen-activated protein kinases (MAPKs), thereby blocking the cascade amplification of inflammatory factors, alleviating chronic inflammatory responses, and correcting immune dysregulation. Additionally, they can influence the polarization direction of macrophages and dynamically regulate the balance among different functional subsets of T cells, restoring immune homeostasis. Their clinical translation faces multiple challenges, including poor druggability, a lack of systematic safety data, the absence of precise pharmacodynamic biomarkers, complexities in clinical trial design, and unclear industrialization pathways.
8.Relationship between spleen volume and non-alcoholic fatty liver disease by three-dimensional computed tomography reconstruction

Objective To investigate the association of spleen volume with the risk of non-alcoholic fatty liver disease(NAFLD)as well as their causal relationship.Methods We included 90 NAFLD cases and 47 healthy controls who had received contrast-enhanced computed tomography(CT)scan of the abdomen at the Second Affiliated Hospital of Xi'an Jiaotong University from November 2022 to November 2023.We conducted three-dimensional reconstruction of the spleen through a deep learning network model using a two-stage coarse-to-fine segmentation approach.We compared the two groups using the two-sample t test or Mann-Whitney U test for continuous data and using the chi-square test for categorical data;evaluated the correlation between spleen volume and liver function indicators through Pearson correlation or Spearman rank correlation analyses;determined the factors influencing the development of NAFLD through multivariable Logistic regression analysis;and further assessed the casual relationship between spleen volume and NAFLD using the inverse variance-weighted two-sample Mendelian randomization(IVW-MR)method.Results Spleen volume was significantly larger in NAFLD cases than in controls(272.93±104.16 vs 204.37±81.20 cm3,P<0.001).The Spearman rank correlation analysis showed that spleen volume was positively correlated with the hepatic steatosis index(rs=0.422,P<0.001)and gamma-glutamyl transferase levels(rs=0.211,P=0.047)in patients with NAFLD.The multivariable Logistic regression analysis indicated that spleen volume was an independent risk factor for the development of NAFLD(odds ratio[OR]=1.01,95%confidence interval[CI]:1.00-1.02,P=0.049).The IVW-MR analysis detected a causal relationship between spleen volume and NAFLD(OR=1.16,95%CI:1.05-1.28,P=0.005).Conclusion Increased spleen volume may be a risk factor for the development and progression of NAFLD.Further studies are still needed to investigate the specific mechanism.
9.The diagnostic value of combined clinical imaging model in deep infiltrating endometriosis of uterosacral ligaments

Objective To investigate the diagnostic value of combined clinical imaging model in deep infiltrating endometriosis of uterosacral ligaments(USL-DIE).Methods The preoperative clinical symptoms,signs,transvaginal ultrasound(TVS)and MRI diagno-ses of 102 DIE patients were reviewed and subjected to univariate analysis,and the significant variables were screened and included in the multivariable logistic regression analysis to construct a combined clinical imaging model.The diagnostic value of statistically signifi-cant factors and combined model was assessed and compared using receiver operating characteristic(ROC)curve.Results The inci-dence of USL nodule with tenderness,the detection rates of TVS and MRI for USL-DIE were higher than those of other patients with DIE,and the differences were statistically significant.TVS(P=0.021)and MRI(P<0.001)were independent factors in the diag-nosis of USL-DIE.The combined clinical imaging model had the highest area under the curve(AUC)(0.909)and specificity(0.929)for the diagnosis of USL-DIE.Conclusion The combined clinical imaging model has a higher value for the comprehensive diagnosis of USL-DIE compared with clinical or imaging diagnosis alone.
10.A practical study on basic medical research team construction and student cultivation mode in universities

This article reviews the challenges faced by research teams in China, and drawing on advanced management modes both in China and abroad, proposes a "dual optimization of management modes, stable and efficient platform support, and flexible and precise talent development" mode for research team construction and student cultivation. Specifically, this mode includes promoting flexible team management through enhanced two-way feedback between mentors and students, improving shared experimental platforms to increase resource utilization efficiency, and designing tailored training programs that align with individual student needs. The aim is to enhance team cohesion, improve the communication, collaboration, and innovation in basic medical research teams, and ultimately raise the quality of talent development in universities.
