1.Zingiberis Rhizoma Alleviates Inflammatory Bowel Disease Through Regulating TLR4/MAPK Signaling Pathway in Ly6Chi Monocytes/Macrophages

ObjectiveTo investigate the potential mechanisms of Zingiberis Rhizoma in treating inflammatory bowel disease (IBD) by integrating network pharmacology with in vitro and in vivo experiments. MethodsTraditional Chinese Medicine Systems Pharmacology Database And Analysis Platform (TCMSP), Traditional Chinese Medicine Integrated Database (TCMID) Database were used to obtain the active component targets of Zingiberis Rhizoma. GeneCards was used to obtain the IBD targets. DAVID was used to perform Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analyses on core targets. Cytoscape 3.10.2 was used to establish the "active component-disease target-signaling pathway" interaction network. Mice were randomly assigned to control, model, and Zingiberis Rhizoma (400 mg·kg-1) groups. An IBD model was induced via dextran sulfate sodium (DSS). The colonic tissue was collected post-treatment to assess histology, expression of Ly6C+ monocytes/macrophages, and mRNA levels of Toll-like receptor 4 (TLR4), and inflammatory cytokines. The effect of Zingiberis Rhizoma aqueous extract on RAW264.7 cell viability was evaluated. Furthermore, the effects of the extract at 100, 10, and 1 mg·L-1 on LPS-induced differentiation of RAW264.7 cells into Ly6Chi monocytes/macrophages, mRNA levels of TLR4 and inflammatory cytokines, and protein levels of factors in the TLR4/mitogen-activated protein kinase (MAPK) signaling pathway. ResultsA total of 241 targets were identified for Zingiberis Rhizoma and 6 787 for IBD, with 122 shared targets among Zingiberis Rhizoma, ulcerative colitis (UC), and Crohn's disease (CD). The enrichment analyses yielded 297 GO terms and 88 KEGG pathways. Associations were noted between Zingiberis Rhizoma's active component targets and IBD targets. In vivo experiments: Compared with the control group, the model group showed decreased body weight and disease activity index (DAI)(P<0.01), shortened colon length, damaged mucosal epithelium with inflammatory cell infiltration, raised pathological scores (P<0.05), increased Ly6Chi and Ly6Clo monocytes/macrophages (P<0.05), and up-regulated mRNA levels of TLR4, TNF-α, IL-1β, and IL-6 (P<0.05) and protein levels of TLR4, phosphorylated extracellular signal-regulated protein kinase 1/2 (p-ERK1/2), and phosphorylated p38 MAPK (p-p38 MAPK) (P<0.05). Zingiberis Rhizoma intervention reversed these changes and reduced Ly6Chi monocytes/macrophages (P<0.01). In vitro experiments: compared with the control, LPS increased the proportion and number of Ly6Chi monocytes/macrophages and mRNA levels of TLR4, TNF-α, IL-1β, and IL-6 (P<0.01) and enhanced the expression of TLR4, p-ERK1/2, and p-p38 MAPK (P<0.05). Zingiberis Rhizoma reduced Ly6Chi monocytes/macrophages (P<0.05), down-regulated the mRNA levels of inflammatory cytokines (P<0.05), and suppressed the TLR4/MAPK pathway (P<0.05). ConclusionZingiberis Rhizoma alleviates IBD by suppressing the TLR4/ERK/p38 MAPK signaling pathway and reducing inflammatory cytokine levels in Ly6Chi monocytes/macrophages.

To explore the role of the "Clinical Practice Guidelines" column and others in the We collected papers published by the Lanzhou University Evidence-Based Medicine Center team in the "Clinical Practice Guidelines" column and others from 2018 to 2025. These publications were analyzed across multiple dimensions, including authorship and institutional affiliations, citation metrics, and research themes and content. A total of 59 papers were included in the analysis, with authors representing 70 domestie and international research institutions. The cumulative citation count was 639, with the highest single-paper citation frequency reaching 101. The average citation per paper was 10.8, and total downloads exceeded 30 000. The content focused on key themes such as guideline terminology, development methodology, guideline evaluation, and dissemination and implementation. The evolution of research topics progressed from critiques of common misconceptions and hot topies in the field to multidimensional evaluations of thecurrent state of Chinese guidelines, culminating in the fommulation of industry standards for guidelines. These contributions have provided critical references for translating guideline theory into practice in China and have garnered widespread attention and discussion among scholars in the field. 3.Disease burden of chronic obstructive pulmonary disease in Zhejiang Province from 1990 to 2021

