1.Comparative Study on Effect of Jingui Shenqiwan and Liuwei Dihuangwan on Reproductive Ability and Brain Function of Normal Mice

ObjectiveTo explore the effects of Jingui Shenqiwan (JSW) and Liuwei Dihuangwan (LDW) on the reproductive ability and brain function of normal mice and compare the actions of the two medications. MethodsSeven groups of female and male mice were divided at a ratio of 2∶1. Except for the control group, the other six groups were as follows: a group of both males and females receiving JSW (3.0 g·kg-1), a group of both males and females receiving LDW (4.5 g·kg-1), a group of males receiving water and females receiving JSW, a group of males receiving water while females receiving LDW, a group of females receiving water while males receiving JSW, and a group of females receiving water while males receiving LDW. Each group was administered the drug for 14 days and then caged together at a 2∶1 (female∶male) ratio to detect the number of pregnant mice and calculate the pregnancy rate. Pregnant mice continued receiving the drug until they naturally gave birth, which was followed by the observation of newborn mice, calculation of their average number, and the measurement of the offspring's preference for sugar water and neonatal recognition index. At the end of the experiment, the weights of the thymus and spleen were measured to calculate the organ coefficients, and mRNA or protein expression was analyzed in the brain and testes or ovaries. A 1% sucrose solution was used to examine the euphoria of their brain reward systems, while novel object recognition test (NOR) was applied to assess their memory capabilities. mRNA expression was detected using real-time quantitative polymerase chain reaction (Real-time PCR) assay, and protein expression was analyzed with Western blot. ResultsCompared with the control group, oral administration of JSW to both male and female mice for 14 days significantly increased the pregnancy rate of female mice on day 2 after being caged together (P<0.05), while LDW showed a trend but no statistical significance. Additionally, compared with the control group, JSW could upregulate the gene expression of gonadotropin-releasing hormone (GnRH) in the thalamus, as well as reproductive stem cell factor (SCF) and tyrosine kinase receptor (c-Kit) in the testes and reproductive stem cell marker mouse vasa homologue (MVH) in the ovaries, upregulate the expression of proteins influencing neuronal functional activity, such as brain-derived neurotrophic factor (BDNF), in hippocampal neurons (P<0.05), and enhance sucrose preference in male mice (P<0.05). Compared with the control group, JSW significantly increased sucrose preference and novel object recognition index in offspring mice (P<0.05), which was related to the upregulation of hippocampal dopamine D1 receptor (D1R) and N-methyl-D-aspartate receptor (Nmdar) gene expression. Compared with the control group, both JSW and LDW could upregulate the protein expression of glucocorticoid receptor (GR), BDNF, and tyrosine kinase receptor B (TrkB) in the hippocampus of offspring mice (P<0.05). ConclusionJSW significantly enhances the reproductive ability of normal mice, which is not only related to the release of gonadotropin but also associated with its regulation of brain function. Additionally, JSW has a certain regulatory effect on the brain function of the offspring mice.
2.Comparison of left ventricular reverse remodeling and prognosis after transcatheter aortic valve replacement in aortic stenosis and mixed aortic valve disease

Objective To evaluate the effects of transcatheter aortic valve replacement(TAVR)on left ventricular reverse remodeling(LVRR)and outcomes in patients with mixed aortic valve disease(MAVD)and predominant aortic stenosis(AS).Methods Patients undergoing TAVR at our center between January 2020 and December 2022 were enrolled consecutively.Propensity score matching(PSM)(1∶1 ratio)was used to reduce selection bias.Transthoracic echocardiography(TTE)was used to monitor left ventricular ejection fraction(LVEF)and other structural parameters over time.The study outcome was a composite of cardiovascular death and rehospitalization due to cardiovascular causes.Linear mixed-effects models and logistic regression were utilized for comparing echocardiographic changes across groups and identifying independent risk factors for no-LVRR,respectively.Results After PSM,126 patients were included.MAVD group exhibited larger structural parameters(left ventricular end-systolic/end-diastolic diameter and volume,left ventricular mass index)and a lower left ventricular ejection fraction(LVEF)(all P<0.05).However,more pronounced improvements in left ventricular structure and hemodynamics were observed during follow-up.Multivariate logistic regression analysis indicated that the left ventricular mass index(LVMI)was an independent predictor of left ventricular reverse remodeling(LVRR)after TAVR,whereas persistent moderate or greater mitral regurgitation(MR)and paravalvular leak(PVL)significantly reduced the incidence of LVRR.During a median follow-up period of 23 months,a total of 31 endpoint events occurred,and there was no statistically significant difference in long-term prognosis between the two groups(Log-rank P=0.330).Conclusions Compared to patients in the AS group,those in the MAVD group exhibited more severe left ventricular remodeling before TAVR.However,more significant LVRR was observed during postoperative follow-up.Additionally,the long-term prognosis was comparable between the two groups.
