1.Investigating Molecular Mechanisms of Qijia Rougan Prescription and Its Key Effect or Ingredients Against Hepatic Fibrosis Based on Macrophage M2 Polarization

ObjectiveBased on the regulation of macrophage M2 polarization, this study aims to explore the molecular mechanism and action targets of the Qijia Rougan prescription and its key effector ingredients in anti-fibrosis, thereby providing a basis and reference for the development of new drugs for hepatic fibrosis. MethodsA rat model of hepatic fibrosis was established by subcutaneous injection of 40%CCl4, followed by oral administration of Qijia Rougan granules. The volume of collagen fibers was detected using Masson staining, the fibrosis markers Collagen Ⅰ and α-SMA were detected using immunohistochemistry, the proportion of M2 macrophages was detected by flow cytometry. The expression levels of M2 macrophage phenotype markers CD163 and CD206 were detected using immunofluorescence double staining. Western blot was used to detect the levels of the transforming growth factor-β (TGF-β), platelet derived growth factor subunit B (PDGFB), interleukin-10 (IL-10), phosphorylated Janus kinase 1 (p-JAK1), and phosphorylated signal transducer and activator of transcription 6 (p-STAT6). Real-time fluorescent quantitative PCR was used to detect the relative expression levels of JAK1, STAT6, Arginase 1(Arg1), and Fizz1. Based on the theory of serum pharmacology, liquid chromatography-mass spectrometry and WENN analysis were used to obtain the active ingredients of Qijia Rougan prescription. Molecular docking and molecular dynamics simulation were performed to analyze the effector ingredients and their targets. The identified effector ingredients were interfered with IL-4-induced M2 polarization of RAW264.7 macrophage in vitro to validate the targets. ResultsQijia Rougan prescription significantly reduced the content of fibrosis markers α-SMA and Collagen Ⅰ, as well as collagen fiber content (P<0.05). It decreased the proportion of M2 macrophages and the levels of related cytokines IL-10, TGF-β and PDGFB, and up-regulated the levels of p-JAK1 and p-STAT6 (P<0.05). A total of 1 214 compounds were identified from Qijia Rougan prescription, medicated serum and blank serum, and 29 ingredients were finalized by Venn analysis, including 15 blood-entry prototypes and 14 drug metabolites. Molecular docking showed that enoxolone and berberine bound more strongly to JAK1, with binding free energies of -9.6 kcal·mol-1(1 cal≈4.184 J) and -9.1 kcal·mol-1, respectively. Molecular dynamics simulations showed that JAK1-enoxolone and JAK1-berberine exhibited stable simulation trajectories within 100 ns, with essentially identical conformations and high protein overlap before and after simulation. Their binding free energies were -25.18 5.0.81 kcal·mol-1 and -27.39 7.0.85 kcal·mol-1, respectively. The number of hydrogen bonds formed between JAK1 and enoxolone ranges from 0 to 5, and most of the time can be maintained at 2-3. In vitro intervention with enoxolone or berberine significantly reduced p-JAK1 and p-STAT6 levels (P<0.05). ConclusionQijia Rougan prescription inhibits M2 macrophage polarization in hepatic fibrosis. Enoxolone and berberine are the key effector ingredients of Qijia Rougan prescription to inhibit macrophage M2 polarization through targeting JAK1 and modulating the JAK1/STAT6 signaling pathway, thereby ameliorating hepatic fibrosis. This study provides a basis for prescription optimization, clinical application and new drug development, as well as a reference for monolithic anti-hepatic fibrosis research.
2.Correlation Between Adverse Reactions in Adjuvant Endocrine Therapy and Cytokines for Early-Stage Luminal-Type Breast Cancer Patients