Objective:
To analyze the disease burden of chronic obstructive pulmonary disease (COPD) and changes in its risk factors among residents in Zhejiang Province from 1990 to 2021, so as to identify key priorities for COPD prevention and control.
Methods:
Data on COPD mortality and disability-adjusted life years (DALY) for residents in Zhejiang Province from 1990 to 2021 were collected from the Global Burden of Disease (GBD) 2021 database. Standardized mortality and standardized DALY rate were calculated using the GBD 2021 world population standard structure. Premature mortality was computed via the life table method. The average annual percent change (AAPC) was applied to analyze trends in COPD mortality, DALY rate, and premature mortality. Changes in deaths of COPD risk factors were evaluated using population attributable fraction (PAF).
Results:
From 1990 to 2021, the standardized COPD mortality in Zhejiang Province decreased from 272.40/100 000 to 70.56/100 000 (AAPC=-4.395%), and the standardized DALY rate declined from 4 167.37/100 000 to 1 071.89/100 000 (AAPC=-4.396%). Similar downward trends were observed in both males (AAPC=-3.933%, -4.173%) and females (AAPC=-4.785%, -4.480%), all P<0.05. Crude mortality and DALY rates increased with age, and the crude mortality and DALY rates of various age groups in Zhejiang Province showed decreasing trends from 1990 to 2021 (all P<0.05). The premature mortality declined from 4.37% to 0.60% from 1990 to 2021 (AAPC=- 6.206%), with consistent trends across males and females (AAPC=- 6.144%, - 6.379%, all P<0.05). From 1990 to 2021, particulate matter pollution showed the largest reduction in PAF (- 56.76%), while ambient ozone pollution had the largest increase (103.07%) in Zhejiang Province. By 2021, smoking became the leading risk factor for deaths of COPD (PAF=43.32%).
Conclusions
The standardized mortality, standardized DALY rate, and premature mortality for COPD show consistent declining trends in Zhejiang Province from 1990 to 2021. However, risk factors such as smoking and ambient ozone pollution require intensified focus to further reduce disease burden of COPD.
4.A preliminary study on the diagnostic value of 18F-FDG PET/CT in primary pulmonary enteric adenocarcinoma

Objective To explore the diagnostic value of 18F-fluorodeoxyglucose(FDG)PET/CT in primary pulmonary enteric adenocarcinoma.Methods The clinical and imaging data of 9 patients with primary pulmonary enteric adenocarcinoma who under-went 18F-FDG PET/CT examination were retrospectively analyzed,including lesion distribution,morphology,maximum standardized uptake value(SUVmax),clinical symptoms and signs,gastroscopy finding,puncture pathological results,and serum tumor markers[carbohydrate antigen 72-4(CA72-4),cytokeratin 19 fragment antigen 21-1(CYFRA21-1),carcinoembryonic antigen(CEA),carbo-hydrate antigen 199(CA199)].Results Pathological examination confirmed a diagnosis of primary pulmonary enteric adenocarcinoma after excluding gastrointestinal primary tumors through clinical evaluation.In all nine patients,18F-FDG PET/CT examination did not reveal any evidence of digestive system malignancies,and gastrointestinal microscopy was negative.Primary lesions were observed as masses or nodular types in 6 cases(5 in the left lung and 1 in the right lung),while 3 cases exhibited diffuse bilateral pulmonary involvement(manifested as multiple patchy opacities,nodules,ground-glass opacities,and consolidations).All pulmonary primary lesions showed increased 18F-FDG uptake,with SUVmax ranging from 2.7 to 12.8,mean 8.6±3.7.The six masses-or nodular-type primary lesions showed maximum diameters ranging from 2.1 to 10.5 cm,mean(5.23±3.06)cm.Four cases demonstrated hilar and mediastinal lymph node metastases,intrapulmonary metastases,and distant metastases,while 1 case showed only distant metastasis.Elevated levels of serum tumor markers were observed as follows:CA72-4 in 7 cases(10-273.3 U/mL),CEA in 7 cases(5-147.4 ng/mL),CA199 in 6 cases(31.22-4 364 U/mL),and CYFRA21-1 in 5 cases(8.31-99.7 ng/mL).Conclusion When pathological biopsy of a pulmonary lesion suggests primary pulmonary enteric adenocarcinoma after excluding gastrointestinal primary tumors,and 18F-FDG PET/CT shows no gastrointestinal masses,this may support the diagnosis of primary pulmonary enteric adenocarcinoma.
5.ALKBH5 mediated m6A modification of NLRP3 promotes cardiomyocytes pyroptosis in mice with myocardial infarction

