1.Evaluation of cardiac morphology and function of fetuses with different types of complete transposition of the great arteries using fetal heart quantification

Objective:To study the cardiac morphology and function of fetuses with different types of complete transposition of the great arteries(cTGA)by using fetal heart quantification(fetal HQ).Methods:A retrospective study was conducted on 50 fetuses diagnosed with cTGA through fetal echocardiography at Sir Run Run Shaw Hospital,Zhejiang University School of Medicine from July 2020 to December 2024. These cases were categorized into simple cTGA group( n=31)and complex cTGA group( n=19)based on the presence of concomitant cardiac anomalies. A control group of 160 normal fetuses with matched gestational ages was selected for comparison. Utilizing fetal HQ technology,the cardiac longitudinal diameter,transverse diameter,area,global sphericity index(GSI),left and right ventricular end-diastolic area(LVEDA,RVEDA),left and right ventricular fractional area change(LVFAC,RVFAC),left and right ventricular global longitudinal strain(LVGLS,RVGLS),and segmental sphericity index of 24 segments for both left and right ventricles(LVSI,RVSI)were measured. The analysis focused on comparing the differences among the simple cTGA group,complex cTGA group,and the control group. Results:Compared to the control group,the simple cTGA group exhibited significantly lower fetal GSI,LVEDA,RVFAC,and RVGLS(all P<0.05). Statistically significant differences were observed in LVSI segments 1-4 and 10-17,as well as RVSI segments 1-7,9,and 15-23 compared to the control group(all P<0.05). In comparison with the control group,the complex cTGA group demonstrated significantly reduced fetal GSI,LVFAC,LVGLS,RVFAC,and RVGLS(all P<0.05). Significant differences were noted in LVSI segments 5-8 and 10-14,along with RVSI segments 1-14 and 24 compared to the control group(all P<0.05). When compared to the simple cTGA group,the complex cTGA group showed significantly lower LVFAC,LVGLS,RVFAC,and RVGLS(all P<0.05),while GSI and LVEDA were significantly higher(all P<0.05). Statistically significant differences were observed in LVSI segments 3-4,6-8,and 17,as well as RVSI segments 10-19 between the complex cTGA group and the simple cTGA group(all P<0.05). Conclusions:The comprehensive parameters provided by Fetal HQ facilitate the assessment of cardiac morphology and function in cTGA fetuses,enabling a deeper understanding of the alterations in cardiac structure and function across different types of cTGA. This advanced analysis offers valuable reference information for clinical guidance during pregnancy.
2.Study on KAP of drug safety behavior among residents in Hezhou city

Objective To investigate the knowledge,attitude,and behavior(Knowledge-Attitude/belief-Practice,KAP)of residents in Hezhou City regarding the safety risks of using traditional Chinese medicine(TCM).Methods A stratified cluster random sampling method was used to select residents from different areas of Hezhou City.A questionnaire based on the KAP model was designed for the survey.Descriptive analysis,univariate analysis,and multiple linear regression analysis were conducted to explore the factors influencing the KAP of TCM safety behavior.Results Among the 2 000 questionnaires distribu-ted,all were conducted online.Of these,98 were incomplete,resulting in a total of 1 902 valid responses,with a valid response rate of 95.10%.In the survey of 2 000 residents of Hezhou City,the average knowledge score was(54.34±2.50),the average behavior score was(64.32±2.44),and the average attitude score was(62.44±3.07).The differences in knowledge scores among residents with different places of residence,educational levels,medical insurance status,monthly incomes,and other de-mographic characteristics were statistically significant(P<0.05).The differences in behavior scores among residents with differ-ent medical insurance status and occupations were statistically significant(P<0.05).The differences in attitude scores among residents with different monthly incomes and medical insurance status were also statistically significant(P<0.05).Results of multiple linear regression analysis indicated that place of residence,medical insurance(self-paid),educational level(junior high school and below),and monthly income(below 2 000 yuan,2 001-4 000 yuan)were influencing factors for the knowledge score on safe use of proprietary Chinese medicines(P<0.05).Medical insurance(self-paid)and occupation(enterprise work-er,freelancer,student)were influencing factors for the behavioral risk score regarding safe use of proprietary Chinese medicines(P<0.05).Medical insurance(self-paid)and monthly income(below 2 000 yuan)were influencing factors for the attitudinal risk score regarding safe use of proprietary Chinese medicines(P<0.05).Conclusion The overall KAP level regarding the safety of using traditional Chinese medicine among residents in Hezhou City is relatively low,and medication safety is closely re-lated to place of residence,medical insurance,monthly income,occupation,etc.

