Irritable bowel syndrome (IBS) is a functional bowel disorder characterized primarily by abdominal pain and altered defecation habits. In recent years, traditional Chinese medicine (TCM) has made progress in multiple aspects of IBS research and treatment, including syndrome distribution, development of TCM formulas, clinical efficacy evaluation, external therapies, and psychosocial regulation. However, it still faces challenges such as over-reliance on symptomatic manifestations rather than biomarkers for diagnostic criteria, and the lack of high-quality evidence-based data supporting the efficacy of TCM formulas in treating IBS. This paper proposed that TCM diagnosis and treatment of IBS should adhere to the strategy of integrating the holistic concept with syndrome differentiation and treatment, combining TCM external therapies such as acupuncture, moxibustion and acupoint application), and emphasizing individualized diagnosis and treatment for psychosomatic abnormalities. Future research should integrate multi-omics technologies, artificial intelligence and other methods to deepen the understanding of the pathogenesis of IBS and the mechanisms of TCM formulas, so as to promote the standardization and internationalization of TCM in the diagnosis and treatment of IBS.
2.Exploration of the Staged and Phased Differentiation and Treatment Approach for Gastric Cancer Immunotherapy Based on the Theory of Shaoyang (少阳) and Shaoyin (少阴) as the Pivots

Based on the theory that Shaoyang (少阳) and Shaoyin (少阴) function as the principal pivots, and integrating the laws of qi-fire transformation, yin-yang alternation, and the waxing and waning of healthy qi and pathogenic factors, as well as the action characteristics of immune checkpoint inhibitors (ICIs), this study systematically expounds the dynamic evolution laws of the Shaoyang and Shaoyin pivots and constructs a stage- and phase-based differentiation and treatment framework for gastric cancer immunotherapy. In the neoadjuvant treatment stage, the core pathogenesis is the dysfunction of the Shaoyang pivot and the disorder of qi and fire transformation, often accompanied by stagnation of cold-deficiency in the middle jiao (焦). In the postoperative adjuvant treatment stage, the dominant factors are the depletion of Shaoyin fire and the damage of yin fluid, accompanied by the lingering of residual pathogen in the Shaoyang level and the obstruction of collaterals by phlegm and stasis. In the advanced stage, the critical conditions are the exhaustion of the Shaoyin pivot, the blockage of orifices by turbid toxins, and the separation of yin and yang, leading to the rampant of pathogenic toxins and the emergence of various complicated syndromes. The treatment should be guided by the principle of regulating the double pivot. In the neoadjuvant treatment stage, it is recommended to activate the Shaoyang pivot and warm and promote the qi movement in the middle jiao. In the postoperative adjuvant treatment stage, clearing the residual pathogenic factors from the pivot and nourishing the Shaoyin. In the advanced stage, rescuing the Shaoyin pivot and opening the orifices to transform turbidity. Furthermore, stage-based treatment should serve as the foundation for dynamically assessing the patient's immune status and phase-specific changes in immunity, thereby promoting immune activation while preventing and managing immune overactivation and drug resistance.
3.Cultivation mode of rehabilitation therapy talents in local colleges and universities under the background of New Medicine

ObjectiveTo explore the training path of rehabilitation therapy professionals in local colleges and universities under the background of New Medicine. MethodsTaking Jining Medical University as an example, combining the interdisciplinary integration, intelligent driving and people-oriented New Medicine attributes of rehabilitation therapy, this study focused on the current practical problems in rehabilitation education, accurately aligned with the requirements of new medicine construction, reshaped the curriculum system with new concepts, empowered practical innovation with new technologies, and cultivated humanistic literacy with new connotations. By deeply integrating digital construction with educational practice, it carried out the construction and practice of the New Medicine talent training model in rehabilitation therapy, helping students form a stereoscopic knowledge structure, possess the ability to apply intelligent rehabilitation technology and solve complex rehabilitation problems, and become high-quality rehabilitation therapy talents with both morality and ability. ResultsThe overall construction of the rehabilitation therapy major established a model of Dual fusion foundation, Three element empowerment and Three dimensional soul casting, including reform and innovation in curriculum system, practical mode and moral education mechanism. It has formed a health oriented "vertical and horizontal integration-discipline fusion" dual-fusion stereoscopic curriculum system, an intelligent led "theory and practice-science and education-industry and education" three element integration practical mode, and a confucian medical culture "curriculum-practice-evaluation" three-dimensional integrated moral education mechanism. Through practical testing, this model achieved significant results and was widely recognized, including a significant improvement in the quality of talent cultivation, prominent achievements in resource construction, and prominent achievements in health services, providing strong support for the construction of regional rehabilitation medical systems. ConclusionThe reform of the rehabilitation treatment talent training model at Jining Medical University under the background of New Medicine provides a replicable and promotable practical paradigm for the training of rehabilitation talents in similar universities through reshaping the curriculum system, strengthening practical innovation, and cultivating humanistic literacy.

