1.CAR-T cell therapy for gastric cancer:advances and challenges from target discovery to clinical translation

Gastric cancer is a malignant tumor with high prevalence worldwide and limited therapeutic options.Chimeric antigen receptor T-cell(CAR-T)therapy has emerged as a promising approach for gastric cancer treatment;however,its application faces substantial challenges.This review provides comprehensive summary of the recent advances in CAR-T cell therapy for gastric cancer,systematic analysis of critical break throughs and core challenges from target discovery to clinical translation,and outlining of future perspectives.We describe the criter-ia for ideal target selection and highlight the current research landscape of major targets,including CLDN18.2 that demonstrated efficacy,and targets facing distinct challenges,including HER-2,CEA,EpCAM,and MUC1.This review also finely dissects three central barriers restrict-ing CAR-T cell efficacy,and discusses corresponding countermeasures:overcoming the immunosuppressive tumor microenvironment through strategies such as local delivery,armored CAR-T cells,and combination therapies;engineering approaches including affinity modula-tion and logic-gate designs to mitigate on-target/off-tumor toxicity;and optimization of manufacturing processes and reduction of costs via early leukapheresis,rapid production platforms,and universal CAR-T cell strategies.Future multidimensional,integrative,and innovative strategies are pivotal for achieving comprehensive break throughs in CAR-T cell therapy for solid tumors.
2.Synthesis of A New Naphthalenesulfonamide-based"Turn-on"Fluorescent Probe for Rapid Detection of Glyphosate

Widespread utilization of glyphosate has led to environmental residues,posing potential threats to ecological systems and human health.Traditional methods for detection of glyphosate are limited by specialized equipment and operational techniques,resulting in inefficient responses.Therefore,it is urgent to develop a convenient,sensitive and accurate detection method for detection of glyphosate.Herein,a new naphthalenesulfonamide-based"Turn-on"fluorescent probe was synthesized using 2-chloroaniline and dansyl chloride as raw materials through a one-step process,which showed a good linear relationship between the glyphosate concentration in concentration range of 0.003-70 μmol/L and the fluorescence intensity(R2=0.995),with a detection limit of 2.73 nmol/L(S/N=3).Analytical techniques such as nuclear magnetic resonance(NMR)spectroscopy and high-resolution mass spectrometry(HRMS)were used to investigate the interaction mechanism between the fluorescent probe and glyphosate.The results indicated that a nucleophilic substitution reaction occurred between the probe and the secondary amine(—NH—)of glyphosate,inducing a photoinduced electron transfer(PET)effect which enhanced the fluorescence intensity by 11.2 times.The probe showed good anti-interference ability towards coexisting metal ions,anions and pesticides in water.When applied to determination of glyphosate in the samples such as tap water,river water(Xiangxi River Reservoir),soil,soybeans,and corn,the spiking recoveries ranged from 94.7%to 109.9%,demonstrating the high accuracy and broad applicability of this detection method.A portable test strip based on this fluorescent probe was developed for rapid semi-quantitative analysis of glyphosate.The developed method was rapid,sensitive,and portable,providing theoretical and technical support for on-site measurement of environmental contaminants.
3.Exploration on the pathogenesis and treatment of gastric polyps from the theory of kenang

Gastric polyps, as a common digestive system disease, have shown an increasing detection rate and are regarded as one of the precancerous states of gastric cancer. In TCM, gastric polyps can be classified as diseases such as "stomachache" and "fullness and distension in the stomach". TCM holds that the disease is often related to the accumulation of pathogenic factors such as phlegm and blood stasis in the body to form "kenang", which has the characteristics of insidiousness, stubbornness, and recurrence. Deficiency of healthy qi and disorder of qi movement can lead to the adhesion of phlegm and blood stasis, which highly aligns with the etiology and pathogenesis of gastric polyps. The treatment principles are promoting blood circulation to remove blood stasis, regulating qi movement, and strengthening the body resistance to consolidate the essence. Understanding the etiology, pathogenesis, and treatment principles of gastric polyps from the theory of "kenang" has important guiding significance for clinical diagnosis and treatment, providing new ideas and methods for the treatment of gastric polyps.
4.Therapeutic effect of external counterpulsation on cardiac rehabilitation in patients with coronary heart disease-a meta-analysis

Objective:To evaluate the clinical rehabilitation effect of external counterpulsation(ECP)on patients with coronary heart disease(CHD)by meta-analysis.Methods:We searched the databases of CNKI,WanFang,VIP,CBM,PubMed,Web of Science,Cochrane Library and Embase for randomized controlled trials(RCTs)and prospective cohort studies upon rehabilitative effect of ECP on CHD patients before May 2024.And meta-analysis was conducted to calculate the pooled MD and 95%CI using the random(P<0.5 or I2≥50%)or fixed effect models(other situations).Results:13 eligible literatures were finally included in the meta-analysis.Compared with participants in control group,those in trial group had significant higher left ventricular ejection fraction(MD=4.15,95%CI 2.55~5.76,P<0.001),stroke volume(MD=9.11,95%CI 7.59~10.64,P<0.001),peak oxygen uptake(MD=5.42,95%CI 2.53~8.32,P<0.001),6-min walking distance(MD=31.14,95%CI 24.89~37.40,P<0.001),metabolic equiv-alent(MD=0.58,95%CI 0.45~0.71,P<0.001),exercise duration time(SMD=0.77,95%CI 0.55~0.99,P<0.001),oxygen pulse(MD=0.88,95%CI 0.68~1.09,P<0.001),and significant lower left ventricular end-diastolic diameter(MD=-3.19,95%CI-5.20~-2.61,P<0.001).Conclusion:This study showed that ECP could effectively improve heart function,exercise capacity and tolerance of CHD patients.

