Background::Heterotaxy (HTX) is a thoracoabdominal organ anomaly syndrome and commonly accompanied by congenital heart disease (CHD). The aim of this study was to analyze rare copy number variations (CNVs) in a HTX/CHD cohort and to examine the potential mechanisms contributing to HTX/CHD.Methods::Chromosome microarray analysis was used to identify rare CNVs in a cohort of 120 unrelated HTX/CHD patients, and available samples from parents were used to confirm the inheritance pattern. Potential candidate genes in CNVs region were prioritized via the DECIPHER database, and PNPLA4 was identified as the leading candidate gene. To validate, we generated PNPLA4-overexpressing human induced pluripotent stem cell lines as well as pnpla4-overexpressing zebrafish model, followed by a series of transcriptomic, biochemical and cellular analyses. Results::Seventeen rare CNVs were identified in 15 of the 120 HTX/CHD patients (12.5%). Xp22.31 duplication was one of the inherited CNVs identified in this HTX/CHD cohort, and PNPLA4 in the Xp22.31 was a candidate gene associated with HTX/CHD. PNPLA4 is expressed in the lateral plate mesoderm, which is known to be critical for left/right embryonic patterning as well as cardiomyocyte differentiation, and in the neural crest cell lineage. Through a series of in vivo and in vitro analyses at the molecular and cellular levels, we revealed that the biological function of PNPLA4 is importantly involved in the primary cilia formation and function via its regulation of energy metabolism and mitochondria-mediated ATP production. Conclusions::Our findings demonstrated a significant association between CNVs and HTX/CHD. Our data strongly suggested that an increased genetic dose of PNPLA4 due to Xp22.31 duplication is a disease-causing risk factor for HTX/CHD.
3.High tibial osteotomy on varus knee osteoarthritis with medial meniscus posterior root injury

Objective To explore clinical effect of distal tibial tubercle-high tibial osteotomy(DTT-HTO)in treating knee osteoarthritis(KO A)with medial meniscus posterior root tear(MMPRT).Methods A retrospective analysis was performed on 21 patients with varus KOA with MMPRT from May 2020 to December 2021,including 3 males and 18 females,aged from 49 to 75 years old with an average of(63.81±6.56)years old,the courses of disease ranged from 0.5 to 18.0 years with an average of(5.9±4.2)years,and 4 patients with grade Ⅱ,14 patients with grade Ⅲ,and 3 patients with grade Ⅳ according to Kellgren-Lawrence;14 patients with type 1 and 7 patients with type 2 according to MMPRT damage classification.The distance of medi-al meniscusextrusion(MME)and weight-bearing line ratio(WBLR)of lower extremity were compared before and 12 months after operation.Visual analogue scale(V AS),Western Ontarioand and McMaster Universities(WOMAC)osteoarthritis index,and Lysholm knee score were used to evaluate knee pain and functional improvement before operation,1,6 and 12 months after operation,respectively.Results Twenty-one patients were followed up for 12 to 18 months with an average of(13.52±1.72)months.MME distance was improved from(4.99±1.05)mm before operation to(1.87±0.76)mm at 12 months after operation(P<0.05).WBLR was increased from(15.49±7.04)%before operation to(62.71±2.27)%at 12 months after operation(P<0.05).VAS was decreased from(7.00±1.14)before operation to(2.04±0.80),(0.90±0.62)and(0.61±0.50)at 1,6 and 12 months after operation.WOMAC were decreased from preoperative(147.90±9.88)to postoperative(103.43±8.52),(74.00±9.54)and(47.62±9.53)at 1,6 and 12 months,and the difference were statistically significant(P<0.05).Lysholm scores were increased from(46.04±7.34)before oepration to(63.19±8.93),(81.10±6.41)and(89.29±3.04)at 1,6 and 12 months after operation(P<0.05).Conclusion For the treatment of varus KOA with MMPRT,DTT-HTO could reduce medial meniscus pro-trusion distance,improve the ratio of lower limb force line,and effectively reduce knee pain and improve knee joint function.
4.Effects of Different Sequential Enzymatic Cleavage of Trypsin and LysC on Proteomic Sample Preparation