6.Clinical efficacy of Huangkui capsules in the treatment of targeted drug-related proteinuria in patients with hepatocellular carcinoma

Objective To investigate the therapeutic effect of Huangkui capsules on targeted drug-related proteinuria in patients with hepatocellular carcinoma (HCC). Methods A retrospective analysis was conducted on clinical data of HCC patients with targeted drug-related proteinuria from June 2023 to December 2024 at Zhongshan Hospital, Fudan University. According to the treatment plan, patients were divided into the conventional treatment group and the Huangkui combination treatment group (Huangkui capsules combined with conventional treatment), and the clinical efficacy between the two groups was compared. The logistic regression analysis was used to identify the main factors affecting treatment efficacy. Results The Huangkui combination treatment group (n=29) showed a significantly higher overall effective rate (79.3% vs 42.3%, P=0.005), and an earlier proteinuria improvement (median time: 3 months vs 6 months, P=0.008) than the conventional treatment group (n=26) . The multivariate logistic regression analysis showed angiotensin-converting enzyme inhibitor (ACEI) or angiotensin Ⅱ receptor blocker (ARB) using (OR=0.190, 95%CI 0.045-0.808, P=0.025), targeted drug adjustment (OR=0.132, 95%CI 0.030-0.581, P=0.007), and Huangkui capsules using (OR=0.168, 95%CI 0.039-0.730, P=0.017) were protective factors for treatment efficacy of targeted drug-related proteinuria. Conclusions On the basis of conventional treatment, additive treatment with Huangkui capsules can alleviate targeted drug-related proteinuria faster and more effectively in HCC patients.
7.Rehmanniae Radix Iridoid Glycosides Protect Kidneys of Diabetic Mice by Regulating TGF-β1/Smads Signaling Pathway

