1.Expert consensus on the clinical application of parenteral direct thrombin inhibitors in special populations

OBJECTIVE To form an expert consensus addressing clinical issues regarding the use of parenteral direct thrombin inhibitors (DTIs) in special populations. METHODS Led by the Sichuan Academy of Medical Sciences & Sichuan Provincial People’s Hospital(the Affiliated Hospital of UESTC), a multidisciplinary working group was formed comprising experts from multiple fields, including clinical pharmacy, cardiac surgery, obstetrics, pediatrics and evidence-based medicine. Through literature review and the Delphi method, clinical questions regarding the efficacy and safety of parenteral DTIs used in special populations were identified. A structured design was adopted using the “Population-Intervention-Comparison-Outcome” (PICO) framework;systematic searches were conducted in CJFD, PubMed, Embase and other databases. Relevant evidence from randomized controlled trials,cohort studies and systematic reviews were included and synthesized. Evidence quality was assessed using the Grading of Recommendations Assessment,Development and Evaluation (GRADE) approach, and recommendations were formulated through three rounds of Delphi surveys and expert consensus meetings. RESULTS &CONCLUSIONS Seven clinical questions were ultimately selected (with a consensus rate exceeding 90%), resulting in the formulation of seven recommendations on the use of parenteral DTIs in special populations, including children, pregnant women, patients with hepatic or renal impairment, patients with mesenteric venous thrombosis, and individuals with thrombophilia. These recommendations clarify the preferred agents, dosing ranges, monitoring parameters, and safety management strategies for parenteral DTIs in these special populations. This expert consensus, which is formulated based on the best available evidence, provides evidence-based guidance for standardized and individualized use of parenteral DTIs in special populations.
2.Association of sedentary types with anxiety and depressive symptom among college freshmen

Objective:
To analyze the association of sedentary types with symptom of depressive and anxiety among college freshmen, so as to provide a reference for improving the mental health of college students.
Methods:
From October to November 2022, all college freshmen at three colleges and universities in Anhui Province were selected by a cluster sampling method. The Generalized Anxiety Disorder-7 (GAD-7), the Patient Health Questionnaire-9 (PHQ-9), and the Youth Leisure-Time Sedentary Behavior Questionnaire (YLSBQ) were used for the investigation. A binary Logistic regression analysis was conducted to investigate the relationship of different types of sedentary behavior with anxiety and depressive symptom.
Results:
The detection rates of anxiety and depressive symptom among college freshmen were 32.8% and 49.9%, respectively. The results of the binary Logistic regression model analysis showed that after controlling for gender, family location, parental education level, self rated family economic status and number of intimate partners, high level overall, video based, and social based sedentary time were associated with an increased risk of anxiety ( OR =1.26, 1.56, 1.27) and depressive symptom ( OR =1.42, 1.94, 1.29) among college freshmen; the association between moderate level sedentary time and depressive symptom was statistically significant ( OR =0.83) (all P <0.05). The overall trends of the association between sedentary behavior with symptom of anxiety and depressive were similar in both boys and girls.
Conclusions
Sedentary behavior is associated with an increased risk of anxiety and depressive symptom in college students. Reducing video based and social based sedentary behaviors is beneficial for mental health promotion in college students.
3.Research on dry and wet durability of reusable surgical gowns