5.Preliminary study of cardiac morphology and function in fetuses with tetralogy of Fallot using fetal heart quantification

Objective:To evaluate the fetal heart shape and function in tetralogy of Fallot (TOF) by fetal heart quantitative analysis (fetal HQ).Methods:A total of 52 fetuses with TOF diagnosed by fetal echocardiography and 200 normal fetuses matched with their gestational weeks from March 2020 to March 2022 at Sir Run Run Shaw Hospital, Zhejiang University were retrospectively evaluated. The basic parameters of fetal cardiac blood vessels in the two groups were measured by fetal HQ technology and conventional M-mode ultrasound technology: aortic valve diameter(AV), pulmonary artery valve diameter(PV), main pulmonary artery diameter (MPA) and Z-score. The overall morphometric measurements including end-diastolic length diameter, transverse diameter, area, and global spherical index (GSI) of the fetal heart in the 4-chamber view(4CV), area and length of the right and left ventricles and their ratios. Measurements of left and right ventricular function included ejection fraction (EF), fraction area change rate (FAC), tricuspid annular plane systolic excursion (TAPSE), left and right ventricular global longitudinal strain (GLS), and left and right ventricular end-diastolic diameter (ED), spherical index (SI), and fractional shortening rate (FS) of 24-segments. The differences of above parameters between TOF group and control group were compared. In addition, the relationships between the absolute value of left and right ventricular GLS of TOF fetus and PV/AV, PV Z-score and MPA Z-score were analyzed. The optimal critical values of GSI, left ventricular EF and left ventricular FAC of TOF fetus were determined by ROC curve, and their corresponding sensitivity and specificity were obtained.Results:Compared with control group, there were significant differences in 4CV end-diastolic length, area, GSI, left ventricular area, left ventricular length, left ventricular EF, left ventricular FAC and left ventricular GLS in TOF group (all P<0.05). There were significant differences in ED between left ventricular 15-24 segments and right ventricular 1-21 segments (all P<0.05). There were significant differences in SI between left ventricular 1-16 segments, 21-24 segments and all segments of right ventricle (all P<0.05). The differences in FS were statistically significant (all P<0.05) when comparing all segments of the left ventricle and 1-2 segments of the right ventricular, and the remaining parameters were not statistically significant (all P>0.05). The left ventricular GLS absolute value of TOF fetuses was positively correlated with PV/AV, PV Z-score and MPA Z-score( rs=0.338, 0.441 and 0.458, all P<0.05), the right ventricular GLS absolute value of TOF fetuses was positively correlated with PV AV, PV Z-score and MPA Z-score( rs=0.418, 0.368 and 0.338, all P<0.05). The optimal critical values of GSI, left ventricular EF, and left ventricular FAC in the diagnosis of fetal TOF were 1.19, 59.05%, and 44.4%, respectively. At this time, the sensitivities of diagnosis of TOF were 0.78, 0.75, and 0.80, respectively. The specificities were 0.88, 0.88 and 0.83, respectively. The areas under ROC curve were 0.89, 0.88 and 0.89, respectively. Conclusions:Fetal HQ technology can provide a simple and reliable quantitative evaluation of fetal heart shape and function, and provide certain theoretical parameters for the study of fetal heart shape and function.
6.Mechanism of Shenci capsule combined with cisplatin in reversing DDP resistance of human lung adenocarcinoma via PI3K/AKT/mTOR signa-ling pathway

AIM:To investigate the effect of Shenci capsule combined with cisplatin ( DDP) in reversing DDP resistance by PI3K/AKT/mTOR signaling pathway in nude mice bearing A 549/DDP tumor.METHODS:The patient-de-rived lung adenocarcinoma A 549/DDP cell xenograft model was established .The tumor-bearing nude mice were randomly divided into control group , Shenci capsule group , DDP group and Shenci capsule combined with DDP group .The mice in control group was treated with normal saline , while the mice in other groups were treated with different drugs for 21 d.After treatment, the mice were killed and lung cancer tissues were collected .Flow cytometry was used to analyze the cell cycle and apoptosis.FQ-PCR was used to determined the mRNA levels of PTEN , P-glycoprotein, PI3K, AKT and mTOR in A549/DDP lung tumor .RESULTS:Compared with control group , the cell proliferation in all the drug treatment groups was inhibited .Compared with other drug treatment groups , Shenci capsule combined with DDP blocked the cell cycle of A 549/DDP cells at G2/M phase, promoted the apoptosis rate , increased the mRNA expression of PTEN and inhibited the mRNA expression of P-glycoprotein, PI3K, AKT and mTOR.CONCLUSION:Shenci capsule increases the sensitivity of A 549/DDP resistant cells in nude mice to DDP by blocking PI 3K/AKT/mTOR signaling pathway , increasing the expression of PTEN or inhibiting P-glycoprotein-mediated resistance pathway .
7.Characteristics of homocysteine in patients with Coronary Heart Disease and the effect of folic acid on its intervention

