Aim To investigate the effects of SAHA on the expression of P-gp and the pharmacokinetic pa-rameters of its substrate phenytoin sodium in rats under hypoxic environments. Methods Wistar rats were randomly divided into the normioxic group, the hypoxic model group, and the low-,medium-and high-dose vorinostat(SAHA)groups.Liver tissues were col-lected,and the expression levels of P-gp and HDAC5 were detected by Real-time PCR and Western blot. The morphological changes of liver tissues were ob-served by HE staining.Following intragastric adminis-tration of 50 mg·kg-1 phenytoin sodium to each group, blood samples were collected, and the plasma concentration of phenytoin sodium was determined u-sing UFLC-MS/MS to calculate pharmacokinetic pa-rameters.Results Compared with the normoxic group, the expression of HDAC5 in the liver tissues of hypoxia model rats increased, while the expression of P-gp decreased. After SAHA treatment,HDAC5 expression decreased, and P-gp expression increased.Among the SAHA groups, the medium-dose group showed the most significant effect, and HE staining re-sults indicated that this concentration did not cause damage to rat liver tissues.Compared with the normox-ic group, the AUC,Cmax, and T1/2 of phenytoin sodium in hypoxia model rats were significantly raised. After administration of the medium dose of SAHA,the AUC,Cmax,MRT,and T1/2 were significantly reduced, while CLZ/r was significantly increased. Conclusions Un-der hypoxic environments, the expression of P-gp in rat liver tissue is significantly downregulated, leading to increased systemic exposure of phenytoin, reduced clearance, and consequently elevated blood concentra-tions,raising the risk of central nervous system toxici-ty.In contrast,SAHA suppresses HDAC5 expression, thereby activating P-gp transcription and enhancing its efflux function. This results in decreased systemic ex-posure and improved clearance of phenytoin,signifi-cantly reducing drug accumulation in body and ulti-mately lowering the risk of adverse effects.

Aim To evaluate the toxicity and mecha-nism of Epimedium sagittatum(Sieb.et Zucc.)Maxim aqueous extract(ESMAE)on HepaRG cells based on serum toxicology. Methods MTT assay was used to detect the activity of HepaRG cells after treatment with the serum containing ESMAE from SD rats.Western blot was used to detect the effects of the serum contai-ning drug on the expression of endoplasmic reticulum stress-related proteins(PERK,eIF-2α,ATF-4,GRP78,CHOP)and pyroptosis related proteins(NL-RP1,caspase-1,cleaved caspase-1,GSDMD,GSDMD-N).MTT assay was used to detect the activity of Hep-aRG cells after treatment with the liver homogenate containing ESMAE from SD rats. Results Twenty percent serum containing drug significantly decreased the viability of HepaRG cells, with the cells exhibiting swelling, rupture, and vesicle-like pyroptosis. The ex-pression levels of endoplasmic reticulum stress-related proteins PERK,eIF-2α,ATF-4,GRP78,and CHOP significantly increased. The expression levels of pro-teins involved in the NLRP1-mediated classical pyrop-tosis pathway were significantly increased. Finally the liver homogenate containing drug decreased the cell ac-tivity,and cells exhibited swelling, rupture, and vesicle-like pyroptosis. Conclusions After administration of ESMAE,the serum containing drug and the liver ho-mogenate containing drug of rats show toxicity to Hep-aRG cells, and can induce endoplasmic reticulum stress and activate the NLRP1/caspase-1/GSDMD pyroptosis pathway in HepaRG cells.

Objective To explore the efficacy differences between 3 Tesla magnetic resonance imaging with zero echo time(3T MRI ZTE)and high resolution computed tomography(HRCT)in the detection of pulmonary nodules and the classification diagnosis of the lung imaging reporting and data system(Lung-RADS)in sub-health populations. Methods Clinical and imaging data of 93 patients with pulmonary nodules(126 nodules in total)admitted to some hospital from July to December 2023 were retrospectively analyzed. The 126 nodules were categorized into a benign nodule group(n=51)and a malignant nodule group(n=75)using pathological findings as the gold standard. All the patients underwent examinations by 3T MRI ZTE and HRCT to compare the detection rates of the two measures for pulmonary nodules;the missed and misdiagnosis rates of 3T MRI ZTE,HRCT and Lung-RADS grading were contrasted with the postoperative pathological diagnosis results as the gold standard;comparison analyses of 3T MRI ZTE signs and HRCT signs were performed between the two groups and the patients with different Lung-RADS grades;3T MRI ZTE,HRCT and Lung-RADS grading were compared with the receiver operating characteristic(ROC)curve in terms of diagnosis efficacy for pulmonary nodules, and the consistency analysis was carried out. Results No discernible statistical variation was observed in the detection rates of pulmonary nodules between 3T MRI ZTE and HRCT(P>0.05).Lung-RADS grading had the highest rates of missed diagnosis and misdiagnosis, and 3T MRI ZTE and HRCT had similar detection rates. The malignant nodule group was different from the benign nodule group in the 3T MRI ZTE and HRCT signs in terms of lesion size, spiculation sign, lobulation sign,calcifica-tion,pleural indentation sign, cavity sign, boundary and bronchial cut-off sign, with the differences being statistically signi-ficant(all P<0.05).For the patients of Lung-RADS grade 3,the 3T MRI ZTE and HRCT signs had significant differences in terms of lesion size, spiculation sign, lobulation sign, calcification, pleural indentation sign, cavity sign and bronchial cut-off sign(all P<0.05).For the patients of Lung-RADS grade 4A,the 3T MRI ZTE and HRCT signs had significant differen-ces in terms of lesion size, calcification, boundary and bronchial cut-off sign(all P<0.05).For the patients of Lung-RADS grade 4B,the 3T MRI ZTE and HRCT signs had significant differences in terms of lesion size and calcification(all P<0.05).For the patients of Lung-RADS grade 4X,there were no significant differences found between the 3T MRI ZTE and HRCT signs(all P>0.05).HRCT had the highest sensitivity, specificity, accuracy,AUC value, predictive values and Kappa value for benign and malignant nodules,3T MRI ZTE had the values slightly lower than those of HRCT,and Lung-RADS grading had the lowest values when compared with HRCT and 3T MRI ZTE.Conclusion HRCT and 3T MRI ZTE are complementary for the evaluation of pulmonary nodules, and the differences in imaging signs between them show graded dependence.3T MRI ZTE and HRCT have no significant differences in the detection rate of pulmonary nodules, while HRCT gains advanta-ges in differentiating benign and malignant pulmonary nodules, and references are provided for the screening and clinical early diagnosis of pulmonary nodules.[Chinese Medical Equipment Journal,2025,46(9):52-59]