Aim To investigate the effects of m6A demethylase ALKBH5 on cardiomyocytes pyroptosis in mice with myocardial infarction(MI).Methods The MI model of left anterior descending coronary artery ligation surgery was established by knocking down ALKBH5 using adeno-associated virus,and the hypox-ia model of mouse cardiomyocytes(HL-1)was estab-lished by knocking down small interfering RNA.The effects of ALKBH5 on the pyroptosis of MI mice and hypoxic HL-1 cells were observed.Subsequently,mechanism studies were conducted at the cellular lev-el,and the binding of ALKBH5 and IGF2BP2 to NL-RP3 mRNA was detected through RNA pull down and RNA immunoprecipitation(RIP)experiments.The MeRIP-qPCR method was used to determine the effects of ALKBH5 on the mRNA m6A level of NLRP3.Acti-nomycin D for RNA stability experiments were conduc-ted to detect the effects of ALKBH5 and IGF2BP2 on the stability of NLRP3 mRNA.Results Knocking down ALKBH5 in vivo and in vitro both inhibited NL-RP3 inflammasome activation and alleviated pyroptosis in MI mice and hypoxic HL-1 cells.Mechanistically,the results showed that NLRP3 mRNA could bind to ALKBH5 protein in HL-1 cells;knocking down ALK-BH5 could increase the m6A level of NLRP3 and re-duce the stability of NLRP3 mRNA;subsequently,it was confirmed that NLRP3 mRNA and IGF2BP2 pro-tein bound to each other;knocking down IGF2BP2 in-creased the mRNA stability of NLRP3.The Rescue ex-periment showed that knocking down IGF2BP2 re-versed the decrease in NLRP3 mRNA expression caused by knocking down ALKBH5.Conclusions ALKBH5 mediated m6A modification of NLRP3 pro-motes cardiomyocytes pyroptosis in mice with myocardi-al infarction.
6.Epidemic characteristics of human brucellosis and awareness status among occupational populations in Xuzhou City, Jiangsu Province, 2023

Objective:To investigate the epidemic characteristics of human brucellosis and the awareness of brucellosis prevention and control knowledge among occupational populations in Xuzhou City, Jiangsu Province.Methods:The data of brucellosis cases reported from January to December 2023 in the China Disease Prevention and Control Information System's Infectious Disease Surveillance System where the current residence was Xuzhou City, were collected. The epidemiological distribution characteristics across the three dimensions (time, region, and population) were analyzed, and the etiological characteristics of Brucella strains collected from sentinel hospitals in Xuzhou City during the same period were analyzed. Meanwhile, in September 2023, a survey was conducted on the awareness of brucellosis prevention and control knowledge among occupational populations in 7 counties (cities, districts) with a high incidence of brucellosis in Xuzhou City. Binary logistic regression was used to analyze the influencing factors. Results:In 2023, a total of 124 human brucellosis cases were reported in Xuzhou City, with an annual incidence rate of 1.37/100 000. Brucellosis occurred in all months of the year, with a peak incidence from May to August, accounting for 46.77% (58/124). The incidence rate of brucellosis in Feng County, Pei County, Suining County, Xinyi City and Pizhou City (1.86/100 000) was higher than that in Gulou District, Yunlong District, Quanshan District, Jiawang District and Tongshan District (0.71/100 000), with a statistically significant difference (χ 2 = 19.15, P < 0.001). Among 124 cases of brucellosis, 81 cases (65.32%) were males and 43 cases (34.68%) were females. The incidence rate of brucellosis in males (1.78/100 000) was higher than that in females (0.96/100 000), with a statistically significant difference (χ 2 = 11.08, P < 0.001). The 55 - 64 age group reported a relatively higher number of cases (45 cases, 36.29%), with an incidence rate of 3.97/100 000. The main occupation was farmers (85 cases, 68.55%). In 2023, a total of 9 strains of Brucella were collected from the sentinel hospitals in Xuzhou City, all of which were Brucella melitensis, mainly classified as biotype 3 (7/9). In 2023, a total of 984 occupational population with brucellosis were surveyed in Xuzhou City, and the overall awareness rate of brucellosis prevention and control knowledge was 3.25% (32/984). By binary logistic regression analysis, the awareness rates of brucellosis prevention and control knowledge among veterinarians and those with high school or vocational school education, college education or above were relatively high [ OR (95% CI): 21.24 (1.52 - 297.74), 13.32 (3.48 - 50.96), 18.90 (3.68 - 97.13), P < 0.05]. Conclusions:In 2023, the incidence rate of human brucellosis in Xuzhou City is at a relatively high level, with men, middle-aged and elderly people, and farmers as the main population. The awareness rate of brucellosis prevention and control knowledge among occupational population is relatively low, and it is necessary to strengthen publicity, education, and behavioral intervention.
7.Changes in voltage-dependent anion channel 3 in an animal model of sepsis-induced myocardial injury

