Objective To analyze the global literature related to functional cure of hepatitis B from 2019 to 2023 by using bibliometric analysis methods,so as to help researchers understand the research hotspots and trends in this field.Methods The literature related to the topic of functional cure of hepatitis B included in the Science Citation Index Expanded(SCI-Expanded)of the Web of Science Core Collection from 2019 to 2023 was searched.By using VOSviewer and CiteSpace visual analysis tools,analyses were conducted from the perspectives of publication trends,international research cooperation networks,and keyword emergence,and were elaborated with the specific contents of the related literature to elucidate research hotspots and trends.Results A total of 600 eligible papers in this field were included.Keyword co-occurrence and thematic clustering suggested that the main research directions of functional cure were:serum biomarkers for prediction and monitoring of functional cure,functional cure and immunity,nucleoside analog discontinuation,interferon therapy,and long-term prognosis of functional cure.The research contents of the ESI highly cited original research papers were similar to the clustering of the above,but showed more attention on the novel agents for functional cure.The content of the keyword emergence map showed that hotspots of interest changed from virologic mechanisms and serum markers,to nucleoside analog discontinuation and interferon therapy,and finally to immunologic mechanisms and new drug.Conclusion The research hotspots and trends of functional cure of hepatitis B were focused on virological mechanism,serum markers,immunological mechanism,nucleoside analog discontinuation,interferon therapy,and long-term prognosis after cure.
3.Salvianolic acid B mediates Elovl6/Echs1/Acot1 pathway to regulate fatty acid metabolism and attenuates OGD/R injury in H9c2 cells

Aim To observe the mechanism of salvian-olic acid B(SalB)against oxygen glucose deprivation/re-oxygenation(OGD/R)injury in H9c2 cardiomyo-cytes.Methods The protective concentration of SalB against OGD/R-injured H9c2 cardiomyocytes was screened by CCK-8 assay.The levels of lactate dehy-drogenase(LDH),aspartate transaminase(AST)and creatine kinase(CK)were detected by ELISA kit.The mechanism of action of SalB on OGD/R-injured H9c2 cardiomyocytes was explored using high-through-put sequencing of the transcriptome.The binding of SalB to differential proteins was assessed using molecu-lar docking assays.Fatty acid content was determined using free fatty acid kits.The relative expressions of mRNA and protein of differential genes were verified by RT-qPCR and Western blot.The causal relationship between the target of action of SalB and heart failure was examined by Mendelian randomization experiment.Results SalB protected OGD/R-injured H9c2 cardio-myocytes and significantly reduced the levels of CK,LDH and AST compared with the blank control group.One hundred differential genes were screened by tran-scriptome sequencing,which were mainly involved in fatty acid elongation,central carbon metabolism of cancer,tryptophan metabolism pathways.Molecular docking showed that SalB had good binding energy to differential proteins.The mRNA and protein expression of core differential genes Elovl6,Echs1 and Acot1 were consistent with the transcriptome sequencing results.SalB reduced fatty acidsafter OGD/R injury.Mende-lian randomization experiments suggested that SalB might reduce the risk of heart failure through fatty acid metabolism,thereby reducing the risk of heart failure.Conclusion SalB can protect H9c2 cardiomyocytes after OGD/R injury by down-regulating Elovl6,Echs1 and Acot1 expression through the fatty acid metabolism pathway.
4.Clinical Observation of the Effect of Kidney Tonifying and Blood Stasis Removing Formula on Nutritional Status and Quality of Life in Patients with Osteoporosis Combined with Hemodialysis

