1.Analysis of the efficacy of traditional Chinese medicine for diabetic retinopathy based on evidence body quality assessment

OBJECTIVE To evaluate the quality of evidence in the systematic evaluation/meta-analysis of traditional Chinese medicine (TCM) for diabetes retinopathy (DR) based on the GRADE system. METHODS Chinese and English databases were searched to obtain the relevant studies of systematic evaluation/meta-analysis of traditional Chinese medicine in the treatment of DR. The search time was from the establishment of each database to January 13th, 2024. According to the inclusion and exclusion criteria, literature screening was conducted. After extracting relevant information from the included literature, the GRADE system was used to evaluate the quality level of the evidence body in the included studies, and the evidence of the outcome indicators was integrated and summarized. RESULTS A total of 51 studies were ultimately included, encompassing 135 outcome indexes. Among these, 19 indicators (14.1%) were of high quality, 87 (64.4%) were of medium quality, 26 (19.3%) were of low quality, and 3 (2.2%) were of very low quality. Overall, the evidence quality of the outcome indicators in the included studies was medium to low quality. The integrated results of evidence on the efficacy of outcome indexes showed that compared with conventional Western medicine, calcium dobesilate or placebo, TCM had significant advantages in improving overall efficacy, reducing bleeding spot area, reducing macular foveal thickness, and increasing visual improvement rate. In addition,the combination of TCM and conventional Western medicine or calcium dobesilate was significantly more effective than using conventional Western medicine or calcium dobesilate alone. CONCLUSIONS The overall quality of the evidence in the systematic evaluation/meta-analysis study on the treatment of DR with TCM is medium to low quality. Based on existing research findings, TCM demonstrates good clinical efficacy in the treatment of DR.
2.Analgesic effect and mechanism of punicalagin on neuropathic pain in rats

OBJECTIVE To investigate the analgesic effect and potential mechanism of punicalagin on neuropathic pain (NP) rats based on the hypoxia-inducible factor-1α (HIF-1α)/nucleotide-binding domain leucine-rich repeat and pyrin domain-containing receptor 3 (NLRP3) signaling pathway. METHODS Male SD rats were randomly divided into sham operation group (18 rats) and modeling group (72 rats). NP rat model was established by chronic constriction injury (CCI) of sciatic nerve. The successfully modeled rats were divided into NP group, 2-methoxyestradiol group (HIF-1α antagonist 10 mg/kg), punicalagin group (300 mg/kg), and punicalagin+dimethyloxaloglycine group (punicalagin 300 mg/kg+HIF-1α agonist 175 mg/kg), with 18 rats in each group. Rats in each group were injected intraperitoneally and/or intragastrically with the corresponding solution or 1% dimethyl sulfoxide/normal saline, once a day, for 14 consecutive days. After the last administration, the mechanical withdrawal threshold (MWT), thermal withdrawal latency (TWL), the levels of tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β) and IL-6 in spinal cord tissue were detected; the morphological changes in the spinal dorsal horn were observed. Apoptosis rate of spinal dorsal horn neurons, the co-localization of NLRP3/ionized calcium binding adapter molecule 1 (Iba-1) (calculated by the number of NLRP3+/Iba-1+ cells) and the protein expressions of HIF-1α, NLRP3, apoptosis-associated speck-like protein containing a CARD (ASC) and caspase-1 in spinal cord tissue were detected. RESULTS Compared with the sham operation group, neurofibril in spinal dorsal horn of rats in NP group was thickened and wound into knots, and vacuolar degeneration containing silver granules was observed; the MWT and TWL were reduced or shortened; the levels of TNF-α, IL-1β and IL-6 in spinal cord tissue, the apoptosis rate of spinal dorsal horn neurons, the number of NLRP3+/Iba-1+ cells, and protein expressions of HIF-1α, NLRP3, ASC and caspase-1 were significantly increased or up-regulated (P<0.05). Compared with the NP group, the above indexes were significantly improved in the 2-methoxyestradiol group and punicalagin group (P<0.05), while dimethyloxaloglycine could significantly reverse the improvement effect of punicalagin on the above indexes (P<0.05). CONCLUSIONS Punicalagin can relieve pain in NP rats, and its analgesic effect may be achieved by inhibiting HIF-1α/NLRP3 signaling pathway and blocking the activation of ma0o4e@163.com NLRP3 inflammasome in spinal dorsal horn microglia.
3.Effects of di(2-ethylhexyl) phthalate on glucose homeostasis in rats due to impaired autophagy flux of islet β cells mediated by oxidative stress