Objective To explore the changes of durability properties of reusable surgical gowns when used in dry and wet conditions.Methods Reusable surgical gowns made of single-layer polyester fiber or 3-layer composite material were selected as test samples,and a Martindale abrasion and pilling tester was used as the basic test platform and modified to form fixtures suitable for the wet state environment.The reusable surgical gowns underwent abrasion experiments in wet and dry conditions to observe the changes in their fiber structure,and were subjected to water penetration resistance and swelling strength tests.Results Visually the reusable surgical gowns had few changes of the microscopic textile fiber structure in dry and wet conditions,and the gowns made of single-layer polyster fiber gained advantages over the outer layers of those of 3-layer composite material in abrasion resistance with the same friction cycles.In dry and wet conditions,the hydrostatic pressure values of the gowns of single-layer polyster fiber gradually decreased with the increase of the degree of abrasion,which were always lower than those of the gowns of 3-layer composite material;the swelling strength of the gowns of single-layer polyster fiber was always greater than that of the gowns of 3-layer composite material,which decreased with the deterioration of the wear more significantly than that of the gowns of 3-layer composite material.Conclusion The reusable surgical gowns made of single-layer polyester fiber or 3-layer composite material have few differences in durability and protective properties at the early stages of ablation in dry and wet conditions.The durability of the gowns decreases as the degree of wear increases,while the trend of the decrease is slowing down until the fabric breaks down and completely loses its barrier effect.[Chinese Medical Equipment Journal,2025,46(6):28-33]
4.Establishment and application of a luciferase immunosorbent assay for the detection of antibodies to Crimean-Congo hemorrhagic fever virus

The purpose of this study was to establish a luciferase immunosorbent assay(LISA)using the Crimean-Congo hemorrhagic fever virus(CCHFV)glycoprotein C(Gc),a specific antigen,for the detection of CCHFV IgG antibodies.Three antigenic fragments based on CCHFV glycoprotein C were designed,and three recombinant plasmids were constructed by liga-tion with the NanoLuc luciferase(NLuc)expression vector pNLF1-N through molecular cloning.The accuracy of the sequences in the recombinant plasmids was confirmed through sequencing.The recombinant plasmids were transfected into eukaryotic cells to obtain fusion proteins containing specific antigens and luciferase,and the expression of the fusion proteins was verified by western blotting,thereby facilitating the establishment of the CCHFV-LISA detection technique.The assay's sensitivity,specificity,and stability were evaluated and compared with those of a commercial CCHFV IgG antibody test kit.Three recom-binant antigen fragments of CCHFV Gc—NLuc-Gc-Full,NLuc-Gc-C1,and NLuc-Gc-C2—were expressed,with molecular weights of 80.1 kDa,62.8 kDa,and 53.9 kDa,respectively.The optimal fragment for CCHFV detection was NLuc-Gc-C2.The sensitivity of the CCHFV-LISA was 90.9%,and the specificity was 100%;the findings were highly concordant with those for the commercial CCHFV enzyme-linked immunosorbent assay kit.Repeatability tests indicated no statistically significant differ-ences in inter-and intra-assay variability within the same batch.The LISA was highly specific,sensitive,and user-friendly in detecting IgG antibodies against the CCHFV.Therefore,this method may facilitate serological diagnosis and epidemiological studies in endemic regions,and provide essential technical support for disease surveillance and early warning.
5.Relationship between spleen volume and non-alcoholic fatty liver disease by three-dimensional computed tomography reconstruction

Objective To investigate the association of spleen volume with the risk of non-alcoholic fatty liver disease(NAFLD)as well as their causal relationship.Methods We included 90 NAFLD cases and 47 healthy controls who had received contrast-enhanced computed tomography(CT)scan of the abdomen at the Second Affiliated Hospital of Xi'an Jiaotong University from November 2022 to November 2023.We conducted three-dimensional reconstruction of the spleen through a deep learning network model using a two-stage coarse-to-fine segmentation approach.We compared the two groups using the two-sample t test or Mann-Whitney U test for continuous data and using the chi-square test for categorical data;evaluated the correlation between spleen volume and liver function indicators through Pearson correlation or Spearman rank correlation analyses;determined the factors influencing the development of NAFLD through multivariable Logistic regression analysis;and further assessed the casual relationship between spleen volume and NAFLD using the inverse variance-weighted two-sample Mendelian randomization(IVW-MR)method.Results Spleen volume was significantly larger in NAFLD cases than in controls(272.93±104.16 vs 204.37±81.20 cm3,P<0.001).The Spearman rank correlation analysis showed that spleen volume was positively correlated with the hepatic steatosis index(rs=0.422,P<0.001)and gamma-glutamyl transferase levels(rs=0.211,P=0.047)in patients with NAFLD.The multivariable Logistic regression analysis indicated that spleen volume was an independent risk factor for the development of NAFLD(odds ratio[OR]=1.01,95%confidence interval[CI]:1.00-1.02,P=0.049).The IVW-MR analysis detected a causal relationship between spleen volume and NAFLD(OR=1.16,95%CI:1.05-1.28,P=0.005).Conclusion Increased spleen volume may be a risk factor for the development and progression of NAFLD.Further studies are still needed to investigate the specific mechanism.