Objective:To explore the effect of kidney-strengthening and blood stasis-eliminating formula on nutritional status and quality of life in patients with osteoporosis combined with hemodialysis.Methods:From January 2024 to January 2025,60 patients with liver and kidney yin deficiency and blood stasis obstruction were selected from the Pizhou City Traditional Chinese Medicine Hospital's Hemodialysis Center.60 patients were randomly divided into two groups,30 in each group.The matched group received general treatment plus paricalcitol,while the treatment group received the same treatment plus Yishen Zhuyu Fang(a traditional Chinese medicine formula for kidney and blood stasis).The treatment course lasted 12 weeks.Changes in TCM syndrome scores,the Modified Subjective Global Assessment(MQSGA),the Kidney and Life Quality(KDQOLTM-36)scale,lipid levels,and nutritional indicators were compared before and after treatment.Results:After treatment,the TCM syndrome scores in both groups decreased(P<0.05),with a more pronounced decrease in the treatment group(P<0.05).The KDQOLTM-36 score in the treatment group increased(P<0.05),while the increase was not significant in the matched group(P>0.05),indicating that the treatment group performed better than the matched group(P<0.05).Both groups experienced a decrease in the MQSGA score(P<0.05),although the difference was not statistically significant(P>0.05),the treatment group showed a more noticeable decline.The TC,TG,LDL,and TG/HDL levels in the treatment group all decreased significantly,while HDL levels increased(P<0.05).In contrast,the TC and HDL levels in the matched group did not change(P>0.05),while the TG,LDL,and TG/HDL levels increased(P<0.05).The treatment group performed significantly better than the matched group(P<0.05).The TRF,Hb,and ALB levels in the treatment group increased(P<0.05),while the increase was not significant in the matched group(P>0.05).The total effective rate of the treatment group was 90%,higher than the matched group(23.3%)(P<0.05).Conclusion:The formula of tonifying the kidney and removing stasis can improve the nutritional status and quality of life of patients with liver and kidney yin deficiency and blood stasis obstruction in hemodialysis combined with osteoporosis.
5.Mechanism of Fraxetin Regulating TLR4/STAT3 Signaling Pathway to Inhibit Proliferation,Migration and Invasion of Ovarian Cancer Cells

Objective To investigate the effect of fraxetin(FXT)on the malignant biological behavior of ovarian cancer cells by regulating Toll-like receptor 4(TLR4)/signal transducer and activator of transcription 3(STAT3)pathway.Methods Ovarian cancer cell lines SKOV3 and SW626 and ovarian epithelial cell line IOSE80 were treated with different concentrations of FXT(0,10,20,40,60,80 and 100 μmol/L),and the cytotoxicity of FXT was detected by methylthiazolyl diphenyl-tetrazolium bromide(MTT)assay.SKOV3 and SW626 cells were randomly divided into control group,FXT low,medium,high dose(40,60,80 μmol/L)group and FXT(80 μmol/L)+colivelin(STAT3 agonist,0.5 μmol/L)group.Colony formation assay and Transwell assay were used to detect cell proliferation,invasion and migration.The expressions of epithelial-mesenchymal transition and TLR4/STAT3 pathway related proteins were measured by Western blot.Results Compared with IOSE80 cells,SKOV3 and SW626 cell survival gradually decreased with increasing FXT concentration,and the differences were statistically significant(F=134.283,146.831,P<0.001).Compared with the control group,the relative colony lineage rates of SKOV3 and SW626 cells in the FXT low-,medium-and high-dose groups,invasion rate and mobility were reduced(t=4.433~45.909),E-cadherin were increased(t=5.879~17.345),and the expression of N-cadherin,zinc finger transcription factors(snail),vimentin,TLR4,phosphorylated(p)-STAT3/STAT3,cell cycle protein D1(Cyclin D1)and MYC oncogenes(Cancer-myc,C-myc)expression were sequentially reduced(t=7.348~50.117),and the differences were statistically significant(all P<0.01),respectively.Compared with the high-dose FXT group,the relative colony formation rate,cell invasion rate and migration rate of SKOV3 and SW626 cells in the FXT+colivelin group increased(t=9.224~20.703),while the expression of E-cadherin protein decreased(t=3.104,5.041),the expression of N-cadherin,snail,vimentin,TLR4,p-STAT3/STAT3,Cyclin D1,and C-myc increased(t=8.403~42.175),and the differences were statistically significant(all P<0.05),respectively.Conclusion FXT may exert antitumor effects by antagonizing the activation of TLR4/STAT3 signaling pathway and inhibiting ovarian cancer cell proliferation,migration,invasion and epithelial mesenchymal transition.