Background Di(2-ethylhexyl) phthalate (DEHP) is the most prevalent environmental endocrine disruptor among phthalate acid esters (PAEs) worldwide. Previous studies have indicated that exposure to DEHP may disrupt glucose metabolism. Objective To investigate the impact of DEHP on glucose homeostasis in rats, focusing on oxidative stress-induced impairment of autophagy in islet β cells. Methods Forty male SD rats were randomly assigned to four groups, receiving DEHP doses of 0, 187, 375, and 750 mg·kg−1 for 12 weeks. Oral glucose tolerance (OGTT) and insulin tolerance tests (ITT) were conducted 24 h after the final exposure. Pancreatic microstructural alterations were assessed using hematoxylin and eosin (HE) staining and transmission electron microscopy (TEM). Commercial ELISA kits were employed to quantify the levels of insulin, adenosine triphosphate (ATP), and adenosine monophosphate (AMP) in rat serum, as well as the protein expression level of activated caspase-3 in pancreatic tissue. Additionally, commercial microplate kits were utilized to measure the concentration of reduced glutathione (GSH) in serum, the activity of superoxide dismutase (SOD) using water-soluble tetrazolium salt-1, the content of malondialdehyde (MDA) by thiobarbituric acid method, and the level of reactive oxygen species (ROS) in pancreatic tissue by chemical fluorescence method. Reverse transcription polymerase chain reaction (RT-PCR) was used to measure sequestosome1 (SQSTM1/p62), Beclin1, microtubule-associated protein 1 light chain 3 (LC3), and cysteinyl aspartate specific proteinase-8 (Caspase-8) mRNA levels. Western blot analysis was applied to detect the protein relative expression levels of p62, Beclin-1, LC3-I, LC3 II, AMPK, p-AMPK, mTOR, p-mTOR, ULK1, and Caspase-8. Results Compared to the 0 mg·kg−1 DEHP group, the 750 mg·kg−1 DEHP group exhibited a significant increase in fasting blood glucose levels at 2, 4, 6, and 12 weeks (P<0.05). The OGTT showed that, following high-glucose gavage, the 187 mg·kg−1 DEHP group had elevated blood glucose at 30 min (P<0.05), the 375 mg·kg−1 DEHP group showed increased glucose levels at 15, 30, and 180 min (P<0.05), and the 750 mg·kg−1 DEHP group exhibited elevated levels at 15, 30, 60, and 180 min (P<0.05). The 375 and 750 mg·kg−1 DEHP groups demonstrated significantly increased OGTT area under the curve (AUC) values (P<0.05). In contrast, ITT results indicated no significant differences in blood glucose levels or AUC among the DEHP exposure groups at all time points (P>0.05). Compared to the 0 mg·kg−1 DEHP group, the 750 mg·kg−1 DEHP group exhibited significantly higher HOMA-IR levels and markedly lower HOMA-ISI values (P<0.05). HE and TEM showed that in each DEHP exposure group, the number of islet cells decreased, the islet area reduced, and chromatin condensation occurred. The endocrine granules in the cytoplasm of islet β cells decreased, and there were varying degrees of widening of the nuclear membrane gap, flattening and expansion of the Golgi complex, and expansion of the endoplasmic reticulum. Ribosome separation was observed, and autophagosomes were visible. In the 375 and 750 mg·kg−1 DEHP groups, the mitochondria were deformed to varying degrees, and some cristae structures disappeared, presenting vacuolization. Moreover, the chromatin condensation in the nuclei was more severe in the 750 mg·kg−1 DEHP group. The serum SOD activity was significantly elevated in the 750 mg·kg−1 DEHP group (P<0.05). Both the 375 mg·kg−1 and 750 mg·kg−1 DEHP groups exhibited a significant increase in the relative ROS content in pancreatic tissue (P<0.05). In DEHP-treated groups, the MDA content increased (P<0.05), while the GSH content decreased (P<0.05). Additionally, in the 750 mg·kg−1 DEHP group, the AMP/ATP ratio in serum was significantly raised (P<0.05), and the expression of cleaved Caspase-3 protein in pancreatic tissue was also significantly increased (P<0.05). The relative mRNA levels of p62, Beclin-1, LC3, and Caspase-8 in the pancreatic tissue of rats exposed to DEHP were significantly elevated (P<0.05). The relative expression levels of p-AMPK/AMPK, p-ULK1/ULK1, and Beclin-1 proteins in the DEHP-treated groups were significantly increased (P<0.05). In the 375 mg·kg−1 and 750 mg·kg−1 DEHP treatment groups, the relative expression levels of p62, LC3 II/LC1, and Caspase-8 proteins were significantly increased (P<0.05), while the relative expression level of p-mTOR/mTOR was significantly decreased (P<0.05). Conclusion DEHP can disrupt glucose homeostasis by inducing oxidative stress, which subsequently activates autophagy via the ROS/AMPK/ULK1 pathway, impairing autophagic flux and promoting apoptosis of islet β cells, ultimately decreasing their function and number.
4.Changes in the levels of miR-34a and miR-29b in lens epithelial cells of patients with age-related cataract and their clinical significance

