ObjectiveTo develop a classification model based on complete blood count (CBC) parameters combined with clinical factors to predict severe respiratory infections caused by Human adenovirus (HAdV) in pediatric patients. MethodsFrom September 2023 to September 2024, the CBC parameters and related clinical data from pediatric patients diagnosed with HAdV infection were collected. Principal component analysis and random forest models were used to identify potential predictors of severe cases. ResultsA total of 668 pediatric patients were included, with 564 cases assigned to the training cohort and 104 cases to the validation cohort. Severe cases were defined as pneumonia and/or fever lasting ≥5 days (pneumonia or prolonged fever, PorPF). Principal component analysis and feature importance analysis (Mean Decrease Gini value) identified the monocytosis ratio (PMono), red blood cell count (RBC), and platelet count (PLT) as the most critical CBC parameters. Logistic regression analysis revealed that oxygen therapy (OR = 4.367, 95% CI: 1.568–12.161) and increased work of breathing (OR = 3.904, 95% CI: 2.146–7.101) were relative risk factors for PorPF. Meanwhile, higher PMono (OR = 0.696, 95% CI: 0.640–0.757), RBC (OR = 0.201, 95% CI: 0.124–0.325), and PLT (OR = 0.990, 95% CI: 0.987–0.994) were protective factors. When PMono was used as a predictive marker for PorPF, the area under the receiver operating characteristic curve (AUC) was 0.648 and 0.705, respectively. A random forest model incorporating four risk factors [PMono, RBC, PLT, and hematocrit (HCT)] was constructed to classify PorPF and general cases, achieving AUCs of 0.688 and 0.768, respectively. ConclusionsPMono, RBC, and PLT may serve as characteristic CBC indicators for predicting pneumonia or prolonged fever in children with HAdV infection. A risk factor model built using PMono, RBC, PLT, and HCT offers a relatively simple and accurate approach to predicting severe cases in pediatric HAdV infections.

The major histocompatibility complex(MHC)is closely related to immune regulation.MHC shows distinct genetic polymorphism, and there are also species differences in MHC restriction. The human MHC is called human leukocyte antigen(HLA),and the mouse MHC is called H-2.The construction of humanized MHC transgenic mouse models is an important strategy to overcome the differences in MHC among species and simulate the characteristics of a human immune response.MHC transgenic mice are mainly divided into MHC Ⅰ or MHC Ⅱ single-transgenic mouse models and MHC Ⅰ and MHC Ⅱ double-transgenic mouse models. The development of HLA Ⅰ transgenic mouse model went through three stages,at present, the strategy of knocking out H-2Kb and H-2Db or murine β2m is adopted to eliminate the competitive inhibition of HLA Ⅰ molecules by endogenous H-2 class Ⅰ molecules. In the construction of an HLA Ⅱtransgenic mouse model, the β strand of murine origin is knocked out and HLA Ⅱ class genes are inserted. With the optimization of construction strategies,MHC transgenic mouse models have been applied to epitope vaccine development, tumor treatment, and genetic disease-association studies, becoming a powerful tool for preclinical trials. In this paper,we summarize the relevant data on MHC transgenic mouse models,as well as the construction strategies used for MHC transgenic mouse models and their application in vaccine development and disease treatment.

Thyroid associated ophthalmopathy (TAO) is an organ-specific autoimmune disease. TAO is clinically classified into active and inactive stage, and the accurate judgment is the key point of treatment choice. Clinical activity score (CAS) is often used for the assessment of TAO activity, which is subjective to some extent. With the development of imaging techniques, ultrasonography, CT, MRI and radionuclide imaging have gradually been applied into the diagnosis and treatment of TAO. What′s more, the imaging is an important complement to CAS from the aspects of anatomical and functional metabolism, which can better assess the activity and the therapy response of TAO. The clinical value of medical imaging in activity and treatment efficacy evaluation of TAO is reviewed in this article.

Chinese Journal of Radiation Oncology 2024;33(8):772-777
Radiation therapy is an essential component of comprehensive treatment for breast cancer. In comparison to photons, proton beam, with its unique Bragg peak, limits the dose distribution to normal tissues around the tumor while improving dose coverage within the target area. Therefore, it could be an appealing therapeutic option for breast cancer treatment. Current researches indicate that proton therapy for breast cancer treatment has a distinct dosimetric advantage in protecting the heart and lungs. However, there are potentially increased risks of side-effects such as skin reactions and rib fractures, etc. Future Subsequent studies should seek a better understanding of the relative biological effectiveness changes along the proton beam range to improve the proton plan evaluation. Additionally, efforts should be directed towards identifying patients who would derive maximum benefit from proton therapy, addressing concerns related to its limited accessibility and high cost. This review provides an overview of the current clinical experiences, research progress and controversies in proton therapy for breast cancer.

The aim is to evaluate the effect of therapeutic communication on cervical cancer patients’ preoperative anxiety and hope level. The convenience sampling method was used to select the inpatients who will receive radical surgery for cervical cancer in the department of obstetrics and gynecology of the Second Affiliated Hospital of Xi’an Jiaotong University from November 2016 to November 2019 as the research object. 50 patients were grouped by the random number table method: 25 patients were in the intervention group, and 25 patients were in the control group. Patients in the intervention group were given therapeutic communication on the basis of routine nursing, and patients in the control group were given routine nursing. Both groups were investigated with the Self-Rating Anxiety Scale (SAS) and Herth Hope Index (HHI) on the first day of admission and the day before surgery. Before the intervention, there was no statistically significant difference between the two groups (P>0.05) . After the intervention, the anxiety level of the intervention group was lower than that of the control group (P<0.05), and the hope level was higher than that of the control group (P<0.05). It can be seen that therapeutic communication can alleviate preoperative anxiety of cervical cancer patients, improve their hope level, promote patient recovery, and ease tense medical relationship.