Objective To observe changes in voltage-dependent anion channel 3(VDAC3)in a mouse model of sepsis-induced myocardial injury and to explore its potential mechanism.Methods Twenty male C57BL/6J mice were divided randomly into a Sham group and Sepsis group,respectively(n=10 mice per group).Sepsis was induced by the cecal ligation and puncture(CLP).Serum levels of interleukin(IL)-6,tumor necrosis factor(TNF)-α,creatine kinase MB(CK-MB),and cardiac troponin T(cTnT)were detected by enzyme-linked immunosorbent assay.Pathological changes in heart tissue were observed by hematoxylin and eosin staining.Structural and functional changes in the heart were evaluated by echocardiography.Changes in total glutathione,reduced glutathione(GSH),oxidized glutathione,and malondialdehyde(MDA)in heart tissue were detected by spectrophotometry.The morphological structure of mitochondria in mouse cardiomyocytes was observed by transmission electron microscopy.Expression levels of IL-6,IL-1β,VDAC3,glutathione peroxidase 4(GPX4),solute carrier family 7 member 11(SLC7A11),lipocalin-2(LCN2),and prostaglandin-endoperoxide synthase 2(PTGS2)mRNA were detected by real-time quantitative polymerase chain reaction and the localization and expression of VDAC3 and GPX4 proteins in mouse heart tissue were detected by immunofluorescence staining.The correlations between VDAC3 mRNA and GPX4,SLC7A11,PTGS2,LCN2,IL-6,and IL-1β mRNA were analyzed.Expression levels of VDAC3,GPX4,and SLC7A11 proteins were detected by Western blot.Results IL-6,TNF-α,CK-MB,and cTnT levels were significantly higher in the Sepsis group compared with the Sham group(P<0.05).In the Sepsis group,myocardial fibers were torn,the ventricular wall was thickened and edematous,the mitochondrial membrane was ruptured,and mitochondrial cristae were broken or absent.GSH levels were significantly reduced in the Sepsis group(P<0.05)and the lipid peroxide MDA was increased in the Sepsis group(P<0.05)compared with the Sham group.VDAC3,GPX4 and SLC7A11 mRNA and protein levels were all lower in the Sepsis group compared with the Sham group(P<0.05),while expression levels of IL-6,IL-1β,LCN2,and PTGS2 mRNA were increased(P<0.05).VDAC3 mRNA was positively correlated with GPX4 and SLC7A11 mRNA levels,and negatively correlated with LCN2,PTGS2,IL-6,and IL-1β.Conclusions VDAC3 expression decreases in myocardial injury,and it may participate in the occurrence of sepsis-induced myocardial injury by regulating ferroptosis.
8.The value of ultrasound elastography in the evaluation of carotid plaque stability and prognosis in patients with cerebral infarction

Objective:To explore the value of ultrasound elastography (USE) in evaluating the stability and prognosis of carotid plaque (CP) in patients with cerebral infarction(CI).Methods:A total of 94 patients with CI admitted to Dongying District People′s Hospital of Dongying City from January 2022 to December 2023 were retrospectively selected as the study objects, all of whom were treated with carotid artery dissection. The presence of CP was confirmed by pathological examination. The head and neck of patients were examined by conventional ultrasound and USE before surgery, respectively, and the diagnostic value of the two methods on CP stability was compared. The difference of strain value and strain rate among different plaque types was compared, and the correlation between different plaque types and the score of ESSEN Stroke Risk Scale (ESRS) was analyzed.Results:The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of conventional ultrasonic detection of CP were 75.00%, 71.43%, 74.00%, 87.10% and 52.63%; and of USE were 90.28%, 89.29%, 90.00%, 95.59%, 78.13%. The strain values and strain rates of stable plaques were lower than those of vulnerable plaques : (0.73 ± 0.11) × 10 3 vs. (2.42 ± 0.57) × 10 3, (6.57 ± 0.84) s -1 vs. (9.36 ± 2.55) s -1, there were statistical differences ( P<0.01). The ESRS scores of patients with vulnerable plaques were mostly ≥3 scores, and those with stable plaques were mostly<3 scores, there was statistical differences ( P<0.01). The scores of National Institutes of Health Stroke Scale (NIHSS) and modified Rankin Scale (mRS) 3 months after the onset of stable plaque were lower than those of vulnerable plaque patients: (26.80 ± 8.47) scores vs. (34.67 ± 8.98) scores, (3.97 ± 0.84) scores vs. (4.55 ± 1.61) scores, there were statistical differences ( P<0.01 or <0.05). Conclusions:USE has significant advantages in detecting CP stability in CI patients, which can efficiently and accurately assess the risk of plaque and provide important information for clinical use.
9.Clinical Efficacy of Suyin Jiedu Prescription in the Treatment of Stage 3-5 Chronic Kidney Disease of Kidney Deficiency and Turbid Toxin Type and Its Effect on Serum Klotho