3.CCTA-based comparative study of quantitative parameters between culprit and non-culprit plaques in patients with acute coronary syndrome

Objective To comparatively analyze the quantitative parameters of culprit and non-culprit plaques in patients with acute coronary syndromes(ACS)with a noninvasive technique of coronary computed tomography angiography(CCTA),and to explore the correlation between the quantitative parameters of plaques and culprit plaques.Methods Data were restrospectively collected from 51 ACS patients admitted in some hospital from January to December 2021.The coronary atherosclerotic plaques shown by invasive catheter angiography(ICA)that caused hemodynamic abnormalities and were treated with stenting were defined as offender plaques and enrolled into a culprit plaque group(56 culprit plaques),and other plaques were regarded as non-culprit plaques and divied into a non-culprit plaque group(164 non-culprit plaques).The relationship between culprit plaque and luminal stenosis was analyzed using the chi-square test,and quantitative plaque metrics were compared between the groups.The corelation between quantitative indicators and culprit plaque was analyzed using univariate Logistic regression,and multifactorial Logistic regression analysis was carried out.Statistical analysis was performed with SPSS 23.0 software.Results Analysis of plaque imaging data showed that culprit plaque was significantly associated with the degree of luminal stenosis(P<0.05).Statistically significant differences were found beween the two groups and between the culprit plaques resulting in moderate or greater luminal stenosis and non-culprit plaques in total plaque length,total plaque load,calcification load,fibrolipid volume,fibrolipid load,necrotic core volume and necrotic core load(all P<0.05).Univariate Logistic regression analysis showed total plaque length,total plaque load,fibrolipid volume,fibrolipid load,necrotic core volume and necrotic core load were all significantly correlated with culprit plaques(all P<0.05);multifactorial Logistic regress indicated total plaque length and fibrous lipid load were independent risk factors for culprit plaques,and the probability of culprit plaques increased to 1.093 and 1.101 times for every one unit increase in total plaque length and fibrolipid load,respectively.Conclusion Such quantitative parameters of plaques as total plaque length,total plaque load,fibrolipid volume,fibrolipid load,necrotic core volume and necrotic core load reflects the characteristics of culprit plaques effectively.Quantitative plaque parameter analysis based on CCTA facilitates clinicians for early assessment of patients with coronary atherosclerosis,which enables early intervention and prevention of acute coronary syndromes.[Chinese Medical Equipment Journal,2025,46(6):65-71]
4.Proficiency evaluation of large language models in medical laboratory technology education

Objective:To assess the professional knowledge proficiency of mainstream large language models (LLMs) in medical laboratory education and to explore their potential as educational aids for medical laboratory technology students.Methods:A comprehensive evaluation was conducted using 400 authentic questions from the 2023 Chinese National Clinical Medical Laboratory Technician Qualification Examination. Five LLMs (Copilot, Grok, Yuanbao, Doubao, and Kimi) were tested through two-round interactions using zero-shot prompting and interaction-optimized prompting strategies. The accuracy of answers and the quality of generated content were evaluated. Performance disparities were analyzed using Cochran's Q test. Content quality was scored through the CLEAR framework (completeness, lack of false information, evidence-based reasoning, appropriateness, relevance).Results:In the first-round test, Doubao achieved the highest overall accuracy rate (375/400). The overall accuracy rates of