In mass spectrometry-based proteomics experiments,achieving high-throughput and efficientproteolytic digestion is crucial to ensure optimal protein cleavage and enhance the depth of protein identi-fication (including the number of identified proteins and the coverage of protein amino acid sequences) .Trypsin is the most widely used protease in mass spectrometry-based proteomics due to its ability to spe-cifically cleave the carboxyl terminus of arginine and lysine.However,it was found that Trypsin has some missed enzymatic efficiency for the cleavage of lysine residues.Therefore,in actual proteomics sample preparation,a combination of Trypsin and LysC will be used to ensure adequate cleavage of lysine resi-dues.Our study revealed that the commonly employed LysC-Trypsin tandem cleavage method exerts an impact on the enzymatic cleavage of protein samples by Trypsin due to the subsequent cleavage of Trypsin by initially added LysC.Consequently,we adjusted the order of LysC and Trypsin tandem digestion,with Trypsin cleavage being performed first followed by the addition of LysC to target any missed lysine resi-dues.We comprehensively compared and analyzed three distinct sequential digestion methods,namely Trypsin-Trypsin (T-T),LysC-Trypsin (L-T),and Trypsin-LysC (T-L),in terms of their effects on pro-tein sample preparation quality.The results demonstrated that the Trypsin-LysC sequential digestion ap-proach not only minimizes missed protein lysine/arginine cleavage sites without increasing experimental costs,at the same time yielding peptides with a moderate amino acid sequence length.The use of Tryp-sin-LysC digestion enhances the adsorption and separation of peptide samples in RP-HPLC,as well as improves the depth of protein detection and amino acid sequence coverage during tandem mass spectrome-try analysis.This research work offers a novel technical solution and serves as a valuable reference for proteome sample preparation.
5.Chemical constituents from Cleidion brevipetiolatum and their anti-inflammatory activities

AIM To study the chemical constituents from Cleidion brevipetiolatum pax et Hoffm.and their anti-inflammatory activities.METHODS The extract from C.brevipetiolatum was isolated and purified by silica gel,MCI,Rp-18,Sephadex LH-20,preparative HPLC,then the structures of obtained compounds were identified by physicochemical properties and spectral data.The MTT and Griess methods were used to evaluate the anti-inflammatory activities of compounds.RESULTS Seventeen compounds were isolated and identified as cleomiscosin C(1),scopoletin(2),fraxedin(3),isofraxidin(4),luteolin(5),apigenin(6),chrysoeriol(7),1-hydroxy-2-O-β-D-glucopyranosyl-4-allylbenzene(8),1-O-β-D-glucopyranosyl-4-allylbenzene(9),trans-1-(4-propenyl)-phenol-β-D-glucopyranosyl(10),benzyl-O-β-D-glucopyranoside(11),2-phenylethyl-1-O-β-D-glucopyranoside(12),(+)-syringaresinol(13),aurantiamide(14),(S)-(+)-2-cis-abscisic acid(15),loliolide(16),and hydroxychavicol(17).The ethanol extract of C.brevipetiolatum and its ethyl acetate portion showed NO inhibition with IC50 values of(44.11±5.29),(24.25±3.59)μg/mL,respectively.The IC50 values of compounds 2,and 5-7 were 3.55-14.53 μmol/L.CONCLUSION Compounds 1-7 and 13-17 are isolated from this plant for the first time.Simple coumarins and flavones from this plant show good inhibition of the production of NO.
6.Clinical characteristics of psoriasis and current status of medical care for patients in county areas of China

