1.Thrombotic thrombocytopenic purpura induced by penpulimab and sintilimab

A 52-year-old male patient with recurrent Hodgkin′s lymphoma was treated with a combination therapy of gemcitabine, oxaliplatin, and sintilimab for 6 cycles, and sintilimab monotherapy for 15 cycles. Because of disease progression, the therapy was switched to decitabine (10 mg intravenous infusion on day 1-5) and penpulimab (200 mg intravenous infusion on day 8), with 21 days as one cycle. On the 5th day after the last administration of the 10th cycle, the patient experienced discomfort such as headache and poor appetite; on the 7th day, he suddenly developed unconscious and faint. The patient had a body temperature of 38.1 ℃ and mild yellowish skin and mucous membranes throughout the body. Laboratory tests showed platelet count 9×10 9/L, red blood cell count 4.1×10 12/L, hemoglobin 113 g/L, prothrombin time 15.6 s, blood creatinine 101.6 μmol/L, total bilirubin 61.1 μmol/L, and creatine kinase isoenzyme 43 U/L. Based on laboratory and imaging examinations, immune checkpoint inhibitors causing acquired thrombotic thrombocytopenic purpura was considered. The patient underwent 3 times of plasma exchanges, and received intravenous infusion of methylprednisolone 80 mg once daily and other symptomatic treatments for 5 days. The patient′s yellowish skin and mucous membranes throughout the body disappeared. The glucocorticoid was decreased gradually. Two months later, the patient′s laboratory test indicators such as platelet count and total bilirubin returned to normal.
2.Comprehensive evaluation of compound trace element preparations used in Beijing

Objective:To compare the types and contents of trace elements in compound trace element preparations used in parenteral nutrition in Beijing against the doses recommended by the guidelines, so as to facilitate optimal choice of preparations for different populations in clinical practice.Methods:The available compound trace element preparations were determined according to the drug list from the Medical Insurance Recruitment and Procurement System of Beijing and the recommended doses of trace elements in the guidelines released by Chinese domestic and foreign nutrition societies were collected and compared.Results:The compound trace element preparations used in Beijing comprised 4 varieties and 6 specifications, with the daily cost ranging from 11.32 to 471 Chinese Yuan. There were 3 varieties of trace element preparations for adults, including multi-trace elements injection (Ⅱ), multi-trace elements injection (Ⅲ), and multi-trace elements injection. Among those, multi-trace elements injection (Ⅲ) met the guideline recommendations best and the price was moderate. For pediatric populations, multi-trace elements injections (Ⅰ) satisfied better the trace element requirements for preterm infants and infants from term to 3 months, while multi-trace elements injections (Ⅲ) outperformed in meeting the trace element requirements for 3-to-12-month infants and children. However, there were still some differences in the trace element contents between the two preparations and the guideline recommendations.Conclusion:The currently available varieties and specifications of trace element preparations basically meet the recommendations of evidence-based guidelines, but there is still the need to improve the instructions for and evidence on the pediatric application of certain preparations.
3.A comprehensive evaluation of commercial triple-chambered bag products for parenteral nutrition in China

Objective:An inceasing number of commercial multi-chambered bag products for parenteral nutrition (PN) have been launched in China, with some being included in the National Medical Insurance Catalogue (NMIC). In this study, the commercially available triple-chambered bag (3CB) products for PN in China were comprehensively analyzed in terms of basic characteristics, applicable population, and costs to guide the rational use of these products in clinical settings.Methods:The marketing information of 3CB products was collected. The inclusion of these products in NMIC and the Catalogue of Selected Drugs for National Centralized Procurement was investigated, along with the collection of manufacturer's instructions and pricing information in Beijing. Two researchers extracted and calculated the information of 3CB products, including energy density, types and levels of each component, glycolipid ratio, osmotic pressure, and price. The applicable population and economy of these products were also compared.Results:As of April 2024, 11 3CB products, including 23 specifications, had been approved in China. Eight products were included by NMIC and 2 by the Catalogue of Selected Drugs for National Centralized Procurement. The energy density, protein content, and lipid content dramatically differed among 3CB products (energy density, 254.0–493.7 kJ/100 ml; protein, 2.2–5.6 g/100ml; lipid, 2–4 g/100 ml; and glycolipid ratio, 62∶38–40∶60). Five products could only be infused via the central veins. The sources of fat emulsions included soybean oil-based long-chain fat-milk, medium-long-chain fat-milk, olive oil-based fat-milk, and fish oil-based fat-milk. All the 3CB products contained 8 essential amino acids and some non-essential amino acids, with or without tyrosine, taurine, aspartic acid, and glutamic acid. The average daily total cost of 3CB PN was 74.78–585.82 yuan. Since Some 3CB products had been listed in NMIC, they were affordable for urban patients covered by Urban Employee Basic Medical Insurance but might be economically unacceptable for rural residents who are insured by the Basic Medical Insurance for Urban and Rural Residents and uninsured patients. Conclusions:A variety of 3CB products have been commercially available in China and can meet the individual PN needs of most patients. Although some products have been listed by NMIC, the economic burden can be heavy for certain patient populations.
4.Predictive value of atherogenic index of plasma combined with novel inflammatory indexes for MACE in ACS patients