AIM: To investigate the changes of microRNA-34a(miR-34a)and microRNA-29b(miR-29b)levels in lens epithelial cells of age-related cataracts(ARC)patients and their clinical significance.METHODS: A total of 65 ARC patients(study group)and 53 cases of clear lens anterior capsulorhexis(control group)who visited our hospital from February 2023 to February 2024 were gathered. Pearson was applied to test the correlation between miR-34a and miR-29b. Multifactor Logistic regression was applied to determine the factors affecting the occurrence of ARC.RESULTS: Compared with the control group, the expression levels of miR-34a and miR-29b in lens epithelial cells of the research group showed a significant decrease trend(all P<0.05). There was a positive correlation between miR-34a and miR-29b in the lens epithelial cells of ARC patients(r=0.472, P<0.05). MiR-34a and miR-29b were influence factors for ARC(all P<0.05).CONCLUSION: The levels of miR-34a and miR-29b in lens epithelial cells of ARC patients are significantly reduced, which is associated with the occurrence of ARC.
5.Herbal Textual Research on Tribuli Fructus and Astragali Complanati Semen in Famous Classical Formulas

By systematically combing ancient and modern literature, this paper examined Tribuli Fructus and Astragali Complanati Semen(ACS) used in the famous classical formulas from the aspects of name, origin, production area, harvesting and processing, clinical efficacy, so as to provide a basis for the development of famous classical formulas containing such medicinal materials. The results showed that the names of Tribuli Fructus in the past dynasties were mostly derived from its morphology, and there were nicknames such as Baijili, Cijili and Dujili. The name of ACS in the past dynasties were mostly originated from its production areas, and there were nicknames such as Baijili, Shayuan Jili and Tongjili. Because both of them had the name of Baijili, confusion began to appear in the Song dynasty. In ancient and modern times, the main origin of Tribuli Fructus were Tribulus terrestris, and ancient literature recorded the genuine producing areas of Tribuli Fructus was Dali in Shaanxi and Tianshui in Gansu, but today it is mainly cultivated in Anhui and Shandong. The fruit is the medicinal part, harvested in autumn throughout history. There is no description of the quality of Tribuli Fructus in ancient times, and the plump, firm texture, grayish-white color is the best in modern times. Traditional processing methods for Tribuli Fructus included stir-frying and wine processing, while modern commonly used is purified, fried and salt-processed. The ancient records of Tribuli Fructus were spicy, bitter, and warm in nature, with modern research adding that it is slightly toxic. The main effects of ancient and modern times include treating wind disorders, improving vision, promoting muscle growth, and treating vitiligo. The mainstream base of ACS used throughout history is Astragalus complanatus. Ancient texts indicated ACS primarily originated from Shaanxi province. Today, the finest varieties come from Tongguan and Dali in Shaanxi. The medicinal part is the seed, traditionally harvested in autumn. Modern harvesting occurs in late autumn or early winter, followed by sun-drying. Ancient texts valued seeds with a fragrant aroma as superior, while modern standards prioritize plump, uniform and free of impurities. Traditional processing methods for ACS included frying until blackened and wine-frying, while modern practice commonly employs purification methods. In terms of medicinal properties, the ancient and modern records are sweet and warm in nature. Due to originally classified under Tribuli Fructus, its effects were thus regarded as equivalent to those of Tribuli Fructus, serving as the medicine for treating wind disorders, additional functions included tonifying the kidneys and treating vitiligo. The present record of its efficacy is to tonify the kidney and promote Yang, solidify sperm and reduce urine, nourish the liver and brighten the eye, etc. Based on the textual research results, it is suggested that when developing the famous classical formulas of Tribuli Fructus medicinal materials, we should pay attention to the specific reference object of Baijili, T. terrestris and A. complanatus should be identified and selected, and the processing method should be in accordance with the requirements of the formulas.
6.Study on the chemical constituents of the active parts of Piper wallichii