Questionnaire survey and symposium were carried out on the implementation of GB 8369.1-2019 Transfusion sets for single use—Part 1:Gravity feed,and then analyses were performed on its implementation by relative enterprises,inspection and testing organizations and regulatory authorities,applicability,coordination and implementation constraints accordingly.Some suggestions were put forward,and it was of great significance for comprehensively grasping the overall situation,efficacy and problems of the standard implementation,promoting the updating and upgrading of the standard and facilitating the implementation of compulsory standards.[Chinese Medical Equipment Journal,2025,46(9):70-74]
7.Clinical effect analysis of rigid laryngoscopy performed in two body positions

Objective To explore the optimal body position of patients during rigid laryngoscopy.Methods 120 outpatients who underwent rigid laryngoscopy from December 2023 to March 2024 were divided into the conventional body position group(58 cases)and the anterior tilted position group(62 cases)by the random number table method.Patients in the conventional body position group were examined in the conventional body position(upright lower head position)and patients in the anterior tilted position group were examined in the anterior tilted position(body tilted forward by 60° and jaw lifted).The vocal folds were scored on the Friedman scale,the visual analogue scale(VAS)score was used to evaluate the comfortable degree of patients during the examination,the incidence of gag reflex sensitivity and the rate of complete pharyngeal exposure were compared between the two groups of patients.Results In the conventional body position group,there were 26 cases of Friedman's difficult exposure and 32 cases of easy exposure.The VAS scores of difficult exposure and easy exposure patients were 2.5(1.0,5.8)and 1.0(1.0,3.0),respectively;The incidence of gag reflex sensitivity was 42.3%and 34.4%,respectively;and the rates of complete pharyngeal exposure were 76.9%and 100.0%,respectively.In the anterior tilted position group,there were 28 cases of Friedman difficult exposure and 34 cases of easy exposure.The VAS scores of patients with difficult and easy exposure were 1.5(1.0,7.0)and 2.0(1.0,4.0),respectively;The incidence of gag reflex sensitivity was 46.4%and 29.4%,respectively;And the rates of complete pharyngeal exposure were 82.1%and 91.2%,respectively;The differences were not statistically significant(P>0.05).Conclusion When rigid laryngoscopy is performed in the upright lower head position or anteriorly tilted position,there is no significant difference in the comfortable degree scores,incidence of gag reflex sensitivity,and the rate of complete pharyngeal exposure in the patients in the two positions,and the examiners can choose the appropriate position according to their own examination habits.
8.Effect of Satya mode intervention on self-efficacy and quality of life in patients with chronic heart failure