Objective:To investigate the clinical characteristics of psoriasis and status quo of medical care for patients in county areas of China.Methods:This study was a cross-sectional investigation. Based on the “Qianxian Wuyin” Project (a national project for upgrating ability for psoriasis care at county level), an online questionnaire survey was conducted in the dermatology departments of 459 county hospitals in 404 pilot administrative counties across China from February to June 2023. The questionnaire included demographic information of patients (gender, ethnicity, age, place of residence, education, marital status), and clinical characteristics of psoriasis (disease course, type, comorbidities, body surface area (BSA) and previous treatment. The Dermatology Life Quality Index (DLQI) and Psoriasis Area and Severity Index (PASI) were applied for assessing the quality of life and disease severity, and completed by patients or guardian and doctors, respectively.Results:A total of 16 935 patients completed the questionnaire. The age of patients was 1-102(44.17±11.58)years, and 71.0% (12 036/16 935) were 30-59 years old. The ratio of male to female was 2.21∶1; 24.3%(4 117/16 935) of patients had high school education; there were 9 940 patients(58.7%) with previous or current smoking and/or alcohol use; 42.8%(7 218/16 855) of patients had a disease course of 1-5 years. There were 15 630 patients(92.3%) with DLQI≥10, 8 346 patients(49.7%) with PASI≥10, 15 017 patients(89.2%) with BSA≥10%. The plaque type was the most common disease type ( n=14 965, 88.7%), and spotting type ranked the second ( n=1 141, 6.8%). The most common initial site was the trunk ( n=12 309, 72.9%). Among the comorbidities, hypertension was the most common one ( n=1 681, 10.0%). There were 7 650 reports of treatment response to conventional topical drug therapy and 3 112 reports of treatment response to systemic drug therapy, with 6 269 (81.9%) and 2 493 (80.1%) reporting poor or no response, respectively. Conclusions:The survey shows that in the county areas of China, the majority of psoriasis patients are severe patients with short course of disease, plaque type is the most common type, and hypertension is the most common comorbidity; and the conventional treatment is less effective for most patients.
7.Analysis of factors influencing the prognosis of patients with chronic liver disease

Objective To explore the risk factors for the prognosis of patients with chronic liver disease(CLD).Methods Ninety-eight CLD patients admitted to Zhumadian Central Hospital and completed follow-ups from June 2016 to June 2017 were selected as the research subjects.The patients were divided into a death group(n=30)and a non-death group(n=68),with death or not as the outcome indicator.Univariate and multivariate logistic regression were used to analyze the risk factors for the prognosis of CLD patients.Results Body mass index(BMI).nutritional risk screening 2002(NRS 2002)score,sarcopenia(SAR),and serum albumin,alanine transaminase(ALT),and aspartate transaminase(AST)levels of patients with CLD were associated with their prognosis(P<0.05).Multivariate logistic regression analysis showed that NRS 2002 score ≥ 3,comorbid SAR,reduced serum albumin level,increased ALT level,and increased AST level were independent risk factors for the prognosis of CLD patients(P<0.05).Conclusion CLD patients with NRS 2002 score ≥3,comorbid SAR,reduced serum albumin level,elevated ALT level,and elevated AST level are at higher risk of death.
8.Applied effects of airway lavage of fiberoptic bronchoscope under CT location combined with ultrasound monitoring diaphragmatic movement in mechanical ventilation of pediatric severe pneumonia

Objective:To investigate the applied effect of airway lavage of fiberoptic bronchoscope under computed tomography(CT)location combined with ultrasound monitoring diaphragmatic movement in mechanical ventilation of pediatric severe pneumonia.Methods:A total of 70 pediatric patients with severe pneumonia who received mechanical ventilation in The People's Hospital of Liupanshui City and Liupanshui Maternal and Child Health Care Hospital from October 2022 to October 2023 were selected,and they were divided into a control group and an observation group according to the random number table method,with 35 cases in each group.The control group did not undergo CT examination for lung before lavage with bronchoscopy,while the observation group received CT examination before airway lavage.Both two groups simultaneously adopted B ultrasound to monitor diaphragm movement.Heart rate,respiration,oxygen saturation,respiratory mechanics index,pediatric early warning score(PEWS)and modified pneumonia survivor index(mPIRO)score were detected before and after lavage,and the movement and thickness of diaphragm of children were measured by B ultrasound.Results:The heart rate and respiratory frequency of two groups after treatment were significantly lower than those before treatment,and the blood oxygen saturation was significantly higher than that before treatment.The heart rate and respiratory frequency of the observation group were respectively(80.23±10.07)times/min and(18.11±5.03)times/min,which were significantly lower than those of the control group,and the blood oxygen saturation was significantly higher than that of the control group,and the differences were statistical significance(t=3.397,3.939,7.719,P<0.05),respectively.The pulmonary pressure,peak pressure,plateau pressure,average airway pressure,chest pressure and abdominal pressure of two groups after treatment were significantly lower than those before treatment,and the levels of the above indicators in the observation group were significantly lower than those in the control group,with statistical significances(t=12.323,8.141,16.733,12.298,11.375,18.248,P<0.05),respectively.The PEWS scores and total scores of body temperature,pulse,respiration and blood pressure of two groups after treatment were significantly lower than those after treatment,and the above scores in the observation group were significantly lower than those in the control group,with statistical significance(t=67.309,58.148,35.997,30.218,5.271,P<0.05),respectively.The mPRIO scores and total scores of age,test indicators of laboratory,symptom duration,respiratory distress index and treatment plan in two groups after treatment were significantly higher than those before treatment,and the above scores in the observation group were significantly higher than those in the control group,with statistical significances(t=7.432,7.153,7.268,5.088,4.645,4.091,P<0.05),respectively.The diaphragm movement and thickness of two groups after treatment were significantly higher than those before treatment,and the above indicators in the observation group were significantly higher than those in the control group,with statistical significances(t=63.444,6.803,P<0.05),respectively.Conclusion:Compared with conventional airway lavage,the airway lavage of fiberoptic bronchoscope under CT location combined with B ultrasound of monitoring diaphragm has higher accuracy and reliability.
9.The efficacy of artificial(nonbiological)extracorporeal liver support system for primary biliary cholangitis and the effect of this therapy on prognosis