ObjectiveTo investigate the protective effect of Rehmanniae Radix iridoid glycosides (RIG) on the kidney tissue of streptozotocin (STZ)-induced diabetic mice and explore the underlying mechanism. MethodsTwelve of 72 male C57BL/6J mice were randomly selected as the normal group, and the remaining 60 mice were fed with a high-fat diet for six weeks combined with injection of 60 mg·kg-1 STZ for 4 days to model type 2 diabetes mellitus. The successfully modeled mice were randomized into model, metformin (250 mg·kg-1), catalpol (100 mg·kg-1), low-dose RIG (RIG-L, 200 mg·kg-1) and high-dose RIG (RIG-H, 400 mg·kg-1) groups (n=11). Mice in each group were administrated with corresponding drugs, while those in the normal group and model group were administrated with the same dose of distilled water by gavage once a day. After 8 weeks of intervention, an oral glucose tolerance test (OGTT) was performed, and the area under the curve (AUC) was calculated. After mice were sacrificed, both kidneys were collected. The body weight, kidney weight, and fasting blood glucose (FBG) were measured. Biochemical assays were performed to measure the serum levels of triglycerides (TG), total cholesterol (TC), serum creatinine (SCr), and blood urea nitrogen (BUN). Enzyme-linked immunosorbent assay (ELISA) was employed to determine the serum level of fasting insulin (FINS), and the insulin sensitivity index (ISI) and homeostatic model assessment for insulin resistance (HOMA-IR) were calculated. The pathological changes in kidneys of mice were observed by hematoxylin-eosin staining and Masson staining. The immunohistochemical method (IHC) was employed to assess the expression of interleukin-1 (IL-1), interleukin-6 (IL-6), tumor necrosis factor-α(TNF-α), transforming growth factor-β1 (TGF-β1), and collagen-3 (ColⅢ) in the kidney tissue. The protein levels of TGF-β1, cell signal transduction molecule 3 (Smad3), matrix metalloproteinase-9 (MMP-9), and ColⅢ in kidneys of mice were determined by Western blot. ResultsCompared with the normal group, the model group showcased decreased body weight and ISI (P<0.01), increased kidney weight, FBG, AUC, FINS, HOMA-IR, TC, TG, SCr, and BUN (P<0.01), glomerular hypertrophy, capsular space narrowing, and collagen deposition in the kidney, up-regulated protein levels of IL-1, IL-6, TNF-α, TGF-β1, ColⅢ, and Smad3 (P<0.01), and down-regulated protein level of MMP-9 (P<0.01) in the kidney tissue. Compared with the model group, the treatment groups had no significant difference in the body weight and decreased kidney weight (P<0.05, P<0.01). The FBG level declined in the RIG-H group after treatment for 4-8 weeks and in the metformin, catalpol, and RIG-L groups after treatment for 6-8 weeks (P<0.01). The AUC in the RIG-L, RIG-H, and metformin groups decreased (P<0.05, P<0.01). The levels of TC, SCr, and BUN in the serum of mice in each treatment group became lowered (P<0.05, P<0.01). The level of TG declined in the RIG-L, RIG-H, and metformin groups (P<0.05, P<0.01). The serum level of FINS declined in the catalpol, RIG-L, and metformin groups (P<0.01). Compared with the model group, the treatment groups showed decreased HOMA-IR (P<0.01), increased ISI (P<0.01), alleviated pathological changes in the kidney tissue, and down-regulated expression of IL-1 and TGF-β1. In addition, the protein levels of IL-6, TNF-α, and ColⅢ in the RIG-H and metformin groups and IL-6 and TNF-α in the RIG-L group were down-regulated (P<0.05, P<0.01), and the protein levels of IL-6, TNF-α, and ColⅢ in the catalpol group and ColⅢ in the RIG-L group showed a decreasing trend without statistical difference. The protein levels of TGF-β1, Smad3, and ColⅢ in the RIG-H and metformin groups were down-regulated (P<0.01). Compared with that in the model group, the protein level of MMP-9 was up-regulated in each treatment group (P<0.01). ConclusionRIG can improve the renal structure and function of diabetic mice by regulating the TGF-β1/Smads signaling pathway.
8.Dual-modal Ultraviolet-Fluorescence Sensor Based on Acetaldehyde Dehydrogenase/Bovine Serum Albumin System for Acetaldehyde Detection in Food

In this study,an ultraviolet(UV)/fluorescence dual-modal sensor was constructed by combining the catalytic properties of aldehyde dehydrogenase(ALDH)with the fluorescence inner filter effect of bovine serum albumin(BSA),realizing highly sensitive and highly selective detection of acetaldehyde in food.Acetaldehyde could react with oxidized coenzyme I(NAD+)to generate acetic acid and reduced coenzyme I(NADH)under catalytic condition by ALDH,and a quantitative relationship between acetaldehyde concentration and UV signal for UV detection based on the characteristic UV absorption peak of NADH at 340 nm was established.Meanwhile,an acetaldehyde detection channel based on fluorescence signal changes was built on the basis of the property that the fluorescence emission of BSA(as a fluorescent indicator)at 340 nm could be effectively quenched by the generated NADH,thus forming a″one-reaction dual-signal″detection mode.The experimental results showed that the acetaldehyde concentration in the range of 0.01?5.0 mg/L had a good linear relationship with both UV and fluorescence signals,and the limit of detection(LOD)was 0.003 mg/L for the UV mode and 0.005 mg/L for the fluorescence mode,and no significant fluctuations were observed when 50-fold concentrations of acetaldehyde analogs and common interfering substances were added.When the dual-modal sensor was applied to detection of acetaldehyde in food samples such as yogurt and wine,the relative error between its quantitative results and those of high-performance liquid chromatography(HPLC)was less than±4.0%.The dual-modal cross-validation strategy could improve the detection reliability through signal mutual verification,providing an innovative solution for the rapid detection of acetaldehyde contamination in food.
9.Analysis of Methadone-Related Poisoning Cases