Objective To investigate the correlation between cytokines and the side effects of estrogen deprivation associated with adjuvant endocrine therapy in female patients with early luminal breast cancer. Methods A retrospective analysis was performed on the data of 70 patients with early-stage breast cancer who underwent both the Endocrine Symptom Scale (FACT-ES) assessment and cytokine testing at the Breast Clinic of the Department of Medical Oncology, Cancer Hospital, Chinese Academy of Medical Sciences, between April 2023 and June 2023. Results Binary logistic regression analysis revealed that breast cancer patients experiencing hot flashes had significantly higher serum levels of IL-5 compared with those without hot flashes (3.17 pg/ml vs. 2.33 pg/ml, OR: 1.307-7.66, P=0.011), and the patients experiencing irritability had significantly lower serum levels of IL-10 (0.83 pg/ml vs. 1.37 pg/ml) and INF-γ (19.91 pg/ml vs. 35.93 pg/ml) compared with those without irritability(OR: 0.855-0.983, P=0.015). Conclusion The elevated IL-5 may be associated with the occurrence of hot flashes, while lower levels of IL-10 and INF-γ may be linked to irritability in patients with early-stage Luminal breast cancer treated with adjuvant endocrine therapy.

Sepsis is a life-threatening organ dysfunction disease caused by a dysregulated host response to infection.It has com-plex pathophysiological changes,and in severe cases,it can rap-idly develop into septic shock and multiple organ dysfunction or multiple organ failure.At present,the pathological mechanism of sepsis and its induced organ dysfunction is complex and the in-fluencing factors are numerous.So far,there is still a lack of specific and effective treatment strategies.RNA modify-N6-methyladenosine(m6 A)is one of the most common post-tran-scriptional modifications on eukaryotic RNAs.It is involved in the regulation of the occurrence and development of a variety of inflammatory diseases,including sepsis,and even multiple organ dysfunction induced by sepsis by affecting the metabolism of RNAs.It includes cardiac dysfunction,acute lung injury(ALI)and acute kidney injury(AKI).Therefore,this article will dis-cuss the effect of m6A modification on the function of immune cells,and its important role in sepsis and its induced multiple or-gan dysfunction diseases by regulating inflammatory signals,py-roptosis,mitochondrial damage and ferroptosis.This will provide new therapeutic targets and strategies for the clinical prevention and treatment of sepsis and its induced multiple organ dysfunc-tion diseases.
4.The relationship between the serum levels of vascular endothelial growth factor, matrix metalloproteinase-9, S100 calcium binding protein with glycolipid metabolism, pregnancy outcome in pregnant women with gestational diabetes

