1.Effects of subanesthetic dose of esketamine on postoperative anxiety and recovery in patients undergoing laparo-scopic cholecystectomy

OBJECTIVE To investigate the effects of subanesthetic dose of esketamine on postoperative anxiety and recovery in patients undergoing laparoscopic cholecystectomy. METHODS A total of 200 patients scheduled for laparoscopic cholecystectomy at Suzhou Ninth Hospital Affiliated to Soochow University from January 2023 to December 2024 were randomly assigned to control group (n=100) and observation group (n=100). One minute before the initiation of anesthesia, patients in the control group received intravenous injections of Propofol emulsion injection, Sufentanil citrate injection, and Succinylcholine chloride injection. On this basis, patients in the observation group received an intravenous injection of Esketamine hydrochloride injection. The anxiety status of patients in both groups was compared, along with their general intraoperative conditions (including sufentanil dosage, duration of pneumoperitoneum, operative time, anesthesia time, and extubation time), postoperative recovery, incidence of adverse reactions, and the need for dezocine rescue analgesia. Heart rate and mean arterial pressure, entropy index (state entropy and response entropy), inflammatory marker levels [interleukin-6 (IL-6) and C-reactive protein (CRP)], numerical rating scale (NRS) for pain intensity were compared between the two groups at different time points. RESULTS No significant differences were found between the two groups in pneumoperitoneum duration, operative time, anesthesia time,extubation time, incidence of postoperative dry mouth, entropy index or length of stay in the post-anesthesia care unit (P>0.05). Compared with the control group, the observation group showed significantly lower postoperative STAI-S scores, reduced intraoperative sufentanil consumption, decreased incidence of postoperative nausea, vomiting, and shivering, the need for dezocine rescue analgesia, as well as lower plasma IL-6 and CRP levels at 24 h after surgery, and NRS (P<0.05). The heart rate and mean arterial pressure of patients in the observation group at the start of surgery, end of surgery, and during extubation were all significantly higher than those in the control group (P<0.05). CONCLUSIONS Subanesthetic dose of esketamine can effectively alleviate postoperative anxiety, reduce intraoperative opioid consumption, suppress postoperative inflammatory response, relieve postoperative pain, and promote recovery in patients undergoing laparoscopic cholecystectomy.
2.New advances in the treatment of neonatal diabetes mellitus with sulfonylureas

Neonatal diabetes mellitus (NDM) is a rare monogenic disorder primarily caused by insufficient insulin secretion resulting from mutations in the KCNJ11 and ABCC8 genes. Sulfonylureas, represented by glibenclamide, have become the standard therapy for this type of NDM by precisely closing the mutated ATP-sensitive potassium channels in pancreatic β cells, thereby restoring insulin secretion. Clinical studies confirm that sulfonylureas enable over 90% of patients to successfully transition from insulin to oral treatment, achieving long-term stable glycemic control and improving neurological outcomes to a certain extent. In terms of safety, severe hypoglycemia induced by sulfonylureas is relatively rare and gastrointestinal reactions are mild; moreover, sulfonylureas show good long-term tolerability, and have no adverse effects on child growth and development. In the future, by further refining the full-chain management pathway of “rapid genetic diagnosis-early intervention-specialized dosage forms-long-term follow-up”, the clinical application of sulfonylureas is expected to provide NDM patients with an optimized treatment regimen and maximize their health benefits.
3.Progress in clinical application of anti-CD20 monoclonal antibody in refractory nephrotic syndrome in children