OBJECTIVE To explore the clinical efficacy of Suyin Jiedu Prescription in the treatment of Stage 3-5 non-dialysis chronic kidney disease of kidney deficiency and turbid toxin type and its effect on serum Klotho and inflammation levels.METHODS A total of 102 patients with Stage 3-5 non-dialysis chronic kidney disease of kidney deficiency and turbid toxin type who met the inclu-sion criteria were randomly divided into a treatment group and a control group,51 cases in each group;a total of 84 cases were comple-ted,including 44 cases in the control group and 40 cases in the treatment group.The control group was given conventional Western medicine basic treatment,and the treatment group was given Suyin Jiedu Prescription on the basis of the control group treatment.The treatment course of both groups was 16 weeks.The levels of renal function indexes[hemoglobin(Hb),serum albumin(Alb),urea ni-trogen(BUN),serum creatinine(Scr),estimated glomerular filtration rate(eGFR),24 h urine protein(UTP)]and TCM syndrome scores were observed in the two groups before and after treatment.The levels of serum Klotho,tumor necrosis factor α(TNF-α)and interleukin 18(IL-18)were detected by ELISA.The clinical efficacy between the two groups after treatment was compared.RE-SULTS After treatment,BUN and Scr levels of the two groups of patients were significantly decreased(P<0.05),eGFR was signifi-cantly increased(P<0.05,P<0.01),the treatment group was better than the control group(P<0.05);the TCM syndrome scores of the two groups were significantly decreased(P<0.05,P<0.01),and the treatment group was better than the control group(P<0.05,P<0.01);the serum Klotho level in the treatment group was significantly increased(P<0.05),the IL-18 level was significantly de-creased(P<0.01),and the serum Klotho,TNF-α,and IL-18 levels in the treatment group were better than those in the control group(P<0.05,P<0.01).The total clinical effective rate in the treatment group was higher than that in the control group(P<0.01).CONCLUSION Suyin Jiedu Prescription can effectively improve renal function and relieve clinical symptoms in patients with Stage 3-5 non-dialysis chronic kidney disease of kidney deficiency and turbid toxin type,which may be related to increasing Klotho level and reducing inflammation in the body.
10.Prevalence and influencing factors of chronic somatic comorbidities in patients with schizophrenia in Shanghai communities

Objective To investigate the prevalence of chronic somatic comorbidities in schizophrenic patients in Shanghai communities,and to explore the factors influencing comorbidities.Methods Based on Shanghai community-based severe mental disorders cohort,5 422 patients with schizophrenia(SCZ)were included in the study.12 common chronic somatic diseases,defined by patients'self-report,were selected to analyze the prevalence of comorbidity,and Logistic regression model was used to analyze the factors influencing the number of somatic comorbidities.Results The total prevalence of somatic comorbidity was 37.0%in 5 422 patients with SCZ,with the highest prevalence of hypertension(22.6%)and diabetes mellitus(13.1%)among 12 somatic diseases selected.Older age was the main factor associated with chronic somatic comorbidities in community schizophrenic patients.The risk of 1-2 comorbidities in patients aged≥60 years was 3.34(95%CI:2.74-4.07)times higher than those aged<45 years,while the risk of≥3 comorbidities was 3.27(95%CI:2.11-5.09)times higher,correspondingly.Female gender,marriage,smoking,and longer duration of illness were also risk factors for comorbidity.Women after menopause had higher risk of comorbidity than perimenopausal women.Conclusion Cardiovascular and metabolic diseases were common somatic comorbidities among schizophrenic patients in Shanghai communities.Older age,female gender,marriage,smoking,and longer duration of illness were risk factors for increasing number of comorbidities.