Objective To screen the mutations of ABCC8 gene in probands of maturity-onset diabetes of the young pedigrees, and investigate it sgenetic and clinical characteristics. Methods Whole exome sequencing were performed to screen ABCC8 mutations in 56 MODY probands who were admitted to Department of Endocrinology and Metabolism,Shanghai Sixth People's Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from July 2021 to December 2023.The mutations were verified by Sanger sequencing, and all participants were genotyped.Clinical phenotypes of the mutation carriers were compared with non-DM controls within the families. The identified mutations were evaluated by bioinformatic softwires. Then the pharmacogenomic characteristics of the mutation carriers were analyzed. Results Two heterozygous mutations D655V and R825Q were identified in two MODY probands and their families respectively, and the D655V was a novel mutation.Bioinformatics studies showed that both mutations were deleterious and pathogenic. In comparison with non-DM controls in the two families, mutation carriers with diabetes exhibited significantly lower fasting insulin/fasting plasma glucose,two-hour postprandial insulin/two-hour postprandial insulin plasma glucose, homeostatic model assessment-β(P<0.05).Treatment with oral hypoglycemic agents such as metformin or insulin in these mutation carriers resulted in a moderate reduction in plasma glucose levels. However, switching to targeted Sulfonylurea's(SUs)proved to be more effective. Conclusions In this study, the prevalence of MODY12 is 3.6%in these MODY pedigrees. The remarkable hypoglycemic efficacy of SUs suggests that both D655V and R825Q were activating mutations of ABCC8,and maybe the cause of MODY12 characterized by impaired insulin secretion.

BACKGROUND:Percutaneous retrograde screws and minimally invasive percutaneous plates are extensively used in the treatment of unilateral pubic ramus fractures. However, the above internal fixations have the disadvantages of mistakenly penetrating the hip joint and damaging the medial pelvic trophoblastic artery.A new type of retrograde superior pubic ramus intramedullary nail offers advantages, such as fewer radiographic exposures and shorter surgery times, but the validation of the biomechanical properties of the three endoprostheses is lacking.OBJECTIVE:To compare the biomechanical differences post-treatment of upper and lower pubic ramus fractures using percutaneous retrograde screws, minimally invasive percutaneous plates, and retrograde superior pubic ramus intramedullary nail under various conditions through finite element analysis.METHODS:Pelvic CT data were collected from one volunteer and imported into Mimics 20,Geomagic Wrap 2021,and SolidWorks 2019 to create geometric models of three types of internal fixation for treating unilateral pubic ramus fractures:hollow screw group, plate group, and intramedullary nail group. Each model was imported into Ansys 2022 software, where it was set up in vertical,book-opening,and shear conditions for preprocessing and calculation to compare and analyze the mechanical stability of the implants.RESULTS AND CONCLUSION:(1)In vertical conditions, the biomechanical stability of the hollow screw treatment for unilateral pubic fractures was superior to that of the steel plate and intramedullary nail, with the smallest peak stress in the implant, which was four times lower compared to the other two groups.(2)In book-opening conditions, the steel plate treatment for unilateral pubic fractures showed better biomechanical stability, particularly in reducing stress at the fracture ends of the upper pubic branch, with stress values five times higher in the other two groups.(3)In shear conditions, the intramedullary nail treatment for unilateral pubic fractures exhibited good biomechanical stability, maintaining the best pelvic displacement, with a 25%greater displacement observed in the other two groups.(4)These findings reveal the biomechanical differences of different implants:the peak stress of implants is the smallest under vertical working conditions with percutaneous retrograde screws.Minimally invasive percutaneous plates reduce the stress at the broken end of superior pubic branch under open-book working condition.Retrograde superior pubic ramus intramedullary nail of superior pubic branch maintains pelvic displacement under shear working condition. The surgeon can combine the biomechanical differences of the implants with the actual situation of pubic injury in clinical practice to make the best implant treatment.