Anti-CD20 monoclonal antibodies for the treatment of refractory nephrotic syndrome (RNS) in children. The first- generation rituximab is the most widely used in clinical practice; it shows definite efficacy in children with RNS, is recommended by guidelines, particularly for achieving a high remission rate in minimal change nephrosis, and can significantly reduce the cumulative use of glucocorticoids and immunosuppressants. The second-generation ofatumumab has potential as an alternative treatment for patients who are intolerant or resistant to rituximab, while the third-generation obinutuzumab has shown efficacy in complex cases such as rituximab resistance or post-transplant recurrence. However, there is still controversy regarding the optimization of rituximab treatment dosage and whether ofatumumab and obinutuzumab offer greater advantages than rituximab for the treatment of RNS in children. The most common adverse reaction induced by anti-CD20 monoclonal antibodies is infusion reactions, and long-term adverse events mainly include increased risks of sustained immunosuppression and infections. Rituximab has significant economic advantages for the treatment of RNS, but additional pharmacoeconomic research based on China’s healthcare environment is needed to evaluate the cost-effectiveness of ofatumumab and obinutuzumab in this population. Given that the current use of ofatumumab and obinutuzumab in this field is considered off-label use, clinical application should only proceed after a rigorous evaluation of the patient’s benefits and risks.
4.Study on mechanism of Wenshen Jianpi recipe regulating autophagy by p70S6K signaling pathway on alleviating podocyte injury in diabetic nephropathy rats

Aim To detect the mechanism of Wenshen Jianpi recipe(WSJPR)regulating the autophagy by p70S6K signaling pathway on alleviating podocyte inju-ry in diabetic nephropathy(DN)rats.Methods DN model rats induced by streptozotocin were divided into five groups with six rats in each group:model control group,low dose group(7.5 g·kg-1·d-1),medium dose group(15 g·kg-1·d-1),high dose group(30 g·kg-1·d-1),and positive control group(25 mg·kg-1·d-1).In addition,six normal rats were used as negative control group(isotonic NaCl solution 10 mL·kg-1·d-1).All the rats were given continuous ga-vage for eight weeks.Fasting blood glucose,urine al-bumin/creatinine ratio(UACR)and blood viscosity were determined.The changes of podocyte ultrastruc-ture and autophagosome in each group were observed by transmission electron microscopy(TEM).The pro-tein levels of signaling pathway factor p70S6K and au-tophagy factor p62 in renal tissues of rats in each group were detected by Western blot.Besides,p62 expres-sion was observed by immunohistochemistry.Results WSJPR could decrease fasting blood glucose and UACR,and improve the indexes of blood viscosity in rats.TEM indicated that WSJPR could significantly improve the podocyte ultrastructure and autophagy level in DN rats.Western blot showed that the expression level of signaling pathway factor p70S6K and autophagy factor p62 in the kidney of DN rats increased signifi-cantly compared with blank control group(P<0.01).The expression level of p70S6K and p62 in WSJPR groups decreased compared with model control group(P<0.05).Among them,the medium-dose group of WSJPR had the most significant change.Immunohisto-chemical results showed that the level of autophagy fac-tor p62 in kidney tissue of DN rats increased compared with the control group.WSJPR had a certain inhibitory effect on p62 expression in DN rats.Conclusion WSJPR might restore cell homeostasis by inhibiting p70S6K level,reducing the expression of autophagy factor p62 and enhancing autophagy level in renal tis-sue of DN rats.
5.Montelukast sodium inhibits airway inflammation through PHD2/HIF-1α pathway in asthmatic mice

Objective The study aimed to investigate whether montelukast sodium could alleviate airway inflammatory responses in asthmatic mice by affecting the PHD2/HIF-1α pathway.Methods An allergic asthma model was established by ovalbumin(OVA)induction,and 18 female BALB/c mice were randomly divided into a control group(Con group),an asthma group(OVA group),and an asthma group with montelukast sodium intervention(30 mg/kg montelukast sodium by oral administration 1 h before OVA challenge,Mon group).HE staining was used to analyze the pathological changes in the lungs of mice.Blood cell analyzer and kits were used to determine the number of inflammatory cells and the levels of cytokines,the content of lactic acid and pyruvic acid in the lungs,respectively.RT-PCR and Western blot were used to detect the mRNA and protein expression of HIF-1α,PHD2,E-cad and p120 in the lungs of mice.Results Compared with the Con group,there was a significant increase in the number of eosinophils,lymphocytes,neutrophils and monocytes,the levels of IL-5,IL-13,complement factor D(CFD)and contents of lactate and pyruvate in the lungs of mice in the OVA group.Lung HIF-1α,PHD2,p120 and E-cad mRNA levels were reduced,meanwhile HIF-1α and PHD2 protein expression were upregulated but E-cad and p120 protein expression were downregulated(all with P<0.05).After montelukast sodium intervention,the number of eosinophils and monocytes and CFD expression were significantly decreased in the lungs of Mon group,the contents of lactate and pyruvate were basically restored to normal,and the mRNA and protein expression of HIF-1α,PHD2,p120 and E-cad were effectively improved.Conclusion Montelukast sodium could alleviate the airway inflammatory responses in the lungs of asthmatic mice by regulating the PHD2/HIF-1α signaling pathway.
6.Treating Childhood Adolescent Depression from the Perspective of"Moving the Pivot and Removing Stale Qi"