Objective:To investigate the effect of Satya mode intervention on self-efficacy,quality of life,mental health and coping style in patients with chronic heart failure.Methods:This randomized controlled study enrolled 136 patients with chronic heart failure admitted in the Second Affiliated Hospital of Xi'an Jiaotong University School of Medicine between December 2022 and May 2023.Patients were randomly divided into control group(n=68,routine psychological intervention)and intervention group(n=68,additional Satya mode intervention).After 6-week intervention,self-efficacy[chronic disease self-efficacy scale(CDSES),general self-efficacy scale(GSES)scores],quality of life[Minnesota living with heart failure questionnaire(MLHFQ)score],mental health[self-rating anxiety scale(SAS),Connor-Davidson resilience scale(CD-RISC)scores]and coping style[simpli-fied coping style questionnaire(SCSQ)score]were compared between the two groups.Results:Compared to pa-tients in the control group after intervention,those in the intervention group had significantly higher scores of CD-SES[(44.67±3.03)points vs.(41.56±2.96)points],GSES[(26.91±1.45)points vs.(23.11±1.39)points],CD-RISC[(73.48±3.61)points vs.(56.45±4.12)points]and SCSQ-positive[(12.39±1.20)points vs.(11.02±0.83)points](P<0.001 all),and significantly lower scores of MLHFQ[(43.52±3.44)points vs.(48.77±3.76)points],SAS[(31.03±2.46)points vs.(47.86±3.23)points]and SCSQ-negative[(8.35±1.18)points vs.(10.42±1.23)points](P<0.001 all).Conclusion:Satya model intervention may improve self-efficacy,quality of life,mental health and coping style in patients with chronic heart failure.
9.Sinicization of the Moyamoya Health Behavior Scale for Adolesent Patients and its reliability and validity

Objective:To translate the Moyamoya Health Behavior Scale for Adolescent Patients into Chinese and to test its reliability and validity.Methods:Based on the Brislin translation model, the scale was forward-translated, back-translated, cross-culturally adapted, and pretested to develop the Chinese version of the Moyamoya Health Behavior Scale for Adolescent Patients. A convenience sampling method was used to recruit 220 adolescent inpatients with moyamoya disease from the Department of Neurosurgery, Zhongnan Hospital of Wuhan University, between January 2023 and December 2024. Reliability and validity analyses were conducted on the scale.Results:A total of 220 questionnaires were distributed, and 206 valid questionnaires were returned, yielding a valid response rate of 93.64%. The Chinese version of the scale comprised 12 items. The item-level content validity index ranged from 0.875 to 1.000, and the scale-level content validity index was 0.917. Exploratory factor analysis extracted three common factors: treatment implementation for moyamoya disease, health-promoting behaviors for moyamoya disease, and health coping behaviors for moyamoya disease, with a cumulative variance contribution of 69.741%. Confirmatory factor analysis showed that the overall model fit was acceptable. The Cronbach's α coefficient for the total scale was 0.872, with subscale Cronbach's α coefficients ranging from 0.810 to 0.871. The test-retest reliability coefficient for the total scale was 0.983, and the split-half reliability coefficient was 0.766.Conclusions:The Chinese version of the Moyamoya Health Behavior Scale for Adolescent Patients demonstrated good reliability and validity and can be used as an assessment tool for health behaviors in this patient population.
10.The value of CT combined with peripheral blood eosinophils in differentiating eosinophilic asthma and eosinophilic bronchitis

Objective To explore the application value of CT combined with peripheral blood eosinophils(EOS)for distinguishing eosinophilic asthma(EA)from eosinophilic bronchitis(EB).Methods The clinical characteristics,peripheral blood EOS detection and imaging features of 523 patients(328 cases of EA and 195 cases of EB)were retrospectively analyzed.Univariate analysis identi-fied statistically significant parameters,which were further utilized in binary logistic regression to construct an imaging model and a combined model incorporating clinical characteristics,peripheral blood EOS and imaging features.Receiver operating characteristic(ROC)curves were plotted to evaluate the performance of each model.Results Statistically significant differences were observed between the two groups in age,body mass index(BMI),peripheral blood EOS count,peripheral blood EOS percentage,and 11 ima-ging features,including bilateral lung,bronchial wall thickening and bronchial mucus plugs(P<0.05).The area under the curve(AUC)of the imaging model and the combined model were 0.891 and 0.918,respectively.Conclusion The combined application of CT and peripheral blood EOS detection can improve the efficiency,simplicity,and accessibility of distinguishing EA from EB.