Objective:To investigate the relationship between the serum levels of vascular endothelial growth factor (VEGF), matrix metalloproteinase-9 (MMP-9), S100 calcium binding protein B (S100B) with glycolipid metabolism, pregnancy outcome in pregnant women with gestational diabetes.Methods:The clinical data of 153 pregnant women with gestational diabetes (research group) and 153 healthy pregnant women (control group) in the Second Affiliated Hospital of Xi ′an Medical University from January 2020 to October 2023 were retrospectively analyzed. The serum levels of VEGF, MMP-9 and S100B were measured by enzyme linked immunosorbent assay, and the fasting blood glucose, triglyceride, total cholesterol, high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), fasting insulin and glycated hemoglobin were measured, and the homeostasis model assessment insulin resistance index (HOMA-IR) was calculated. The adverse outcomes of pregnant women with gestational diabetes were recorded. Pearson method was used to analyze the correlation between glycolipid metabolism indexes and VEGF, MMP-9, S100B in pregnant women with gestational diabetes. Multivariate Logistic regression was used to analyze the independent risk factors of adverse pregnancy outcome in pregnant women with gestational diabetes. Receiver operating characteristic (ROC) curve was drawn to analyze the predictive value of VEGF, MMP-9 and S100B on adverse pregnancy outcome in pregnant women with gestational diabetes. Results:The fasting blood glucose, fasting insulin, glycated hemoglobin, HOMA-IR, triglyceride, total cholesterol, LDL-C, VEGF, MMP-9 and S100B in research group were significantly higher than those in control group: (9.42 ± 0.65) mmol/L vs. (4.13 ± 0.46) mmol/L, (16.58 ± 2.37) mU/L vs. (13.41 ± 2.05) mU/L, (7.28 ± 0.46)% vs. (4.35 ± 0.39)%, 4.83 ± 0.42 vs. 2.71 ± 0.37, (3.41 ± 0.67) mmol/L vs. (2.85 ± 0.63) mmol/L, (5.54 ± 1.56) mmol/L vs. (5.12 ± 1.50) mmol/L, (3.14 ± 0.97) mmol/L vs. (2.86 ± 0.93) mmol/L, (184.02 ± 30.25) ng/L vs. (156.33 ± 26.41) ng/L, (45.78 ± 7.56) μg/L vs. (29.36 ± 5.03) μg/L and (117.51 ± 25.12) ng/L vs. (89.74 ± 22.46) ng/L, the HDL-C was significantly lower than that in control group: (1.34 ± 0.27) mmol/L vs. (1.42 ± 0.30) mmol/L, and there were statistical differences ( P<0.01 or <0.05). Pearson correlation analysis result showed that VEGF, MMP-9, S100B in pregnant women with gestational diabetes were positively correlated with fasting blood glucose, fasting insulin, glycated hemoglobin, HOMA-IR, triglyceride, total cholesterol and LDL-C ( P<0.01), negatively correlated with HDL-C ( P<0.01). Among 153 pregnant women with gestational diabetes, 49 had adverse pregnancy outcome, and 104 had good pregnancy outcome. The VEGF, MMP-9 and S100B in pregnant women with adverse pregnancy outcome were significantly higher than those in pregnant women with good pregnancy outcome: (212.75 ± 28.63) ng/L vs. (170.49 ± 26.58) ng/L, (52.37 ± 7.14) μg/L vs. (42.68 ± 6.35) μg/L and (136.83 ± 23.62) ng/L vs. (108.41 ± 21.35) ng/L, and there were statistical differences ( P<0.01). Multivariate Logistic regression analysis result showed that VEGF, MMP-9 and S100B were independent risk factors for adverse pregnancy outcome in pregnant women with gestational diabetes ( OR = 7.013, 5.382 and 6.129; 95% CI 5.206 to 9.447, 3.449 to 8.398 and 3.520 to 10.673; P<0.01). ROC curve analysis result showed that the area under the curve of VEGF, MMP-9 combined S100B in predicting adverse pregnancy outcome in pregnant women with gestational diabetes was significantly larger than that of VEGF, MMP-9 and S100B alone (0.945 vs. 0.863, 0.847 and 0.801; P<0.05 or <0.01), with sensitivity of 89.80% and specificity of 91.30%. Conclusions:The high serum levels of VEGF, MMP-9 and S100B are associated with abnormal glycolipid metabolism and adverse pregnancy outcome in pregnant women with gestational diabetes, and the combination of the three indexes has a high predictive value for adverse pregnancy outcome.
5.Association of Cardiac Magnetic Resonance Structural Parameters With Myocardial Fibrosis and Their Predictive Value for Major Adverse Cardiovascular Events in Patients With Hypertrophic Cardiomyopathy