Objective To investigate the characteristics of homocysteine in patients with coronary heart disease (CHD) and the intervention effect of folic acid.Methods90 patients with coronary heart disease form internal medicine-cardiovascular department in the third affiliated Liwan hospital of Guangzhou Medical University from December 2015 to December 2016 as the study group.They were randomly divided into group A and group B, 45 patients in each group.The group A received routine treatment and Group B on the basis of conventional treatment plus folic acid.Another 90 healthy people who came to our hospital during the same period were selected as control group.The plasma homocysteine level of study group and control group were compared and plasma homocysteine level and angina status(frequency and duration of disease) of A and B group were compared before and after treatment.ResultsThe plasma homocysteine level of study group was significantly higher than that of the control group (P<0.05).After treatment, plasma homocysteine level,frequency and duration of disease of two groups were better than that of before treatment and B group was lower than that of A group(P<0.05).ConclusionThe results of plasma homocysteine levels can be used as an important basis for the clinical diagnosis of coronary heart disease and to determine its efficacy.
8.The research of causative genes and phenotypic features in Chinese families with thoracic aortic aneurysm and dissec-tion

Objective This study aimed at exploring the causative genes and summarizing the clinical characteristics in two Chinese families with thoracic aortic aneurysm and dissection ( TAAD ) .Methods The whole exome capture and high throughput sequencing were applied to identify the causative gene.Family members were examined for features of syndromic ge-netic diseases by clinician and geneticist.Results Four known TAAD candidate genes were identified in family TAA01:rs140598(FBN1), rs185661462(MYH11), rs77620762(MYLK3), and rs111426349(TGFBR1).The TGFBR1 mutation (c.1459C>T) had been confirmed to co-segregate with the TAAD phenotype in all affected family members.Early onset of aortic root dilatation was significant in this family , and the average age at diagnosis of aortic root dilatation or aneurysm was23. 2 years.ACTA2(c.445C>T) was proved in family TAA02, and livedo reticularis was confirmed.Conclusion The causa-tive genes were identified via whole exome capture and high throughput sequencing in two TAAD families .Early onset of aortic root aneurysm was proved in TAA01, while livedo reticularis was found in TAA02.
9.Effect of aorta-iliac bypass total thoracoabdominal aorta aneurysm repair to spinal cord function.

OBJECTIVETo evaluate the effect of aorta-iliac bypass total thoracoabdominal aorta aneurysm repair to spinal cord function.
METHODSThis was a prospective study. From June 2014 to April 2015, 31 patients underwent total thoracoabdominal aorta aneurysm repair were treated with aorta-iliac bypass technique. There were 23 male and 8 female patients with a mean age of (36±12) years. A 4-branched tetrafurcate graft was used. The aorta-iliac bypass was established, then distal descending aorta was perfused in a retrograde fashion via bypass graft. Thoracic and abdominal aorta were replaced in a staged fashion. Evoked potentials (EP) monitoring was adopted to assess the spinal cord ischemia throughout the procedure. The intraoperative evoked potentials results, clinical outcomes and follow-up results of this technique were evaluated.
RESULTSThe EP wave disappeared after proximal descending aorta clamped and gradually recovered after the patent segmental arteries reattached. Motor evoked potentials disappeared for (56±18) minutes, somatosensory evoked potentials disappeared for (50±19) minutes. The EP wave was restored to normal at the end of operation in all cases. The somatosensory evoked potentials remained unchanged in 2 cases (false negative). One case died after operation. There were acute kidney dysfunction in 3 cases, and pulmonary haemorrhage in 1 case. No spinal cord injure occurred. The median follow-up after operation was 8 months (ranging from 1 to 11 months). There was no delayed neurologic deficit or relative death.
CONCLUSIONSThere is a transient function loss of spinal cord during the aorta-iliac bypass total thoracoabdominal aorta aneurysm repair. But the process is reversible. The technique of the aorta-iliac bypass is practicable.

10.Zhu Lian's characteristics and experiences in clinical practice of acupuncture and moxibustion.

This paper aims at discussing the clinical characteristics and experiences of ZHU Lian, the renowned contemporary acupuncture master from the following three aspects: "characteristics of clinical manipulations and techniques", "thoughts on diagnosis and treatment" and "examples of clinical cases". The study has shown that ZHU Lian invented the slow insertion technique by rotating needle and the embedding needle technique, improved moxibustion technique with moxa roll and proposed the three keys on the treatment with acupuncture and moxibustion, as well as discovered new acupoints for treatment. The pioneering and distinguished achievements she con tributed play the great demonstrating and driving role in the development of clinical study and practice of acupuncture and moxibustion.