OBJECTIVE To analyze the chemical constituents of the active parts of Piper wallichii. METHODS The petroleum ether-extract fraction was prepared from the methanol extract of P. wallichii. Separation and purification were performed using semi-preparative high-performance liquid chromatography. The structures of the compounds were identified by nuclear magnetic resonance spectroscopy. RESULTS Nineteen compounds were isolated from the petroleum ether-extract fraction from the methanol extract of P. wallichii, identified as 3-acetoxybenzyl benzoate (1), 2-acetoxybenzyl benzoate (2), 2-methoxybenzyl benzoate (3), 3-methoxybenzyl benzoate (4), 4-hydroxy-3-methoxybenzyl benzoate (5), 3-hydroxybenzyl benzoate(6), benzyl benzoate (7), ganschisandrine (8), lancifolin A (9), (7R,8R,3′R)-7-acetoxy-3′,4′-dimethoxy-3,4-methylenedioxy-6′-oxo- Δ1′,4′,8′-8.3′-lignan (10), (7S,8R,3′S)-Δ8′-3′,6′-dihydro-3′-methoxy-3,4-methylenedioxy-6′-oxo-8.3′,7.O.4′-lignan (11), (7R, 8R,3′S)-Δ8′-3′,6′-dihydro-3′-methoxy-3,4-methylenedioxy-6′-oxo-8.3′,7.O.4′-lignan (12), isodihydrofutoquinol A (13), licarin A (14), licarin B (15), 2-(2′,5′-dimethoxyphenyl)-3,4- dimethyl-5-(3″,4″-dimethoxyphenyl)- tetrahydrofuran (16), galgravin (17), velutin (18), and piyunin A (19). CONCLUSIONS Compound 1 is a new benzyl benzoate compound. Compounds 3-5, 8 and 9 are isolated from the Piper genus for the first time, while compounds 2, 6, 10-13 and 15-19 are isolated from P. wallichii for the first time.
7.Trend in disease burden of lung cancer in cancer registration areas of Guizhou Province from 2017 to 2021

Objective:
To investigate trends of incidence, mortality, and years of life lost (YLL) rate of lung cancer in cancer registration areas of Guizhou Province from 2017 to 2021, so as to provide references for formulating lung cancer prevention and control strategies and reducing the disease burden of lung cancer.
Methods:
The qualified lung cancer registration data from cancer registration areas of Guizhou Province from 2017 to 2021 were collected, the crude incidence and mortality of lung cancer were calculated by urban/rural areas, genders and ages. The standardized incidence and standardized mortality was calculated using the age structure of the standard population from the Fifth National Population Census in 2000. YLL was calculated using the standard life table from the Global Burden of Disease Study 2019. The disease burden of lung cancer was assessed using incidence, mortality, and YLL rate, and the trend in the disease burden of lung cancer from 2017 to 2021 was calculated using annual percent change (APC).
Results :
From 2017 to 2021, the crude incidence, standardized incidence, crude mortality, standardized mortality, YLL and YLL rate in Guizhou Province were 53.13/100 000, 37.58/100 000, 42.77/100 000, 29.44/100 000, 98.19 thousand person-years and 10.95‰, respectively. The standardized incidence and standardized mortality of lung cancer were higher in rural areas than in urban areas (39.45/100 000 vs. 34.23/100 000, 30.68/100 000 vs. 27.18/100 000). The standardized incidence and standardized mortality of lung cancer were higher in males than in females (49.34/100 000 vs. 26.47/100 000, 41.31/100 000 vs. 18.28/100 000). The crude incidence and crude mortality of lung cancer increased with age, peaking in the 80-<85 age group (360.84/100 000) and the ≥85 age group (414.85/100 000), respectively. From 2017 to 2021, the standardized incidence demonstrated downward trends in the total population, urban areas and males (APC=-6.590%, -5.829%, and -6.729%, all P<0.05). The standardized mortality demonstrated downward trends in urban areas and females (APC=-3.710% and -5.378%, both P<0.05). The YLL rate also showed downward trends in urban areas and females (APC=-3.957% and -3.631%, both P<0.05).
Conclusions
From 2017 to 2021, the overall disease burden of lung cancer in registration areas of Guizhou Province showed a decreasing trend. However, the disease burden remained relatively heavier in rural areas and males, with a relatively gradual change.