Objective:To explore the predictive value of atherogenic index of plasma(AIP)combined with neutro-phil/HDL-C ratio(NHR),lymphocyte/HDL-C ratio(LHR)and monocyte/HDL-C ratio(MHR)for major adverse cardiovascular events(MACE)in patients with acute coronary syndrome(ACS).Methods:This retrospec-tive study enrolled 320 patients underwent coronary angiography in Department of Cardiovascular Medicine,Harri-son International Peace Hospital between January 2021 and December 2022,including 215 ACS patients and 105 pa-tients without ACS;according to Gensini score,ACS patients were divided into low risk group(n=50),medium risk group(n=70)and high risk group(n=95).According to the presence of MACE within 1 year,patients were divided into MACE group(n=55)and no MACE group(n=160).After admission,blood routine,blood lipids,C-reactive protein(CRP)and endothelin-1(ET-1)levels were measured,then NHR,LHR,MHR and AIP were calculated.Spearman method was used to analyze the association of above-mentioned indexes with Gensini score;stepwise Logistic regression was used to analyze the influencing factors of MACE within 1 year in ACS patients;and ROC curve was used to analyze the predictive value of single and combined detection of above indexes for MACE within 1 year in ACS patients.Results:Compared to patients in no ACS group,those in ACS group had significantly higher BMI[(23.59±0.85)kg/m2 vs.(21.57±1.16)kg/m2],proportions of hypertension(57.67%vs.13.33%),diabetes(23.26%vs.8.57%),NHR[(12.09±3.46)vs.(3.81±1.29)],MHR[(0.70±0.18)vs.(0.33±0.10)],LHR[(0.79±0.21)vs.(0.40±0.09)],AIP[(0.21±0.06)vs.(0.11±0.02)],CRP[(9.82±3.09)mg/L vs.(2.20±0.58)mg/L],ET-1[(31.25±10.34)μg/L vs.(10.60±1.96)μg/L](P<0.001 all).Compared to those in no MACE group,patients in MACE group had significantly higher NHR[(17.33±3.87)vs.(10.12±2.68)],MHR[(0.93±0.24)vs.(0.61±0.13)],LHR[(1.05±0.27)vs.(0.71±0.16)],AIP[(0.28±0.04)vs.(0.17±0.02)],CRP[(15.52±3.83)mg/L vs.(7.69±2.40)mg/L],ET-1[(46.68±9.51)μg/L vs.(25.47±4.66)μg/L]levels(P<0.001 all).Spearman correlation analysis showed that NHR,MHR,LHR and AIP were significant positively correlated with Gensini score(r=0.837~0.868,P<0.001 all).Stepwise Logistic regression analysis showed that NHR(OR=1.225,95%CI 1.016~1.550,P=0.035),AIP(OR=2.632,95%CI 1.055~6.566,P=0.038)were independent risk factors for MACE within 1 year in ACS patients.ROC curve shows that AUC of AIP combined with NHR,MHR and LHR predicting MACE within 1 year in ACS patients was 0.786(95%CI 0.725~0.839),which was significantly higher than those of NHR(AUC 0.768,95%CI 0.706~0.823),LHR(AUC 0.749,95%CI 0.686~0.806)and AIP(AUC 0.764,95%CI 0.701~0.819)alone(Z=2.597,2.687,1.965,P<0.05 or<0.01).Conclusion:AIP combined with NHR,MHR and LHR have certain predictive value for MACE within 1 year in ACS patients.
5.Survey of influenza awareness of community residents in nanchang during routine epidemic prevention and control