Based on the theory of"the spleen and stomach as the pivot",Professor Xiong Lei's clinical thinking of"moving the pivot and removing stale qi"to treat depression in children and adolescents is summarized.It is believed that the key mechanism of de-pression in children and adolescents is the unfavorable pivot of the spleen and stomach,and the disharmony between qi and blood of the five organs,which leads to the growth of stale qi,the clouding of the clear orifices by turbid evils,and the malfunction of the spirit.If the pivot is unfavorable,move it,and if the stale qi is endogenous,remove it.Professor Xiong,based on the concept of"treating with orchids and removing stale qi"from The Yellow Emperor's Inner Classic,utilizes aromatherapy in Chinese medicine,taking"moving the pivot and removing stale qi"as the basic treatment,and proposes Xiangzhi Shuxin Formula as the basic formula,which applies five methods of removing dampness,resolving phlegm,eliminating accumulation,removing stasis,and dispersing fire and relieving depres-sion.An experimental case is attached to explain his clinical thinking.
7.Effects of 4C management mode on rehabilitation and quality of life in patients with acute heart failure in CCU

Objective:To explore the effects of 4C management mode on rehabilitation and quality of life in patients with acute heart failure(AHF)in cardiovascular intensive care unit(CCU).Methods:This randomized controlled study enrolled 110 patients with AHF admitted in CCU of Guangdong Provincial Hospital of Traditional Chinese Medicine between February 2021 and February 2023.Patients were randomly divided into intervention group(n=55)and control group(n=55).Patients in the control group received routine intervention,while those in the inter-vention group received additional 4C management mode,both groups were intervened for 3 months.The indexes of cardiac function,self-efficacy,exercise tolerance,blood oxygen saturation(SpO2),mental state and quality of life were compared between the two groups.Results:Compared to those in the control group after intervention,patients in the intervention group had significantly lower interventricular septal thickness(IVST)[(9.20±0.10)mm vs.(10.30±0.17)mm,P<0.001],left ventricular end-systolic volume index(LVESVI)[(35.21±0.74)ml/m2 vs.(39.70±1.09)ml/m2,P<0.001],left ventricular end-diastolic volume index(LVED VI)[(45.35±0.71)ml/m2 vs.(56.76±1.48)ml/m2,P<0.001],scores of self-rating anxiety scale(SAS)[(32.10±1.57)points vs.(45.36±1.94)points,P<0.001],self-rating depression scale(SDS)[(35.03±1.52)points vs.(47.70±2.12)points,P<0.001]and Minnesota living with heart failure questionnaire(MLHFQ)[(51.27±2.85)points vs.(56.46±3.53)points,P<0.001],and significantly higher score of general self-efficacy scale(GSES)[(28.71±3.84)points vs.(20.34±1.56)points,P<0.001],6 min walking distance(6MWD)[(428.53±5.50)m vs.(362.14±6.18)m,P<0.001]and SpO2[(99.11±0.87)%vs.(94.23±0.73)%,P<0.001].Conclusion:4C management mode may improve the cardiac function,self-efficacy,mental state and quality of life in patients with acute heart failure in CCU.

