This article adopts Professor CHEN Chaozu′s " sanjiao composed by membrane-striae" theory as its foundation to explore the relationship between irritable bowel syndrome and functional/structural abnormalities of the membrane-striae. Sanjiao encompasses both the tangible membrane and the intangible striae. These striae permeate the entire body, and their pathological changes comprehensively reflect qi,body fluids, and fasciae. Based on the physiological function of the membrane-striae in regulating qi and fluids, the pathogenesis of irritable bowel syndrome is characterized by a disharmony of membrane-striae and an imbalance of the qi-fluid interactions. In the early stage, external pathogens, emotional factors,or dietary stimuli often cause membrane-striae constriction and disordered qi-fluid circulation. In the middle stage, stagnant fluids gradually transform into phlegm retention, leading to membrane-striae obstruction. In the late stage, deficiency of vital qi becomes predominant, manifesting as laxity of membrane-striae with impaired control or weakened conduction. The treatment of irritable bowel syndrome should adopt " unblocking" as the guiding principle. In the early stage, therapy should focus on eliminating pathogenic factors and soothing membrane-striae to promptly restore qi-fluid circulation, thereby attaining unblocking through spasm relief. In the middle stage, treatment should focus on resolving tangible obstructions in membrane-striae,achieving unblocking via dredging. In the late stage, the emphasis should shift to reinforcing healthy qi,particularly by strengthening spleen-kidney yang qi,and achieving unblocking through supplementation. Concurrently, throughout the entire treatment process, the regulation of mental state and easing of emotional tension should be integrated to alleviate patient′s anxiety, achieving the goal of holistic treatment of both body and mind.

Objective:To analyze the effect of high serum ferritin(SF)before transplantation on erythrocyte, granulocyte and platelet implantation in unrelated cord blood transplantation(UCBT)patients with myelodysplastic syndrome(MDS)and acute myeloid leukemia(AML).Methods:The medical records of 60 patients with MDS and AML who underwent UCBT in the First Affiliated Hospital of University of Science and Technology of China from January 2020 to December 2022 were retrospectively collected.According to the SF level before transplantation, they were divided into high SF group(SF ≥ 1 000 μg/L,n=20)and non-high SF group(SF<1 000 μg/L,n=40).The red blood cell(RBC)infusion volume before transplantation, implantation time of RBC,granulocyte and platelet, implantation risk and prognosis were analyzed and compared between the two groups. Results:There was no correlation between the level of SF before transplantation and RBC infusion. After transplantation, the median implantation time of RBC in the high SF group was 28.5(14-149)d,which was longer than 21(10-83)d in the non-high SF group(P<0.05).The median time of granulocyte engraftment in the high SF group was 16.5(12-63)d,while that in the non-high SF group was 16(12-49)d,with no statistical difference between the two groups(P>0.05).The median platelet engraftment time in the high SF group was 45(12-206)d,while that in the non-high SF group was 35.5(14-149)d,with no statistical difference between the two groups(P>0.05).Kaplan-Meier cumulative implantation probability analysis showed that the rate of erythroid implantation in the non-high SF group was higher than that in the high SF group(P<0.05),while there was no significant difference in the rates of granulocyte and platelet implantation between the two groups(P>0.05).The 1-year overall survival rates of the non-high SF group and high SF group were 95%and 90%,respectively,with no statistical difference between the two groups(P>0.05).Conclusion:SF levels before cord blood transplantation in MDS and AML patients have an impact on post transplant erythroid implantation.Detecting and intervening of iron load in patients before transplant may be beneficial for improving implantation and prognosis.

Aim To explore the mechanism of Gano-derma lucidum polysaccharides(GLPS)promoting my-elin repair and regeneration in mice with chronic demy-elination induced by cuprizone(CPZ).Methods A total of 40 C57BL/6 mice were randomly divided into four groups:Normal+NS,Normal+GLPS,CPZ+NS and CPZ+GLPS.A chronic demyelination model was established using 0.2%CPZ.Open field and elevated plus maze tests were performed to observe the behavior-al changes in the mice.Immunofluorescence staining and Western blot were used to detect changes in myelin basic protein expression in the corpus callosum.ELISA was performed to measure the levels of TNF-α,IL-6,IL-1β and IL-10 in brain homogenates.Immunofluo-rescence staining was also used to observe the expres-sion of ionized calcium binding adapter molecule 1(Iba1)and neural-glial antigen 2(NG2).RT-qPCR and Western blot were conducted to assess the mRNA and protein expression levels of MBP,iNOS,COX-2,JAK2 and STAT3.Results Mice in the CPZ+NS group showed a significant decrease in body weight, cognitive behavior abnormalities, and impaired myelin regeneration. The expression of pro-inflammatory fac-tors increased, while anti-inflammatory factors de-creased.Additionally,Iba1 and NG2 expression in-creased,and the JAK2/STAT3 signaling pathway was activated. After GLPS intervention, the mouse body weight increased, myelin regeneration occurred,cogni-tive behavior was improved, the expression of inflamma-tory factors decreased,anti-inflammatory factors in-creased,NG2 expression was further elevated, and the proliferation of microglia as well as the activation of the JAK2/STAT3 signaling pathway was inhibited.Conclu-sions GLPS can improve cognitive behavior abnormali-ties and inflammatory responses in chronic demyelinated mice by inhibiting the JAK2/STAT3 signaling pathway, thereby promoting myelin repair and regeneration.

Objective To explore the relationship between community health follow-up management and the mental health of the long-term care insurance residents, and to provide a basis for the construction of an integrated community home care service mode for disabled elders. Methods The residents were selected through cluster sampling who participated in LTCI home care from Jan 1 to Dec 31,2021.After a year of participation, the subjects'mental health was assessed face-to-face by trained community doctors using the Self-Rating Anxiety Scale and the Self-Rating Depression Scale.By referring to residents'electronic health records combined with on-site questionnaire survey, community doctors collected the demographic information and health follow-up management provided by primary medical and health institutions. The multivariate logistic regression were conducted to evaluate the association between follow-up care and mental health outcomes. Results The study consisted of 399 LTCI-enrolled individuals,57.64%(n=230)received follow-up care by family physicians. The prevalence of anxiety and depression among participants was 19.80%(n=79)and 67.67%(n=270),respectively.Univariate analysis found that community health follow-up management could underscore the potential impact of follow-up care in mitigating anxiety(χ2=38.926,P<0.001)and depression(χ2=14.598,P<0.001)among LTCI enrollees.Multivariate analysis revealed that follow-up care was an independent protective factor against anxiety(adjusted OR=0.351,95%CI:0.176-0.701,P=0.003).However,follow-up care did not significantly impact depression prevalence.Additionally,LTCI grade and education level were also identified factors influencing the mental health of participants(P<0.05).Conclusion Community health service centers provide health follow-up management that plays a positive role in alleviating the anxiety symptoms of disabled residents under long-term care insurance home care.It is an effective way to improve the quality of LTCI home care services.