Doubao and Yuanbao significantly outperformed Copilot and Kimi ( P<0.001). After the second-round interactive optimization, the accuracy rate of Kimi significantly improved ( P<0.05), whereas other LLMs showed slight improvements ( P>0.05). Doubao still had the highest overall accuracy rate (380/400). The overall accuracy rates of Doubao and Yuanbao significantly outperformed Copilot ( P<0.005). Evaluation based on the CLEAR framework revealed that Yuanbao, Doubao, and Kimi significantly outperformed foreign models in the dimensions of evidence-based reasoning ( P<0.003) and completeness ( P<0.05), demonstrating standardized citation of authoritative evidence and superior content quality. Conclusions:The tested LLMs possess extensive medical laboratory knowledge. The accuracy of their answers and the quality of the generated content can be improved through single-question input, specifying evidence requirements, and enabling advanced reasoning functions. Domestic LLMs are comparable to foreign LLMs in terms of accuracy, and have significant advantages in the dimensions of evidence-based reasoning and completeness. LLMs can serve as auxiliary tools for learning professional knowledge in medical laboratory technology.
5.Genetic diversity analysis of oxacillinase in 241 clinical isolates of Pseudomonas aeruginosa

Objective:To analyze the carriage status, subtype distribution and flanking gene sequence characteristics of oxacillinases (OXA enzyme) in 241 clinical strains of Pseudomonas aeruginosa, and assess their roles in the drug resistance of Pseudomonas aeruginosa and ability to horizontally transfer across species. Methods:Clinical P. aeruginosa isolates were collected from four hospitals in Sanya, Tangshan, Zhangjiakou, and Beijing. The prevalence of oxacillinases and their flanking gene sequences was analyzed by whole-genome sequencing (NGS) and bioinformatic approaches. Results:A total of 241 isolates of P. aeruginosa were gathered, and 35 blaOXA subtypes were identified through screening of 252 blaOXA genes. These genes were classified into three subfamilies: blaOXA-50-like (241, 95.6%), blaOXA-1-like (9, 3.6%) and blaOXA-10-like (2, 0.8%). Among these, 11 subtypes (11, 31.4%) were novel blaOXA subtypes. Nine of these belonged to the blaOXA-50-like subfamily and were designated as blaOXA-1244, blaOXA-1245, blaOXA-1246, blaOXA-1250, blaOXA-1252, blaOXA-1253, blaOXA-1254, blaOXA-1255, and blaOXA-1256. The remaining two belonged to the blaOXA-10-like subfamily and were named blaOXA-1247 and blaOXA-1248. Compared to the amino acid sequence of OXA-10, the newly identified subtype OXA-1247 exhibited a mutation at position 117, where a valine was replaced by a leucine. This change was thought to improve the enzyme′s ability to hydrolyze carbapenems. In the analysis of the flanking sequences of the blaOXA genes, Class I integrons were identified in four bacterial strains. The variable regions of these integrons carried three distinct patterns of resistance gene cassettes: aac( 6′) -Ib-blaOXA-1247-ant( 3′′) -Ia, aac( 6′) -Ib-blaOXA-1248 and aac( 6′) -Ib- blaIMP-45-blaOXA-1-catB3. Among these, the strain BJ2326 carried a class I integron that was connected to the downstream IS CR1 element to form a composite class I integron structure, additionally carrying the resistance gene blaPER-1. Out of the 223 non-wild-type P. aeruginosa strains, 127 strains exhibited non-wild-type profiles to the four beta-lactam antibiotics MEM, CAZ, FEP, and TZP, with the combination of MEM+CAZ+FEP being the most prevalent, representing 57.0% of the total. Conclusions:The blaOXA genes in 241 clinical P. aeruginosa strains showed diversity. Some blaOXA genes had a co-transfer risk with the metallo-β-lactamase resistance gene blaIMP-45. Among the 11 newly discovered blaOXA subtypes, the new subtype OXA-1247 may have carbapenemase activity and potential for horizontal transfer.