Objective To analyze the characteristics of methadone-related poisoning cases and provide a reference for forensic identification.Methods A total of 71 cases of methadone-related poisoning re-ported from 1998 to 2023 in China and 26 cases of methadone-related deaths reported from 2013 to 2018 in Italy were retrieved from databases including PubMed,Wanfang and CNKI.The general infor-mation,forensic pathological and toxicological characteristics were analyzed.Results Among the 71 methadone-related poisoning cases in China,there were 54 cases(76.06%)of poisoning without death and 17 cases(23.94%)of death from poisoning.There were 54 male cases(76.06%),and 51 cases(71.83%)aged 19 to 39 years old.There were 35 cases(49.30%)of poisoning caused by methadone alone,and 32 cases(45.07%)were poisoning caused by methadone combined with other substances or drugs including heroin and benzodiazepines.Most of the poisoned showed coma,respiratory depres-sion and miosis.Signs of asphyxia were often found by autopsy.The mass concentration of methadone detected in the blood of 6 deceased ranged from 0.112 to 3.000 mg/L.Among the 26 methadone-related deaths in Italy,22 cases were male(84.62%).There were 6 cases(23.08%)caused by methadone alone,and 20 cases(76.92%)died from methadone combined with other substances or drugs.The mass concentration of methadone in blood ranged from 0.181 to 4.059 mg/L.Conclusion The propor-tions of poisoning cases caused by methadone alone and methadone combined with other substances or drugs are comparable in China.The majority of deceased caused by methadone poisoning shows typi-cal triad of coma,respiratory depression and miosis,which helps forensic experts determine the cause of death related to methadone.Additionally,it is necessary to increase the routine testing of the con-centration of methadone and its combined substances or drugs in deceased,and collect data for the in-terpretation of the results of related cases.
10.Serum Nesfatin-1 and Klotho levels and their predictive value for secondary mild cognitive impairment in elderly patients with type 2 diabetes

Objective To investigate serum levels of food intake inhibitory factor-1(Nesfatin-1)and Klotho and their predictive value for secondary mild cognitive impairment(MCI)in elderly patients with type 2 diabetes mellitus(T2DM).Methods A total of 118 elderly patients with T2DM diagnosed and treated in the hospital from April 2023 to March 2024 were selected as the T2DM group,and they were divided into the non-MCI group(n=71)and the MCI group(n=47)according to the Montreal Cognitive Assessment(Mo-CA)scale.In addition,110 healthy people in the same hospital during the same period were selected as the control group.The clinical data of the patients were collected.Serum Nesfatin-1 and Klotho levels were detec-ted by enzyme-linked immunosorbent assay.Spearman and Pearson correlation analysis were used to analyze the correlation of serum Nesfatin-1 and Klotho levels with MoCA score and related clinical indicators in elder-ly patients with T2DM.Multivariate Logistic regression analysis was used to analyze the influencing factors for secondary MCI in elderly patients with T2DM.Receiver operating characteristic(ROC)curve was used to evaluate the predictive value of serum Nesfatin-1 and Klotho levels for secondary MCI in elderly patients with T2DM.Results Compared with control group,the serum levels of Nesfatin-1 and Klotho were significantly decreased in T2DM group(P<0.05).The serum levels of Nesfatin-1 and Klotho in MCI group were signifi-cantly lower than those in non-MCI group(P<0.05).Compared with the non-MCI group,the levels of fast-ing plasma glucose(FPG),insulin resistance index(HOMA-IR),reactive oxygen species(ROS)and C-reac-tive protein(CRP)were significantly increased in the MCI group(all P<0.05),and were negatively correla-ted with serum Nesfatin-1 and Klotho levels(all P<0.05).The serum levels of Nesfatin-1 and Klotho were positively correlated with MoCA score(P<0.05).Increased levels of FPG and ROS and decreased levels of Nesfatin-1 and Klotho were risk factors for secondary MCI in T2DM patients(P<0.05).The area under the curve of serum Nesfatin-1,Klotho and their combination for predicting secondary MCI in T2DM patients was 0.803,0.829 and 0.932,respectively.The combined prediction of serum nesfatin-1 and Klotho was better than each index alone(Zcombined-Nesfatin-1=3.421,P=0.001,Zcombined-Klotho=2.980,P=0.003).Conclusion The serum lev-els of Nesfatin-1 and Klotho are decreased in T2DM patients,which are significantly correlated with secondary MCI in T2DM patients,and both of them have high predictive value for secondary MCI in T2DM patients.