Objectives:To investigate the association between cardiac magnetic resonance(CMR)structural parameters and myocardial fibrosis in patients with hypertrophic cardiomyopathy(HCM),and to assess their predictive value for major adverse cardiovascular events(MACE).Methods:A total of 120 patients with HCM who underwent CMR examination at Affiliated Hospital of Guangdong Medical University between December 2021 and December 2023 were retrospectively enrolled.Patients were divided into the MACE group and the non-MACE group.MACE included acute myocardial infarction,angina pectoris,stroke,heart failure,severe arrhythmia.The CMR parameters were compared between the two groups.The correlation between CMR structural parameters and the myocardial fibrosis index(number of myocardial fibrotic segments/total segment number×100%)was evaluated using the locally weighted scatterplot smoothing method(LOWESS).Multivariate Cox regression was performed to identify the influencing factors of MACE.Threshold effect analysis of CMR structural parameters was conducted,and receiver operating characteristic(ROC)curves were generated to evaluate the diagnostic performance of CMR structural parameters for MACE.Results:During a follow-up period of 12(5,30)months,a total of 78(65.0%)patients developed MACE(the MACE group),and the remaining 42(35.0%)patients were assigned to the non-MACE group.Compared with the non-MACE group,patients in the MACE group had significantly larger left atrial diameter,left ventricular end-diastolic maximum diameter,maximal interventricular septal thickness,maximal left ventricular wall thickness,and higher myocardial fibrosis index(all P<0.05).The LOWESS analysis showed significant positive correlations between the above CMR structural parameters and the myocardial fibrosis index(all Poverall<0.05).The multivariate Cox regression showed that the echocardiographic interventricular septal thickness OR=2.622,95%CI:1.537-3.033),left ventricular posterior wall thickness(OR=1.597,95%CI:1.353-2.420),as well as CMR-derived left atrial diameter(OR=1.623,95%CI:1.314-2.595),left ventricular end-diastolic maximum diameter(OR=1.927,95%CI:1.594-2.981),maximal interventricular septal thickness(OR=2.085,95%CI:1.426-2.563),and maximal left ventricular wall thickness(OR=2.306,95%CI:1.760-2.805)were independent predictors of MACE(all P<0.05).The threshold effect analysis revealed that the left atrial diameter,left ventricular end-diastolic maximum diameter,maximal interventricular septal thickness,and maximal left ventricular wall thickness were all positively associated with the risk of MACE(all Poverall<0.05).The ROC curve analysis demonstrated that among individual CMR parameters,maximal interventricular septal thickness had the highest predictive performance(AUC=0.86,95%CI:0.74-0.95;sensitivity was 83.67%,specificity was 78.87%).Combined structural parameters(left atrial diameter,left ventricular end-diastolic maximum diameter,maximal interventricular septal thickness,and maximal left ventricular wall thickness)provided superior predictive accuracy(AUC=0.91,95%CI:0.84-0.99;sensitivity was 86.22%,specificity was 76.26%).Conclusions:HCM patients with MACE exhibit distinct clinical and imaging features compared with those without MACE.CMR structural parameters,including left atrial diameter,left ventricular end-diastolic maximum diameter,maximal interventricular septal thickness,and maximal left ventricular wall thickness,are closely related to myocardial fibrosis and strongly associated with MACE risk.A composite index integrating these parameters shows high predictive value for MACE.

Objective:To evaluate the value of ultrasound radiomics combined with machine learning for early diagnosis of seronegative Hashimoto’s thyroiditis (SN-HT) .Methods:This retrospective study included 164 patients from Liaoning Provincial People’s Hospital , Lixin County People’s Hospital, Linghai Dalinghe Hospital, Fengcheng Phoenix Hospital, who underwent thyroidectomy for solitary nodules with normal thyroid function between Nov. 2016 and Jan. 2024. Postoperative pathology confirmed Hashimoto’s thyroiditis (HT) in some cases, who were further categorized into antibody-positive and antibody-negative groups based on serum antibody status. Patients without Hashimoto’s thyroiditis served as the control group. A total of 298 ultrasound images were analyzed. Radiomics features were extracted from hypoechoic non-nodular areas within 0.5 cm surrounding the tumor. Two senior pathologists and two senior ultrasound physicians independently assessed lymphocytic infiltration, eosinophilic changes of follicular epithelium, and the proportion of hypoechoic areas in pathology and ultrasound images, respectively. A machine learning model, CCH-NET, was developed using linear regression and t-distributed stochastic neighbor embedding (t-SNE) techniques. The dataset was divided into a training set (80%) and a validation set (20%) to compare the diagnostic accuracy of CCH-NET with that of senior ultrasound physicians. Results:In internal validation, CCH-NET achieved a diagnostic accuracy of 88.89% for both antibody-positive and antibody-negative groups, significantly higher than the 66.67% accuracy of senior ultrasound physicians ( P<0.01). In external validation, CCH-NET achieved 75.00% and 66.67% accuracy for the two groups, compared to 50.00% by senior ultrasound physicians. For the control group, both methods achieved 93.33% accuracy. The AUC of CCH-NET was 0.848, outperforming senior ultrasound physicians (0.681) ,demonstrating superior diagnostic performance. Conclusion:The radiomics-based CCH-NET model, using non-nodular hypoechoic areas as a specific indicator, can accurately identify early SN-HT in euthyroid patients. It significantly outperforms senior ultrasound physicians, improving diagnostic accuracy and reducing missed diagnoses.