6.Correlation between skeletal muscle mass index and islet β cell reserve function in patients with newly diagnosed type 2 diabetes mellitus

Objective To investigate the correlation between skeletal muscle mass index(ASMI)and islet β cell reserve function in patients with newly diagnosed type 2 diabetes mellitus(T2DM).Methods A total of 100 patients with newly diagnosed T2DM were included in this study.All the patients were admitted to the Department of Endocrinology in the First People's Hospital of Yinchuan between June 2022 and November 2023.They were divided into two groups according to their skeletal muscle mass index(ASMI):patients with T2DM accompanied by sarcopenia(Sar,n=50)group,and patients with simple T2DM(T2DM,n=50)group.Additionally,a control(NC)group consisting of 50 healthy participants was selected.Fasting C-peptide levels,liver and kidney function,blood lipid profiles,and other indicators were assessed in all the individuals.The correlation between ASMI and other indicators was analyzed,and the influencing factors for ASMI and T2DM combined with sarcopenia were analyzed respectively.Results The levels of HbA1c,FPG,and TG were higher,while FC-P and Scr levels were lower in the T2DM group and Sar group compared with the NC group(P<0.05).FPG was higher,while ASMI,FC-P,BMI were lower in the Sar group than in the T2DM group(P<0.05).Spearman correlation analysis revealed a negative correlation between ASMI and FPG and HbA1c(P<0.05),whereas a positive correlation was observed with BMI,ALT,Scr,SUA and FC-P(P<0.05).Multiple linear regression analysis indicated that BMI,HbA1c and FC-P were influencing factors for ASMI(P<0.05).Furthermore,logistic regression analysis demonstrated that BMI,HbA1c,FC-P were influencing factors for T2DM with sarcopenia(P<0.05).Conclusions The level of ASMI may be related to the reserve function of islet β cells.
7.Experimental study of the ultraviolet A-riboflavin cross-linking corneal stro-mal lenticule punctal plug for the treatment of dry eyes in rabbits

Objective To investigate the efficacy of the ultraviolet A-riboflavin cross-linking corneal stromal lenticule punctal plug for the treatment of dry eyes in rabbits.Methods Thirty-two New Zealand white rabbits(64 eyes)were se-lected and randomly divided into a normal group,a model group,a conventional plug group and a cross-linking plug group,with 8 rabbits(16 eyes)in each group.Rabbits in the normal group were not treated.In the model group,moderate-to-se-vere dry eye rabbit models were constructed but no treatment was given.Rabbits in the conventional and cross-linking plug groups were implanted with conventional and cross-linking plugs after model construction,respectively.The tear film func-tions of the experimental rabbits,including tear secretion volume(SIT),corneal fluorescein sodium staining,tear film break-up time(BUT),and tear meniscus height(TMH),were detected before the intervention,1 day,1 week,2 weeks,and 4 weeks after the intervention.In addition,the corneas of each group were collected for hematoxylin and eosin(HE)staining to observe the changes of the corneal epithelium 4 weeks after the intervention.Finally,the non-cross-linking and cross-linking corneal stromal lenticules were placed separately in 1 g·L-1 of type Ⅰ collagenase to observe the change of the lenticule diameter over time.Results In the cross-linking plug group,the SIT,BUT and TMH values 2 and 4 weeks after the intervention were significantly higher than those before the intervention(all P<0.01).In the conventional plug group,the SIT,BUT and TMH values 4 weeks after intervention were significantly higher than those before the intervention(all P<0.01).After 4 weeks of intervention,the corneal staining of rabbits in the cross-linking plug group was better than that in the conventional plug group and model group.The HE staining results showed that in the cross-linking plug group,the corneal epithelial thickness of rabbits was basically normal,epithelial cells were basically arranged in order,and a single layer of columnar epithelial cells was observed on the basal layer.The dissolution time of the corneal stromal lenticule after cross-linking was significantly increased,compared with that of the non-cross-linking one.Conclusion The ultraviolet A-riboflavin cross-linking corneal stromal lenticule punctal plug has a more stable therapeutic effect on the rabbit dry-eye model than the conventional one,so it is expected to provide a novel approach for the clinical treatment of dry eyes.