Objective To evaluate the clinical efficacy of plasma exchange(PE)and double plasma molecular absorption system(DPMAS)in the treatment of primary biliary cholangitis(PBC)and the effect of this therapy on prognosis.Methods The clinical data on 526 PBC patients in our hospital from December 2013 to January 2022 were retrospectively analyzed.The patients were divided into different groups according to different therapies and then matched with propensity.The changes in symptoms,laboratory indexes and MELD scores were compared between two groups before and after treatment,and the clinical efficacy of artificial liver treatment for PBC patients was assessed.The effect of this treatment on the survival outcomes in these patients via comparing the cumulative survival rates at 3,6 and 12 months between the two groups.Results The efficiency was better in the group with artificial liver treatment in addition medical therapy than the group with medical treatment alone,the difference was statistically significant(76.7%vs.55.8%,χ2 = 4.214,and P = 0.040).Cox proportional risk regression showed that TBIL was an independent risk factor affecting the 3-,6-,or 12-month survival in PBC patients.Conclusions Artificial liver support system can effectively relieve symptoms,reduce levels of ALT,AST and TBIL,improve blood coagula-tion function,and lower MELD scores in PBC patients.This therapy revealed a trend of improvement in 3-,6-,or 12-month survival outcomes.
10.Compliance and correctness of hand hygiene of health care workers in intensive care units of a tertiary first-class hospital

Objective To study the compliance and correctness of hand hygiene(HH)of staff in intensive care units(ICUs)of a tertiary first-class hospital,and provide theoretical basis for HH intervention.Methods In April 2023,staff in 17 ICUs of this hospital were performed on-site survey by infection control staff,and monitoring forms about HH compliance and correctness were filled out.Results A total of 874 HH opportunities were ob-served with the concealed observation method,501 HH opportunities were implemented,the compliance rate was 57.32%,273 HH opportunities were correctly implemented,with an correct rate of 54.49%.The compliance and correct rate of HH among staff in different ICUs varied significantly.Compliance and correct rates of HH among staff with different jobs were statistically different:HH compliance rate of cleaners(31.97%)was lower than that of nurses(63.83%),doctors(58.77%)and other personnel(58.14%);HH correct rate of cleaners(30.77%)was lower than that of nurses(58.17%).The causes for not implementing HH among staff with different jobs and at different HH opportunities were statistically different:the rate of not implementing any HH measures after con-tact with patients(84.75%)was higher than before contact with patients(41.27%),before clean and sterile ma-nipulation(30.00%),as well as after contact with blood and body fluid(45.45%).The rate of not implementing any HH measures after contact with the patient's surrounding environment(66.67%)was higher than before con-tact with patient as well as before clean and sterile manipulation.The rates of incomplete HH steps and insufficient HH time among staff with different jobs were statistically different:The rates of incomplete HH steps of other per-sonnel(82.35%)was higher than that of doctors(52.63%).The rates of insufficient HH time of doctors(82.46%)and nurses(78.18%)were higher than that of cleaners(51.85%).Conclusion The implementation of HH among different occupational groups and at different HH implementation opportunities in ICU is significantly different,which should be intervened based on their characteristics.