Vascular calcification is a highly regulated process of ectopic calcification in cardiovascular system while no effective intervention can be clinically performed up to date.As vascular calcification undergoes a common regulatory mechanism within bone formation,bone morphogenetic protein 7(BMP-7)main-tains contractile phenotype of vascular smooth muscle cells and further inhibits vascular calcification via promoting the process of osteoblast differentiation,reducing ectopic calcification pressure by increasing bone formation and reducing bone resorption.This work systematically reviews the role of BMP-7 in vascular calcifi-cation and the possible mechanism,and their current clinical application as well.The current proceedings may help develope early diagnostic strategy and therapeutic treatment with BMP-7 as a new molecular marker and potential drug target.The expec-tation could achieve early prevention and intervention of vascular calcification and improve poor prognosis on patients.
9.Effect of high-frequency electric surgical knife in blepharoplasty

Objective To compare the application effects of high-frequency electric surgical knife and scalpel in blepharoplasty.Methods A retrospective analysis was performed for 35 patients(70 eyes)who were admitted to the Shanghai Ninth People's Hospital,Shanghai JiaoTong University School of Medicine from January to July 2021,and one side eye was randomly selected as the high-frequency electric surgical knife group(operated by high-frequency electric surgical knife)and the other side eye as the scalpel group(operated by scalpel),with 35 eyes in each group.The effect of blepharoplasty in both eyes after surgery was evaluated.The swelling and ecchymosis of the patients were observed 1 day,1 week,1 month and 3 months after surgery.The Manchester scar scale(MSS)was used to evaluate the scarring of the patients 1 week,1 month and 3 months after surgery.The excised tissues were stained with HE staining and TUNEL staining to assess the degree of necrosis and the extent of thermal damage to the skin and muscle.Results There was no statistically significant difference in swelling,ecchymosis,pain or scarring after surgery between the two groups(P>0.05).The eyelids of the high-frequency electrosurgical group showed coagulative necrosis of muscle tissue,and the scalpel group had almost no coagulative necrosis.There was statistically significant difference in the proportion of coagulable necrotic muscle after surgery between the two groups(P<0.05),and the depth of thermal damage in the high-frequency electric surgical knife group was greater than that in the scalpel group(P<0.05).Conclusion The high-frequency electric surgical knife in blepharoplasty is simple to operate,and safe to use,with timely intraoperative incision and coagulation,which can obtain good surgical field and effect.
10.Results of one-year blood pressure follow-up after proximal and total renal artery denervation

Objective To compare the efficacy of renal proximal renal artery denervation(pRDN)and full-length renal artery denervation(fRDN)for treatment of hypertension.Methods Fifty-six hypertensive patients were enrolled and randomly assigned to full-length renal artery denervation group(n=25)and proximal renal artery denervation group(n=31).After the procedure,24-hour ambulatory blood pressure monitoring(24 h-ABPM)at 6 months and office blood pressure at 12 months was recorded for statistical analysis.Results The blood pressure at follow-up reduced significantly in both groups,while there was no significant difference between groups.The baseline office blood pressure in fRDN group and pRDN group was(180±15)/(104±10)mmHg and(180±12)/(103±8)mmHg,respectively,which decreased to(142±9)/(82±7)mmHg and(143±10)/(83±6)mmHg at 12 months postoperatively(P<0.001 within groups and P>0.05 between groups).The baseline 24 h-ABPM in the two groups was(162±13)/(95±8)mmHg and(160±12)/(94±8)mmHg,respectively,which decreased to(142±11)/(83±7)mmHg and(141±8)/(81±7)mmHg at 6 months postoperatively(P<0.001 within groups and P>0.05 between groups).However,there was no significant difference in the reduction of office blood pressure and ambulatory blood pressure between the two groups.No treatment-related adverse events were observed.Conclusions pRDN has similar antihypertensive effect to fRDN.