8.The effectiveness and safety of a fully biodegradable occluder for the treatment of ventricular septal defects through percutaneous and transthoracic surgical approaches

Objective:To analyze the efficacy and safety of a fully biodegradable occluder for the treatment of ventricular septal defects through percutaneous and transthoracic strategies.Methods:A case series study was conducted.The clinical data of 38 pediatric patients with a ventricular septal defect treated with a fully biodegradable occluder at Central China Fuwai Hospital from January 2023 to July 2024 were retrospectively analyzed.Among these patients, 15 received the percutaneous approach(percutaneous approach group) and the other 23 adopted the transthoracic approach(transthoracic approach group).The diagnosis was confirmed by transthoracic echocardiography before operation in all patients.The percutaneous approach was defined as establishing a venous-arterial track through X-ray and then placing an occluder under the guidance of transthoracic echocardiograph.The transthoracic approach was achieved by establishing a delivery track with a special hollow bougie through a small right subaxillary incision under the real-time guidance of esophageal ultrasound and then delivery and put an occluder.The clinical data of the patients, including general characteristics, electrocardiograms, echocardiograms and the biodegradable occluder system were collected and analyzed.Categorical variables were tested using the chi-square or Fisher′s exact test.Continuous variables were verified using the t-test or Mann-Whitney U test. Results:The patients were aged (5.7±3.9) years on average, with an average weight of 19.5(14.9, 25.9) kg.There were 39.5%(15 cases) of males among the patients included.The average size of the ventricular septal defect was 4.1(4.0, 5.0) mm.A simple perimembrane ventricular septal defect was detected in 29 patients (76.3%), concomitant membranous aneurysm in 4 patients (10.5%), an intracristal ventricular septal defect in 3 patients (7.9%), and a muscular ventricular septal defect in 2 patients (5.3%).Preoperative aortic and tricuspid valve regurgitations accounted for 5.3%(2/38) and 81.6%(31/38), respectively.The average age was (9.0±3.9) years in the percutaneous approach group and (3.6±1.9) years in the transthoracic approach group.In terms of the cardiac structure, the percutaneous approach group had smaller Z values of the left atrial anterior-to-posterior diameter (-0.5±0.6 vs.0.5±1.0, P<0.01) and the left ventricular end-diastolic diameter (-0.5±1.1 vs.0.8±0.8, P<0.01), and a smaller ventricular septal defect [4.0(3.9, 4.2) mm vs.4.5(4.0, 5.5) mm, P=0.01] than the transthoracic approach group.Regarding the operation, the percutaneous approach group had a larger difference between the waist diameter of the selected occluder and the diameter of the ventricular septal defect [2.8(2.0, 3.0) mm vs.2.0(1.5, 2.5) mm, P=0.02], shorter operative time [(61.5±27.3) minutes vs.(91.5±31.4) minutes, P=0.01], and a shorter hospital stay [8(5, 9) days vs.12(9, 15) days, P<0.01] than the transthoracic approach group.Both groups achieved immediate occlusion postoperatively, with no residual shunts and no grade Ⅲ atrioventricular conduction block.Five new cases of bundle branch blocks and 1 case of trivial aortic valve regurgitation occurred in the transthoracic approach group. Conclusions:Both percutaneous and transthoracic approaches are safe and effective in interventional closure of ventricular septal defects, but the former is more applicable to slightly older or heavier patients with a smaller ventricular septal defect, who need a larger occluder.