Objective To investigate influenza awareness of community residents in Nanchang under routine epidemic management using the Knowledge-Attitude-Practice(KAP)framework.Methods A structured questionnaire,including four do-mains(disease overview,prevention,diagnosis,and treatment),was designed and administered via street and online surveys from December 2023 to February 2024.Statistical analyses,including chi-square tests and logistic regression,were performed to identify determinant factors.Results A total of 303 valid questionnaires were retrieved,with an effective rate of approximately 94.7%.Community residents in Nanchang demonstrated a certain degree of understanding of the overview,prevention,diagno-sis,and treatment of influenza,although some gaps were identified.Univariate analysis revealed statistically significant differ-ences in influenza cognition scores among urban and rural residents(P<0.05),residents with different educational levels(P<0.05)and exposure levels to influenza education(P<0.05).Multivariate regression analysis identified these as influencing fac-tors for influenza awareness.Conclusion Collaborative efforts from the government,individuals,and various societal sectors are essential to enhance the public's overall ability to prevent and control influenza.
6.Phylogenetic analysis of influenza B in the Yellow River Delta region,China,in 2021-2024

This study analyzed the variations and evolution characteristics of influenza B Victoria(BV)virus in the Yellow River Delta region of China during 2021-2024.Throat swabs were collected from people with influenza-like illness(ILI)from 2021 to 2024 in Binzhou and Dongying,China.Viral isolation was performed,and 22 representative influenza BV isolates were selected for whole genome sequencing.Phylogenetic analysis of whole-genome sequences was performed in MegAlign and MEGA software.A total of 27 674 samples were obtained,and the overall positivity rate of influenza virus(A/H3N2,A/H1N1,BV)was 11.1%.Our surveillance data indicated that influenza B virus was detected in 2 years,which showed positivity rates of 28.2%and 1.7%,respectively.Statistically significant differences in the positivity rates of influenza BV viruses were observed(x2=3 641.791,P<0.001).The median pairwise sequence identities ranged from 98.7%to 99.3%for eight segments of 22 viral sequences.The isolates for the monitoring years 2021-2024 were located in clades V1A.3a.1 and V1A.3a.2.Intra-lineage reassortments were discovered in B/shandongbincheng17/2022.The NA gene of one isolate exhibited an increase in the 488NLTV N-glycoproteome site.The K338R mutation occurred in the PA gene.Three locus deletion or insertion mutations occurred in the MP gene.The BV influenza epidemic was prevalent every other year;the intensity ranged from strong to weak;and the duration ranged from long to short in the Yellow River Delta region of China during 2021-2022.Influ-enza B virus formed intra-lineage reassortments,and showed significant mutations in NA gene,PA gene,and MP gene.
7.A preliminary study on the diagnostic value of 18F-FDG PET/CT in primary pulmonary enteric adenocarcinoma

Objective To explore the diagnostic value of 18F-fluorodeoxyglucose(FDG)PET/CT in primary pulmonary enteric adenocarcinoma.Methods The clinical and imaging data of 9 patients with primary pulmonary enteric adenocarcinoma who under-went 18F-FDG PET/CT examination were retrospectively analyzed,including lesion distribution,morphology,maximum standardized uptake value(SUVmax),clinical symptoms and signs,gastroscopy finding,puncture pathological results,and serum tumor markers[carbohydrate antigen 72-4(CA72-4),cytokeratin 19 fragment antigen 21-1(CYFRA21-1),carcinoembryonic antigen(CEA),carbo-hydrate antigen 199(CA199)].Results Pathological examination confirmed a diagnosis of primary pulmonary enteric adenocarcinoma after excluding gastrointestinal primary tumors through clinical evaluation.In all nine patients,18F-FDG PET/CT examination did not reveal any evidence of digestive system malignancies,and gastrointestinal microscopy was negative.Primary lesions were observed as masses or nodular types in 6 cases(5 in the left lung and 1 in the right lung),while 3 cases exhibited diffuse bilateral pulmonary involvement(manifested as multiple patchy opacities,nodules,ground-glass opacities,and consolidations).All pulmonary primary lesions showed increased 18F-FDG uptake,with SUVmax ranging from 2.7 to 12.8,mean 8.6±3.7.The six masses-or nodular-type primary lesions showed maximum diameters ranging from 2.1 to 10.5 cm,mean(5.23±3.06)cm.Four cases demonstrated hilar and mediastinal lymph node metastases,intrapulmonary metastases,and distant metastases,while 1 case showed only distant metastasis.Elevated levels of serum tumor markers were observed as follows:CA72-4 in 7 cases(10-273.3 U/mL),CEA in 7 cases(5-147.4 ng/mL),CA199 in 6 cases(31.22-4 364 U/mL),and CYFRA21-1 in 5 cases(8.31-99.7 ng/mL).Conclusion When pathological biopsy of a pulmonary lesion suggests primary pulmonary enteric adenocarcinoma after excluding gastrointestinal primary tumors,and 18F-FDG PET/CT shows no gastrointestinal masses,this may support the diagnosis of primary pulmonary enteric adenocarcinoma.
8.Relationship between macrophage activation related factors and clinical symptoms of schizophrenia