Acupuncture and moxibustion have their unique advantages in treating tumors,but the theoretical framework of acupuncture and moxibustion intervention in tumors needs to be improved,and the academic system still needs to be sorted out.In this paper,we try to describe the commonalities between Chinese medical theories and modern medical technologies in order to consolidate the theoretical foundation of acupuncture intervention in tumor.Specifically:① Acupuncture regulates immune function and energy metabolism,which is a specific form of tonifying the spleen and stomach to support positive qi.② Tumors belong to the broad definition of"sores and ulcers"in traditional Chinese medicine,which is a special kind of chronic inflammation,and acupuncture intervenes in tumors by intervening in inflammatory reactions from ulcers.③ Tumor is a"dynamic pseudo-organ"with yin and yang attributes and"life attributes".Acupuncture and moxibustion can regulate various cells and factors in the tumor environment in a multi-targeted way,so as to make it tend to the state of yin and yang equilibrium.④ Acupuncture and moxibustion have unique advantages in the reduction of toxicity,enhancement of efficacy,tonicity,rejuvenation,treatment of complications,etc.,which complement the allopathic therapies of modern medicine,and can help to realize survival with tumors.

Critically ill patients often experience significant skeletal muscle wasting due to prolonged bed rest, metabolic disorders, inflammatory responses and malnutrition, which affects the patient's mobility and may also lead to increased mortality. Timely and accurate assessment of muscle status is important for optimizing treatment strategies and improving patient prognosis. There are various limitations in the current methods of assessing muscle mass, and muscle ultrasound, as a noninvasive, convenient, low-cost and suitable technique for bedside monitoring, has received increasing attention for its application in muscle assessment of critically ill patients. However, there are still a number of challenges in its practical application, such as the lack of uniform standards for the measurement method, the high dependence on the operation, and the reproducibility of the data that needs to be optimized, and so on. The aim of this article is to systematize the research progress of muscle ultrasound in muscle assessment of critically ill patients, and to discuss the advantages and limitations of its clinical application, in order to provide a scientific basis for future research and clinical practice.

10.Analysis of long-term efficacy of second-course radiotherapy for pelvic recurrence of rectal cancer

Objective:To evaluate the long-term efficacy of second-course radiotherapy for patients with locally recurrent rectal cancer (LRRC) following pelvic radiotherapy.Methods:Data of 34 patients with in-field pelvic recurrence of rectal cancer that could not be resected with R0 surgery admitted to Peking University Third Hospital from October 2012 to April 2024 were retrospectively analyzed. All patients received second-course intensity-modulated radiotherapy and the median radiation dose for the first course was 50 Gy (range 45-66 Gy), and for the second course, the median dose was 45 Gy (range 10-60.4 Gy). Regular follow-up visits and imaging examination were performed during follow-up. The recurrence of all patients was analyzed. According to the site of recurrence, all patients were divided into the presacral, pelvic wall and central recurrence groups. Local recurrence-free survival (LRFS) and overall survival (OS) rates were estimated using the Kaplan-Meier method. The survival among groups was compared by two-sided log-rank test.Results:The median follow-up period was 22 months. Among 34 patients, recurrence was observed in 38 sites, primarily at the central region (24 sites, 63%) and 23 patients (68%) had initial recurrence. The median OS was 47 months, and the median LRFS was 19 months. The 1-, 2-, 3- and 5-year OS rates were 93.1%, 75.3%, 69.1% and 38.4%, respectively. The 1- and 2-year LRFS rates were 69.8% and 27.2%. There were no significant differences in OS among the presacral, pelvic wall and central recurrence groups ( P=0.616). Conclusions:For patients with locally recurrent rectal cancer that cannot be resected with R0 margins, second-course radiotherapy offers higher clinical efficacy compared with alternative therapeutic options previously reported.