7.Effect of pristimerin on adriamycin resistance of osteosarcoma cells by regulating Hippo/YAP signal pathway

Objective To investigate the effect of pristimerin(PM)on adriamycin(ADM)resistance in osteosarcoma cells and its mechanism.Methods The ADM resistant cells MG-63/ADR was established in vitro by using the ADM increasing concentration gradient long-term culture method,CCK-8 was used to detect the effects of different concentrations of ADM on the viability of osteosarcoma cells MG-63 and MG-63/ADR cells to verify the drug resistance of ADM;subsequently,MG-63/ADR cells were randomly divided into the MG-63/ADR group(normal culture),low-dose PM group(adding 0.5 μmol/L PM),middle-dose PM group(adding 1.0 μmol/L PM)and high-dose PM group(adding 2.0 μmol/L PM).The cell viability of each group under different concentrations of ADM treatment was detected by CCK-8;the sensitivity of cells to ADM in each group was detected by plate clone formation assay;the cell morphology of each group was observed under microscope;the cell apoptosis level was detected by flow cytometry;Western blot was used to detect the expression of phosphorylated mammalian STE20-like protein kinase 1(p-MST1),MST1,phosphorylated large tumor suppressor 1(p-LATS1),LATS1,phosphorylated Yes associated protein(p-YAP)and YAP proteins.Results With the intervention of 25,125,625,3 125 and 15 625 ng/mL ADM,the activity of MG-63/ADR cells was significantly higher than that of MG-63 cells(P<0.05),which indicated that the ADM resistant cell model was successfully established.Compared with the MG-63/ADR group,the number of cells in the low,middle and high-dose PM groups decreased,and the cells gradually shrank,the spacing between cells became larger,and the cell viability,cell clonal formation rate and p-YAP/YAP level decreased(P<0.05),the apoptosis rate and the levels of p-MST1/MST1 and p-LATS1/LATS1 significantly increased(P<0.05).Conclusion The inhibitory effect of PM on ADM resistance in osteosarcoma cells may be related to the activation of Hippo pathway and the downregulation of YAP signal.
8.Analysis of risk factors for bile leakage after laparoscopic common bile duct exploration with primary closure

Objective To explore the risk factors for bile leakage after laparoscopic common bile duct exploration(LCBDE)with primary closure.Methods The clinical data of 560 patients with choledocholithiasis who underwent LCBDE with primary closure in Tianjin Hospital of Integrated Traditional Chinese and Western Medicine from September 2021 to September 2023 were retrospectively analyzed,and the patients were divided into the bile leak group and the non-bile leak group according to the occurrence of postoperative bile leakage.The risk factors affecting the occurrence of postoperative bile leakage were analyzed by multivariate analysis.Results A total of 64 cases(11.4% )experienced varying degrees of bile leakage,including 55 cases of grade A bile leakage,7 cases of grade B,and 2 cases of grade C.The thin common bile duct(OR=0.07,P<0.001),history of hypertension(OR=4.56,P<0.001),and high BMI(OR=1.17,P=0.002)were the risk factors for postoperative bile leakage in patients with choledocholithiasis.Conclusion Patients with thin common bile duct,hypertension and obesity are more likely to occur postoperative bile leakage.Patients with choledocholithiasis who have the above high-risk factors should be cautious in choosing LCBDE with primary closure.
9.The effect of learning input on core competencies of rural order-oriented medical students and the moderating role of professional commitment

Objective:To explore the influence of learning input and professional commitment on the core competence of rural order-oriented medical students,and to provide theoretical reference for improving the quality of the grassroots health workforce.Methods:520 rural order-oriented medical students from two medical colleges in S province were selected as research subjects.Stratified regression method was used to analyze the effect of learning input on the core competence of rural order-oriented medical students,and simple slope test was used to analyze the moderating effect of professional commitment on the aforementioned relationship.Results:The overall mean score of core competence of rural order-oriented medical students in S province was(3.39±0.549),learning input had a significant positive effect on the core competence of rural order-oriented medical students(β=0.358),and there was a moderating effect of professional commitment on the relationship between the two,which made the effect of learning input on core competence stronger(β=0.206).Conclusion:Improve the policy of training rural order-oriented medical students,strictly control the quality of admission,increase the construction and investment in grass-roots bases,emphasize students'vocational quality education,and enhance their core competencies.
10.Establish and evaluate effectiveness of three-tier review rules for parenteral nutrition prescriptions

Objective:To establish a three-tier review rule for parenteral nutrition orders by pharmacists,improve the qualification rate of parenteral nutrition orders,and promote the rational and standardized use of parenteral nutrition.Method:Pharmacists from different positions participate in a manual review intervention model for parenteral nutrition orders at three-tier review,establishing standardized and simplified parenteral nutrition review rules to reduce review difficulty and improve review efficiency.Result:After implementation,the qualified rate for parenteral nutrition orders increased from 71.09%to 95.59%,with a qualified rate of 100%for an industrial multi-cavity bag.The simplicity of parenteral nutrition components,cation concentration,carbohydrate-to-lipid ratio,non-protein calorie-to-nitrogen ratio and route of administration have been significantly improved.Conclusion:The involvement of pharmacists in the three-tier review of parenteral nutrition can reduce the risk of single-tier review,promote collaboration between pharmacists in different roles,improve the accuracy and efficiency of review,and reduce the risk of inappropriate use of parenteral nutrition.