8.Efficacy and safety of a domestic hair follicle extraction system in extracting hair follicles from patients with androgenetic alopecia: a multicenter, prospective, randomized, self-controlled clinical trial

Objective:To compare the efficacy and safety of a domestic hair follicle extraction system versus traditional follicular unit excision (FUE) in extracting hair follicles for the treatment of androgenetic alopecia (AGA) .Methods:A multicenter, randomized, self-controlled clinical trial was conducted on AGA patients aged 18 - 59 years who were recruited from the Huashan Hospital, Fudan University, the Affiliated Hangzhou First People's Hospital, and the China-Japan Friendship Hospital between June 2023 and September 2024. Each patient's scalp was randomly divided into two sides (experimental side vs. control side) using an envelope method. The experimental side underwent robotic hair transplantation with a domestic hair follicle extraction system, and the control side underwent traditional FUE. Hair follicles were extracted from the safe donor area in the occipital region, and implanted into the ipsilateral hair loss area. The primary outcome was the hair transection rate which was calculated immediately after follicular extraction. The secondary outcomes included the hair follicle unit loss rate and the change in hair density at the recipient site on postoperative day 14. Safety was evaluated by assessing the incidence of folliculitis at the donor site on postoperative day 14 and the overall incidence of adverse events. Surgical outcomes were evaluated at 9 months after surgery. Comparisons of evaluation indicators among groups were performed by using a paired t test or Wilcoxon signed-rank test. Results:A total of 55 patients with AGA (51 males and 4 females, aged 32.71 ± 5.75 years) completed the hair follicle transplantation and postoperative follow-up. The hair transection rate ( M[ Q1, Q3]) was 6.65% (4.56%, 10.16%) in the experimental group and 5.28% (3.04%, 8.89%) in the control group (difference = 1.24%, 95% CI: -0.24%, 2.65%) . The hair follicle unit loss rate was 2.00% (1.00%, 3.50%) in the experimental group and 0.50% (0, 2.00%) in the control group, with a significant difference between the two groups ( P = 0.008) . On postoperative day 14, there was no significant difference in the hair density between the experimental group and control group (72.20 ± 25.95 per cm 2vs. 76.49 ± 30.84 per cm 2, P = 0.173) . At 9-month follow-up, both groups showed improvement in the investigator's overall score in the recipient areas. Seven adverse events occurred in 7 subjects (12.72%) in each group, and all were mild folliculitis. Conclusion:The domestic hair follicle extraction system demonstrated comparable efficacy and safety to the traditional FUE in hair transplantation.

This study investigated the disease characteristics and clinical traits of Nocardia infection,and analyzed the antibiotic resistance phenotypes,antibiotic resistance genes,and evolutionary characteristics of the strains,to provide a basis for Nocardia diag-nosis and treatment.A total of 31 Nocardia strains from a hospital in Hebei Province were collected from 2020 to 2023.The strains were identified through mass spectrometry and whole genome sequencing.Antibiotic susceptibility testing was conducted with the broth macrodilution method,and whole genome sequencing data were used to predict antibiotic resistance genes and comprehensively ana-lyze antibiotic resistance phenotypes.The 31 strains of Nocardia comprised 21 strains of Nocardia farcinica,3 strains of Nocardia ter-penica,three strains of Nocardia brasiliensis,two strains of Nocardia cyriacigeorgica,and two strains of Nocardia nova.The ceftriaxone susceptibility of 21 Nocardia farcinica strains was 85.7%,and all 31 strains were susceptible to imipenem,except for three strains of Nocardia brasiliensis.Rifampicin,aminoglycoside,and β-lactam resistance genes were found in Nocardia farcinica.Pathogenic tests should be carried out in a timely manner for suspected Nocardia infections.In clinical treatment of Nocardia infection,infected strains should be confirmed,and antibiotics should be used rationally according to the antibiotic susceptibility test results.

This paper focuses on the application of health big data in cancer screening. Firstly, the sources and characteristics of health big data are introduced, then the commonly used epidemiological designs and analytical techniques in hospital-based cancer screening studies are summarized and the application scenarios of such studies are described. Finally, the challenges and future development in the application of health big data are analyzed to provide reference for the future studies.