9.Clinical Characteristics and Survival Analysis of 34 Patients with Aggressive NK-Cell Leukemia

Objective:To explore the clinical characteristics and prognosis risk factors of aggressive NK-cell leukemia(ANKL).Methods:The clinical and laboratory data of 34 patients with ANKL and 15 patients with chronic lymphoproliferative disorders of NK cells(CLPD-NK)admitted to the First Affiliated Hospital of Zhengzhou University from September 2019 to December 2024 were retrospectively analyzed.The Kaplan-Meier method was used to calculate survival rates,and the Cox proportional hazards regression model was used to analyze prognostic factors.Results:Compared with CLPD-NK patients,ANKL patients had a younger median age of onset,a higher proportion patients with EBV-DNA≥500 copies/ml,hepatosplenomegaly and hemophagocytic syndrome.They also presented with a higher peak of fever,a shorter median survival time,lower WBC count,PLT count,ALB and Fib values,while having higher LDH,AST,TG,ferritin,CRP and PCT levels.There were statistically significant differences in the morphology and expression of HLA-DR,CD56,CD57,CD16 and CD158 on abnormall cells between ANKL patients and CLPD-NK patients.Multivariate survival analysis revealed that combined with asparaginase treatment could improve patients' survival,and CRP≥ 15 mg/L and Fib<2.0 g/L were independent risk factors affecting the overall survival of patients with ANKL.Conclusion:The differences in clinical features and laboratory tests between patients with ANKL and CLPD-NK aid in the diagnosis of ANKL.CRP and Fib levels can be used to predict the prognosis of patients,and combined asparaginase therapy can enhance the overall survival of patients.

Objective:To investigate the clinical features and multi-slice spiral computed tomography(CT)imaging features of acute appendicitis in soldiers who have been living in plateau for a long time.Methods:The clinical features and imaging data of 56 cases of acute appendicitis in soldiers who have been living in plateau for a long time confirmed by surgery from February 2022 to August 2024 were retrospectively analyzed.Results:In 56 cases with acute appendicitis in soldiers who have been living in plateau for a long time,the appendectomy position results showed:anterior ileum 4 cases(7.14%),lower ileum 10 cases(17.86%),posterior cecum 16 cases(28.57%),lower cecum 9 cases(16.07%),lateral cecum 2 cases(3.58%),posterior ileum 6 cases(10.71%),high(subhepatic)9 cases(16.07%),and left lower abdominal 0 cases,retroperitoneal appendicitis 0 cases,which was suggested that the anatomical position variation of appendicitis in soldiers with acute appendicitis who have been living at high altitude for a long time was relatively large.The direct manifestations of multi-slice spiral CT showed:appendectomy enlarged diameter>6 mm in 49 cases(87.50%),appendicular wall thickening>2 mm in 42 cases(75.00%),ppendiceal dilation lumen and effusion in 29 cases(51.79%),appendix indistinctness in 3 cases(5.36%),lppendix fecalith:27 cases(48.21%),gas in the appendix in 16 cases(28.57%).Indirect findings of multi-slice spiral CT showed that,periappendiceal exudation with shadow in 32 cases(57.14%),appendiceal cellulitis with peripheral abscess in 9 cases(16.07%),peritonitis and ascites in 13 cases(23.21%),ileocecal intestinal wall thickening in 22 cases(39.29%),mesenteric lymph node enlargement in 16 cases(28.57%),reflexive intestinal stasis in ileocecal region was observed in 19 cases(33.93%).Conclusion:In the officers and soldiers with acute appendicitis who lived at high altitude for a long time,multi-slice spiral CT showed the direct manifestations of appendiceal thickening,tube wall thickening,lumen dilatation,fluid accumulation,etc.,and the indirect manifestations were periappendiceal exudation with shadow,appendiceal cellulitis with peripheral abscess,ileocecal intestinal wall thickening,reflexes of small intestine and mesenteric lymph node enlargement.Multi-slice spiral CT has the advantages of clear and intuitive,high safety,high resolution and simple operation in the diagnosis of acute appendicitis.