Objective:To evaluate the value of ultrasound radiomics combined with machine learning for early diagnosis of seronegative Hashimoto’s thyroiditis (SN-HT) .Methods:This retrospective study included 164 patients from Liaoning Provincial People’s Hospital , Lixin County People’s Hospital, Linghai Dalinghe Hospital, Fengcheng Phoenix Hospital, who underwent thyroidectomy for solitary nodules with normal thyroid function between Nov. 2016 and Jan. 2024. Postoperative pathology confirmed Hashimoto’s thyroiditis (HT) in some cases, who were further categorized into antibody-positive and antibody-negative groups based on serum antibody status. Patients without Hashimoto’s thyroiditis served as the control group. A total of 298 ultrasound images were analyzed. Radiomics features were extracted from hypoechoic non-nodular areas within 0.5 cm surrounding the tumor. Two senior pathologists and two senior ultrasound physicians independently assessed lymphocytic infiltration, eosinophilic changes of follicular epithelium, and the proportion of hypoechoic areas in pathology and ultrasound images, respectively. A machine learning model, CCH-NET, was developed using linear regression and t-distributed stochastic neighbor embedding (t-SNE) techniques. The dataset was divided into a training set (80%) and a validation set (20%) to compare the diagnostic accuracy of CCH-NET with that of senior ultrasound physicians. Results:In internal validation, CCH-NET achieved a diagnostic accuracy of 88.89% for both antibody-positive and antibody-negative groups, significantly higher than the 66.67% accuracy of senior ultrasound physicians ( P<0.01). In external validation, CCH-NET achieved 75.00% and 66.67% accuracy for the two groups, compared to 50.00% by senior ultrasound physicians. For the control group, both methods achieved 93.33% accuracy. The AUC of CCH-NET was 0.848, outperforming senior ultrasound physicians (0.681) ,demonstrating superior diagnostic performance. Conclusion:The radiomics-based CCH-NET model, using non-nodular hypoechoic areas as a specific indicator, can accurately identify early SN-HT in euthyroid patients. It significantly outperforms senior ultrasound physicians, improving diagnostic accuracy and reducing missed diagnoses.
9.Astragaloside Ⅳ attenuates pathological myocardial hypertrophy and fibrosis in mice via EGR1-SIRT1-PPARα-SCAD signaling pathway

Aim To elucidate whether Astragaloside Ⅳcould ameliorate pathological myocardial hypertrophy and fibrosis via the EGR1-SIRT1-PPARα-SCAD signa-ling pathway in TAC mice.Methods After randomi-zing mice into groups,the Sham+AS-Ⅳ group and TAC+AS-Ⅳ group were intragastrically administered 20 mg·kg-1AS-Ⅳ once daily,whereas the Sham+NS group and TAC+NS group were given equivalent saline.Six weeks post-surgery,an evaluation of cardiac function was conducted,heart weight index was compu-ted,morphological alterations in heart were noted,vari-ations in collagen and myocardial hypertrophy indexes were analyzed,ATP content,free fatty acid content,hydroxyproline content,SCAD expression,and enzyme activity were measured,and an initial investigation into the protein expression of EGR1-SIRT1-PPARα-SCAD in myocardial tissues was undertaken.Results After AS-Ⅳ intervention,the heart weight index of TAC mice decreased(P<0.01),LVAWd,LVAWs,LVPWd and LVPWs values decreased(P<0.01,P<0.05),EF%and FS%values increased(all P<0.01),myocardial hypertrophy markers and collagen area decreased,FFA content,HYP content and collagen expression de-creased(all P<0.01),SCAD enzyme activity and ex-pression increased(P<0.01,P<0.05),and ATP content increased(P<0.01).The expression of EGR1 protein decreased,and the expression of SIRT1 and PPARα protein increased(all P<0.01).Conclu-sions AS-Ⅳ may improve fatty acid oxidation via the EGR1-SIRT1-PPARα-SCAD signaling pathway,thereby ameliorating pathological myocardial hypertrophy and fibrosis in TAC model mice.