Objective:To investigate the relationship between macrophage activation related factors and clini-cal symptoms of schizophrenia(SCZ).Methods:Outpatient or inpatient SCZ patients(n=166)and normal con-trols(n=71)meeting the diagnostic criteria of DSM 4th edition were selected as subjects.The psychopathological symptoms were assessed by the Positive and Negative Syndrome Scale(PANSS),and the concentrations of α-Na-Galases,MAF and IL-18 were determined by enzyme-linked immunosorbent assay(ELISA).The correlation be-tween biological indicators and clinical symptoms was analyzed and the mediation effect was tested.Results:The concentrations of α-NaGalases(P<0.001)and MAF(P<0.01)in SCZ group were lower than those in normal control group.In SCZ group,IL-18 was negatively correlated with α-NaGalases concentration(r=-0.24,P<0.01).α-NaGalases was positively correlated with MAF concentration(r=0.67,P<0.001),and the total score of PANSS positive symptom scale was positively correlated with IL-18(r=0.21,P<0.05)and MAF concentration(r=0.22,P<0.01).The mediating effect of α-NaGalases and MAF was statistically significant,and the relative mediating effect accounted for 25.47%.Conclusion:The increase of IL-18 level may indicate the occurrence of positive symptoms of schizophrenia,and α-NaGalases and MAF may negatively regulate the inflammatory damage effect of IL-18 on SCZ,thereby reducing the positive symptoms.
9.Clinical efficacy of surgical treatment combined with IFX and UST on Crohn's anal fistulae

Objective To evaluate the efficacy of combining surgical treatment with biological agents for perianal fistulizing Crohn's disease(pfCD).Methods Sixty patients with perianal fistulizing Crohn's disease(pfCD)admitted to our hospital from May 2021 to December 2023 were randomly allocated into two groups:Treatment Group A(n=30)and Treatment Group B(n=30).Treatment Group A received pfCD surgery combined with inflix-imab(IFX)and azathioprine(AZA),while Treatment Group B underwent pfCD surgery along with ustekinumab(UST)and AZA.The CDAI score,PDAI score,and Assche score were monitored for both groups,and postoperative MRI examinations were conducted to evaluate the healing of pfCD.Results There were statistically significant differences in CDAI,PDAI,and Assche scores between pre-treatment and post-treatment comparisons within treat-ment groups A and B(P<0.05),as well as in the magnitude of change at each time point.Comparisons of CDAI,PDAI,and Assche scores at 8,16,24,and 32 weeks,and PDAI scores at 40 weeks between groups A and B using independent samples t-tests did not yield statistically significant results(P>0.05).However,significant differences were observed for CDAI and Assche scores at 40 weeks(P<0.05).Significant changes in CDAI,PDAI,and Assche scores were noted at 8,16,24,32,and 40 weeks post-treatment within both groups A and B(P<0.05).Multiple comparisons using the LSD method revealed that the changes in these scores at each time point were statistically significant(P<0.05).The data indicate a temporal trend in the changes of CDAI,PDAI,and Assche scores,with group B showing a more rapid decline compared to group A.In terms of fistula response rates,both groups A and B achieved 100%(30/30).However,the clinical healing rate of fistulas was higher in group B at 86.7%(26/30)compared to 76.7%(23/30)in group A.Conclusion The combination of surgical treatment with IFX/UST plus AZA is safe and effective for treating pfCD.However,the long-term efficacy of combining surgical treatment with UST appears to be superior.
